Abstract
A process for the preparation of a composition having improved vitamin C activity is described. Vitamin C salts are partially converted into corresponding aldonic acid salts (L-threonates, L-xylonates and L-lyxonates) by heating between 40 and 80 ° C under oxidizing conditions.
Description
This invention relates to a method for producing a preparation with improved vitamin C activity.
More particularly, the invention relates to a method for preparing a mixture for improving the effectiveness of certain therapeutically active compounds. 1 preventive or curative drug therapy, one usually wishes to initially establish a physiologically effective level of a drug or other therapeutically active compound in the human or animal host body, and then maintain this effective level for an extended period of time, as necessary to achieve the desired physiological result. An improvement in either the absorption rate ("uptake rate") or the retention (reduction in the excretion rate) or both usually provides important physiological and therapeutic benefits. It is also very advantageous if unwanted side effects of the therapeutically active compound can be reduced or eliminated.
In order to initially illustrate the implementation and principles of the invention for specialists in the field, the invention will now be described with reference to the application of the invention to the improvement of vitamin C therapy.
Previous researchers have identified over 300 different metabolic mechanisms in which vitamin C is involved in physiological reactions. These mechanisms range from the antiscorbutic action first observed by Dr. Robert Lind in 1740 to the more recently discovered antioxidant free radical eliminating turnovers, to co-reaction with enzymes in the formation of collagen, underlining the energy metabolism of the polynuclear leukocytes and transport of iron - the absorption.
The clinical effects of such metabolic reactions have been widely recognized and reported. For example, the free radical scavenging effect is believed to enable the body to convert carcinogens into non-toxic derivatives that are eliminated in the urine, and therefore to ameliorate the effects of smoking and exposure to other environmental pollutants and temperature extremes. Animal studies have shown that body enzymes convert ascorbates into oxidation products that have demonstrated inhibition of tumor growth.
There is therefore little scientific doubt that establishing and maintaining an effective level of vitamin C and its derivatives in the human body provides important health benefits. The presence of vitamin C in significant concentration has been observed in the adrenal glands, ovaries, brain, pituitary gland, liver, spleen, blood cells, blood serum, and extracellular lung fluids.
Most animals have a liver enzyme that enables them to actually manufacture vitamin C in situ by converting blood sugar to ascorbic acid. However, humans do not have this enzyme. As a consequence, the vitamin C required by the human body for the various metabolic reactions discussed above must be taken in with the human diet. Furthermore, the human body does not have the ability to store vitamin C. If it is unmetabolized, it is excreted again. A low level of vitamin C and its derivatives in the human body produces various undesirable physiological responses, and an extremely low level produces extreme responses that can lead to death, e.g. from scurvy. Quite apart from these "normal" needs for vitamin C, it is important in some therapeutic cases to establish and maintain higher than normal vitamin C levels in the body. These above-normal concentrations are difficult to establish and maintain because the human body only has a certain tolerance for vitamin C (ascorbic acid) with resultant diarrhea and other side reactions such as stomach irritation and inflammation if these tolerances are exceeded.
Methods have now been discovered for producing a preparation with improved vitamin C activity. The preparation improves the efficiency, i.e. establishment and/or maintenance of the body's level of therapeutically active compounds which are usually eliminated from the body without metabolism via the kidney's tubular secretion mechanism for organic anions. Such therapeutically active compounds have a molecular weight below approximately 5,000, have an acid functional group and a pKa of 6 at physiological pH = 7.4. The mixtures produced according to the invention comprise such therapeutically active compounds and at least one compound selected from the group consisting of L-threonine, L-xylonic and L-lyxonic acid, and the non-toxic edible salts, aldone-lactones and aldone-lactides hence.
The present invention therefore relates to a method for the production of a preparation with improved vitamin C activity, characterized by the fact that vitamin E salts are partially transferred to corresponding aldonic acid salts (L-threonates, L-xylonates and L-lyxonates) by heating to between 40 and 98 °C under oxidizing conditions.
