（技術分野）
本開示は、外科用機器に関し、より具体的には、身体の組織を挟み、その組織に創傷治療物質を適用することが可能な外科用装置に関する。
（関連技術の説明）
組織の切断を必要とする外科手順は、切断部位で出血を生じ得る。例えば、縫合、血管へのクリップの適用、および、外科用締め具の使用、ならびに、電気焼灼および他の組織治癒技術のような、首尾よく出血を制御するための種々の技術が開発されている。
外科用締め具を使用する外科用デバイスは、対向する顎構造体の間に組織を把持もしくは挟み、次いで、外科用締め具を用いて、組織を接合することを必要とする。これらのデバイスは、当該分野で周知である。いくつかの機器においては、締め具によって接合された組織を切断するためのナイフが提供される。これらの締め具は、代表的には、外科用ステープルの形状であるが、二つの部品からなるポリマー性締め具もまた利用される。
この目的のための機器は、各々が、組織を捕捉もしくは挟むために使用される、二つの細長い部材を備え得る。代表的には、これらの部材の一方は、少なくとも二本の横の列（ｌａｔｅｒａｌ ｒｏｗ）に整列された複数の締め具を収容するカートリッジを有する。他方、もう一方の部材は、締め具がカートリッジから駆出されたときに、締め具の脚部を形成するための表面を規定するアンビルを備える。二つの部品からなる締め具が使用される場合、アンビルは、カートリッジから駆出される、締め具の嵌合部分（例えば、受器）を有する。一般に、ステープルを行なう操作は、カートリッジ保有部材を通して長手方向軸に移動するプッシャーによって達成され、このプッシャーは、ステープルが、カートリッジから連続して排出するように作用する。ナイフは、ステープルの列の間で、プッシャーと共に移動して、ステープルの列の間にあるステープルされた組織を長手軸方向に切断し得る（すなわち、ナイフ切断線を形成する）。このような機器は、特許文献１および特許文献２（これらの文献の各々の全内容は、本明細書中に参考として援用される）に開示されている。
特許文献３に開示される、より後のステープラーは、切開またはナイフ切断線の各側に二列のステープルを提供する。これは、カートリッジアセンブリを提供することによって達成され、このカートリッジアセンブリにおいては、カム部材が、溝を有する２組のねじれ型のステープルの間の、細長いガイド経路を通って動く。ステープル駆動部材は、溝の内部に位置し、そして、カムを長手軸方向に動かすことによって接触して、ステープルの駆出を達成するような様式で位置決めされる。ステープラーの他の例は、特許文献４、特許文献５および特許文献６（これらの文献の各々の全内容は、本明細書中に参考として援用される）に開示される。
発熱エネルギー、好ましくは、高周波エネルギーを用いて、組織および血管を加熱し、凝血または焼灼をもたらすことによって、止血の改善をもたらすためには、電気焼灼デバイスが好ましい。単極性デバイスは、切断機器もしくは焼灼機器と関連した一本の電極と、通常は、患者に外部から付着された遠隔の戻り電極とを利用する。双極性の機器は、二本の電極を利用し、そして、焼灼電流は、一般に、機器の組織治療部分（例えば、エンドエフェクタ）の二本の電極間の組織に限定される。
ステープル機器と電気焼灼デバイスの構造的局面と機能的局面とを組み合せた、止血用電気外科ステープルデバイスは、一般に、発熱エネルギーの使用前、使用中、または使用後のいずれかに、組織にステープルを止めるために、発熱エネルギーを用いて、ナイフ切断線および外科用締め具、またはその近くで、凝固または焼灼をもたらすことによって、止血の改善を提供する。
ステープル付け、電気焼灼、または両方の技術の組み合わせは、一般に、ナイフ切断線に沿った出血の制御に非常に都合よくできているが、他の技術も、外科手順の間の出血を制御するために使用されることが想定される。

従って、従来の技術を使用せずに、止血、組織の接合、または、溶接を提供するための外科用装置を提供することが、本開示の一つの局面である。
