Abstract
Description
The invention relates to medicine, namely to dermatology, and can be used to treat androgenetic alopecia.
В., 2000].Alopecia (baldness) is a pathological hair loss. There are cicatricial alopecia due to the destruction of hair follicles due to inflammation, skin atrophy or scarring, and non-cicatricial alopecia, which occurs without previous skin lesions. Non-cicatric alopecia include focal alopecia, androgenetic alopecia, diffuse alopecia [Adaskevich VP, Melyazel O.D., Tikhonovskaya IV, 2000].
3).According to statistics, the ratio of different types of alopecia is as follows: 30% of alopecia areata, 40% of androgenetic alopecia, 20% of telogen prolapse, the rest is scar alopecia and others (A.G. Hajigoroeva, journal Natural Pharmacology and Cosmetology, No. 1, 2005, p. .3).
The most common form of hair loss is androgenetic alopecia, a hereditary decrease in cosmetically valuable hair induced by androgens in men and women with a genetic predisposition. This condition is also commonly called male pattern baldness or common baldness.
The urgency of the problem of alopecia is that, affecting the aesthetic perception of a person by society, it leads to neurotization, a decrease in social activity and a deterioration in the quality of life.
Various approaches to the treatment of androgenetic alopecia are known.
A known method of treating androgenetic alopecia by oral administration of the antiandrogenic drug finasteride (Patent RU No. 2173152, IPC A61K 31/58, A61P 17/14, published on 09/10/2001). The need for long-term use of the drug to maintain treatment results (journal Natural Pharmacology and Cosmetology, No. 1, 2005, p. 3, 6) increases the risk of side effects such as impotence, loss of libido, gynecomastia, teratogenesis in pregnant women.
The external use of minoxidil (Rogein) as a treatment for androgenetic alopecia is known (A.G. Hajigoroeva, journal Natural Pharmacology and Cosmetology, No. 1, 2005, pp. 3-6), which stimulates the growth of follicular and germinal hair cells, activates follicular growth in the anagen phase. The disadvantages of this method include the possibility of developing side effects: hypotension, headache, dizziness, weakness, dryness and peeling of the skin, allergic dermatitis, urticaria.
A known method of treating androgenetic alopecia by oral administration of zinc preparations (patent RU 2053770, IPC A61K 31/41, A61K 31/315 publ. 02/10/199). Zinc reduces the sensitivity of androgen receptors in the cells of the hair follicles. However, the low effectiveness of zinc preparations can be due to both their low bioavailability and the participation of zinc in the processes of testosterone biosynthesis at a systemic level, which can offset the therapeutic effect in the follicles.
A known method of stimulating hair growth by applying to the scalp in the area of the hairline means, which is an aqueous solution containing trace elements: copper, zinc, indium, silicon, cobalt, manganese and chromium. The product is applied daily for 4-12 months or more, wetting the head, without rinsing for 7-10 days; in parallel, the solution is taken orally (patent RU No. 2227014, IPC 7 A61K 7/06, A61P 17/04, published on 04/20/2004). The disadvantages of this method are the multicomponent composition of the solution used, which leads to polypharmacy, to the load of tissues with an excessive amount of many trace elements, of which not all have a pathogenetically substantiated effect; creating hygienic discomfort for the patient (lack of the ability to flush for 7-10 days); course duration (daily application for 4-12 months or more).
Known as a prototype is a method of treating alopecia, which is closest to the claimed by the combination of essential features and the achieved technical result. The known method includes the introduction into the skin of the scalp of a zinc-containing preparation. The introduction of the drug is carried out by injecting peripherally in the amount of 20-30 injections, with further application to the head of a mixture of zinc-containing trace elements with anesthetic for at least 4 hours (patent RU No. 2242811, IPC 7 A61K 33/30, A61K 33/10, publ. March 20, 2005). The disadvantages of this method is the relatively low effectiveness of the therapeutic effect, the presence of pain and the risk of trauma to the skin of the scalp of the patient due to the use of the injection method of drug administration. In addition, the method is characterized by discomfort due to the need for a long stay of the patient in the treatment room.
The problem to which the invention is directed, is to develop a highly effective method for the treatment of androgenetic alopecia.
Technical results achieved by the implementation of the claimed invention are: a) in the exclusion of pain and trauma to the scalp; b) to increase the density of hair, the average diameter of all hair, the number of terminal, thick and medium hair, as well as hair in the anagen phase, and c) to reduce the number of velus, thin hair and hair in the telogen phase.
These technical results are achieved in that a method of treating androgenetic alopecia involves the introduction of a zinc-containing preparation into the skin of the scalp, carried out by electroporation, with zinc hyaluronate being used as the zinc-containing preparation.
It is preferable that electroporation is carried out by rectangular current pulses with a duration of 100 microseconds, a frequency of 200 Hz, sinusoidal modulation.
It is also preferable that the duration of the series of pulses was 1.5 seconds, the pause was 0.5 seconds, and the duration of exposure was no more than 18 minutes.
It is also preferable that the current power is regulated in the range from 0.1 to 100 V according to the subjective sensations of the patient until pain appears.
It is also preferable that the course of treatment be at least 10 procedures, with the first 6 procedures carried out with a frequency of 3 times a week, and the next 4 procedures with a frequency of 2 times a week.
