Abstract
Description
The invention relates to medicine and can be used to form motor skills in children with cerebral palsy.
66-91).A known method of treating cerebral spastic paralysis by exposure to low-intensity laser radiation (LIL) on the skin projection of the motor points of spastic muscles (Illarionov V.E. Technique and methods of laser therapy procedures. - M., 1994, pp. 66-91).
The disadvantage of this method is the lack of a holistic restoration of the function of the affected motor analyzer, since it affects only its part — spastic muscles — and only passively, without active training.
A61N 5/067,2001 г.).The closest in technical essence and the achieved result to the proposed one is a method of treating cerebral palsy by means of physiotherapy exercises and stimulating effects on nerve endings (SU patent No. 2201773, class A61N 5 / 067,2001).
A stimulating effect on the nerve endings is provided by scanning laser therapy of the red range on the skin projections of nerves, blood vessels and distal receptor fields, passively active gymnastics is performed 2-3 hours after laser therapy. Physiotherapy exercises include passive-active gymnastics and consists of 3 parts:
1. Exercises for muscles innervated by cranial nerves.
2. Respiratory gymnastics.
3. Exercises aimed at training basic motor skills.
A disadvantage of the known method for treating cerebral palsy is that the effect is local to the skin projections of the peripheral nerves, and sensory information enters the cerebral cortex. And therapeutic exercises are carried out after a while. At the same time, gymnastics is aimed at relaxing and restoring the function of individual limbs, rather than restoring motor skills in general. In addition, the known method is not intended for the treatment of cerebral palsy in children.
The technical result to which the present invention is directed is to increase the effectiveness of the treatment of cerebral palsy in children by restoring motor skills.
The specified technical result is achieved by the fact that in the method of treating cerebral palsy in children, including the formation of motor skills by means of physiotherapy and stimulating the nerve endings, according to the invention, the stimulating effect on the nerve endings is carried out with the Brain Port device for transling brain electrical stimulation while performing active exercises aimed at teaching new motor skills using a technique divided into three sets and exercises, depending on the patient’s motor skills, while the exposure time is 20-25 minutes for 10 days daily, while the first set of exercises is aimed at creating the ability to sit independently while holding your body in space, the second set of exercises is aimed at forming the skill to maintain the vertical position of the body in space and the ability to control the position of the body during acceleration or deceleration of rectilinear movement, as well as during rotations: deviations of the head, body and eyes to the side, etc. oppositely movement, and the third set of exercises aimed at learning walk.
Translingual electrical stimulation of the brain is performed using the Brain Port instrument (User Guide. Wicab, Inc. USA). The Brain Port device can be placed in a fixture made, for example, on the basis of the nipple, to prevent part of the device from biting, since the electrodes are placed on the child’s tongue, and the base of the electrodes is a very fragile jumper that needs protection from biting the patient’s teeth.
The device operates on the principle of biological feedback, transmitting information about the spatial position of his body through the electrical stimulation of the tongue to the patient’s brain, improving the coordinating function and spatial orientation. This stimulation is based on the excitation of natural flows of nerve impulses to the brain stem and cerebellum through the lingual branches of the cranial nerves and introduces changes in the functions of these structures. There is a change in neural activity in the corresponding nuclei of the brain stem. This causes a sequential cascade of changes in neighboring nuclei and / or passive transmission of biological compounds in the intercellular space. This includes activation of the vestibular nuclei, the cochlear complex, sublingual nuclei, and cerebellar worm.
The proposed method for the treatment of cerebral palsy in children allows you to directly affect the structure of the brain stem, vestibular nuclei, cerebellum legs, which are responsible for balance, through the linguistic branches of the cranial nerves, introducing changes in the function of these structures, which entails changes in muscle tone, improving coordinator function, equilibrium. This allows you to effectively restore motor skills in children, because the child is taught new motor skills, since he did not know how to sit, stand, walk, and do not restore motor skills, subsequently lost trauma, stroke and other CNS lesions.
A method of treating cerebral palsy in children is based on the formation of new motor skills in children with cerebral palsy by improving vestibular function, the formation of static and statokinetic installation reflexes, repaying posotonic automatisms, and improving central nervous system control over muscles.