Reaksjonstemperaturen som anvendes er fortrinnsvis 60-80°C. The reaction temperature used is preferably 60-80°C.
It is possible to administer a physiologically effective dose of this composition to a human or an animal.
It is further possible that the aldone compound is administered in a first step to establish an effective level thereof in the body, and then the therapeutically active compound is administered.
L-askorbinsyre 2-O-sulfat, L-askorbinsyre 2-O-fosfat, L-askorbinsyre 3-O-fosfat, L-askorbinsyre 6-heksadecanoat, L-askorbinsyre monostearat, L-askorbinsyre dipalmitat, o.l. As used herein, the term "compound having vitamin C activity" shall mean vitamin C (L-ascorbic acid) and any derivative thereof which exhibits anti-scorbutic activity. Such derivatives include e.g. oxidation products such as dehydroascorbic acid and edible salts of ascorbic acid such as calcium, sodium, magnesium, potassium and zinc ascorbates, esters of vitamin C with organic and inorganic acids, e.g. L-ascorbic acid 2-O-sulphate, L-ascorbic acid 2-O-phosphate, L-ascorbic acid 3-O-phosphate, L-ascorbic acid 6-hexadecanoate, L-ascorbic acid monostearate, L-ascorbic acid dipalmitate, etc.
Metabolites of ascorbic acid and its derivatives include aldonic acid, the aldone lactones, the aldone lactides and the edible salts of the aldonic acids. As will be seen, the mixtures produced according to the present invention are characterized by the presence of at least one or more of these metabolites corresponding to three special aldonic acids: L-threonic acid, L-xylonic acid and L-lyxonic acid.
Aldon-laktonene har strukturformelen The aldone lactones have the structural formula
hvori R er hydrogen eller -CH-OH og n = 1 til 3. wherein R is hydrogen or -CH-OH and n = 1 to 3.
The presence of one or more of these metabolites in the mixtures produced according to the invention is both a good way to identify such mixtures, and is also necessary to achieve the desired result, improvement in absorption and/or retention of vitamin C or another therapeutically active compound.
Lengre oppvarming ved oksyderende betingelser frembringer lavere jodaskorbat-analyser. A suitable method for preparing the improved vitamin C mixture comprises heating L-ascorbic acid with a non-toxic metal compound, e.g. calcium carbonate, sodium bicarbonate etc., under oxidizing conditions at elevated temperature, e.g. 40-89°C, to convert a significant part of the ascorbic acid to its corresponding salt, e.g. calcium or sodium ascorbate and drying the reaction mixture so that a solid product with essentially neutral pH (e.g. 6.0 - 7.5) is produced. Preferably, a slight stoichiometric excess of the metal salt reactant is used. The resulting product has an iodoascorbate assay in the range 50-480 mg/ 500 mg sample depending on process parameters, so that the higher ascorbate activity is preferred for practical reasons. Longer heating under oxidizing conditions produces lower iodoascorbate analyses.
The mixtures produced according to the invention prove to be usable for the administration of vitamin C to patients who have a low ascorbic acid tolerance. In particular, patients who have a tendency to form kidney stones are particularly exposed to difficulties when consuming ascorbic acid and its usual derivative, calcium ascorbate, which gives an elevated oxalate level in the urine. There are indications that the compositions prepared according to the present invention can be administered without increasing the oxalate level in the urine to the level that is common when using compositions and methods according to prior art. These compositions are particularly suitable as a means of establishing and maintaining an adequate ascorbate level in the body in such subjects who are prone to kidney stones. The vitamin C mixtures produced according to the invention can also be used in the treatment of inflammatory diseases, such as arthritis.
