Description
The device relates to medicine, namely, to ophthalmology, and is intended for carrying out a comprehensive analysis of the state of the ocular blood flow in the orbital artery and the vessels of the anterior part of the brain.
A comprehensive analysis of blood flow is necessary to obtain the fullest possible information about the blood supply to the eye and to form effective diagnostic conclusions on this basis.
1971).Currently, for the study of the blood supply to the organ of vision, a reoophthalmography is known (LA Katsnelson Rheography of the Eye. M .: Medicine; 1977. 120 p., Lazarenko VI. Functional Rheography of the Eyes. Krasnoyarsk: Raster, 2000. 160 p. ) and ophthalmoplethysmography (A.Ya. Bunin, Hemodynamics of the eye and methods of its research. Moscow: Medicine, 1971).
2004; 4: 3-5).These contact methods for estimating pulse blood volume in intraocular vessels require a transducer to be placed on the perilimbal area of the conjunctiva using local anesthesia. The transpalpebral ultrasound method is also known - color Doppler mapping (Machekhin VA, Vlazneva I. N. Study of the blood supply to the eye using color ultrasound dopplerography. Siberian National Medical Journal. 2009. 4: 100-103; Kiseleva TN. Ultrasonic methods blood flow studies in the diagnosis of ischemic lesions of the eye. Bulletin of Ophthalmology (2004; 4: 3-5).
2015. 4: 4-10).In addition, contactless optical methods are known - optical coherent tomography - angiography (Kurysheva NI, Trubilina AV, Maslova EV. Optical coherent tomography - angiography and pattern-electroretinography in the early diagnosis of glaucoma. Glaucoma news. 2017. 1: 66-69) and laser Doppler flowmetry (Kiseleva TN, Adzhemyan NA Methods for assessing ocular blood flow in vascular eye disease. Regional blood circulation and microcirculation. 2015. 4: 4-10).
The above-mentioned transpalpebral and optical methods do not allow for a comprehensive assessment of the state of blood flow in the eye vessels, examining the blood supply of each vessel separately. At the same time, using the electric impedance method of the study, it is possible to evaluate the blood supply not in the individual arteries, but in the vascular system of the eye as a whole. Significant advantages of the electrical impedance method are also the lack of contact with the surface of the eye, the short duration of the study (2-5 minutes), mobility and low cost of equipment.
(R. Patterson, "Electrical Impedance Tomography: Methods, History, and Applications (Institute of Physics Medical Physics Series)", Physics in Medicine and Biology. 2005. 10:2427-2428). Electrical impedance methods are now known, including methods for obtaining impedance distribution in a human body through non-invasive electrical sounding, calculations and reconstruction algorithms (R. Patterson, "Electrical Impedance Tomography: Methods, History, and Applications (Institute of Physics, Medical Physics Series) ", Physics in Medicine and Biology. 2005. 10: 2427-2428). In Russia, the rheography method, the electrical impedance method for studying pulse oscillations in the blood supply of vessels of various organs and tissues, is widely used, based on the graphic registration of changes in the total electrical resistance of tissues (Ronkin MA, Ivanov LB Rheography in clinical practice. M .: MBN 1997. - 403 s.). In the study of the brain using leads, proposed H.H. Yarullin (Yarullin Kh.H. Clinical reoencephalography. L., "Medicine", 1967. - p. 68-88), which allows to estimate the blood supply of the main main arteries: anterior cerebral, middle cerebral, posterior cerebral and vertebral arteries. The disadvantage of this method is the impossibility of registering the pulse volume of the orbital artery.
The known method of transpalpebral reoophthalmography (Luzhnov P.V., Shamaev D.M., Iomdina E.N., Tarutta E.P., Markosyan G.A., Shamkina L.A., Sianosyan A.A. Transpalpebral tetrapolar reo-ophthalmography in problems of assessing the parameters of the circulatory system of the eye. Bulletin of the Russian Academy of Medical Sciences. - 2015. - T. 70. - No. 3. -P. 372-377.), involving the use of a device for attaching electrodes during its implementation (RU 153381, 11.12.2014 ). The method is designed to obtain quantitative indicators of uveal ocular blood flow. The disadvantage of this method and device for its implementation is the impossibility of simultaneous evaluation of the blood supply of the orbital artery and cerebral arteries.
The task of the utility model is to develop a device for a comprehensive assessment of blood circulation in the vessels of the eye and brain, namely in the orbital artery, from which all vessels of the eye branch off, and in the vessels of the anterior cerebral arteries, the anterior cerebral artery and middle cerebral artery.