（要旨）
本開示は、身体の組織を挟み、その組織に創傷治療物質を適用することが可能な外科用装置に関する。
本開示の一局面によれば、その遠位端においてディスポーザブル充填ユニットを受容するように適合された第一の半セクションと；第一の半セクションと並列した関係の第二の半セクションと；第一の半セクションの遠位端と選択的かつ作動可能に係合できるディスポーザブル充填ユニットと；標的の外科手術部位に創傷治療物質を送達するための、創傷治療物質適用器アセンブリとを備える外科用装置が提供される。
ディスポーザブル充填ユニットは、カートリッジと；カートリッジ内に支持される複数の配置可能な針であって、この針の各々は、内部を延びる管腔を備え、その外周には、少なくとも一つの穴が形成されている、針と；カートリッジから標的組織内へと各針を配置するための、各針と作動可能に関連した針プッシャーと；針プッシャーの各々に駆動力を送達して、カートリッジから針を配置するために、カートリッジ内に並進可能に配置された作動部材と、を備える。
適用器アセンブリは、第一の半セクションおよび第二の半セクションのそれぞれの遠位端上に支持される、第一のレザバおよび第二のレザバと；各レザバと流体連絡する創傷治療物質の供給源と、を備える。
創傷治療物質の供給源は、少なくとも一つの、創傷治療物質の加圧型キャニスタを備え得る。各キャニスタは、第一の半セクションおよび第二の半セクションのうちの一方の上に支持され得る。
各針プッシャーは、それぞれの針の管腔と流体連絡するために、内部に形成された開口部を備える。
第一の半セクションの遠位端は、レザバからカートリッジへの創傷治療物質の伝送（ｔｒａｎｓｍｉｓｓｉｏｎ）を可能にするために、レザバとレザバ上に支持されたカートリッジとの間に提供される複数の開口部を備える。第二の半セクションの遠位端の上には、複数の針受容開口部を備えるプレートが支持され得る。従って、各針の遠位端は、外科用装置の作動中に、第二の半セクションのそれぞれの針受容開口部に入り得る。
第二の半セクションの遠位端上に支持されたレザバ内に創傷治療物質を閉じ込めるために、エラストマー材料が提供され得る。従って、外科用装置の起動時には、各針の遠位端は、エラストマー材料を貫通し、そして、各針の管腔が、第二の半セクションの遠位端上に支持されたレザバと流体連絡する。
創傷治療物質は、接着剤、シーラント、止血剤および医薬品のうちの少なくとも一つである。
ディスポーザブル充填ユニットは、針を引き戻された状態に付勢するために、針の少なくとも一つと作動可能に関連した付勢部材を備え得る。外科用装置はさらに、第一の半セクションの遠位端と第二の半セクションの遠位端との間に挟まれた組織を切断するための、作動スレッド（ａｃｔｕａｔｉｏｎ ｓｌｅｄ）上に支持されたナイフブレードを備え得る。このナイフブレードは、カートリッジ内に提供された長手軸方向のスロットに沿って移動し得る。針のうちの少なくともいくつかは、カートリッジの長手軸方向のスロットのいずれかの側に配置される。
適用器アセンブリは、創傷治療物質の供給源とレザバとの間に、流体的に挿入された（ｆｌｕｉｄｌｙ ｉｎｔｅｒｐｏｓｅｄ）バルブを備え得る。
本開示の別の局面によれば、標的の外科手術部位において組織を手術する方法が提供される。この方法は、外科用装置を提供する工程を包含する。この外科用装置は、第一の半セクションの遠位端内に選択的かつ作動可能に支持されることが可能なディスポーザブル充填ユニットを備え、このディスポーザブル充填ユニットは、複数の配置可能な針を作動可能に支持するカートリッジと、針の各々に駆動力を送達してカートリッジから針を配置するために、カートリッジ内に並進可能に配置された作動部材と、を有し、針の各々は、内部を通って延びる管腔を備え、そして、その外周には、少なくとも一つの穴が形成されている。この外科用装置はさらに、標的の外科手術部位に創傷治療物質を送達するための、創傷治療物質適用器アセンブリを備える。この適用器アセンブリは、第一の半セクションおよび第二の半セクションのうちの一方の遠位端の上に支持された少なくとも一つのレザバと、レザバの各々と流体連絡する創傷治療物質の供給源とを備える。