171, p 103 (1970)). The effectiveness of the aldonin compounds in improving the absorption, tolerance and/or retention rates of vitamin C compounds is also generally applicable to improving such properties of therapeutically active compounds having a molecular weight below about 5,000, and having an acid functional group and pKa< 6. The the physiological mechanisms responsible for these improvements appear to be inhibition of the normal elimination of such unmetabolized therapeutically active compounds from the body via the renal tubular secretion mechanism for organic anions and the improved absorption of these components through the cell walls of the body tissues. The aldonin compounds obviously work so that they provide both improved absorption and inhibition of the kidneys' excretion effect. A general description of the different secretion mechanisms in the kidneys is contained in the article by Hirsch and Hook, Journal of Pharmacology and Experimental Therapeutics (vol. 171, p 103 (1970)).
The amount of the aldonine compound which is necessary in the mixtures prepared according to the invention is a therapeutically effective proportion. The exact amount will vary somewhat depending on the precise nature of the therapeutically active compound and other factors that will occur to those skilled in the art. Generally, the reduced renal secretion rate and/or increased absorption rate in the body is somewhat improved by even very small amounts of the aldonin compounds. These properties will be improved by increasing the proportions of the aldonine compound, and the upper limit is determined by practical considerations such as avoiding undue dilution of the therapeutically active ingredient to the point that a satisfactory minimum dosage is not administered. According to the best available information, the proportion of the aldonine compound in the mixtures prepared according to the invention may vary from less than 1% by weight to 24% by weight. Practical therapeutic effects have been observed at concentrations of the aldonine compound in the range of about 0.10% by weight, and the preferred range here is about 1% by weight to about 7% by weight of the aldonine compound.
The mixtures can be prepared by simple physical mixing of the components in the mixtures. In the case of vitamin C, the mixtures can alternatively be prepared in situ by the techniques illustrated in the working examples.

The following examples are given for the purpose of illustrating the practice of the invention.
EKSEMPEL 1 EXAMPLE 1
To an 80 gallon (300 liter) steam heated stainless steel reaction vessel was added 60 Ibs (27 kg) of hot water (44°C). Ascorbic Acid - U.S.P., 110.23 Ibs (50.00 kg) was added in one portion to the hot water. The resulting slurry was mechanically stirred and heated with steam (pressure 15 p.s.i = 103 kPa) until a temperature of 70°C was reached.
To the aqueous slurry of ascorbic acid was added 23 lbs (10.4 kg) of calcium carbonate. The stepwise addition of the carbonate required 3-4 minutes. The reaction mixture appeared gray in colour, and due to development of CO2 there was a lot of foaming. After 8 minutes of stirring, most of the foaming had ceased and the reaction mixture appeared reddish brown in color. The temperature of the solution was 80°C. Stirring and heating were continued for 15 minutes until the temperature of the reaction mixture reached 98°C where it was held for an additional 20 minutes, after which an additional 8.25 lbs (3.75 kg) of calcium carbonate was added, with stirring. After foaming ceased, the reaction mixture was then pumped to a steam heated double drum dryer (surface temperature about 250°F = 121°C). The pump drying step required 35 minutes. The dried product was light brown in color and the yield was about 120 Ibs (54.5 kg) of product.
Optionally, air can be bubbled through the reaction mixture to promote the turnover of the ascorbic acid.
Analyzes were performed immediately on 5.00 g of samples dissolved in 500 ml of distilled water.
The material collected during the drying process showed 400 mg of anhydrous calcium ascorbate per 500 mg by standard iodine titration technique. The same aqueous solution showed a pH of 7.0.
EKSEMPEL 2 EXAMPLE 2
The following example describes clinical tests for the comparison of products from example 1 (the test material) with L-ascorbic acid and citric acid (placebo), by measuring the ascorbate level in serum and intracellularly, the ascorbate excretion and the oxalate excretion in the urine at different times after intake of standard doses of the test, L-ascorbic acid and placebo.
Sammendrag av protokollen Summary of the protocol
Tolv menn i alderen 27 til 45 ble studert. Twelve men aged 27 to 45 were studied.
All were instructed to be on a low vitamin C diet for a week prior to the study (no citrus products and no large amounts of green leafy vegetables).
