Изобретение относится к области медицины, в частности к стоматологии, и предназначено для выявления на стоматологическом приеме лиц с риском наличия кариеса и камнеобразования в ротовой полости на основе лабораторной оценки состояния полости рта.The invention relates to medicine, in particular to dentistry, and is intended to identify at a dental appointment persons with a risk of caries and stone formation in the oral cavity based on a laboratory assessment of the condition of the oral cavity.
Состояние органов полости рта в немалой степени зависит от состава и свойств окружающей их жидкости, а именно слюны. Слюна играет важнейшую роль в поддержании физиологического равновесия процессов минерализации и деминерализации в эмали зубов. В полости рта слюна присутствует в виде тонкой пленки жидкости на различных поверхностях. Слюна человека может служить источником информации об общем состоянии организма. Очень важными являются исследования ротовой жидкости в условиях физиологии полости рта с целью выявления интегрированных параметров, установления их критических значений для предупреждения возникновения патологических изменений. Исследование ротовой жидкости в тесном взаимодействии с неблагоприятными факторами в настоящее время является актуальной проблемой стоматологии, с ним связаны вопросы донозологической диагностики, прогнозирования и индивидуальной профилактики заболеваний полости рта. Имеющиеся сейчас методы диагностики патологических изменений в полости рта в определенной степени сложны, трудоемки и недостаточно информативны. Поэтому важным является поиск простых, информативных, доступных методов экспресс-диагностики заболеваний зубов, основанных на анализе изменений физико-химических параметров слюны.The condition of the organs of the oral cavity to a large extent depends on the composition and properties of the surrounding fluid, namely saliva. Saliva plays a crucial role in maintaining the physiological balance of the processes of mineralization and demineralization in tooth enamel. In the oral cavity, saliva is present as a thin film of liquid on various surfaces. Human saliva can serve as a source of information about the general condition of the body. Studies of the oral fluid under physiological conditions of the oral cavity are very important in order to identify integrated parameters and establish their critical values to prevent the occurrence of pathological changes. The study of oral fluid in close interaction with adverse factors is currently an urgent problem of dentistry, it is associated with questions of prenosological diagnosis, prognosis and individual prevention of diseases of the oral cavity. Currently available methods for diagnosing pathological changes in the oral cavity are to a certain extent complicated, time-consuming and not sufficiently informative. Therefore, it is important to search for simple, informative, accessible methods for the rapid diagnosis of dental diseases, based on the analysis of changes in the physicochemical parameters of saliva.
Известны способы диагностики ряда заболеваний по исследованию ротовой жидкости: одонтогенных остеофлегмоны и аденофлегмоны (патент РФ 2230327, МПК G01N 33/84), острого одонтогенного остеомиелита (заявка РФ 2002113510, МПК G01N 33/84); десквамативного глоссита по содержанию норадреналина (заявка РФ 2004132590, МПК G01N 33/48), воспалительного процесса в органах дыхания (патент РФ 2140080, МПК G01N 33/487), хронического гастродуоденита и функциональной диспепсии у детей (патент РФ 2270610, МПК А61В 10/00, G01N 33/483). Также известны способы прогнозирования течения заболеваний по ряду параметров ротовой жидкости, в частности, воспалительных заболеваний (патент РФ 2286572, МПК G01N 33/52) и заболеваний, сопровождающихся нарушением микрофлоры полости рта (патент РФ 2229130, МПК G01N 33/48). Все эти способы позволяют диагностировать заболевание организма по ряду специфических параметров ротовой жидкости, что нередко требует разработки и внедрения новых методик анализов. Methods are also known for predicting the course of diseases by a number of parameters of the oral fluid, in particular inflammatory diseases (RF patent 2286572, IPC G01N 33/52) and diseases accompanied by disturbance of the microflora of the oral cavity (RF patent 2229130, IPC G01N 33/48). All these methods make it possible to diagnose an organism’s disease by a number of specific parameters of the oral fluid, which often requires the development and implementation of new analysis methods. Moreover, the above methods are not applicable for the diagnosis and prevention of diseases of the oral cavity.

Задачей заявляемого изобретения является разработка лабораторного способа ранней диагностики заболеваний ротовой полости (кариеса зубов и камнеобразования) по элементному составу слюны и выявления предрасположенности к данным заболеваниям, в том числе и в случаях отсутствия клинических признаков проявления соответствующих заболеваний полости рта.The objective of the invention is to develop a laboratory method for the early diagnosis of diseases of the oral cavity (dental caries and stone formation) by the elemental composition of saliva and to identify a predisposition to these diseases, including in cases where there are no clinical signs of manifestation of the corresponding diseases of the oral cavity.

Для определения параметров ротовой жидкости были проведены следующие исследования. Материалом исследования служила слюна 90 пациентов. Забор слюны осуществляли утром, натощак, до чистки зубов в стерильную пробирку с плотно закрывающейся крышкой. Собранную слюну центрифугировали в течение 10 минут при скорости 3000 об/мин в лабораторной медицинской центрифуге ОПЛ-8. Элементный состав образцов ротовой жидкости определяли методом атомно-эмиссионного спектрального анализа с индуктивно связанной плазмой [Томпсон М., Уолш Д.Н. Руководство по спектрометрическому анализу с индуктивно связанной плазмой. - М.: Недра, 1988. - 174 с.]. Проверка достоверности полученных результатов проведена по методикам, адаптированным в стоматологии для изучения ротовой жидкости [Питаева А.Н., Коршунов А.П., Дистель В.А. и др. Физико-химические методы исследования смешанной слюны в клинической и экспериментальной стоматологии (учебное пособие). - Омск. 2001. - 71 с.].The following studies were performed to determine the parameters of the oral fluid. Validation of the obtained results was carried out according to the methods adapted in dentistry for the study of oral fluid [Pitaeva AN, Korshunov AP, Distel V.A. and other Physico-chemical methods for the study of mixed saliva in clinical and experimental dentistry (study guide). - Omsk. 2001. - 71 p.].
В ходе исследования были выделены три группы людей:The study identified three groups of people:
1) группа 1 - пациенты с кариесом зубов (30%).1) group 1 - patients with dental caries (30%).
2) группа 2 - пациенты, имеющие зубные отложения в полости рта (40%).2) group 2 - patients with dental deposits in the oral cavity (40%).
3) группа 3 - контрольная группа (30%).3) group 3 - control group (30%).
Взятую в качестве сравнения группу лиц, резистентных к заболеваниям, составили люди с одинаковым соматическим статусом «практически здоровые», а также без заболеваний пародонта и слизистой оболочки полости рта.The group of persons resistant to diseases, taken as a comparison, was composed of people with the same somatic status of “practically healthy,” as well as those without periodontal and oral mucosa diseases.
Следующий этап исследования включал определение элементного состава ротовой жидкости при изучаемых заболеваниях. Как видно из приведенных данных (табл.1, 2), при возникновении различных заболеваний полости рта происходит существенное отклонение параметров ротовой жидкости от нормы. Так, при образовании зубных камней уменьшается содержание ионов кальция, но увеличивается содержание фосфора и электролитных компонентов - ионов натрия и калия. При наличии кариеса наблюдаются такие же изменения, однако, содержание натрия остается в норме.As can be seen from the above data (Tables 1, 2), when various diseases of the oral cavity occur, a significant deviation of the parameters of the oral fluid from the norm occurs.
