Description

【考案の詳細な説明】 技術分野 本考案は、脈管内ステントの改良に関する。
背景技術 種々の状況において狭窄された脈管部分を拡張するため
に、または脈管部分を通る開放通路を維持するために手
段が設けられることが望ましい。
例えば、これらの状況としては、食道癌または食道転移
により発生される食道狭窄によるもの、または胆臓系、
尿管系および気管支系の癌から結果として生じた狭窄に
よる状況を挙げることができる。
バルーン拡張はこれらの場合に食道の通路を拡張しかつ
維持するための良く知られた方法であるが、この方法は
その一時的な作用のために繰り返して作業されなければ
ならず、かつまたそれは重症の狭窄の患者に効果がな
い。
他の方法として、種々の人工食道が食道狭窄の場合に使
用されたが、それらは収縮および弛緩しないので、癌に
より引き起こされた狭窄区域にそれらを挿入する過程に
おいて食道破裂の割合が高く(30ないし40%)、そし
て、食道の破裂が原因となって縦隔洞炎による死亡率が
高くなる。加えて、患者は人工食道の内径(10ないし12
mm)が狭いため、嚥下時に大きな困難をきたし、また、
食物を摂取する際にしばしば人工食道の閉塞を引き起こ
す。
この困難を克服する手段として、ステントを使用して通
路を拡大して保持するための装置がアメリカ合衆国特許
第4,214,587号により呈示されている。しかしながら、
その発明の装置は通路を拡大するのに一時的な作用しか
有さず、脈管内の管腔が長時間経過後に、再び狭くなる
という問題を依然として有していた。
この欠点を改善するために、アメリカ合衆国特許第4,58
0,568号が提供されている。前記発明のステントは、屈
曲部により接合れた真っすぐな部分の無端列を含んでい
る閉止ジグザグ形状に形成されたワイヤを含んでいる。
ステントは真っすぐな部分が相並んで、かつ挿入のため
互いに近接して配置されるより小さな第1の形状に弾性
的に圧縮可能であり、また、より大きな第2の形状とな
って、前記真っすぐな部分がそれを開放して維持するた
めに通路の壁に対して押圧することもできる。
しかしながら、第4,580,568号の発明のステントが長時
間にわたって使用される場合において、増殖された細胞
がステントを厚く覆うため、管腔が狭くなってしまうと
いう問題を有する。癌細胞が前記真っすぐな部分を通過
することができるため、癌による狭窄の場合に効果がな
い。
考案の開示 本考案の主たる目的は、狭窄された脈管部分を拡張しか
つ管腔内で移動することなしに長時間にわたって脈管部
分を通る開放通路を維持する自己膨張ステントを提供す
ることにある。
本考案の他の目的は、癌細胞がステントに入り込むのを
阻止する自己膨張脈管内ステントを提供することにあ
る。
前記目的を達成するために、本考案は複数のユニット構
造により形成される筒状フレーム;真っすぐな部分から
なる無端列を含みかつ屈曲部により接合される閉止され
たジグザグ形状に形成されかつ向かい合わせに多段形状
に配置される前記ユニット構造;前記ユニット構造を接
続する接続部材;および前記フレームの外側のまわりに
巻き付けられるメッシュからなるステントを提供する。
そして本考案による他のステントは、複数のユニット構
造により形成される筒状フレーム;真っすぐな部分から
なる無端列を含みかつ屈曲部により接合される閉止され
たジグザグ形状に形成されかつ向かい合わせに多段形状
に配置される前記ユニット構造;前記ユニット構造を接
続する接続部材;および前記ユニット構造と同一構造を
有し、そして前記フレームの上方および下方部分の端部
に配置される反移動部材;および前記フレームの外側の
まわりに巻き付けられるメッシュからなる。
図面の簡単な説明 第1図は、本考案によるフレームのユニット構造を示す
展開図である。
第2図は、本考案によるフレームを示す第1実施例の斜
視図である。
第3図は、本考案の第1実施例の前記フレームを示す展
開図である。
第4図は、本考案による第1実施例を示す斜視図であ
る。
第5図は、本考案による第1実施例のステントが管腔内
に配置されかつ拡張される状態を示す説明図である。
第6図は、フレームを示す本考案の第2実施例を示す斜
視図である。
第7図は、本考案の第2実施例のステントが管腔内に配