この方法はさらに、外科用装置の第一の半セクションの遠位端と第二の半セクションの遠位端との間に組織の隣接する層を挟む工程と；外科用装置を始動させる工程であって、この外科用装置を始動させる工程は、作動部材を遠位に進めて、挟まれた組織に複数の針を打ち込む工程、および、複数の針を通して、創傷治療物質の供給源から、挟まれた組織へと創傷治療物質を分配する工程を包含する、工程と、を包含する。
創傷治療物質の供給源は、加圧され得る。従って、外科用装置を始動させた際に、創傷治療物質は、その供給源から排出される。従って、外科用装置を始動させた際に、各針の遠位端は、望ましくは、第二の半セクションの遠位端に入る。
外科用装置は、第一の半セクションおよび第二の半セクションの各々の遠位端の上に支持されたレザバを備え得る。従って、創傷治療物質は、レザバの各々から、針の各々の管腔へと送達され得る。
外科用装置は、さらに、第一の半セクションの遠位端と第二の半セクションの遠位端との間に挟まれた組織を切断するために、第一の半セクションおよび第二の半セクションの遠位端に沿って並進移動可能なナイフブレードを備え得、外科用装置を始動させた際に、このナイフブレードは、第一の半セクションおよび第二の半セクションの遠位端に沿って遠位に進められ、第一の半セクションの遠位端と第二の半セクションの遠位端との間に挟まれた組織を切断する。

（実施形態の詳細な説明）
本開示は、止血、組織の接合または溶接を提供するために、生物学的および／もしくは合成の生体適合性シーラント、止血剤および接着剤（本明細書中では、まとめて、創傷治療物質と呼ぶ）を使用する外科用装置を提供する。創傷治療物質の使用は、短期および長期の止血およびシーリングを提供し、そして／または、ナイフ切断線に沿った出血を減少もしくは防止する。
図面および以下の説明において、用語「近位」は、従来どおり、操作者により近い装置の端部を指し、一方で、用語「遠位」は、操作者からより遠い装置の端部を指す。
ここで、図面を参照すると（図面において、類似の参照番号は、類似の構造的要素を特定する）、図１および２には、好ましい実施形態に従って構築された創傷治療物質適用器を有する外科用装置１０が図示される。外科用装置１０は、カートリッジ、すなわち、針を放出する半セクション１１ａ（本明細書においては、以後、第一の半セクションと呼ぶ）と、アンビル、すなわち、針を受容する半セクション１１ｂ（本明細書においては、以後、第二の半セクションと呼ぶ）を備える。当業者に容易に明らかであるように、装置１０は、その製造および組立てに関連する費用を実質的に減少させるような様式で構築される。
図１〜３を参照すると、第一の半セクション１１ａと一体になった装置１０は、デバイスを把持および支持するためのハンドルを規定する本体部分１２を備える。保持チャネル１４は、その遠位端に隣接する本体部分１２の内腔１５内に設置される。保持チャネル１４は、ディスポーザブル充填ユニット２０を支持するような寸法および構成である。
図４、７および９に示されるように、ディスポーザブル充填ユニット２０は、複数のスロット２２ａ、２２ｂを有するカートリッジ２２を備え、この複数のスロット２２ａ、２２ｂは、複数の外科用針２４を保持する。このディスポーザブル充填ユニット２０はさらに、駆動力によって起動されたとき、スロット２２ａ、２２ｂの外へと針２４を押し出すか、または、スロット２２ａ、２２ｂを通して外へと針２４を配置するように適合および構成される複数の針プッシャー２６を備えている。針プッシャー２６もまた、プッシャー２６内に形成された中央開口部１５４を介して、針２４の内部へと創傷治療物質「Ｗ」を供給するような構成および寸法である（図９を参照のこと）。ディスポーザブル充填ユニット２０はさらに、例えば、ここでは、スレッドとして示されている作動部材２８（図３、４および８）を備え、この作動部材２８は、針２４の周囲にある穴１２１から、または、この穴１２１を通して、創傷治療物質「Ｗ」を外科的適用部位へと適用または分配するために、カートリッジ２２を長手軸方向に平行移動して、駆動力をプッシャー２６に伝達するように設置されている（図９を参照のこと）。作動部材２８はスレッドとして示されているが、図１０を参照して以下により詳細に記載されるように、作動部材２８は、カムローラ、カムスライドなどであることが想定され、そして、これは、本開示の範囲内である。
