Description
PCIでは、非常に重要な点は、介入装置−たとえばバルーン又はステント−の正しいサイズ設定にある。特に、ステントの正しい長さを決定することは非常に重要である。短すぎるステントは、全病巣を網羅せず、端部での斑を傷つける恐れがある(端部効果、再狭窄を起こしやすくなる)。他方長すぎるステントは、望ましくない構造(他のステント、側枝根等…)と重なる恐れがある。PCI中での理想的な長さについての文献、研究、及び指針が多数存在し、これは、使用されるステントの種類(金属型又は薬剤溶出型)によって変化しうる。しかし正しいステント長さが特にカギとなる問題であることは通底する。
− たとえば冠動脈血管内超音波(IVUS)のような血管内装置の利用(非特許文献2)
− 定量的測定アルゴリズムが適用可能な病巣の3D表現を生成する再構成又はモデル化ソフトウエアと結合させた複数の血管造影像の利用(非特許文献3)
である。
a) 当該ルーラーは特に、わずかなコントラスト剤で見える前記チップと狭窄部の画像を取得する蛍光シーケンスから計算された拡大像内で重ねられて良い。
b) 当該ルーラーは、前記チップが前記狭窄部へ入る前だがコントラスト剤が現れた蛍光シーケンスの場所に重ねられて良い。
c) 当該ルーラーは、対応する血管造影の画像内で重ねられて良い。
Claims (15)
- 装置のサイズ決定の支援を供する方法であって:
管のX線画像を取得する手順;
ガイドワイヤの放射線を透過しないワイヤチップのX線画像を取得する手順;
前記管を通り抜けるときの前記ワイヤチップを示すX線画像中において前記ワイヤチップをセグメント化する手順;及び
前記ワイヤチップのサイズに基づいて前記管のサイズに関するサイズ設定情報を供する手順;
を有する方法。 - 前記ワイヤチップの画像が、前記ワイヤチップの拡大像である、請求項1に記載の方法。
- 前記ワイヤに対して平行に延在する疑似的ルーラーが、前記画像に重ね合わせられる、請求項1又は2に記載の方法。
- 前記ルーラーが、前記ワイヤチップの長さの分数倍で得られる、請求項3に記載の方法。
- 前記ルーラーが、曲げられたルーラーである、請求項4に記載の方法。
- 前記ワイヤチップがセグメント化及びモデル化される、請求項1乃至5のうちのいずれか1項に記載の方法。
- 前記ルーラーが、わずかなコントラスト剤によって見える前記ワイヤチップと管の両方を取得する蛍光シーケンスから計算される像に重ね合わせられる、請求項3乃至6のうちのいずれか1項に記載の方法。
- 前記ルーラーが、コントラスト剤は存在するが、前記ワイヤチップが前記管に入り込む前に、蛍光シーケンスの一部で重ね合わせられる、請求項3乃至6のうちのいずれか1項に記載の方法。
- 前記ルーラーが、対応する血管造影画像内で重ね合わせられる、請求項1乃至6のうちのいずれか1項に記載の方法。
- 前記ワイヤチップの画像から取得された前記ワイヤチップサイズが、前記ワイヤチップの実際のサイズと比較される、請求項1乃至9のうちのいずれか1項に記載の方法。
- 前記管の収縮が推定される、請求項10に記載の方法。
- 前記ワイヤチップの角度が推定される、請求項10に記載の方法。
- 管のX線画像を取得する手段;
放射線を透過しないワイヤチップを有するガイドワイヤが前記管へ導入されるときに前記放射線を透過しないワイヤチップを有するガイドワイヤの画像を取得する手段;
前記ワイヤチップのX線画像を取得する手段;
前記ワイヤチップが前記管を通り抜けるときに、前記ワイヤチップをセグメント化する手段;及び
前記ワイヤチップのサイズに基づいて前記管のサイズに関するサイズ設定情報を供する手段;
を有するX線画像化システム。 - 擬似的ルーラーを計算する計算手段を有する、請求項13に記載の画像化システム。
- 前記ルーラーを重ね合わせ像中に表示する表示手段を有する、請求項14に記載の画像化システム。
