Description
に際し、切除端部を重ね合わせて多数の鎹状をした門型
のステープルを打ち込み、かつ反対側から突出する各ス
テープルの両足片を相互に対向方向に向けて折り曲げる
ことにより切除部位の止血ならびに組織の縫合をおこな
うための縫合器ならびに該縫合器に使用される縫合用ス
テープルの改良に関する。
することはいうまでもなく、このような観点からこれま
でに、たとえば第9図に示したような縫合器が開発され
ている。
上部アーム1と下部アーム7とからなり、上部アーム1
には下部アーム7との対接面の中心に、その長さ方向に
連続させた溝部2を有するとともに、該溝部2の両側に
は下部アーム7との対向面方向に向けて貫通させたステ
ープル穴3が各々所定の間隔1aを介して縦列状に設けら
れており、ステープル穴3より押出片5を後退させた状
態にて該ステープル穴3内に門型をしたワイヤー等のス
テープル11がその両足片を下部アーム7との対接面方向
に向けたまま支承させた後、縫合部位を上部アーム1と
下部アーム7をもって第10図の矢印(A−A)方向に接
近させ、下部アーム7の先端突起部10を上部アーム1の
ガイド穴4内に挿通案内させつつ挟んだ状態において、
たとえばペンチ状の加圧手段により第10図の矢印(B−
B)方向に加圧し、押出金具6の各押出片5を上部アー
ム1のステープル穴3内に侵入させて、各ステープル穴
3内に装備した鎹状のステープル11の足片を縫合部位の
組織内に同時的に刺し込む。
加圧力をさらに強めることによりステープル11の両足先
端部のみが縫合部位組織を貫通した後、下部アーム7の
各対応するステープル足片受け部9において相互に対向
方向に折り曲げられて縫合部位を縫合するものである。
1の多数のステープル穴3内にステープル11を一本宛そ
れぞれ各足片を下部アーム7との対接面方向に向けて装
入保持させるために著しく手間と時間を要し、一個所の
縫合終了後、別の個所の縫合作業迄には可成りの時間、
施術が中断されることになる。そのために別の縫合器を
複数用意して使用する必要があり、施術作業も煩雑とな
ることが多かった。
11の充てん作業をおこなう場合には多数のステープル穴
3のうちいずれかに対する充てん忘れや充てんミスを生
じ易く、そのために縫合作業後の止血効果が不十分とな
る危険もある。
テープルの足片同志が衝突して予期しない不規則な方向
に折り曲がる結果、縫合の不完全や止血効果の不完全を
おこすおそれがある。
であって、具体的には同一軸を支点として相互に開閉自
在とした一対の上部アームと下部アームとからなり上部
アームには多数の門型ステープルを収容可能としたカー
トリッジを着脱自在に嵌装するとともに、下部アームに
は上記上部アームとの対接面に、上記上部アームのカー
トリッジに保持された各門型ステープル…に対応する位
置に、ステープルの両足片の間隔に略等しいか、あるい
はいくぶん長めの長さを有する凹部であって、該凹部は
その長さ方向の中間部が最深部となるようにやや湾曲し
て形成されたところの、ステープル足片を受け入れる折
り曲げ凹部を形成してなる外科手術用縫合器に関する。
また本考案は上部アームに嵌装されるカートリッジの各
ステープル穴と、これに対応する下部アームの折り曲げ
凹部とは、その配列位置が、それぞれの長さ方向軸線が
互いに平行であって、しかも端部が互いに接近するよう
に斜め方向に順設されていることを特徴とした外科手術
用縫合器にも関する。

出金具を介して各ステープル穴内の門型ステープルを一
斉に下部レバーに向けて押し出す。

あらかじめ門型ステープルを充てんした別のカートリッ
ジを装着して再度の縫合作業に備える。
の実施例をもとに説明すると、この縫合器は第1図にお
いて軸39を支点として相互に開閉自在とした一対の上部
アーム31と下部アーム36とからなり、上部アーム31のテ
ール部31aには軸40を支点に上部ハンドル41が、また下
部アーム36のテール部36aには軸42を支点に下部ハンド
ル43がそれぞれ回動自在に設けられ、さらに上部ハンド
ル41と下部ハンドル43とは軸44により相互に矢印C−C
方向に回動自在に構成され、上部ハンドル41と下部ハン
ドル43とを矢印C−C方向に回動させるにつれて軸40と
42との軸間距離が大きくなる結果上部アーム31と下部ア
ーム36とが相互に矢印D−D方向に接近し、かつ突出部
37が凹部32内に案内されて上・下アーム31、36が相互に
密接するように構成されている。

