Abstract
Description
The invention relates to medicine, in particular to surgery (urology), and is used for instrumental removal of stones from the urinary tract.
П. Known extractor V.P. Pashkovsky for extracting stones from the ureters [1], containing a bearing cable woven from metal strings, a four-jaw basket-catcher and a conductor with olive, moreover, the branches of the basket are made diverging from the bearing cable at different angles and converging at the base of the conductor at the same angles, and the cross-section conductor - gradually decreasing from its base.
However, the device does not provide for reliable immobilization of the stone in the four-jaw basket, the stone protrudes beyond it and injures the ureter during extraction, stone loss during manipulations is not ruled out. In addition, the divergence of the branches immediately from the carrier cable limits the possibility of grabbing the stone.
The closest in technical essence is an extractor for extracting stones from the ureter [2], containing a supporting cable, a basket-catcher and a flexible conductor. The basket is formed by branches extending from the supporting cable in the form of two branches, divided by at least two additional branches, converging at one point in the place of conductor attachment, the conductor being covered with an elastic sheath.
In the prototype there is no constructive solution for varying the number of branches along the entire length of the basket-catcher, which determines the achievement of a positive effect.
The problem solved by the invention is to achieve a medical and social effect, namely increasing the efficiency of extracting stones from the lower urinary tract in a non-invasive way, which is achieved by increasing the reliability and non-invasiveness of stone reduction.
1.In FIG. 1 shows an isometric view of a 6-branch extractor; in FIG. 2 - the same with a rotation about an axis of 30 about ; in FIG. 3 - two projections of the extractor of FIG. 1.
2. The extractor consists of a support cable 1 with a
The extractor is equipped with branches with various physical properties. So,
The need for the same properties for all branch branches is optional, since for the formation of
The extractor is used as follows. Previously, a tubular guide on the conductor is passed through the cystoscope over the stone in the ureter. The cystoscope and conductor are removed, and the tubular guide is left in the urinary tract. An extractor is introduced into it and put it behind a stone. When the tubular guide is removed, the
The use of the inventive extractor can improve the reliability of the capture and immobilization of the stone in a constructive way, namely by varying the number of branches in different parts of the basket-catcher. Concretization of the point of divergence of the branches of the branches in the unequal part of the basket made it possible to expand the exciting gap to the maximum. This allows the stone, regardless of its size and shape, effortlessly to get into the bosom of the basket-catcher. At the same time, traumatic ureter traumatic scrolling of the extractor with the aim of combining the exciting gap with the stone is virtually eliminated since practically nothing prevents the stone from entering the bosom of the basket: the maximum gap formed by the minimum number of branches repeats the shape of the ureter.
The proposed design does not limit the number of branches in the working part of the extractor, so the bosom of the basket, formed by a dense network of branches, reliably immobilizes the stone, isolating it from the mucous membrane of the ureter. Consequently, atraumatic lowering of the stone increases.
Thus, the claimed design provides high efficiency of extraction of urinary tract stones, due to the absolute reliability of capture and immobilization in the bosom of the trap-catcher of stone of any size and configuration, as well as the non-invasiveness of stone deposition due to its isolation in the bosom of the basket.
