На последнем этапе операции те области, в которых имеется высокая вероятность развития спаек (например, органы серозных полостей, просвет полых органов: матка, маточные трубы, пищевод), обрабатываются стерильным биорастворимым инертным полимерным гелем необходимой вязкости (0,6-0,93 мм/с) для достижения барьерного эффекта с депонированным в них комплексообразующим веществом d-пеницилламином. Количество действующего вещества в смеси 15 мг на 100 мл геля, т.к. именно это количество комплексообразующего препарата позволяет достичь нужного уровня свободных ионов кальция, чтобы каскад реакций спайкообразования не запустился. Полученный имплантат полимерного рассасывающегося водного геля с депонированным в нем комплексообразующим веществом d-пеницилламином с профилактической целью наносится на висцеральный (париетальный) серозный покров с помощью шприца, тупфера, шпателя или вводится в просвет полых органов шприцом или катетером. ПРИМЕР КОНКРЕТНОГО ВЫПОЛНЕНИЯEXAMPLE OF SPECIFIC PERFORMANCE
В качестве профилактического средства нами использовался водный биорастворимый инертный полимерный 3% гель метилцеллюлозы вязкостью 0,9 мм/с с депонированной в нем субстанцией комплексообразующего вещества d-пеницилламина из расчета 15 мг на 100 мл геля. Способ апробирован на модели спаечного процесса брюшной полости. В асептических условиях двум сериям крыс-самцов линии Вистар (по 30 животных в каждой) воспроизводился спаечный процесс путем скарификации слепой кишки до появления «кровавой росы» и гидравлического препарирования париетальной брюшины (Липатов В.А., 2001 г.). Причем первая серия была контрольной и составляла 3 группы животных, а вторая - опытной, включала 5 групп в зависимости от формы и способа введения препарата.As a prophylactic, we used an aqueous bio-soluble
После моделирования спаечного процесса животным опытной серии в места десерозированных участков брюшины имплантировались 2 мл смеси, другим подопытным проводилось курсовое пероральное применение d-пеницилламина на 2 мл изотонического раствора (0,9% NaCl) из расчета 1,5-2 г в сутки. Животные выводились из эксперимента на 14 сутки путем передозировки эфирного наркоза. Крысы подвергались аутопсии, тщательному изучению органов брюшной полости, оценки выраженности спаечного процесса визуально и с помощь методики семантического дифференциала (балльная система, состоящая в оценки распространенности спаечного процесса, деформации органов брюшной полости и выраженности отдельных видов спаек с учетом коэффициента значимости каждого показателя). Париетальная брюшина и органы брюшной полости подвергались гистологическому исследованию по общепринятой методике с фиксацией в нейтральном формалине, заливкой в парафин, окраской срезов гематоксилин-эозином, по Ванну Гизо, импрегнацией серебром по Футу и окраской с помощью ШИК-реакции. The research results were processed statistically (calculated average values, student criterion, conjugation coefficient, Chi-square, linear regression and correlation methods were used).
Статистически достоверных отличий общего состояния, показателей клинического анализа крови и индексов лейкоцитарной формулы между контрольной и опытной группами нами не обнаружено. В контрольной серии спайки имели место у всех животных, выраженность спаечного процесса методом семантического дифференциала составила 3,39±0,18 баллов.