ートリッジ装着口CHが開口され、該カートリッジ装着口
CHにはフランジFを有するカートリッジCAが上部アーム
31の下側から装入される。

れぞれカートリッジCAの上下方向に貫通されている。

斜め方向に順設されている さらに34は上部アーム31の上面側長さ方向に沿わせて設
けられた加圧金具を示し、その下面には上記アーム31の
カートリッジ装着口CHに装着されたカートリッジCAに貫
通して設けられた各ステープル穴33(第2図参照)内に
進退が可能な押出片35が上記各ステープル穴33の位置に
対応させた角度位置にそれぞれ連設されている。
の凹部32に対応する位置に上記凹部32に嵌入が可能な大
きさの突出部37が形成されており、しかも下部アーム36
の上記した上部アーム31に設けた切断用スリットS1およ
び多数のステープル穴33にそれぞれ対応する位置に、同
じく切断用スリットS2、およびステープル足片の折り曲
げ凹部38が形成されている。
略等しいか、いくぶん長目の長さを有するとともに、該
凹部38は第7図に示したようにその長さ方向中間部33a
が最深部となるようにやや湾曲して形成する。
対応する下部アーム36側のステープル穴33との配列につ
いても必ずしも上記したものに限られるものではなく、
このほかに、第6図のカートリッジCAに示したように切
断用スリットS1を境に、片側のステープル穴33(図にお
いて上側)を相互に一定の間隔を介して直線状に配列す
るようにしてもよい。
うに上・下部ハンドル41、43を開いて、上・下部アーム
31、36を相互に離間させた状態とし、上部アーム31のカ
ートリッジ装着口CH内に、あらかじめ各ステープル穴内
にステープル21を充てんしたカートリッジCAを装着し、
さらに上部に加圧金具34を取りつける。
バー41、43を相互に矢印C−C方向に駆動させて上・下
部アーム31、36を矢印D−D方向に相互に接近させ、か
つ突出部37を凹部32内に案内させた後、相互に圧着させ
る(第3図の状態参照)。
の適当な加圧器具を用いて加圧金具34の押出片35を上部
アーム31の内方に侵入させる。
テープルヘッド部21aを上記押出片35によって下方に押
し下げられ、左右の足片21bが組織内の刺し込まれ、貫
通して反対側面より突出した後、下部アーム36の各折り
曲げ凹部38内に入り、湾曲した溝に沿って第7図に示し
たように、中央の最深部をすれ違い通過し、相互に対向
する方向に内方の組織を拘え込むようにして両足片21b
により組織をカシメつけて縫合を完了する(第5図参
照)。
すると足片先端同志が衝突することなく、互いに摺接し
ながらすれ違うことができる。
自在とした一対の上部アームと下部アームとからなり、
上部アームには多数の門型ステープルを収容可能とした
カートリッジを着脱自在に嵌装するようにしたために、
1ヵ所の作業を実施した後、施術中に多数のステープル
を一本宛充てんする必要がなく、あらかじめすべてのス
テープル穴内に完全にステープルを充てんしてある別の
カートリッジを差し替えるだけできわめて短時間のうち
に、しかも完全に装着することができるのでステープル
の充てんミスがなく、縫合作業の継続的実施が迅速にお
こなわれ、縫合による止血効果も十分で完全な縫合作業
をおこなうことができる。
の折り曲げ凹部を、その長さ方向中間部が最深部となる
ようにやや湾曲して形成したために、ステープルの両足
片による組織の抱え込みがより一層十分となり止血効果
を向上させることができる。また上部アームに嵌装され
るカートリッジの各ステープル穴と、これに対応する下
部アームの折り曲げ凹部とは、その配列位置が、それぞ
れの長さ方向軸線が互いに平行であって、しかも端部が
互いに接近するように斜め方向に順設するようにした場
合には、従来のような2列のステープル間に生ずる組織
の壊死や化膿、あるいは穿孔発生等の障害を引き起こす
危険がなく、しかも止血効果が十分に維持される。
足片を相互に喰い違い状となるように互いに逆方向とな
るよう幅方向に斜断した場合には数多いステープルによ
る同時的縫合に際し、ステープルの足片同志が衝突する
ことがなく、縫合および止血の完全を期すことができ
る。

Claims (2)
- 【請求項1】同一軸を支点として相互に開閉自在とした
一対の上部アームと下部アームとからなり、上部アーム
には多数の門型ステープルを収納可能としたカートリッ
ジを着脱自在に嵌装するとともに、下部アームには上記
上部アームとの対接面に、上記上部アームのカートリッ
ジに保持された各門型ステープルに対応する位置に、ス
テープルの両足片の間隔に略等しいか、あるいはいくぶ
ん長めの長さを有する凹部であって、該凹部はその長さ
方向の中間部が最深部となるようにやや湾曲して形成さ
れたところの、ステープル足片を受け入れる折り曲げ凹
部を形成してなる外科手術用縫合器。 - 【請求項2】上部アームに嵌装されるカートリッジの各
ステープル穴と、これに対応する下部アームの折り曲げ
凹部とは、その配列位置が、それぞれの長さ方向軸線が
互いに平行であって、しかも端部が互いに接近するよう
に斜め方向に順設されていることを特徴とした請求項
(1)に記載の外科手術用縫合器。