置されかつ拡張される状態を示す説明図である。
考案を実施するための好適な形態 本考案の原理の理解を促進するあめ、以下の図面に示し
た実施例について説明する。その際、特定の用語が同一
物を説明するのに使用される。しかしその説明は、本考
案の範囲の制限を意図するものではなく、また図示され
た装置における変更およびさらに他の変形、およびここ
に図示されたような本考案の原理の応用は本考案が関連
する技術に熟練した者にとっては普通に想起しうるもの
であると予期されるということを理解すべきである。
第1図は本考案によるフレームのユニット構造を示す展
開図であり、第2図は本考案によるフレームを図示する
第1実施例の斜視図であり、そして第3図は第2図の前
記フレームの展開図である。
第1実施例のフレーム10は、第1図に示されるようなか
なりの数のユニット構造11からなる。ユニット構造11
は、ワイヤから構成され、そして該ワイヤは好ましくは
ステンレス鋼から作られる。ユニット構造11は閉止ジグ
ザグ形状に形成され、それにより図1に示されるように
一連の真っすぐな部分111および屈曲部112を形成する。
ワイヤの両端は溶接により互いに接合され、それにより
屈曲部で接合された真っすぐな部分の無端列を形成す
る。
フレーム10は、互いに同様な複数の前記ユニット構造11
およびこれらユニット構造11の各々に対応する屈曲部
が、多段の形状に向かい合って配置されるような方法に
おいて構成される。このとき、それぞれのユニット構造
は多数の数の接続部材31,33,35,37・・・により一列に
並んで接続される。これらの接続部材は、各ユニット構
造が分離することを阻止するように指定されたユニット
構造の屈曲部間に適宜に接合される。接続部材の最も好
ましい接合位置は、接続部材31に関して、第図3に示さ
れるのと同様な基準点として対角線状に置かれることが
望ましい。すなわち第図3に見られることができるよう
に、構造31,33,35,37および39を有することが好まし
い。また、接続部材は他の屈曲部ごとに接続されること
なく、一ユニット構造11における接続部材の適宜な数、
すなわち図3の第1実施例の10個の接続部材が互いに接
続されるフレーム10は複数の菱形状の空間100を有す
る。ユニット構造または接続部材の数は特別な意味を持
たず、そして患者の状態および/または製造の便宜に応
じて変化させることができる。
空間区域は真っすぐな部分111が収縮されるとき真っす
ぐな部分111を収容し、屈曲部112による弾性変化を作
る。
完成されたフレーム10は外側に突出する突起を備える管
腔内でのステントの移動を阻止するための反移動部材4
1,43を有する。反移動部材41,43は適宜に配置されかつ
接続部材31,33,35,37および39が接続されない屈曲部と
接続される。それゆえ、本考案のステント1は管腔内で
移動しない。
ユニット構造、接続部材、および反移動部材はすべてス
テンレス鋼のような同一材料から作られ、好ましくは金
メッキされる。
フレーム10はメッシュ91内に巻かれ、そしてメッシュの
上方および下方縁部は内側に向かって折り畳まれ、そし
て両縁部はそれぞれ接着性となりそれによりユニット構
造11のワイヤは露出されない。メッシュはナイロンから
作られかつナイロンメッシュはシリコンゴムで被覆され
るのが好ましい。
アメリカ合衆国特許第4,214,587号の方法または本考案
のステントが圧縮されているカテーテルを使用する方法
は位置、すなわち、管腔内の病んだ部分に本考案のステ
ントを配置するために使用されることができる。
ステントは反移動部材の突起411,431により管腔の内壁
に固定され、そしてステントは長時間使用しても移動せ
ず、そしてそれに加えて、患者が感じるかも知れない痛
みは、従来のステントに比して、該ステントの外壁がメ
ッシュ91により巻かれるため、緩和されることができ
る。さらに、メッシュ91を被覆するシリコンゴムは癌細
胞等がステントの内部に入り込むのを阻止することがで
きる。
胃癌または食道癌の患者についての本考案の第1実施例
のを実験の結果として、以下の効果が得られる。
A.胃癌および胃癌の食道拡散を有する患者が1989年7月
