Abstract
Description
The invention relates to medicine, in particular to gastroenterology, physiotherapy and rehabilitation medicine, namely to the treatment of patients with non-erosive gastroesophageal reflux disease, and can be used in a hospital, clinic, offices and health centers.
Gastroesophageal reflux disease, which is based on disorders of the motility and sphincter apparatus of the esophagus, in its frequency, possible serious manifestations and dangerous consequences is recognized as the leading disease of the digestive system. The prevalence of gastroesophageal reflux disease among the adult population is 20-40%, a huge number of people daily experience symptoms of gastroesophageal reflux disease, which significantly worsens the quality of life: work is disrupted, they cannot fully eat and rest, which allows us to consider this problem relevant, having medical -social and economic importance.
Gastroesophageal reflux disease is a multifactorial disease. In the vast majority of patients with GERD, reflux episodes occur due to transient relaxation of the lower esophageal sphincter. The innervation of the lower esophageal sphincter (NPS) is carried out by the sympathetic and parasympathetic nerves. It was found that in patients with gastroesophageal reflux disease, the activity of cholenergic innervation was reduced compared to healthy ones, while the predominance of adrenergic innervation was not revealed.
Leading domestic gastroenterologists consider the approach to monitoring the effectiveness of treatment, focused on the dynamics of symptoms, very rational. Endoscopically distinguish two main types of GERD: non-erosive form and erosive, characterized by the presence of erosion of the mucous membrane of the esophagus. The severity of the subjective symptoms of GERD does not correlate with the severity of the endoscopic picture. For non-erosive gastroesophageal reflux disease, the elimination of clinical manifestations is a decisive criterion for the effectiveness of therapy.
- 2011. - №3. - С.23-26; Blum H., Donath F., Wamke A., Schug В. Pharmakokinetik drug interaction profiles of proton pump inhibitors // Drug Safeti - 2006. - 29(9). - P.-769-784).Patients with GERD represent a significant public health problem in terms of the lack of effectiveness of most of the methods used and the high economic costs of treatment. Despite the wide arsenal of medications used to treat patients with GERD, the number of patients tends to increase, and such formidable complications as peptic strictures, erosive and ulcerative lesions, bleeding, Barrett's esophagus force me to resort to more radical methods of treatment. On the other hand, the growth of drug hepatitis and side effects caused by the main drugs used in the treatment of this pathology (proton pump inhibitors, H 2 blockers) do not allow to overestimate the relevance of the search for non-drug methods of treatment of GERD (Minushkin O.N., Maslovsky L. V., Teplukhina O.YU. Supportive therapy for GERD: resolved, unresolved, controversial issues // Effective pharmacotherapy. - 2011. - No. 3. - P.23-26; Blum H., Donath F., Wamke A., Schug B. Pharmakokinetik drug interaction profiles of proton pump inhibitors // Drug Safeti - 2006. - 29 (9). - P.-769-784).
The closest in technical essence to the proposed method for the treatment of patients with GERD is the method of exposure to the patient with an electromagnetic field of the decimeter wave range (UHF) on the collar zone. When conducting DMV therapy, the Volna-2 apparatus, with a power of 25-30 watts, is used. The impact is carried out by a rectangular radiator 35 × 16 cm, for 8-10 minutes, 8-10 procedures are administered every other day (Efendieva M.T., 2003, dissertation for the Ph.D. Non-pharmacological correction of neurohumoral regulation of the lower esophageal sphincter in various forms of its pathology ”).
The disadvantages of this method are the following:
- главный ориентир выбранной методики - субъективные ощущения больного - появление жжения или покалывания;- the main landmark of the chosen technique - the subjective sensations of the patient - the appearance of a burning or tingling sensation;
д.- the presence of a wide profile of contraindications: uterine fibroids, adenomyosis, coronary heart disease, thyroid disease, acute inflammatory diseases, etc.
The technical result of the proposed method for the treatment of patients with non-erosive gastroesophageal reflux disease is the relief of the clinical manifestations of the disease, a decrease in the number of gastroesophageal refluxes, an improvement in the psychoemotional state and quality of life of patients, a reduction in the duration of treatment, and the possibility of using the method with a concomitant pathology.
