開創手段は、前記の通り、体腔側、体表側両リング1、2及び弾性シート3の張力により、自然状態で閉じようとする切開創を開いた状態に保持すると共に、柔軟な弾性シート3の気密的な接触により創縁を覆って保護するもので、体腔側リング1は、押し潰した状態で前記小切開創から体腔内に挿入可能な可撓性、及び、体腔内で元の状態(リング状態)に自然に復元可能な形状復元性を備えて形成される。本実施の形態では、芯材として、前記弾性及び形状復元性に優れたチタン−ニッケル合金などの線材よりなる超弾性合金11をリング状に形成し、該超弾性合金11に樹脂チューブ(本例においてはポリウレタンチューブ)よりなる保護チューブ12を被覆し、更に、保護チューブ12に被覆して、後記するシリコーン樹脂よりなる弾性シート3の下側端部31を一巻きし、接着あるいは溶着により接続して構成した。
サイズは手術のさいの切開の大きさにより選択され、切開創より無理なく挿入可能で、かつ、十分な開創状態の得られるサイズのものが選ばれる。
尚、本実施の形態においては、切開創20〜40mmに適用するものとして、直径70mm、リング幅4mmの円形断面の円形リングを用いた。

ポート設定部位41は、トロカールを穿刺してポートを設定することが可能な範囲であって、該トロカールを穿刺したさい、穿刺ルートが前記開創手段の弾性シート3の内腔(開創されている範囲)を外れないように、ポート設定が可能なラインとして、該弾性シート3の直径と同等な直径(本例においては、直径40mm)に設定され、弾性キャップ4中央のスペースに設けられる。そして、該ポート設定部位4より5mm程度内側(本例においては、直径30mmの位置)に、ポート設定が推奨されるラインとしてトロカール刺入マーカー42、及び、該ライン上に僅かに突起する円形のポイントマーカー43を90度間隔で4箇所に設け形成する。
このように、ポート設定部位41、トロカール刺入マーカー42を明確に設定することにより、前述したような、トロカールの穿刺による、開創手段などへの不慮の接触が防止でき、意図した操作ができないなど手技への不都合も生じにくい。

この構成により、前述した通り、使用されないポートは確実に閉塞されており、使用するさいにはトロカールの穿刺により容易に開口可能な構成となっている。また、トロカール穿設後に、トロカールを抜去する必要が生じた場合にも、弾性キャップ4全体が弾性部材により形成されること、また、穿刺された部位にある程度の厚み(本例においては、2.5mm)を備えていることにより、抜去後の開口部は閉塞する方向に働き、必要な気密を維持することができる。