にチエオン・バク医大の病院に来た。患者の症状は6回
の抗癌剤後改善された。しかし患者は嚥下困難の発生の
ために再び来訪した。
1990年1月8日に、胃癌の拡散による食道の末端部分の
閉塞された区域が直径20mmおよび長さ8cmの食道バルー
ンにより拡大された後、外径20mmかつ長さ10cmのステン
トが外径10mmの鎧装を通して食道の末端部分に挿管され
た。
手術の結果として、患者は流動食および固形食を取るこ
とができ、そして手術後数日に行われた食道撮影図にお
いて、バリウムがどのような潜流または閉塞なく食道を
通って良好に通過し、そしてステントの移動または食道
の破裂のごとき特殊な合併症は発生しなかった。
その後、患者は病院を出、そして1990年4月に胃癌の全
身に広がった転移で死んだ。しかしながら、患者の死の
直前までステントの機能による問題は生じなかった。
B.胃癌および食道転移を有する患者が1990年3月13日に
嚥下困難のためチエオン・バク医大の病院に来た。
直径20mmおよび長さ8cmの食道バルーンにより拡大され
た後、外径20mmおよび長さ10cmのステントが外径10mmの
鎧装を通して食道の末端部分に挿管された。
手術の結果として、患者は水すら飲むことができなかっ
たがステントが彼の管腔に挿管された後、患者は流動食
および固形食を取ることができ、そしてバリウムは手術
後数日に行われた食道撮影図においてどのような潜流ま
たは閉塞もなく良好に通った。内視鏡が1990年3月27日
に行われたとき、外径10mmの内視鏡が困難なく通され
た。
そして1990年8がつ10日に行われた食道撮影図の検査に
続いて、ステントの移動または閉塞のごとき特殊な合併
症は発生せず、そして加えて、患者は流動食および固形
食をたべることができた。
C.食道癌を有する患者がチエオン・バク医大の病院に入
院しそして放射線療法を取りかつ抗癌剤が5回投与さ
れ、そしてそこで症状は改善されたが、患者は嚥下困難
により再び病院を訪ねた。
食道の閉塞された区域が直径20mmおよび長さ8cmの食道
バルーンにより膨張させられた後、外径20mmかつ長さ12
cmのステントが外径10mmの鎧装を通して食道の閉塞され
た区域に挿管された。
処置の結果として、患者は流動食および固形食を取るこ
とができ、そしてバリウムは食道撮影図においてどのよ
うな潜流または閉塞なく食道を通って良好に通過し、そ
してステントの移動または食道の破裂のごとき特殊な合
併症は発生しなかった。
本考案の医療器具は上記の実験から証明されるように、
指定された区域に挿管されかつ保持されるステントの膨
張可能な力が優れているので、通路の狭窄を拡大保持す
る顕著な効果を得ることができる。
上述したように第1実施例の構造はステントの移動を阻
止することが期待されるが、反移動部材41,43の末端部
分が外方に突出する突起を有するので、突起が患者に痛
みおよび管腔貫通を感じさせる可能性が除去されること
ができず、そしてステントが位置を直すように人工的に
移動されるとき、管腔壁が傷つくかも知れない。
図6は本考案の第2実施例の斜視図であり、そして複数
のユニット構造21,22,23,24が膨張しているリンクであ
るとを示す。ユニット構造21,22,23,24は第1実施例の
ユニット構造と同一である。
この考案の第2実施例によれば、ステント2は複数の接
続されたユニット構造21,22,23,24、およびフレーム20
の下方および上方部分に置かれた反移動部材42および44
により形成される。反移動部材42および44なしのフレー
ム20は、ユニット構造の数および接続部材36,38の長さ
を除いて本考案の第1実施例のフレームとほぼ同一であ
る。ユニット構造の数は本考案において意味を持たな
い。そしてフレーム20の上方および下方部分の端部にお
ける接続部材36,38の長さは本考案の第1実施例の接続
部材31,39の長さの半分である。
ユニット構造21,24の両側部分は第2接続部材32,34を介
して反移動部材42および44と接続される。
フレーム20の上方および下方部分の端部に置かれる反移
動部材42および44はユニット構造21,22,23,24と同一構
造を有する。しかし反移動部材42および44は直径がフレ
ーム20より大きいように形成される。互いに直径が異な