After overnight fasting, blood and 24-hour urine samples were taken. Ascorbate and oxalate levels in white blood cells and 24 hour urine were determined and correlated with serum ascorbate levels.
The 12 men were divided into three groups, and were given the following supplement:
(a) Testgruppe: 4000 mg<*> pr. dag av produktet eks. 1. (a) Test group: 4000 mg<*> per day of the product e.g. 1. (b) Ascorbate group: 3000 mg L-ascorbic acid per day (c) Citric acid group: 3000 mg citric acid per day and night. ;<*> 4000 mg is equivalent in ascorbate (iodine test) to 3000 mg of L-ascorbic acid.
All 12 continued on the low vitamin C diet. Blood samples were taken after 0, 4, 8 and 24 hours after the morning intake of the indicated supplements. Ascorbate and oxalate levels were determined in 24-hour urine.
After a washout period (varying from two days to several days due to the work situation of the participants), the groups were put on other supplements, as follows:
(a) Testgruppen til sitratgruppen. (a) The test group of the citrate group.
(b) Sitratgruppen til askorbatgruppen. (b) The citrate group of the ascorbate group.
(c) Askorbatgruppen til testgruppen. (c) The ascorbate group of the test group.
The supplements were taken at the same level (4000 mg of ex. 1 product, 3000 mg of L-ascorbic acid and 3000 mg of citric acid) by all three groups. The blood samples were again taken after 0, 4, 8 and 24 hours from the time of ingestion. 24 hour urine was also collected from all 12 participants at the end of the period. All samples were again analyzed for their respective concentrations of ascorbate and oxalate levels.

For the determination of oxalate in 24-hour urine, an aliquot of the urine is shaken with an adsorbent that selectively binds the oxalate. The extracted urine is discarded, and the oxalate eluted from the adsorbent with dilute alkali.
Oxalate is oxidized to hydrogen peroxide and carbon dioxide by oxalate oxidase. The hydrogen peroxide reacts with 3-methyl-2-benzothiozolinone hydrazone (MBTH) and 3-(dimethylamino)-benzoic acid (DMAB) in the presence of peroxidase to give an indamine dye with a maximum absorbance at 590 nm.

Resultatene av denne kliniske studie er fremsatt nedenfor: The results of this clinical study are presented below:
The conclusions drawn from this study are:
Asknrhatnivå i serum: Serum ash level:
After 4, 8 and 24 hours and 7 days later, the test groups had higher ascorbate levels in serum than compared to both the citrate group and the L-ascorbic acid group. Ascorbate level in white blood cells: Although all the groups with ascorbate in white blood cells after 8 hours showed an average reduction, the test group had the smallest percentage reduction. Measurement at 4 hours and 24 hours plus the level on the 7th day showed that the test group was able to retain the highest level of ascorbate in white blood cells.
Askorhat i hvite blodlegemer etter 24 timer: Ascorhate in white blood cells after 24 hours:
24 hours after different loads of: citrate, L-ascorbic acid and test give similar results. Both the citrate group and the L-ascorbic acid group showed a reduction in the level of ascorbate in white blood cells. The test groups maintained a much higher level compared to the baseline.
7 døgn etter belastning med T.- askorhinsyre og test: 7 days after exposure to T.- ascorbic acid and test:
Gjennomsnittlig prosentvis forandring av askorbat i hvite blodlegemer er fremdeles høyere i testgruppen enn i L-askorbinsyregruppen. The average percentage change of ascorbate in white blood cells is still higher in the test group than in the L-ascorbic acid group.
Askorbatutskilling i 74 timers urin: Ascorbate excretion in 74-hour urine:
Testgruppene hadde lavere askorbatutskilling enn sitrat- og L-askorbat-gruppene. The test groups had lower ascorbate excretion than the citrate and L-ascorbate groups.
Askorbatutskilling i 7 døgns - 7. 4 timers urin Ascorbate excretion in 7-day - 7.4-hour urine
The test groups have lower ascorbate separation than the citrate and L-ascorbate groups.