The specified technical result is achieved by the fact that the patient is affected by a pulsed magnetic field from the Magnetomed apparatus, with a frequency of 8-10 Hz and an intensity of 4-6 mTl for 12-15 minutes, through an inductor with a solenoid with a diameter of 60 cm, which is located directly above the bottom half of the chest and epigastrium, which allows you to act on the distal esophagus, lower esophageal sphincter and stomach, on the course of treatment of 10-12 daily procedures.
A prerequisite for the appointment of a pulsed magnetic field was the effects obtained by the action of a magnetic field on the plasma membrane of a cell with a change in its permeability and on autonomic regulation. The basis of the action of low-frequency magnetotherapy, in addition to the traditional physicochemical mechanisms of inducing electromotive force, the participation of biogenic magnetism, polarization of nuclei and electrons, is the phenomenon of resonance that occurs between the acting electromagnetic field and the processes occurring in the atoms and molecules of the body. The pathogenetic feature of the use of a low-frequency magnetic field in the treatment of various diseases is due to increased blood circulation in the inflammation zone due to changes in the tone of blood vessels, correction of autonomic dysfunction, and blocking the ability of information to be transmitted by a pathologically altered inflammatory process to the central nervous system.
A feature of pulsed fields is a more pronounced trophic, neuromyostimulating, vegetotropic effect.

Periodically takes antacids and prokinetics with a short effect. An endoscopic examination revealed cardia insufficiency, reflux of the gastric contents into the esophagus, slight swelling and hyperemia of the mucous membrane of the esophagus, which corresponds to grade 0 esophagitis according to the Savari-Miller classification.
The patient underwent a course of treatment with low-frequency magnetotherapy from the Magnetomed apparatus. The exposure was carried out by the following parameters: magnetic field frequency of 10 Hz, intensity 6 mT, procedure time 12 minutes, the procedures were performed daily. The course of treatment was 10 sessions. The tolerability of the procedures was satisfactory; no adverse reactions were noted.
As a result of treatment with the proposed method, a regression of the clinical manifestations of the disease was noted — heartburn, belching, coughing, headaches disappeared, and the patient’s mood improved significantly. The positive dynamics of the patient's condition, along with subjective sensations, were also confirmed by the data of special research methods.
The dynamics of indicators characterizing the vegetative balance was established by the results of a spectral analysis of heart rate variability. The initial state of the autonomic nervous system was characterized by a predominance of activity of the sympathetic nervous system - 1204 ms * 2 and a decrease in the activity of parasympathetic influences of HF - 239 ms * 2. Accordingly, there was an increase in the vagosympathetic balance index to 5.03 (with a norm of 1.02 ± 0.25). After applying the proposed method of treatment, there was a decrease in the sympathetic contour from 1204 ms * 2 to 945 ms * 2 and an increase in parasympathetic modulations from 239 ms * 2 to 227 ms * 2, which led to a decrease in the coefficient of vagosympathetic balance from 5.03 to 4.19. The obtained result can contribute to improving the obturator function of the lower esophageal sphincter, which confirms the pathogenetic validity of the use of magnetotherapy of the above parameters.
As a result of the therapy, an improvement in activity (from 4.4 to 5.6) according to the SAN test was also revealed.
Assessment of the quality of life according to the SF-36 questionnaire showed an increase in the scales of general health, role functioning due to emotional state, role functioning due to physical condition, social functioning and mental health. The proposed method for the treatment of non-erosive gastroesophageal reflux disease was tested on 48 patients aged 23 to 65 years with a disease duration of up to 5 years. As a result of treatment, positive dynamics in the form of relief of the clinical symptoms of the disease (heartburn, belching, regurgitation) and improvement of the psychoemotional state and quality of life were observed in all patients, normalization of the autonomic balance was noted in 75% of patients.
The proposed method of treating patients helps to optimize autonomic regulation - lowering initially elevated sympathetic modulations and increasing parasympathetic influences, which leads to an improvement in the functional state of the lower esophageal sphincter, in particular, stimulates its locking mechanism, preventing the reflux of acidic or alkaline gastric contents into the esophagus. Along with this, there is a regression of the clinical manifestations of the disease (heartburn, belching, regurgitation disappears or decreases), and the psychological status of patients is improved. Long-term results of treatment indicate the preservation of the achieved results for 7-9 months.
The proposed method for the treatment of non-erosive gastroesophageal reflux disease is simple, affordable, has minimal contraindications, can be used both in a hospital and in an outpatient setting, does not require large material costs and does not cause adverse reactions.
