Эффективность лечения больных инфекциями мочеполового тракта в последние годы значительно снизилась. Причинами развития рефрактерности, то есть невосприимчивости к стандартной терапии (от лат. refracterius - упрямый), являются негативные изменения как макроорганизма (снижение естественного иммунитета, большое количество сопутствующих заболеваний, широкое распространение метаболического синдрома и прочее), так и микроорганизма, равно как и недооценка патогенетических факторов при назначении терапии [1]. Эрадикация возбудителя не означает излечение пациента, в 38% случаев развивается резидуальный болевой синдром и диспареуния. Из причин длительно сохраняющегося болевого синдрома и диспареунии у женщин, страдающих резидуальным хроническим уретритом, воспаление женской простаты, наличие которой в настоящее время доказано [2-5], играет основополагающую роль.The effectiveness of treatment of patients with urinary tract infections in recent years has significantly decreased. Блокировать избыточную активность ПСА в паренхиме простаты можно посредством афалы - релиз-активных антител к ПСА. Известен Способ лечения уретрального синдрома у женщин антибактериальным препаратом гентамицин, который вводят в стенку уретры в количестве 80 мг, растворенного в 2 мл 0,25% раствора анестетика, отличающийся тем, что введение осуществляют параллельно стенке уретры, а в качестве анестетика вводят 0,25% раствор новокаина с последующим облучением вульвы излучением инфракрасного лазера длиной волны 0,82 мкм, частотой 3000 Гц, мощностью 10 мВт, продолжительность процедуры 5-6 минут, на курс 8-10 процедур (RU 2509556 С1, 22.06.2012). Положительная динамика симптомов достигла максимума после 10-й процедуры электростимуляции и окончания трехнедельного курса приема афалы. Таким образом, увеличивать продолжительность терапии смысла не имело. Предлагаемый способ апробирован в туберкулезном отделении для больных урогенитальным туберкулезом ФГБУ «ННИИТ» Минздрава России у 7 пациенток. Исходно средний показатель интенсивности боли составил 8,2±3,7; качество жизни было снижено в среднем до 2,7±0,4, диспареуния была у 100% пациенток. После трехнедельного курса приема афалы в сочетании с интравагинальной электростимуляцией интенсивность боли снизилась в среднем до 1,9±0,9, диспареуния прекратилась у 85,7% пациенток, качество жизни возросло в среднем до 4,3±0,2 баллов.Initially, the average pain intensity was 8.2 ± 3.7; the quality of life was reduced on average to 2.7 ± 0.4, dyspareunia was in 100% of patients.