Oksfllflt ntskilling i 74 timers urin Oxflflt excretion in 74 hour urine
Oxalate excretion is greatly reduced in the test group compared to the ascorbic acid group. This means that when you take the test product as a supplement, you have less chance of forming oxalate-containing kidney stones than when you take L-ascorbic acid.
Oksalatntskilling etter 7 døgn og etter 7. 4 timer: Oxalate separation after 7 days and after 7.4 hours:
Prolonged supplementation with test product leads to lower excretion of oxalate in the urine than supplementation with L-ascorbic acid.
EKSEMPEL 3 EXAMPLE 3
To this stirred gruel, finely divided calcium carbonate is added in portions at such a rate that it gives constant evolution of carbon dioxide (by-product of the reaction), while the reaction temperature is kept at around 20°C. The addition of calcium carbonate ceases after about 25 g to 37.5 g have been added (corresponding to from about 20% to 30% of that required for complete reaction with the added L-ascorbic acid).
At this stage the temperature is raised to 80°C. Further addition of calcium carbonate is started while the temperature is kept in the range of 60 to around 70°C. The total amount of calcium carbonate added is 125 g (1.25 mol).
The reaction mixture is transferred to a shallow container which is maintained at a temperature of between 60 and 80°C for a period of from 12 to 36 hours, during which the pH of the mixture rises to a pH range of 6.0 to 7.0. At this stage, excess water is removed under vacuum.
The dry products are light brown in color and dissolve easily in water, with the exception of unreacted calcium carbonate, and give neutral solutions.

EKSEMPEL 5 EXAMPLE 5
The products from Examples 1 and 3 are subjected to qualitative analysis as follows: After filtering off excess insoluble calcium carbonate, calcium ascorbate was removed from the product by chromatography, and the residue was subjected to nuclear magnetic resonance spectroscopy. Plausible possibilities were designed for the components' structures determined by spectroscopy, and these authentic compounds were then synthesized. After the nmr spectra of these authentic compounds were prepared, they were compared with the nmr spectra of the test samples. Coincidence of these spectra was used to identify the components of the test samples.
The techniques used were 1H and 13C nmr. The identified salts of aldonic acid are the calcium salts of L-threonic acid, L-xylonic acid and L-lyxonic acid.

Produktene har de viste sammensetningene: A quantitative analysis of the products from e.g. 1,4 and 6. The products have the compositions shown:
The aldonic acid derivatives comprise derivatives of the acid shown in the following approximate proportions in the residue above.
EKSEMPEL 9 EXAMPLE 9
Fremgangsmåtene i eks. 2 blir gjentatt, med unntak av at testproduktet syntetiseres ved å blande kalsiumaskorbat av reagenskvalitet med: The procedures in ex. 2 is repeated, except that the test product is synthesized by mixing reagent grade calcium ascorbate with:
Test A - threoninsyre (kalsiumsalt) Test A - threonic acid (calcium salt)

Man gjentar testene i eks. One repeats the tests in e.g. 2 using these test compounds, and using pure calcium ascorbate as an additional control.
These tests confirm that the physiological activity of the mixed ascorbate aldonic acid product is due to the aldonin component, and that either of these aldonin components causes the similarly enhanced absorption and retention of the vitamin C component.
EKSEMPEL 10 EXAMPLE 10
This example illustrates the practice of the invention to improve the absorption/retention of aspirin.
Sixteen albino rats of the Wistar strain, eight males (242-333 g) and 8 females (295-345 g) were acclimatized for seven days before this study was started. During this period, the rats were fed Purina (R) Rodent Chow, received water ad lih, and lived in self-cleaning stainless steel cages. The animal room was maintained at 70 ± 2°F (21 ±1°C), 60-80% relative humidity, and had a photoperiod of 12 hours light - 12 hours dark.