The technical result is the possibility of carrying out a comprehensive analysis of the blood supply to the orbital artery and the vessels of the anterior part of the brain for diagnosing, including early, hemodynamic disorders in eye diseases and evaluating the effectiveness of their therapy.
The technical result is achieved through the use of a system of sixteen electrodes, positioned in accordance with the anatomical location of the sites of the analyzed vessels (internal carotid artery, anterior cerebral artery, middle cerebral artery and orbital artery).
The device is an elastic tape with holes to accommodate 16 electrodes, 12 of which are located on the tape itself, and 4 holes are on 4 plates attached to it and protruding beyond its edge. The tape has a pair of Velcro fasteners at the edges, providing, if necessary, changing the length of the tape.
1 и 2.A schematic representation of the device is shown in FIG. 1 and 2.
FIG. 1 - (1) - elastic tape with a hexagonal hole in the center, (2) - Velcro fasteners at the edges of the tape, separated by a notch in the tape, (3) - plates with holes for electrodes protruding beyond its edge, ( 4) - holes for electrodes.
FIG. 2 - preferred dimensions of the device.
Velcro fasteners at the edges of the tape provide, if necessary, a change in the length of the tape in accordance with the anthropometric data of the subject's head and the necessary level of pressing the device to the patient's head. A pair of Velcro fasteners is located on the left and right ends of the tape, structurally they are separated from each other by a notch in the tape, preferably 10.0 ± 1.0 mm wide, which allows you to adjust the tension of the tape separately along its upper and lower edges, thus avoiding the warping effect tape while fixing it on the head of the test. As a result, the perimeter of the tape in the buttoned state can vary in the range of 550-600 mm and is chosen individually for each subject. The distance between the holes of the electrodes can be 20.0 ± 2.0 mm. The attachment points of the extreme electrodes on the left and right sides of the tape (four electrodes on each side) are used to register the pulse volume of the internal carotid artery, the anterior cerebral artery and the middle cerebral artery. The mounting holes for the electrodes can have a diameter of ∅7.0 ± 0.7 mm. Each hole for mounting the electrode can be equipped with an insulating stop, which allows to mount reusable metal electrodes.
Four plates (3), made, for example, of ABS plastic (acrylonitrile butadiene styrene), are fixed on the tape. The location of the plates in the device is selected in accordance with the anatomical location of the orbital artery. Each plate can be located at a distance of 25.0 ± 2.5 mm from the horizontal axis of symmetry of the tape. In each plate there are two holes for fixing the electrodes, one of which is placed outside the contour of the tape, and the other coincides with the corresponding hole on the tape. The mounting holes for the electrodes preferably have a diameter of ∅7.0 ± 0.7 mm. Each hole of the electrode can be equipped with an insulating stop in the case of mounting reusable metal electrodes.
The device is used as follows: a device with electrodes located in it, fixed on an elastic tape and its plates, is mounted on the patient's head in the periorbital region (on the closed eye). The location of the plates is determined in accordance with the patient's nasal bone - the plates should be placed at a distance of 15 ± 3 mm from the nasal septum. Further, fixation on the patient's head is carried out using a Velcro elastic tape fastener. After that, the device is ready for operation and further connection of the device for recording electrical impedance signals.
When using the device, it is possible to simultaneously obtain quantitative indicators of the blood filling of the orbital artery and the blood filling of the vessels of the anterior part of the brain.
An example of the electrode positioning scheme when using the device for examining the left hemisphere of the brain is shown in FIG. 3. The scheme for the study of the right hemisphere is symmetric about the sagittal plane.
Example. Subject N., 29 years old, ophthalmopathology is absent.
Modeling of ocular and eyelid pulse blood filling in diagnosing using transpalpebral rheoophthalmography. 30-42%.The registration of eographically signals at the location of the electrodes shown in FIG. 3. The results of using a device with such an arrangement of a tape with electrodes showed compliance with theoretical calculations obtained on the basis of a mathematical model (Shamaev DM, Luzhnov PV, Iomdina EN Modeling of the ocular and eyelid rheoophthalmography. EMBEC & NBC 2017, IFMBE Proceedings 65 : 1000-1003) for rheophthalmographic studies. The value of the base impedance indicator in the study of the blood filling of the orbital artery was 33% higher than in the study of the anterior part of the eye using the transpalpebral reophthalmography technique. This result is consistent with the theoretical estimate, which gives an increase in the base impedance in the case under consideration in the range of 30-42%.
Thus, the proposed device provides the ability to conduct multichannel electrical impedance studies in ophthalmology with simultaneous assessment of blood supply in the orbital artery without contact with the eye surface, as well as in the vessels of the anterior part of the brain, which can be of great importance in clinical practice - for the diagnosis of eye diseases and control the effectiveness of their treatment.