Therapeutic gymnastics is carried out in combination with translingual electrical stimulation of the brain. Active gymnastics is aimed at the formation of new motor skills in children with cerebral palsy. The procedure takes 20-25 minutes, which is very important in childhood. At the end of the procedure, a persistent positive effect is observed, which lasts for 5-6 hours.
A set of exercises is individually selected for each patient. The training procedure is carried out in combination with translingual electrical stimulation of the brain and lasts 20-25 minutes.
The technique is divided into three sets of exercises, depending on the motor skills of patients.
1. The first set of exercises is aimed at the formation of the ability to sit independently, holding your body in space. All exercises are performed on a chair:
1.1. sitting on a chair resting on legs
1.2. sitting on a chair without resting on legs
1.3. with a change in the angle of inclination of the chair and a shift in the center of gravity.
2. The second set of exercises is aimed at the formation of the ability to maintain the vertical position of the body in space and the ability to control the position of the body during acceleration or deceleration of rectilinear movement (elevator reflex), as well as during rotations (deviations of the head, body and eyes in the direction opposite to movement). All exercises are performed while standing:
2.1. Standing with his back to the wall, legs shoulder-width apart, buttocks pressed against the wall, arms along the torso.
2.2. Standing with your back to the wall, change the position of the legs.
2.3. We teach the patient to break away from the wall.
3. The third set of exercises is aimed at the formation of walking skills
3.1. The patient is taught to walk using 2 multi-support canes (the height of the support should be at the level of the xiphoid process of the patient. At this height, a large load goes to the legs). Movement of arms and legs should be coordinated "right hand-left foot, left hand-right foot."
3.2. The patient is taught to walk with one multi-support cane.
3.3. The patient is taught to walk with one single support cane.
3.4. Independent walking without means of support.
The method is as follows.
Before the start of therapeutic exercises, the patient's motor skills are assessed (according to the Berge scale and GMFCS). Check if the patient can sit on their own and for how long they can sit. Can it stand at the support, how long. It depends on what complex of exercises the patient will receive. Then begins the training procedure. The patient has a device on the tongue, and the current strength, which the patient feels in the form of a slight tingling, is gradually added. As soon as the patient feels tingling, the increase in current strength stops.
The implementation of a method of treating cerebral palsy in children is shown on the example of a specific patient.

2.1. Standing with his back to the wall, legs shoulder-width apart, buttocks pressed against the wall, arms along the torso.
2.2. Standing with your back to the wall, change the position of the legs.
2.3. We teach the patient to break away from the wall.
Then a set of exercises was used, aimed at the formation of walking skills:
3.1. The patient is taught to walk using 2 multi-support canes (the height of the support should be at the level of the xiphoid process of the patient. At this height, a large load goes to the legs). Movements with hands and feet should be coordinated "right hand-left foot, left hand-right foot."
At the end of the first course of treatment, the child has a positive trend. Spastic muscle groups relaxed, the volume of active movements along the joints increased. The coordination of movements has improved. The child can independently walk with support on 2 multi-support canes.
The second course of treatment was started with exercises aimed at the formation of walking skills:
3.2. The patient is taught to walk with one multi-support cane.
3.3. The patient is taught to walk with one single support cane.
At the end of the second course of treatment, the child can walk confidently with support on 2 one-support canes; for short distances within the premises, he can walk with support on 1 one-support canes.

A method for the treatment of cerebral palsy in children was tested at the Department of Medical Rehabilitation of Children on the basis of St. Petersburg GBUZ GB No. 40 for 2.5 years. It was treated 50 children with a diagnosis of cerebral palsy. Spastic diplegia, 3 children: cerebral palsy atonic-astotic form. The level of motor development at admission according to the GMFSC scale: 12 people 4 level, 7 people 2 level, 31 people 3 level, 3 people 1 level. All patients underwent treatment according to this technique for 2 weeks. At the end of the course, all patients showed positive dynamics in the form of a decrease in muscle tone, improved coordination of movement, improved control of the position of the head and body in space. After the first year, 29 children learned to walk using the means of support, 7 people mastered the ability to walk independently, 5 patients learned to sit on their own.
Thus, the application of the proposed method for the treatment of cerebral palsy in children will improve the effectiveness of the treatment of cerebral palsy in children by restoring motor skills.
