Известен способ остеосинтеза таких переломов (Мюллер М., 1996) путем открытой репозиции с использованием зажимов или спиц Киршнера для предварительной фиксации, затем отломки фиксируют винтами к компрессирующей пластине, которую предварительно изгибают по контурам кости. Недостатком способа является то, что сложно адаптировать пластину заданной длины к основным и промежуточным отломкам таким образом, чтобы восстановить нормальную анатомическую протяженность кости и для стабильной фиксации создать компрессию между ее отломками, так как возможности компрессирующей пластины ограничены.A known method of osteosynthesis of such fractures (Müller M., 1996) by open reduction using clamps or Kirschner spokes for preliminary fixation, then the fragments are fixed with screws to the compression plate, which is pre-bent along the contours of the bone. Последовательность осуществления данного способа поясняем на примере оскольчатого внутрисуставного перелома проксимального конца локтевой кости. Задним разрезом в области верхней трети предплечья выделяют область перелома проксимального конца локтевой кости (фиг.1). Рассверливают костный канал под винт в проксимальном (3) и дистальном (4) отломках локтевой кости. Вворачивают компрессирующий винт (1) в оба отломка после максимального растяжения по длине (фиг.2). Укладывают по задней поверхности локтевой кости компрессирующую 3 мм пластину (2), формируя ее таким образом, что изогнутый центральный конец ложится под головку винта, а периферический - вдоль дистального отломка. Этот конец пластины временно фиксируют к отломку костодержателем (фиг.3). Затем последовательно от центра к периферии репонируют промежуточные отломки (5) и фиксируют их к пластине крепежными элементами (6) (например, винтами и проволочными петлями). При этом винты и петли не затягивают до самого конца (фиг.4). Удаляют костодержатель и с помощью винта осуществляют компрессию отломков по длине до полного соприкосновения друг с другом. Дотягивают винты и проволочные петли и фиксируют дистальный отломок к пластине 3-мя винтами (7) (фиг.5). In this case, the screws and loops are not tightened to the very end (Fig. 4). The bone holder is removed and the fragments are compressed along the length of the screw until they are in complete contact with each other. Pull the screws and wire loops and fix the distal fragment to the plate with 3 screws (7) (Fig. 5). A layered suture is applied to the wound.

Задним разрезом в области верхней трети предплечья послойно рассечены мягкие ткани до кости. Выделена область перелома локтевой кости. При ревизии раны обнаружен оскольчатый перелом локтевой кости, который включал проксимальный, дистальный и 5 промежуточных внутрисуставных и околосуставных отломка. Произведена репозиция и фиксация отломков винами и пластиной по описанному способу. Путем полного сгибания - разгибания и ротации в локтевом суставе, также растяжений по длине произведена проверка стабильности фиксации отломков.