再度、図１〜３を参照すると、創傷治療物質「Ｗ」またはその成分は、適用器アセンブリ２によって外科的適用部位へと供給される。適用器アセンブリ２は、第一の半セクション１１ａおよび第二の半セクション１１ｂのそれぞれの遠位端と流体連絡する、一対のレザバ４ａ、４ｂを備える。レザバ４ａ、４ｂは、導管アセンブリ８を介して、感圧（ｐｒｅｓｓｕｒｅ−ａｃｔｕａｔｉｎｇ）流体（気体または液体）キャニスタ６と流体連絡している。好ましくは、流体は気体である。導管アセンブリ８の上部導管８ａおよび下部導管８ｂ（図２および３を参照のこと）は、キャニスタ６から延び、そして、レザバ４ａ、４ｂに加圧された流体を送達するために、適用器アセンブリ２のそれぞれのレザバ４ａ、４ｂ内に入る。各キャニスタ６は、本体部分１２の一部分であるか、または、本体部分１２に固定された、それぞれの取り付け部材またはトラック（ｔｒａｃｋ）９１に、解放可能に設置されている。各キャニスタ６は、ノズル１２４（図３を参照のこと）を備え、このノズル１２４は、内部に収容される加圧された流体の、導管アセンブリ８を介した、レザバ４ａ、４ｂへの流れを制御するためのバルブ１２６を有する。キャニスタ６からの加圧された流体の放出は、外科的な創傷治療物質の適用作業の間に、創傷治療物質「Ｗ」を針２４を通して押し出す。
図６および７に見られるように、レザバ４ａから保持チャネル１４の内部へと創傷治療物質「Ｗ」を提供するために、保持チャネル１４は、保持チャネル１４の下に設置されたレザバ４ａからの創傷治療物質「Ｗ」の通過のために、その底部分６０に沿って形成され、好ましくは最初はシールされた開口部１５０を備える。使用時に、本開示の方法によれば、底部分６０に重なるシール６１（図６を参照のこと）を除去するか、破壊するか、穿通するか、または、他の方法でアクセスもしくは通路を提供した後、創傷治療物質「Ｗ」は、キャニスタ６の加圧された流体により提供される圧力に起因して、開口部１５０を通り、レザバ４ａから保持チャネル１４の内部へと、そして最終的には、針２４の内部を通って流れ、その後、組織１２０に、組織１２０の上に、組織１２０の下に、そして／または組織１２０の層の間に分配される（図９を参照のこと）。
好ましい実施形態において、レザバ４ａ、４ｂのうちの一方は、二成分からなる接着剤（ａｄｈｅｓｉｖｅ）を形成する際に使用される創傷治療物質「Ｗ」の一つの成分（例えば、接着剤（ｇｌｕｅ））を貯蔵し、他方、レザバ４ａ、４ｂのうちのもう一方は、二成分からなる接着剤を形成する際に使用される創傷治療物質「Ｗ」の第二の成分（例えば、促進剤）を貯蔵する。別の好ましい実施形態において、図６に見られるように、各レザバ４ａおよび／または４ｂの第一の区画１５６は、接着剤を形成するための創傷治療物質の第一の成分を貯蔵し、他方、各レザバ４ａおよび／または４ｂの第二の区画は、接着剤を形成する際に使用される創傷治療物質の第二の成分を貯蔵する。好ましくは、第一および第二の成分は、処置部位への、または、処置部位における、組み合わされた成分の適用／送達のために、共通の針へと供給される。
好ましくは、レザバ４ａ、４ｂは、同一であり、そして、第一の成分の第二の成分に対する所定の比（この比は、１：１であり得る）を得、そして／または維持するために、創傷治療物質「Ｗ」のそれぞれの成分の、等しいか、相補的であるか、または適切な容積測定量が入っている。また、好ましくは、創傷治療物質「Ｗ」の二つの成分により形成される接着剤は、止血剤として、そして、組織接着剤として機能する、フィブリン接着剤（ｆｉｂｒｉｎ ｇｌｕｅ）またはフィブリンシーラント（ｆｉｂｒｉｎ ｓｅａｌａｎｔ）である。
