This invention relates to medicine, namely to otorhinolaryngology.
Cochlear AG (ed.), Nieburg, Germany: Hoffinann, 1992, pp 1-72].There is a method of fixing a cochlear implant on the surface of the skull using ligatures passed through tunnels into the bones of the cranial vault along the edges of the bone bed of the cochlear implant [Lehnhardt E., Ramsden RT, Fraysse B., Degunine O., Manrique M. Multilingual surgeon's manual for cochlear implants . Cochlear AG (ed.), Nieburg, Germany: Hoffinann, 1992, pp 1-72].
The disadvantage of this method is, firstly, the need when creating ligature tunnels to position the cutter at an angle of not more than 45 degrees to the surface of the skull. This requires a large surgical field. Secondly, the high probability of the cutter penetrating the cranial cavity and injuring the dura mater, especially in young children.
The aim of this invention is to increase the safety of the fixation method of the cochlear implant, as well as the possibility of its implementation in a limited surgical field.
This goal is achieved in that in the method of fixing a cochlear implant on the surface of the skull by creating a bone bed, a distinctive feature is that grooves are additionally formed in opposite walls of the bed and the implant is fixed using a titanium bracket with bent ends of the legs that are fixed in the grooves.
The method is as follows (drawing). After exposure of the bones of the surface of the skull 1 in the behind-the-ear region, a bone bed 2 is created for the cochlear implant. The walls of the bed are sheer 3. On opposite walls of the bed near the bottom, with a diamond mill with a working part, grooves 4 are cut in a steep form, parallel to the upper edge 5 of the bed, each 1.5 mm deep. Using a disk cutter allows you to work at right angles to the surface of the skull with a surgical field of minimum size. The gap between the cochlear implant and the bed wall is equal to the double thickness of the tape from which the grip is made, i.e. 1mm. One of the grooves is in communication with the surface of the skull in the form of a cut-notch 6. The grip is a U-shaped bracket 7, the ends of the legs of which are bent outward at an angle of 90 degrees 8. The length of the crossbar of the bracket is 1 mm longer than the width of the fixed cochlear implant, the height of the legs of the bracket 1.5 mm more than the size of the part of the cochlear implant protruding above the surface of the skull. The length of the bent ends of the legs of the bracket is 1 mm. In working condition, the grip bends around the cochlear implant from above and from the sides. The distance from the staple bar to the bending point is equal to the distance from the outer surface of the cochlear implant in the bed to the groove in the side wall of the bed, which includes the bent part of the leg. Before placing a cochlear implant in a bed in a groove, a grip with a bent part of the leg is inserted on one wall of the latter. The cochlear implant is located in the bed under the grasp, the bent part of the second leg is lowered into the bed through the recess to the level of the furrow on the opposite wall and moves into the furrow. The recess in the wall of the bed is filled with muscle tissue or bone filings.
Example: Patient K., 27 years old, was admitted to the ENT department with a diagnosis of bilateral deafness. The purpose of hospitalization: the surgical stage of cochlear implantation (placement of a cochlear implant in the temporal region). The cause of deafness is the use of chemotherapeutic drugs. Under endotracheal anesthesia, a vertical skin incision was made in the right behind the ear region, a fascial-periosteal flap was cut out, a mastoid process, part of the scales of the temporal and occipital bones were exposed. Mastoidotomy, posterior tympanotomy, cochleotomy (snail opened) were performed with a milling cutter. In the temporal-occipital region, a bed was prepared in the form of a round recess on the surface of the skull with sheer walls for the cochlear implant C124M from Cochlear (Australia). The diameter of the bed was 1 mm larger than the diameter of the PS, and grooves with a depth of 1.5 mm and a length of 4 mm were made on the opposite walls of the bed with a diamond disk mill of 4 mm in diameter at the bottom of the bed. One of the grooves is connected to the surface of the skull with a cut-notch. After the electrodes were inserted into the cochlea, the cochlear implant was placed in the bed, the bent part of the titanium bracket leg was inserted into the groove without a notch, while the bracket went around the cochlear implant and the second leg was lowered through the saw cut into the groove on the opposite wall of the bed. In this position, the bracket is shifted laterally until the second leg enters the bent portion into the groove. As a result, the cochlear implant was tightly fixed on the surface of the skull. The surgical wound is stitched in layers. In the postoperative period, the patient received antibiotics, the healing of the skin incision occurred by primary intention.
Thus, the proposed method allows you to fix the cochlear implant with a limited surgical field, reduce the likelihood of damage to the dura mater due to the milling cutter in the direction parallel to the surface of the skull, and reduce the duration of surgery.