Gruppe A fikk U.S.P. aspirin i 54 mg/kg i 2,0 ml destillert vann gjennom sonde. The rats were randomly selected into two groups of 8 animals each (4 males and 4 females) and placed in metabolism cages. Group A received the U.S.P. aspirin at 54 mg/kg in 2.0 ml of distilled water by gavage. Group AM received the U.S.P. aspirin at 54 mg/kg + calcium threonate, (a metabolite of ascorbic acid) at 15 mg/kg in 2.0 ml of distilled water, also by gavage. Blood samples were taken at 1, 2, 3 and 4 hours after dosing for serum salicylate analysis.
(3 ml). Urine was collected if it had been passed at the same times, also for salicylate analysis. In an attempt to maximize urine production, the rats were given additional water at 1, 2, 3 and 4 hours by gavage. (3 ml).
Blod og urin ble oppsamlet og analysert ifølge Natlesons metode (Natelson, S. Techniques of Clinical Chemistry, 3. utg. p. 649, Charles C. Thomas, utgiver Blood and urine were collected and analyzed according to Natleson's method (Natelson, S. Techniques of Clinical Chemistry, 3rd ed. p. 649, Charles C. Thomas, publisher
(1971)). The study was terminated after the time of the 4 hour collection.
All statistics were calculated using Statistical Analysis System computer programs (SAS Institute, Inc., Box 8000, Cary, NC 27511). The ANOVA procedure (repeated measures model) was used to determine differences in salicylate concentrations between groups.
Table 1 shows the actual serum salicylate concentrations for each animal in both groups A and AM, at each sampling time. The mean values (X) and standard deviation (SD) are given for each group and time point. Figure 1 is a histogram of the data given in Table 1. The significance level (P) for the differences between the two groups at each sampling time in the serum salicylate concentration is given after each set of histograms. Figure 2 is the time profile for serum salicylate for both groups of rats, the standard deviation is included. The initial absorption rate (gastrointestinal) is calculated from the linearized curve in the first hour after dosing. Data for urinary salicylate are not given in detail but will be discussed below. The groups are not broken down into males and females, because there is no significant difference in serum salicylate in relation to the sex of the animal.
Kurven for AM-gruppen har to faser, hvilket indikerer to prosesser. It is obvious from these data that the AM group had a much higher initial uptake or absorption of U.S.P. aspirin. This group had an uptake rate of 11.76 mg/hour, while the A group had a much lower uptake rate of 4.40 mg/hour. The curve for the AM group has two phases, indicating two processes. The first part of the curve concerns uptake or absorption in the gastrointestinal tract. There is an accelerated uptake in the AM group compared to the A group, and this is obvious from the respective serum profiles. The second part of the curve concerns the distribution to other parts of the body and the excretion in the kidneys. The difference between the curves at this point (two hours) is most likely due to reduced salicylate excretion from the kidneys in the AM group (U.S.P. aspirin + aldonin). This group appears to be approaching a steady state concentration of salicylate at a higher level than the U.S.P. aspirin group that is decreasing at this time.
The elimination rate for the two groups is significantly different. The elimination rate for the plasma concentration in the A group (calculated from log to peak) is 1.30 mg/hour, while the AM group has 0.05 mg/hour. This elimination rate is approximately 26 times higher when the metabolite is not present.
Salicylate analyzes in urine tend to show that less salicylate was excreted over time in the AM group compared to the A group. In the first hour after dosing, the content in the A group's urine was approximately 9.1 mg% and in the AM group 5.0 mg%. This is consistent with the higher serum level in the AM group and lower level in the A group's serum.
The results confirm that the presence of the metabolite initially increases aspirin absorption, acting locally on the epithelial cells of the gastrointestinal tract. As the metabolite itself is absorbed and increases its concentration in the blood, it will have an inhibitory effect on the kidneys. This effect leads to the reduced excretion of aspirin by the renal mechanisms. A delay is expected until the inhibition of nyreutski 11 none is established, and this is reflected in the decrease in the AM profile at 2 hours. The recovery between 2-3 hours is then due to the inhibition of the renal excretion process and continued absorption.
