듀테트라베나진은 테트라베나진에 비해 이의 활성 대사산물의 반감기가 증가한다(예컨대, 미국 특허 번호 8,524,733).Dutetrabenazine ((RR,SS)-1,3,4,6,7,11b-hexahydro-9,10-di(methoxy-D3)-3-(2-methylpropyl)-2H-benzo [a]quinolizin-2-one) is a vesicular monoamine transporter type 2 (VMAT2). The biologically active metabolite formed from deutetrabenazine (alpha-dihydrodutetrabenazine [α-deuHTBZ] and beta-dihydrodutetrabenazine [β-deuHTBZ]), identified together as "deuHTBZ", is VMAT2. It is a strong inhibitor of binding. Dutetrabenazine has an increased half-life of its active metabolite compared to tetrabenazine (eg, U.S. Patent No. 8,524,733).
Dutetrabenazine (deu-TBZ) has been approved by the US Food and Drug Administration under the trade name AUSTEDO® for the treatment of chorea (involuntary muscle movements) associated with Huntington's disease (HD) and for the treatment of tardive dyskinesia (TD) in adults. . The AUSTEDO® formulation is administered orally twice daily (bid) for a total daily dose of at least 12 mg of dutetrabenazine.
Several factors affect gastrointestinal absorption of an orally administered drug, including the drug's solubility at various pHs, and the rate at which the drug is released from the dosage form. The rate of drug release of an oral dosage form is usually measured as the rate of dissolution in vitro, i.e., as the amount of drug released from the dosage form per unit time in, for example, an FDA approved system. Such systems include, for example, the United States Pharmacopoeia (USP) dissolution devices I, II and III.
The therapeutic window of a drug is the period of time during which plasma drug concentrations are within the therapeutically effective plasma drug concentration range. However, because plasma drug concentrations decrease over time, multiple doses of the drug formulation must be administered at appropriate intervals so that plasma drug concentrations remain within the therapeutic window or rise back into the therapeutic window. At the same time, however, there is a need to avoid or minimize plasma drug concentrations that result in undesirable side effects.
Several formulations containing dutetrabenazine are disclosed in U.S. Patent No. 9,296,739. Formulations capable of delivering dutetrabenazine in a controlled manner over an extended period of time allow for once daily ("qd") dosing while maintaining the therapeutic effects currently realized by more advantageous dosing regimens such as AUSTEDO®. will be able to make it possible. A need exists for such alternative formulations.

They are used interchangeably herein to refer to a compound or composition of compounds or substances that induces a physiological and/or physiological effect. The active agent disclosed herein is preferably dutetrabenazine. "Dutetrabenazine" is an optionally deuterium substituted, stable, non-selectively deuterium substituted, stable, non-deuterium compound of tetrabenazine in which six hydrogen atoms on two O-linked methyl groups are replaced with deuterium atoms (ie, -OCD 3 that is not an -OCH 3 moiety). -It is in the form of a radioactive isotope.
As used herein, “formulation” refers to an oral drug form having gastric retention properties as described herein.
As used herein, the viscosity of a polymer means the viscosity measured at 2 weight percent polymer in an aqueous solution at about 20-25°C.
The controlled release solid oral dosage form of the present invention remains in the gastric region (i.e., stomach) for several hours, such as up to 24 hours, thereby significantly prolonging the gastric residence time of the active agent. . Preferably, the controlled release solid oral dosage form of the present invention remains in the stomach for 6-20 hours. Prolonged gastric retention can improve bioavailability, reduce drug waste, and improve availability for drugs that are less soluble in a high pH environment, i.e., the intestinal environment. Without wishing to be bound by any mechanism or theory, prolonged gastric retention may be based at least in part on gas generation by one or more excipients in the formulation and/or mucoadhesive properties of the formulation. To achieve mucoadhesive properties, a number of bioadhesive polymers can be used. These polymers swell in water and adhere to mucosal surfaces. Swelling of certain polymers optionally in combination with additional excipients (i.e., disintegrants) in the formulation provide floating properties upon contact with the gastric environment. The controlled release solid oral dosage forms described herein preferably swell to a size that is at least about twice their unswollen volume upon contact with gastric fluid.
[Timmermans, J. and Moes, A.J., (1990) How well do floating dosage forms float? Int. J Pharm. 62: 207 - 216]. As used herein, “floating” is used in connection with “floating gastroretentive formulations” which exhibit a bulk density lower than that of gastric fluid [Timmermans, J. and Moes, A.J., (1990) How well do floating dosage forms float? Int. J Pharm. 62: 207-216]. Such formulations "float" in that they remain buoyant in the gastric fluid of the stomach for a target period of time. The floating formulation can then be retained in the stomach while releasing the active agent.
As used herein with respect to a polymer in a formulation, the terms "swellable" and "swell" refer to a polymer capable of absorbing fluid and expanding in size, i.e., swelling, upon contact with a fluid environment, such as the stomach environment.
As used herein, the term "method of treatment" or "therapy" (and its different forms) includes preventive (eg, prophylactic), curative or palliative treatment. As used herein, the term “treating” includes alleviating or reducing at least one adverse or adverse effect or symptom of a condition, disease or disorder. As used herein, the term "particle size distribution" refers to the statistical distribution of volume occupancy associated with all particle sizes, expressed as D90, D50 or D10 values. The D90 value of the integrated volume distribution is defined in the context of the present invention as the particle diameter at which 90% by volume of the particles has a diameter smaller than the diameter corresponding to the D90 value. For example, a D90 of 15 μm means that 90% of the particles (by volume) have a diameter of 15 μm or less. A D50 of 10 μm means that 50% of the particles (by volume) have a diameter of 10 μm or less. A D10 of 3 μm means that 10% of the particles (by volume) have a diameter of 3 μm or less. Particle size distribution is determined by laser diffraction measurement. More specifically, the particle size distribution is determined using a Mastersizer 3000 from Malvern Instruments.

