Изобретение относится к медицине, а именно к челюстно-лицевой хирургии, и может быть использовано для хирургического устранения деформации ушной раковины типа «сложенного» (свернутого) уха при значительном дефиците хрящевой ткани.The invention relates to medicine, namely to maxillofacial surgery, and can be used for surgical elimination of deformation of the auricle such as a “folded” (rolled) ear with a significant deficiency of cartilage tissue.
При сложенной ушной раковине ее верхняя часть перегибается вперед и вниз. В зависимости от степени тяжести поражения выделяют 3 степени деформации. При I степени расширенная часть завитка перегибается вниз, и его верхний край расположен на уровне латеральной ножки противозавитка. Вертикальный размер ушной раковины уменьшен, но при разгибании (перемещение в правильное положение) близок к норме. При II степени край завитка находится на уровне верхней части чаши, противозавиток недоразвит. Вертикальный размер раковины уменьшен. Переместить пальцами завиток вверх не удастся из-за дефицита тканей. При III степени край завитка расположен на уровне козелка, недоразвита и смещена кпереди вся верхняя часть ушной раковины. Известны методы устранения врожденной деформации сложенных ушных раковин, показанные в зависимости от степени поражения. Чаще всего используют методику Musgrave (1966), идентичную методике Stephenson в модификации Кручинского. При этом деформированный участок хряща рассекают в нескольких местах, формируют полоски, которые расходятся в стороны, расширяя тем самым восстанавливаемую часть ушной раковины. Способ осуществляют следующим образом (фиг. 1-7).The method is carried out as follows (Fig. 1-7).
На первом этапе в ходе операции при помощи перманентного маркера и инъекционных игл, пропитанных зеленкой, размечают анатомические ориентиры линий разрезов хряща (фиг. 1). Проводят разрез кожи соответственно дорсальной поверхности деформированной ушной раковины, параллельно ходу предполагаемого противозавитка, на всю длину хрящевой части ушной раковины. Тупым путем обнажают надхрящница, рассекают острым путем без повреждения подлежащего хряща. Обнажают хрящевую часть ушной раковины соответственно всей дорсальной поверхности, с переходом на край и переднюю поверхность, таким образом чтобы мобилизовать ее деформированный участок. Повторно осуществляют разметку анатомических ориентиров и линий разрезов хряща. С дорсальной поверхности проводят разрез хряща параллельно нижнему отделу противозавитка ушной раковины, без повреждения целостности надхрящницы с передней поверхности. Отступая 1,5-2 мм друг от друга проводят еще несколько разрезов, параллельных предыдущему, также без нарушения целостности передней надхрящницы. Далее эти разрезы продолжают под углом в сторону верхнего хрящевого края, в виде расходящихся лучей, не пересекающих друг друга. От уровня хрящевой деформации до верхнего края ушной раковины разрезы сквозные. Образованные хрящевые полоски, постепенно расширяющиеся от основания к краю, мобилизуются. Передний край ушной раковины отсекают от височной области. Форма ушной раковины сопоставляется в новом положении, хрящевые полоски фиксируются в новом положении между собой мононитью длительного рассасывания. Для усиления стабильности конструкции в области ранее располагавшейся деформации хрящевые полоски фиксируются к подкожно-жировой клетчатке. Кожные лоскуты по краям раны укладывают обратно поверх сформированной ушной раковины, перераспределяются (фиг. 2). Так как в дальнейшем планируют корректирующие операции, в ходе которых будет необходимость использования дополнительных мягких тканей, то избытки кожи не удаляются. Раны ушивают нерезорбируемой мононитью. Далее укладывают формирующие валики, пропитанные мазями Левомеколь, Гепариновой и Вазилина, выше и ниже противозавитка, фиксируются П-образными швами, плетеной резорбируемой нитью (фиг. 3). Фиксируют давящую асептическую повязку. At a distance of 1.5-2 mm from each other, several more incisions are made, parallel to the previous one, also without violating the integrity of the anterior perichondrium. Further, these incisions continue at an angle towards the upper cartilaginous edge, in the form of diverging rays that do not intersect each other. From the level of cartilaginous deformation to the upper edge of the auricle, the incisions are through. The formed cartilaginous strips, gradually expanding from the base to the edge, are mobilized. The anterior edge of the auricle is cut off from the temporal region. The shape of the auricle is compared in a new position, the cartilaginous strips are fixed in a new position among themselves with a long-absorbable monofilament. To enhance the stability of the structure in the area of previously located deformation, cartilage strips are fixed to the subcutaneous fat. The skin flaps along the edges of the wound are placed back on top of the formed auricle and redistributed (Fig. 2). Since corrective surgeries are planned in the future, during which it will be necessary to use additional soft tissue, excess skin is not removed. The wounds are sutured with non-resorbable monofilament thread. Next, forming rollers soaked in Levomekol, Heparinova and Vasilin ointments are placed above and below the antihelix, fixed with U-shaped sutures and braided resorbable thread (Fig. 3). A pressure aseptic bandage is fixed. 2 weeks after the operation, the forming ridges and sutures are removed (Fig. 4).