Opptakshastigheten for AM var 2,67 ganger høyere enn opptakshastigheten for A. Elimineringshastigheten for AM var imidlertid 26 ganger mindre enn for A gruppen. Den reduserte nyreekskresjonen hadde derfor større betydning for å holde ved like salicylatnivået i serum over tiden, enn øket opptakshastighet. The increased salicylate level in serum in the AM group is more a result of reduced excretion, rather than increased uptake. The uptake rate for AM was 2.67 times higher than the uptake rate for A. However, the elimination rate for AM was 26 times less than for the A group. The reduced renal excretion was therefore more important in maintaining the same level of salicylate in the serum over time, than an increased absorption rate.
The preceding tests show that the addition of the aldonine compound not only increases the absorption rate of aspirin, but also inhibits the kidney's organic anion transport system. This means that the aspirin level in the blood can be maintained for a longer period of time by reducing the rate of elimination via the secretion process that takes place in the proximal tubules of the kidneys.

1. Alle disse hundene led av bevegelsesforstyrrelser. 180 dogs of varying breed and age were given 3X30 mg/kg of the product from ex. 1. All these dogs suffered from movement disorders.
The effect of the supplements was measured by changes in the real symptoms, as they were perceived by new clinical evaluation and the owner's report on the condition of the animals. The effect was measured 7 days after the supplement was given, and then again after 6 weeks. The last evaluation was after more than 6 months.
Diagnosegrupper: Diagnostic groups:
100 hunder kvalifiserer under kriteriene ovenfor. Several different ailments were treated, both acute and chronic. In acute diseases where the symptoms change rapidly, it is difficult to distinguish between the effects of the given supplements and other factors. This test therefore only studied disorders that have a known cause that becomes permanent, where the symptoms had been stable over a period of time and where one would assume they would continue without supplementation. Animals with the following chronic conditions were selected: joint damage with secondary permanent changes, arthrosis, spondylosis, hip dysplasia, older disc herniation with secondary permanent changes, muscle atrophy as a result of reduced function and senile wear and tear changes in the support and movement system. 100 dogs qualify under the above criteria. Age or race were not considered in this report.
Spondylose og ryggprolaps: Spondylosis and back prolapse:
This example shows that the product from e.g. 1 given orally, provides symptomatic relief of the pain of chronic deforming changes in the joint and skeletal system in most cases in this group of patients. Neither calcium ascorbate alone nor L-ascorbic acid alone provides such relief.

10 albinorotter av Wistarstammen, hanner (375-411 g), ble aklimatisert i 10 albino rats of the Wistar strain, males (375-411 g), were acclimated in
7 days before the survey began. During this period, the rats were fed Purina (R) Rodent Chow, received water ad lih and lived in self-cleaning stainless steel cages. The animal room was maintained at 70 ± 2°F (21 ± 1°C), 60-80% relative humidity, and had a 12 hour - 12 hour light-dark photoperiod.
Gruppe A fikk U.S.P. Piroxicam i 0,6 mg/kg, og gruppe AM fikk U.S.P. Piroxicam i 0,6 mg/kg pluss kalsium threonat i 15 mg/kg gjennom sonde. The rats were randomly selected into two groups of 5 each and placed in Nalgene (R) cages. Group A received the U.S.P. Piroxicam at 0.6 mg/kg, and group AM received U.S.P. Piroxicam at 0.6 mg/kg plus calcium threonate at 15 mg/kg by gavage. Plasma samples were taken at 4, 6, 8 and 10 hours after dosing for Piroxicam analysis in plasma.
Selective HPLC was used to analyze Piroxicam, and these data were analyzed via SAS. ANOVA repeated measures procedures were used to determine differences in plasma Piroxicam concentrations between groups.
Table 4 shows the relevant plasma concentrations for each animal in groups A and AM at each sampling time. The mean values (X) for each group and time are given. Figure 3 is the plasma profile for the data given in table 4.
