Дисплазия соединительной ткани - это группа системных заболеваний соединительной ткани, обусловленных нарушением развития соединительной ткани в эмбриональном и постнатальном периодах и проявляющихся различными патологическими признаками так или иначе связанных с дефектами волокнистых структур и/или основного вещества соединительной ткани. Выделяют дифференцированные формы дисплазии соединительной ткани (синдромы Элерса-Данлоса, Марфана, Стиклера, несовершенного остеогенеза и др.) - диагностика возможна за счет ярких клинических проявлений наследственных синдромов, и недифференцированные формы дисплазии соединительной ткани - диагностика требует дополнительных методов исследования в связи с полиморфностью симптоматики.Connective tissue dysplasia is a group of systemic diseases of the connective tissue caused by impaired development of the connective tissue in the embryonic and postnatal periods and manifested by various pathological signs that are somehow related to defects in the fibrous structures and / or the main substance of the connective tissue. При планировании предоперационной подготовки в Национальных рекомендациях по ведению больных с дисплазией соединительной ткани предложено дополнить существующие протоколы и стандарты назначением препаратов магния, витаминов группы В и С, левокарнитина, хондропротекторов и другие мероприятия. Однако до настоящего времени нет четких показаний к изменению схемы предоперационной подготовки у таких больных, что затрудняет обоснование дополнительного расходования денежных средств лечебным учреждением. С другой стороны, возникновение ранних или поздних послеоперационных осложнений ведет к наложению ФОМС или страховой компанией штрафа на лечебное учреждение. В качестве прототипа мы приняли описание "Способа оценки эффективности предоперационной подготовки больных тиреотоксикозом" (патент РФ на изобретение №2191386 от 22.11.2000). Авторы предлагают проводить оценку эффективности предоперационной подготовки больных тиреотоксикозом путем анализа гемодинамики и иммунного статуса, отличающийся тем, что до и после предоперационной подготовки определяют количественные и качественный показатели тканевой гемодинамики щитовидной железы путем ультразвукового доплерографического исследования и уровни концентрации иммуноглобулина G и циркулирующих иммунных комплексов в сыворотке крови и при их нормализации считают предоперационную подготовку эффективной и завершенной, а при значениях определяемых показателей выше нормы в 2-3 раза считают предоперационную подготовку неэффективной и незавершенной.As a prototype, we adopted the description of the "Method for evaluating the effectiveness of preoperative preparation of patients with thyrotoxicosis" (RF patent for the invention No. 2191386 from 11/22/2000). При поступлении проведено биохимическое исследование крови: уровень матриксной металлопротеиназы IX составляет 436,97 нг/мл, а уровень щелочной фосфатазы составляет 544,51 Е/л. Послеоперационная подготовка проведена в соответствии с протоколом лечения. За 6 часов до операции проведено второе биохимическое исследование крови: уровень матриксной металлопротеиназы IX составляет 407,34 нг/мл, а уровень щелочной фосфатазы составляет 569,27 Е/л. Послеоперационная подготовка проведена в соответствии с протоколом лечения. За 6 часов до операции проведено второе биохимическое исследование крови: уровень матриксной металлопротеиназы IX составляет 524,12 нг/мл, а уровень щелочной фосфатазы составляет 505,56 Е/л.