フィブリンシーラントは、タンパク質性の凝固因子（例えば、フィブリノーゲン）の溶液が、タンパク質性の触媒（例えば、トロンビン）の溶液と接触するときに生じる、迅速な重合によって形成される。この迅速な重合は、代表的には、これらの溶液が最初に互いに接触してから二秒以内に開始し、そして、代表的には、接触から十秒以内で軟らかい固まりを生じる。フィブリノーゲンとトロンビンの緊密な相互作用の際の迅速な重合に起因して、適用部位に適用するまで、これらの二つの血液性タンパク質を別々に維持することが重要である。従って、適用器アセンブリ２は、各タンパク質のための別個の導管アセンブリを用い、そして、適用部位に適用する前の二つの成分の混合を防止するために、外科用針２４を区画化することによって、各血液性タンパク質をもう一方の血液性タンパク質とは別々に供給することが好ましい。
創傷治療物質「Ｗ」は、組織を接合、治癒、シール、または、他の方法で処置するためのあらゆる物質であることが企図される。好ましい実施形態において、創傷治療物質は、生体適合性のシーラントであり、そして、これらとしては、限定はされないが、組織との接触の際に硬化するシーラント、紫外（ＵＶ）光への曝露の際に硬化するシーラント、互いに隔離した状態を維持され、そして、合わされる、二つ部分からなる系のシーラント、またはこれらの任意の組み合わせが挙げられる。あらゆる公知の適切な接着剤が使用され得る。一実施形態において、このようなシーラントおよび／または接着剤は硬化可能であることが企図される。例えば、約１０〜１５秒の硬化時間を有し得るシーラントが使用され得る。好ましい実施形態において、シーラントおよび／または接着剤は、生体吸収性および／または生体再吸収性の物質である。別の実施形態において、約３０秒の硬化時間を有するシーラントおよび／または接着剤が使用され得ることが企図される。さらに、創傷治療物質「Ｗ」は、プレキュアーされた接着剤またはシーラントであり得ることが想定される。プレキュアーされた接着剤またはシーラントは、身体組織の水分および／または熱と反応し、それによって、このシーラントまたは接着剤のシーリングおよび／または接着特性を活性化させ得る。
特定の好ましい実施形態において、創傷治療物質はシーラントを含む。このようなシーラントは、望ましくは、ＰＥＧベースの物質である。シーラントおよび／または接着剤として有用な物質のクラスの例としては、生体適合性の光開始剤の存在下でのアクリレートまたはメタクリレート官能性ヒドロゲル、アルキル−シアノアクリレート、イソシアネート官能性マクロマー（アミン官能性マクロマー有りまたは無し）、スクシンイミジルエステル官能性マクロマー（アミンもしくはスルフヒドリル官能性マクロマー有りまたは無し）、エポキシ官能性マクロマー（アミン官能性マクロマー有りまたは無し）、アルデヒド架橋剤の存在下でのタンパク質またはポリペプチドの混合物、Ｇｅｎｉｐｉｎ、あるいは、水溶性カルボジイミド、多価カチオンの存在下でのアニオン性多糖類などが挙げられる。
利用され得るいくつかの特定の物質としては、有機ポリイソシアネートとオキシエチレンベースのジオールもしくはポリオールから誘導された、イソシアネート末端親水性ウレタンプレポリマー（米国特許第６，７０２，７３１号および同第６，２９６，６０７号、ならびに、米国特許出願第２００４／００６８０７８号に開示されるものを含む）；α−シアノアクリレートベースの接着剤（米国特許第６，５６５，８４０号に開示されるものを含む）；アルキルエステルベースのシアノアクリレート接着剤（米国特許第６，６２０，８４６号に開示されるものを含む）；インサイチュで反応し、架橋し得る求電子基と求核基とを有する水溶性前駆体から形成された、生体適合性の架橋ポリマーに基づく接着剤（米国特許第６，５６６，４０６号に開示されるものを含む）；生体吸収性のジアミン化合物と組み合せた、一以上のイソシアネート基で置換されたポリアルキレンオキシド骨格に基づくもの、または、生体吸収性のジイソシアネート化合物と組み合せた、一以上のアミン基で置換されたポリアルキレンオキシド骨格に基づくものを含めて、二つの部分からなる接着剤系（例えば、米国特許出願第２００３／００３２７３４号出願公開に開示されるもの、この文献の内容は、本明細書中に参考として援用される）；ならびに、芳香族ジイソシアネートおよびポリオールから誘導された、イソシアネート末端親水性ウレタンプレポリマー（例えば、米国特許第２００４／０１１５２２９号出願公開に開示されるもの、この文献の内容は、本明細書中に参考として援用される）が挙げられる。
創傷治療物質「Ｗ」が、接着剤、止血剤、シーラント、または、任意の他の組織もしくは創傷を治療する物質のうちの一つまたは組み合わせを含み得ることが想定され、このことは、本開示の範囲内である。外科用の生体適合性創傷治療物質「Ｗ」（本開示に従って使用され得る）としては、器官、組織もしくは構造を付着もしくは保持するように機能する接着剤、流体の漏れを防止するように機能するシーラント、そして、出血を停止もしくは防止するように機能する止血剤が挙げられる。使用され得る接着剤の例としては、タンパク質由来で、アルデヒドベースの接着物質（例えば、ＢｉｏＧｌｕｅＴＭの商品名でＣｒｙｏｌｉｆｅ，Ｉｎｃ．より販売されている、市販のアルブミン／グルタルアルデヒド物質）、および、ＩｎｄｅｒｍｉｌＴＭの商品名でＴｙｃｏ Ｈｅａｌｔｈｃａｒｅ Ｇｒｏｕｐ，ＬＰより、そして、Ｄｅｒｍａ ＢｏｎｄＴＭの商品名でＥｔｈｉｃｏｎ Ｅｎｄｏｓｕｒｇｅｒｙ，Ｉｎｃ．より販売されている、シアノアクリレートベースの物質が挙げられる。使用され得るシーラントの例としては、フィブリンシーラント、ならびに、コラーゲンベースおよび合成ポリマーベースの組織シーラントが挙げられる。市販のシーラントの例は、ＣｏＳｅａｌＴＭの商品名の下で、Ｃｏｈｅｓｉｏｎ ＴｅｃｈｎｏｌｏｇｉｅｓおよびＢａｘｔｅｒ Ｉｎｔｅｒｎａｔｉｏｎａｌ，Ｉｎｃ．より販売されている、合成ポリエチレングリコールベースのヒドロゲル材料である。使用され得る止血物質の例としては、フィブリンベース、コラーゲンベース、酸化した再生セルロースベース、そして、ゼラチンベースの局所用止血剤が挙げられる。市販の止血物質の例は、ＣｏＳｔａｓｉｓＴＭの商品名の下で、Ｔｙｃｏ Ｈｅａｌｔｈｃａｒｅ Ｇｒｏｕｐ，ＬＰより、ＴｉｓｓｅｅｌＴＭの商品名の下でＢａｘｔｅｒ Ｉｎｔｅｒｎａｔｉｏｎａｌ，Ｉｎｃ．より販売されている、フィブリノーゲン−トロンビンを組み合せた材料である。本明細書における止血剤としては、収れん薬（例えば、硫酸アルミニウム）および凝固剤が挙げられる。
医薬品は、一以上の医学的および／または外科的に有用な物質（例えば、薬物、酵素、増殖因子、ペプチド、タンパク質、色素、診断剤または止血剤、モノクローナル抗体、または、狭窄の防止に使用されるあらゆる他の医薬品）を含み得る。
創傷治療物質「Ｗ」は、粘弾性フィルム形成物質、架橋反応性の因子、およびエネルギー硬化性接着剤を含み得る。創傷治療物質「Ｗ」、および、特に、接着剤は、創傷治療物質「Ｗ」に水および／またはグリセリンを適用すると硬化され得ることが想定される。この様式において、水および／またはグリセリンは、接着剤を硬化させ、そして、創傷を水和する。
創傷治療物質「Ｗ」は、創傷治療物質「Ｗ」が、導管８および／または針２４を通って自由に流れ得るように、比較的粘度の低い流体もしくは液体であり得ることが想定される。さらに、創傷治療物質「Ｗ」は、粒子状物質の微細な粉末を含み得ることが想定される。