るフレーム20、および反移動部材42および44を接続する
ために、第2の接続部材32,34が利用可能である。第2
接続部材34は接続部材38の屈曲部381または端部381を反
移動部材44の屈曲部442と接続する。この接続は溶接に
より行われる。
反移動部材42および44の直径は第2の接続部材32,34の
ほぼ水平な部分321,341の長さに依存するので、その長
さを決定することが重要である。膨張可能な部分322,34
2はほぼ水平な部分321,341から膨張されかつ垂直に屈曲
される。
図6においてメッシュがフレーム20の外側のまわりに巻
かれることが省略される。
上述したように形成されるステント2は、力がそれに付
与されないならば膨張されたままである。ステント2を
食道内に配置するために、導入管、食道バルーンおよび
押圧カテーテルのごとき一定の導入構体が圧縮された状
態において狭窄に押し入れられる。
導入管は押圧カテーテルにわたって除去されるので、圧
縮されたステントが自動的に通されかつ膨張される。
図7は管腔内に置かれたステント2が管腔にくっつくよ
うに半径方向に膨張される状態の説明図である。
食道癌を有する患者に使用されることができるステント
が上述され、そしてステントが直径が8mmないし10mmの
間の筒状であるように形成されるならば、このステント
は、他の用途、例えば胆臓、気管支および秘尿系に使用
することができる。

  1. 【請求項1】複数のユニット構造(11,12,13,14,15,1
    6)により形成される筒状フレーム(10); 真っすぐな部分(111)からなる無端列を含みかつ屈曲
    部(112)により接合される閉止されたジグザグ形状に
    形成され、かつ向かい合わせに多段形状に配置される前
    記ユニット構造(11,12,13,14,15,16); 前記ユニット構造(11,12,13,14,15,16)を接続する接
    続部材(31,33,35,37,39);および 前記フレーム(10)の外側のまわりに巻き付けられるメ
    ッシュ(91)からなることを特徴とする自己膨張脈管内
    ステント。
  2. 【請求項2】突起を有する反移動部材(41,43)が前記
    フレーム(10)の中央区域(100)に取り付けられるこ
    とを特徴とする請求の範囲第1項記載の自己膨張脈管内
    ステント。
  3. 【請求項3】前記フレーム(10)、接続部材(31,33,3
    5,37,39)、および反移動部材(41,43)が金メツキされ
    ることを特徴とする請求の範囲第1または2項記載の自
    己膨張脈管内ステント。
  4. 【請求項4】前記メッシュ(91)がシリコンゴムで被覆
    されることを特徴とする請求の範囲第1または2項記載
    の自己膨張脈管内ステント。
  5. 【請求項5】複数のユニット構造(21,22,23,24)によ
    り形成される筒状フレーム(20); 真っすぐな部分(211)からなる無端列を含みかつ屈曲
    部(212)により接合される閉止されたジグザグ形状に
    形成され、かつ向かい合わせに多段形状に配置される前
    記ユニット構造(21,22,23,24); 前記ユニット構造(21,22,23,24)を接続する接続部
    材;および 前記ユニット構造と同一構造を有し、そして前記フレー
    ムの上方および下方部分の端部に配置される反移動部材
    (42および44);および 前記フレーム(20)の外側のまわりに巻き付けられるメ
    ッシュからなることを特徴とする自己膨張脈管内ステン

    ーム(20)の直径より大きい直径を有し、そして第2の
    接続部材(32,34)により接続されることを特徴とする

  7. 【請求項7】前記第2接続部材(32,34)がほぼ水平な
    部分(321,341)および膨張性部分(322,342)からなる
    ことを特徴とする請求の範囲第6項記載の自己膨張脈管

  8. 【請求項8】メッシュが前記フレーム(20)および反移
    動部材(42および44)のまわりに全体的に巻き付けられ
    ることを特徴とする請求の範囲第5,6または7項記載の