Тупым путем мобилизуют кожный лоскут, обнажают хрящевую часть завитка. Имплантируют фрагмент реберного хрящевого аутотрансплантата, восстанавливающий рельеф и размеры, соответственные противоположному условно здоровому уху. Аутотрансплантат фиксируют нерезорбируемой мононитью к хрящевому рудименту. Тупым путем сепарируют ткани подкожно в височной области дистально, обнажая поверхностную височную фасцию. Из марлевой салфетки создают шаблон, по которому определяются размеры фасции, необходимые для покрытия верхнего отдела ушной раковины. Проводят перевязку теменной ветви поверхностной височной артерии ниже места предполагаемого разреза. При помощи косого разреза производят разрез поверхностной височной фасции, соответственно шаблону. Тупым путем мобилизуют фасциальный лоскут на питающей ножке, основанием, обращенным к ушной раковине. Фасциальный лоскут опрокидывают вместе с питающим сосудом на 180 градусов, покрывая хрящевой аутотрансплантат и часть собственного хряща ушной раковины, фиксируют плетеной резорбируемой нитью в новом положении. Создают вакуумную аспирацию путем введения активного дренажа под фасцию. Кожные лоскуты укладывают поверх ушной раковины. Рана ушивают узловыми швами, нерезорбируемой мононитью. Укладывают формирующие валики, пропитанные мазями Левомеколь, Гепариновой и Вазилина, выше и ниже противозавитка, фиксируются П-образными швами, плетеной резорбируемой нитью. Дренаж удаляют. Фиксируют асептическую недавящую, формирующую повязку. The fascial flap is tilted along with the feeding vessel 180 degrees, covering the cartilage autograft and part of the auricle's own cartilage, and is fixed with a braided resorbable thread in the new position. Vacuum aspiration is created by introducing active drainage under the fascia. Skin flaps are placed over the auricle. The wound is sutured with interrupted sutures and non-resorbable monofilament thread. Forming rollers soaked in Levomekol, Heparinova and Vasilin ointments are placed above and below the antihelix, fixed with U-shaped sutures and braided resorbable thread. The drainage is removed. An aseptic non-pressure, forming bandage is fixed. 2 weeks after the operation, the forming ridges and sutures are removed (Fig. 5).
Предлагаемая методика может быть использована для пациентов с II и III степенями выраженности деформации сложенной ушной раковины.The proposed technique can be used for patients with degrees II and III of severity of deformation of the folded auricle.
По предлагаемому способу прооперировано 5 пациентов (дети в возрасте от 6 до 8 лет). Четверым пациентам проведен один (первый) этап, и запланировано продолжение лечения. Одной пациентке были проведены оба этапа, и получены отдаленные результаты - спустя 5 лет после устранения деформации. Во всех случаях у пациентов получен схожий технический результат, а именно: у пациентов, которым был проведен первый этап, на ранних сроках и через год после операции сохранялась форма смоделированных местных тканей, без возникновения вторичных деформаций и без возникновения рубцовых деформаций; у пациентки, которой было проведено оба этапа, спустя 5 лет определяли стойкое сохранение эстетического результата, без возникновения послеоперационных осложнений.Using the proposed method, 5 patients (children aged 6 to 8 years) were operated on. Four patients underwent one (first) stage and continued treatment is planned. One patient underwent both stages, and long-term results were obtained - 5 years after the deformity was eliminated. In all cases, the patients obtained a similar technical result, namely: in patients who underwent the first stage, in the early stages and a year after the operation, the shape of the simulated local tissues was preserved, without the occurrence of secondary deformations and without the occurrence of scar deformities; in the patient who underwent both stages, after 5 years, stable preservation of the aesthetic result was determined, without the occurrence of postoperative complications.

Клинический пример 1 (фиг. 6, 7). Пациент К. 6 лет обратился с эстетическим дефектом - деформацией сложенной ушной раковины (фиг. 6). Проведено обследование, составлен план лечения.Clinical example 1 (Fig. 6, 7). Patient K., 6 years old, presented with an aesthetic defect - deformation of the folded auricle (Fig. 6). An examination was carried out and a treatment plan was drawn up.