The initial rise in the plasma level of Piroxicam is significantly greater in the AM group compared to the A group. There is a two-fold increase in the peak level of Piroxicam in plasma in the AM group at 4 hours. Subsequently, this level began to decrease through the 6-hour point and approach the A group at 8 hours. However, the AM level in plasma starts to rise again after 8 hours. There appears to be an effect of the threonate on Piroxicam plasma levels. Statistical differences were found to be p.< .07 over the 10 hour period. This is a strong indication that a threonate effect is taking place.

Rottene i gruppe A fikk daglig i 3 døgn 1,0 ml destillert vann gjennom sonde før undersøkelsen begynte. The rats were randomly divided into 2 groups of 5 each. The rats in group A received 1.0 ml of distilled water by gavage daily for 3 days before the examination began. The rats in group AM received 15 mg/kg/ml calcium threonate daily by gavage, also for 3 days. The threonate dosages were corrected daily for body weight gain to ensure that the 15 mg/kg dose was maintained.
On day 4, group A rats received 0.6 mg/kg Piroxicam by gavage, and group AM rats received 0.6 mg/kg Piroxicam and 15 mg/kg threonate. Plasma samples were taken at 4, 6, 8 and 10 hours after dosing for Piroxicam analysis in plasma.

These data were analyzed statistically by the SAS procedure. ANOVA repeated measures procedure was used to determine the differences in Piroxicam concentration in plasma between the groups.
Tabell 5 viser de aktuelle Piroxicam konsentrasjonene i plasma for hvert dyr i gruppene A og AM (metabolitt forbehandling). Gjennomsnittsverdiene (X) for hver gruppe er gitt for hver tidsperiode. Figur 4 er plasmaprofilen konstruert ut ifra de Table 5 shows the relevant Piroxicam concentrations in plasma for each animal in groups A and AM (metabolite pretreatment). The mean values (X) for each group are given for each time period. Figure 4 shows the plasma profile constructed based on those
data som er gitt i tabell 5. data given in Table 5.
Forskjellen utgjør i gjennomsnitt omlag 60 % høyere plasmanivå i AM gruppen. It appears from the plasma profiles that the AM pre-treatment group had a significantly higher Piroxicam level in plasma than the non-treatment group. The AM group had a higher uptake rate and maintained a higher plasma level during the 10 hour period. The difference amounts to an average of around 60% higher plasma levels in the AM group. These higher levels are statistically significant at p.< 0.05.
Pretreatment with calcium threonate significantly increased and maintained Piroxicam plasma levels (Feidene) over a ten-hour period.

25 cm<3> T-flasks containing mouse 3T3 fibroblasts were incubated with 100 mg% (1 mg/ml) calcium L-threonate (pH 7.4) for 1 hour at 37°C. A control group was incubated with Riner's as a control.
The incubation fluid was removed and 1.25 mg% ascorbic acid was added to each bottle. (5 µl of 14 C-L ascorbic acid was added, 10.0 mCi/mmol, 0.05 mCi/ml). The pH was adjusted to 7.62 and the bottles were kept at 37°C for 20 minutes.
The flasks were then washed with 4 ml of HBSS(-), and were then trypsinized with 0.1 ml of trypsin-EDTA for 5 minutes. The cells were resuspended in 4 ml of ice-cold HBSS(-) to stop the enzymatic turnover. The cells were centrifuged for 10 minutes at 1000 G. The cells were then washed and resuspended in 4 ml of HBSS(-). The cells were centrifuged again (10 min, 1000 g), resuspended in 1.0 ml distilled H 2 O and sonicated for 90 seconds.
0.5 ml of each sample was used for <14>C counting and 0.5 ml was used for protein analysis using the Bradford method.
The uptake of <l4>C in the cells was calculated, and gave the following results.
5. The uptake of <l4>C-L-ascorbic acid was thus 1.86 times greater in the cells from the bottles containing the threonate than in the cells containing the control solution. These results are statistically significant (Studenf's T-test, alpha = 0.05). These results are shown graphically in fig. 5.
