The proposed device relates to medicine, namely to surgery and can be used for electrocoagulation (destruction) of pathological tissue. The device includes an electrosurgical generator operating in bipolar mode, connected by means of conductors with two electrodes. What is new is that each electrode is a needle and is configured to move in three-dimensional space relative to another electrode. The device allows for the necessary volume of electrocoagulation of pathological tissue, including located deep in the parenchymal organs, with a minimal risk of bleeding.
Description
The proposed utility model relates to medicine, namely to surgery, and can be used for electrocoagulation (destruction) of pathological tissue.
A device for conducting monopolar electrocoagulation (destruction) (I.V. Fedorov, A.T. Nikitin. Clinical Electrosurgery. Practical Guide. GEOTAR Medicine, Moscow, 1997). In this case, the device has one active and one passive electrodes connected by a conductor with an electrosurgical generator. The main disadvantage of this device is the inability to pre-determine the size of the tissue destruction zone, and the possibility of damage to surrounding tissues, due to the fact that the electric current from the active electrode to the passive electrode goes in different directions.
For the prototype of the proposed device, a device for bipolar electrocoagulation of tissue was selected, containing an electrosurgical generator operating in bipolar mode, connected by means of conductors to two electrodes made in the form of a branch of scissors coated with a dielectric, except for the cutting edge of the branch. The disadvantages of this device are:
1. невозможность перемещения каждой части (бранши-электрода) ножниц в трехмерном пространстве относительно друг друга, что ведет к невозможности провести полный необходимый объем электрокоагуляции (деструкции) патологической ткани;1. the inability to move each part (jaw-electrode) of scissors in three-dimensional space relative to each other, which leads to the inability to carry out the full necessary amount of electrocoagulation (destruction) of pathological tissue;
2. невозможность проведения электрокоагуляции (деструкции) патологической ткани, расположенной в глубине паренхиматозных органов, в связи с тем, что это потребовало бы внедрение бранш-электродов в глубину паренхиматозного органа, что в свою очередь, стало бы причиной большого риска возникновения кровотечения;2. the impossibility of conducting electrocoagulation (destruction) of pathological tissue located deep in the parenchymal organs, due to the fact that this would require the introduction of branch electrodes deep into the parenchymal organ, which, in turn, would cause a high risk of bleeding;
3. a thin layer of coagulation (destruction), which also does not lead to the full and necessary volume of coagulation of pathological tissue and increases the risk of bleeding, because a thin layer of coagulated tissue can be easily damaged.
The objective of the proposed utility model is to provide the ability to conduct the full and necessary volume of electrocoagulation (destruction) of pathological tissue, including those located deep in the parenchymal organs, with a minimal risk of bleeding.
The problem is solved in that each electrode is a needle and is configured to move in three-dimensional space relative to another electrode.
Figure 1 shows a diagram of the proposed device.
In figure 1 is indicated:
1. иглы-электроды;1. needle electrodes;
2. проводники;2. conductors;
3. электрохирургический генератор;3. electrosurgical generator;
4. патологическая ткань;4. pathological tissue;
5. ткань, расположенная между концами электродов;5. tissue located between the ends of the electrodes;
6. контакт иглы-электрода и проводника;6. contact of the needle electrode and conductor;
7. The ends of the needle electrodes, free of dielectric coating (active part).
The device consists of two needle electrodes (1) connected by means of conductors (2), (6 - contact of the conductor and needle electrode) with an electrosurgical generator (3) operating in a bipolar mode.
The device operates as follows: needle-electrodes are inserted into the pathological tissue (4) under visual control or under the control of radiation research methods (ultrasound, X-ray, computed tomography, etc.). After applying voltage between the ends of the needle electrodes free of dielectric coating (active part) (7), an alternating electric current of high frequency occurs. This leads to heating of the tissue and its subsequent coagulation (destruction). Moreover, only tissue located between the ends of the electrodes is coagulated (5). After electrocoagulation (destruction) of one section of pathological tissue, if necessary, the needle-electrodes are moved to another section of pathological tissue, and the process of electrocoagulation (destruction) is repeated until the destruction of the focus of the pathological tissue.
Using the proposed device, electrocoagulation (destruction) of hyperfunctioning thyroid formations was performed in 20 patients. The results showed that the hyperfunctioning formations of the thyroid gland were destroyed, and the thyroid function returned to normal, while there were no cases of bleeding.