На первом этапе в ходе операции при помощи перманентного маркера и инъекционных игл, пропитанных зеленкой, были размечены анатомические ориентиры линии разреза хряща. Проведен разрез кожи соответственно дорсальной поверхности деформированной ушной раковины, параллельно ходу предполагаемого противозавитка, на всю длину хрящевой части ушной раковины. Тупым путем обнажена надхрящница, рассечена острым путем без повреждения подлежащего хряща. Обнажена хрящевая часть ушной раковины соответственно всей дорсальной поверхности, с переходом на край и переднюю поверхность, таким образом чтобы мобилизовать ее деформированный участок. Повторно осуществлена разметка анатомических ориентиров и линий разрезов хряща. С дорсальной поверхности проведен разрез хряща параллельно нижнему отделу противозавитка ушной раковины, без повреждения целостности надхрящницы с передней поверхности. Отступая 1,5-2 мм друг от друга проводятся еще несколько разрезов, параллельных предыдущему, также без нарушения целостности передней надхрящницы. Далее эти разрезы были продолжены под углом в сторону верхнего хрящевого края, в виде расходящихся лучей, не пересекающих друг друга. От уровня хрящевой деформации до верхнего края ушной раковины разрезы были сквозные.At the first stage of the operation, anatomical landmarks of the cartilage incision line were marked using a permanent marker and injection needles soaked in brilliant green. A skin incision was made along the dorsal surface of the deformed auricle, parallel to the course of the intended antihelix, along the entire length of the cartilaginous part of the auricle. The perichondrium was exposed bluntly and cut sharply without damaging the underlying cartilage. The cartilaginous part of the auricle is exposed along the entire dorsal surface, with a transition to the edge and anterior surface, so as to mobilize its deformed area. Anatomical landmarks and cartilage cut lines were re-marked. A cartilage incision was made from the dorsal surface parallel to the lower part of the antihelix of the auricle, without damaging the integrity of the perichondrium from the anterior surface. At a distance of 1.5-2 mm from each other, several more incisions are made, parallel to the previous one, also without violating the integrity of the anterior perichondrium. Next, these incisions were continued at an angle towards the upper cartilaginous edge, in the form of diverging rays that did not intersect each other. From the level of cartilaginous deformation to the upper edge of the auricle, the incisions were through.

Так как в дальнейшем планируются корректирующие операции, в ходе которых будет необходимость использования дополнительных мягких тканей, то избытки кожи не удалялись. Пациентка Э. 9 лет обратилась с эстетическим дефектом - деформацией сложенной ушной раковины. Проведено обследование, составлен план лечения. Первый этап хирургического лечения был проведен по предложенной нами, описанной выше, методике.Patient E., 9 years old, presented with an aesthetic defect - deformation of the folded auricle. Через 1 год после первого этапа проведен второй хирургический этап. В ходе операции проведен разрез в височной области, отступя 1 см от края ушной раковины. Тупым путем мобилизован кожный лоскут, обнажена хрящевая часть завитка. Имплантирован фрагмент реберного хрящевого аутотрансплантата, восстанавливающего рельеф и размеры соответственные противоположному условно здоровому уху. Аутотрансплантат фиксирован. Тупым путем сепарируются ткани подкожно в височной области дистально, обнажая поверхностную височную фасцию. Проведена перевязка теменной ветви поверхностной височной артерии ниже места предполагаемого разреза. При помощи косого разреза произведен разрез поверхностной височной фасции, соответственно шаблону. Тупым путем мобилизован фасциальный лоскут на питающей ножке, основанием, обращенным к ушной раковине. Фасциальный лоскут опрокинет вместе с питающим сосудом на 180 градусов, покрывая хрящевой аутотрансплантат и часть собственного хряща ушной раковины, фиксирован. Создана вакуумная аспирация путем введения активного дренажа под фасцию. Кожные лоскуты уложены поверх ушной раковины. Рана ушита. Уложены и фиксированы формирующие валики, пропитанные мазями Левомеколь, Гепариновой и Вазилина. Дренаж удален. Фиксирована асептическая недавящая повязка. Через 2 недели после операции сняты формирующие валики и швы (фиг. 8).1 year after the first stage, the second surgical stage was performed. The fascial flap will tilt together with the feeding vessel 180 degrees, covering the cartilage autograft and part of the auricle's own cartilage, fixed. Vacuum aspiration was created by introducing active drainage under the fascia. Skin flaps are placed over the auricle. The wound is sutured. Forming rollers soaked in Levomekol, Heparin and Vazilin ointments are laid and fixed. Drainage removed. An aseptic non-pressure bandage is fixed. 2 weeks after the operation, the forming rollers and sutures were removed (Fig. 8).