As used herein, “controlled release” and “controlled release” are used interchangeably and refer to the properties of a polymer used in a formulation. Such polymers, when included in a dosage form with an API, can slow the release of the API compared to an immediate release dosage form under physiological conditions and contribute to the release of the active agent over an extended period of time.
Although constant release formulations have proven effective for many different drug therapies, there are clinical situations where such formulations are not entirely satisfactory. It has been observed that in some patients the therapeutic effect of the drug decreases below the therapeutic effect threshold prior to the end of the desired period of therapy, despite maintaining a substantially constant drug release expected to provide a sustained effect.

Dutetrabenazine is a weakly basic compound that is relatively soluble in acidic environments (pH<4) and poorly soluble at pH > 4 (<2.3 mg/mL). Deutetrabenazine is permeable across the small intestine (SI) segment in a rat perfusion model. In humans, in a human [ 14 C]-AME study in which dutetrabenazine was administered as a powder in capsule (PIC), approximately 80% of the radioactive dose was recovered in urine, indicating that the compound is well absorbed in the small intestine and large intestine/colon. suggests The level of absorption of dutetrabenazine in the lower GI of rats was jejunum < middle small intestine < colon < ileum, and the ileal absorption was about 0.0006 cm/sec.

The trial was conducted as an open label, randomized, single dose, partial crossover bioavailability comparison study. The study was designed to determine AUC0-t, AUC0-24, Cmax and Tmax of deutetrabenazine and total deuterated α- and β-dihydrotetrabenazine (deuHTBZ) metabolites. The sample group consisted of 4 male beagle dogs. Each dog received two treatments. The first treatment, Treatment A, consisted of a single 12 mg AUSTEDO® tablet administered in two (bid) doses 8 hours apart; The second treatment, Treatment B, consisted of a single administration of the gastroretentive formulation of
Blood samples were collected before dosing and at scheduled time points for 24 hours after dosing. Samples were analyzed for dutetrabenazine and total deuHTBZ in plasma using LC-MS/MS methods. The PK profiles following oral administration of the two treatments are presented in Table 11.

The gastroretentive formulation tested (Treatment B) was well tolerated, without any adverse clinical signs or vomiting following a single oral dose to dogs. The tested gastroretentive formulations produced peak plasma concentrations (Cmax) and total systemic exposure (AUC) of dutetrabenazine and total deuHTBZ that were lower than AUSTEDO®. These preliminary studies provided favorable indications for the design and in vivo behavior of the formulation. The observed values on the lower side for C max and AUC can be attributed to differences between dogs and humans (dutetrabenazine/deuHTBZ C max ratio is about 10% in dogs and < 1% in humans).

The primary objective was addressed using the following parameters:
(Cmax)- Maximum Observed Concentration (Cmax)
(AUC) (AUC0-t)- Area under the plasma concentration-time from time 0 to the time of the last measurable plasma concentration (AUC) (AUC0-t)

Calculate AUC0-t, AUC0-∞, and AUC0-24h using the trapezoidal rule. Cmax, AUC0-t, AUC0-∞, AUC0-24h data are natural log transformed prior to statistical analysis. Comparison of AUC0-t, AUC0-∞ and AUC0-24h between treatments (test versus reference) was performed using a separate parametric analysis of variance (ANOVA) with a fixed effect term for sequence, duration, treatment group and a random effect of subject within sequence. ) will be performed using the model. Differences between reference and test articles will be assessed by constructing 90% confidence intervals for the test/reference ratio based on least square means from ANOVA for log-transformed Cmax, AUC0-∞ and AUC0-24h. Treatment differences and associated 90% confidence intervals estimated from ANOVA on a logarithmic scale will be inversely transformed to obtain an estimated ratio of the geometric mean between treatment groups and thus a 90% confidence interval for the ratio.
