Abstract
Description
CATHETER VOOR STENT-IMPLANTATIECATHETER FOR STENT IMPLANTATION
The invention relates to a catheter for introducing a stent into a patient. A stent is a tubular member, usually made of metal wire mesh, that can be expanded in section. Such an element is for instance introduced into a blood vessel in a patient whose wall is weakened such that there is a risk of collapse of the blood vessel concerned. The stent is inserted in a compressed or squeezed state and expanded at its intended site, thus forming a reinforcement of the blood vessel wall. Expansion usually occurs with the balloon of a balloon catheter. Stents and catheters for insertion thereof are known per se and are described, for example, in U.S. Patent 5,226,889.
The present invention aims to provide an improvement on the known catheters for stent implantation.
This object has been achieved with the catheter according to the invention as characterized in claim 1. By using the bosses between which the stent is confined, the stent is securely fixed at the balloon level to expand the stent. The stent cannot slide off the balloon. Due to this good fixation of the stent, the use of a so-called sheath is unnecessary, which simplifies the treatment to be carried out with the catheter according to the invention.
A favorable further development is characterized in claim 2. The thickenings can hereby be applied relatively close to each other without limiting the expansion possibility of the balloon. This allows the stent to be securely fixed in the center of the balloon. In the unexpanded state of the balloon, it fits snugly over the thickenings, thereby also achieving a smooth course of the catheter longitudinally over the thickenings.
For proper fixation of the stent, it is sufficient if the thickenings relative to the inner diameter of the pinched stent are such that the stent cannot slide over these thickenings. Preferably, however, the measure of claim 3 is applied. As a result, the stent lies, as it were, in a recess or "bed" defined by the thickenings, whereby the stent does not complicate the insertion of the catheter.
A suitable embodiment is characterized in claim 4. The measure of claim 5 is preferably applied. The two rings, and therefore the position of the stent lying between them, are readily detectable in this way, so that the stent can be accurately positioned.
The measure of claim 6 is preferably applied. The relatively distal balloon member can be used to dilate the blood vessel where the stent is to be deployed, even when a strong narrowing is present. After dilating the blood vessel with the relatively distal balloon, the catheter is advanced slightly with the stent positioned at the dilated blood vessel portion. Thereafter, the balloon around which the stent is deployed can be expanded, causing the stent to expand and the vessel wall portion to provide the desired support. Thus, with a single catheter, dilatation and stent delivery can be performed.
Een geschikte uitvoeringsvorm is gekenmerkt in conclusie 7.A suitable embodiment is characterized in claim 7.
The invention is further elucidated in the following description with reference to the attached figures.
Figure 1 shows a partly broken away perspective view of a catheter according to the invention.
Figure 2 shows a partially sectioned view of the distal end of the catheter of Figure 1. Figure 3 shows the catheter section shown in Figure 2 during a first step of the treatment to be performed therewith.
Figure 4 shows a view corresponding with figure 3 of a second step of the treatment.
Figure 5 shows a view corresponding to Figures 3 and 4 of a third step of the treatment.
Figure 6 schematically shows the treated blood vessel with the stent disposed therein, after the catheter has been removed.
The catheter 1 shown in figure 1 comprises a tubular basic body 4 which in this case is composed of an outer tubular element 5 and an inner tubular element 6 received in a lumen thereof. The inner tubular element 6 has two lumen 16, 17. In the shown embodiment, distal end 3 of the catheter 1 has two balloon members 9, 10. At the proximal end 2, two connecting members 7, 8 are arranged, which respectively communicate with the balloon member 9 and the balloon member 10. By supplying pressurized medium via the connections 7 or 8, the balloon members 9 or 10 can be expanded. The connection between the connecting member 7 and the balloon member 9 is via the lumen 17 of the inner tube-like element 6 and the connection between the connecting member 8 and the balloon member 10 is via the gap 20 between the inner tube-like element 6 and the outer tube-like element 5. The lument 16 serves to receive a guide wire.
The catheter 1 is provided with thickenings 11, 12 on either side of the balloon member 10. A squeezed stent 13 is arranged around the balloon 10, between these thickenings 11 and 12. The stent 13 is enclosed axially on the catheter 1 by the thickenings 11 and 12 so that it cannot slide off the balloon 10.
As shown in particular in Figure 2 et seq., The thickenings 11 and 12 are formed by respective rings 14 and 15 disposed within the balloon 10. The ring 14 is arranged on the continuous inner tube-like element 6 and the ring 15 is mounted on the end of the outer tube-like element 5. Because the rings 14, 15 are arranged inside the balloon 10, they do not impede the development of the balloon 10 and the balloon forms a smooth "skin" over the rings 14, 15, which is beneficial during catheter insertion.
The distal end 3 of the catheter 1 is inserted into a blood vessel 18 into which the stent 13 is to be deployed. The embodiment of the catheter according to the invention shown here, in addition to the balloon 10 for expanding the stent 13, is provided with the balloon 9 for dilating a narrowed blood vessel 18. By using the two balloons 9, 10 it can dilate and apply of the stent with one and the same catheter 1, without catheters having to be exchanged.
3( .In the treatment, the catheter 1 is first introduced into the blood vessel 18 such that the front balloon 9 is located at the location of the narrowing to be dilated. Subsequently, a medium under pressure is supplied through the lumen 17 of the inner tube-like element 6 via the connecting member 7. This pressurized medium enters the interior of the balloon 9 through the opening 21 in the wall of the inner tubular member, causing it to expand. This widens the blood vessel 18 (fig. 3 (.
Deze toestand is weergegeven in figuur 4.The pressure in the balloon 9 is then allowed to drop again, so that it returns to its original shape. The catheter is introduced further, whereby the balloon 10 is positioned at the location of the dilated vessel section. By now supplying medium under pressure to the balloon 10, via connection 8 and the interspace between the hose-like elements 5, 6, which interspace at the end of the outer hose-like element 5 communicates as an annular opening 20 with the interior of the balloon 10, this balloon 10 is expanded and thereby the stent 13 expanded. This condition is shown in Figure 4.
Then, the pressure in the balloon 10 is allowed to drop again, causing the balloon 10 to return to its original small-diameter shape, as shown in Figure 5. The catheter can now be withdrawn, with, as shown in Figure 6, the stent 13 in the blood vessel 18 remains to support its wall.
The catheter and associated stent shown in the figures are only exemplary embodiments. Other per se known embodiments of stents can be used with the catheter according to the invention. Due to the good axial fixation of the stent, the latter remains reliably positioned at the location of the expansion balloon, so that the use of a guide catheter or sheath can be dispensed with.
The catheter according to the invention may comprise any number of balloon-shaped members, at least one of which contains bulges at both ends for enclosing a stent between them.
The rings 14 and 15 in the exemplary embodiment shown are material which is visible under X-rays, so that the position of these rings 14, 15 and consequently the position of the stent located between these thickenings can be made visible in a catheter laboratory.