A comparative analysis of the claimed invention with the prototype showed that in all cases it is different from the known, closest solution:
- введением лекарственного препарата методом электропорации;- the introduction of the drug by electroporation;
- использованием препарата гиалуроната цинка в качестве цинкосодержащего препарата.- using a zinc hyaluronate preparation as a zinc-containing preparation.
In preferred cases, the execution of the claimed invention differs from the well-known, closest technical solution:
- осуществлением электропорациии прямоугольными импульсами тока длительностью 100 микросекунд, частотой 200 Гц, синусоидальной модуляции;- electroporation using rectangular current pulses with a duration of 100 microseconds, a frequency of 200 Hz, sinusoidal modulation;
- длительностью серии импульсов, составляющей 1,5 секунды, длительностью паузы, составляющей 0,5 секунды, и продолжительностью воздействия, составляющей не более 18 минут;- a duration of a series of pulses of 1.5 seconds, a pause of 0.5 seconds, and a duration of exposure of not more than 18 minutes;
- регулированием мощности тока в диапазоне от 0,1 до 100 V по субъективным ощущениям пациента до появления болезненности;- regulation of current power in the range from 0.1 to 100 V according to the subjective sensations of the patient until pain appears;
- выполнением курса лечения, составляющим не менее 10 процедур, первые 6 процедур из которых осуществляют с частотой 3 раза в неделю, а последующие 4 процедуры - с частотой 2 раза в неделю.- the implementation of a course of treatment comprising at least 10 procedures, the first 6 procedures of which are carried out with a frequency of 3 times a week, and the following 4 procedures - with a frequency of 2 times a week.
д.).The use of pulsed current can significantly improve the delivery of zinc in the tissue. The connection of zinc with hyaluronic acid allows the creation of zinc depots in the tissue, which in turn prolongs the positive effect of the procedures up to 4 months after one course. The introduction of zinc hyaluronate preparations by electroporation significantly enhances the diffusion of zinc hyaluronate into the dermis and hair follicles due to the formation of temporary pores in the lipid layers of the epidermal barrier under the action of a pulsed electric current. With electroporation, temporary channels arise in the epidermis, through which you can enter a larger amount of the drug per unit time. Zinc ions accumulated in subepidermal structures locally affect the androgens of the hair follicles. The anions of hyaluronic acid that are able to bind and retain water, which can increase the moisture content of the dermis, are of auxiliary importance. The proposed method provides local accumulation of zinc directly in the problem area. A similar method for the delivery of zinc hyaluronate is not described in the available literature. The inventive method of treating alopecia is the most convenient to use, does not cause a violation of the integrity of the skin, minimizes pain, minimizes the time of the patient and the doctor (the total duration of the procedure is 18 minutes). The method has no side effects that are inherent in currently used oral antiandrogens and minoxidil (impotence, loss of libido, gynecomastia, teratogenesis, hypotension, dryness and peeling of the skin, allergic dermatitis, etc.).
The claimed method is illustrated by the following graphic materials.

Figure 2 presents a histogram of changes in the indicators of the trichogram (the average diameter of all hair, the number of thick hair, the number of medium hair, the number of thin hair) immediately after the electroporation of zinc hyaluronate relative to the values in the control zone.
The claimed method is as follows.
Zinc hyaluronate is introduced into the skin of the scalp by electroporation into the parietal (androgen-dependent) region. Zinc hyaluronate, manufactured by various pharmaceutical companies under various commercial names, is applied to the scalp and evenly distributed over the entire surface of the problem area with an electroporation device sensor, for example, a BTL-5000 NNM ™ device (Great Britain) in the manufacturer's Aquaphoresis mode for 18 minutes . The power of the current is regulated by the subjective sensations of the patient (before the onset of soreness) - up to 100 V. The course of treatment includes at least 10 procedures, with a frequency of 3 times a week the first 6 procedures and 2 times a week for the next 4 procedures.
The technical result of the proposed method for the treatment of androgenetic alopecia using electroporation of the drug zinc hyaluronate is proved by the results of the study.
Under observation were 30 patients (16 women and 14 men) suffering from androgenetic alopecia from 6 months to 16 years. All patients underwent course introduction of zinc hyaluronate into the parietal zone (androgen-dependent) by electroporation. The course of therapy consisted of 10 procedures lasting 18 minutes each, the frequency of procedures - 3 times a week for the first 6 procedures, 2 times a week for the remaining 4 procedures.
Studied initially, immediately after completion, and 4 months after the course of therapy, the indicators of the trichogram, both in the exposure zone and in the intact control zone: hair density, average diameter of all hair, number of terminal, thick and medium hair, number of velus, thin hair hair in the anagen phase and in the telogen phase. The trichogram was performed using an ARAMO SG microvisiograph (Korea) at magnifications of x60 and x200 times. The results of the survey were processed using a specialized diagnostic program for the personal computer Trichoscience rus v.1.6. The results were processed statistically using the Statistika 6.0 program, the significance of differences was evaluated using the paired t-test student.
The obtained technical result of the treatment by introducing zinc hyaluronate into the skin of the scalp by electroporation was expressed as a statistically significant increase in hair density, average diameter of all hair, the number of terminal, thick and medium hair, as well as hair in the anagen phase. At the same time, the number of velus, thin hair and hair in the telogen phase was significantly reduced. The achieved therapeutic effect persisted for 4 months after the course of therapy.