さらに、創傷治療物質「Ｗ」は、例えば、組成物および／または化合物の粒子が外科的な修復部位に適用または曝露されたときに、治癒プロセスを加速するか、もしくは、有益に改変する組成物および／または化合物を含み得ることが企図される。例えば、創傷治療物質「Ｗ」は、修復部位に蓄積される治療剤であり得る。治療剤は、その抗菌性特性、修復、または、再構成、および／または、新しい組織の成長を促進する能力について選択され得る。ゆっくりと組織内へと放出される広域スペクトル抗生物質のような抗菌剤（硫酸ゲンタマイシン、エリスロマイシン、または、誘導体化した糖ペプチド）は、この様式で適用されて、組織修復部位における臨床的な感染および潜在的な感染に対抗するのに役立ち得る。修復および／または組織の成長を促進するために、創傷治療物質「Ｗ」は、一または数種の成長促進因子（例えば、線維芽細胞増殖因子、骨増殖因子、上皮増殖因子、血小板由来増殖因子、マクロファージ由来増殖因子、肺胞由来増殖因子、単球由来増殖因子、マガイニン（ｍａｇａｉｎｉｎ）など）を含み得る。いくつの治療的指標は、血栓症を引き起こす、組織もしくは腎臓プラスミノーゲンアクチベーターを伴うグリセロール、組織を損傷するフリーラジカルを除去するスーパーペルオキシドジスムターゼ（ｓｕｐｅｒｏｘｉｄｅ ｄｉｍｕｔａｓｅ）、癌治療のための腫瘍壊死因子、または、免疫系を増強するための、コロニー刺激因子およびインターフェロン、インターロイキン−２もしくは他のリンホカインである。
カートリッジ２２は、好ましくは、液体の結晶性ポリマー材料から製造されるが、他の材料も企図される。カートリッジ２２は、固定、安定化、および、例えば、カートリッジ２２の内部への回転を防止するために、保持チャネル１４と係合する縁部２３を有する。
図３および４に見られるように、製造および組立てを単純化し、そして、簡単にするために、作動スレッド２８は、好ましくは、所望の構造上の構成へと折り畳まれた、単一の片のシートメタルから一体となって形成される。この構成において、作動スレッド２８は、底部分３０と、二つの直立したカムウェッジ３２および３４と、ナイフブレード３６を支持する一つの直立したシャンク３５とを規定する。ナイフブレード３６は、好ましくは、シャンク３５に対してスポット溶接されているが、他の公知の締付け方法が使用され得る。
カムウェッジ３２および３４は、互いに関して軸方向にずれており、その結果、カートリッジ２２を通したスレッドの平行移動の間中、一方が、他方を先導する。こうすることで、カートリッジ２２内の駆動力は、外科的な駆動動作の間に平衡状態を保つ。長手軸方向のスロット２２ａおよび２２ｂは、カムウェッジ３２および３４の長手軸方向の移動に適応し、一方で、スロット２２ｃは、シャンク３５の長手軸方向の移動に適応する。
作動スレッド２８の底部分は、内部に規定された横方向のスロット４０を有し、このスロット４０は、細長の作動チャネル４４の遠位端に形成された上向きのフランジ４２を解放可能に保持するような寸法および構成である（図４を参照のこと）。ディスポーザブル充填ユニット２０が、保持チャネル１４内に配置され、そして、作動スレッド２８が、その最近位位置に配置されるとき、フランジ４２は、スロット４０と解放可能に係合する。こうして、作動チャネル４４の動きにより、作動スレッド２８が動き、作動スレッド２８は次いで、プッシャー２６と係合して、針２４を配置する。接着剤適用作業の後、ディスポーザブル充填ユニット２０は、スロット４０からフランジ４２を容易に脱係合することによって、保持チャネルから取り外され得る。
引き続き図４を参照すると、作動チャネル４４は、底部分４５と、細長の構成の二つの平行な直立梁４６および４８によって規定される。梁４６および４８の遠位端は、それぞれ、カムウェッジ３２および３４のずらされた方向と適合するようにずらされる。各梁４６、４７の近位端は、始動ノブ５０と関連付けられた設置ブロック４９と係合するように、後方に突出している。作動チャネル４４の直立梁４６、４８の各々の近位端を受容するために、一対のスロット５２（このうち、一方のみが示される）が、設置ブロック４９に形成される。スロット５２には、梁の端部にある開口部５６と係合するために、移動止め５４が提供されており、梁４６、４８を、設置ブロック４９内にロックするように保持する。使用時に、始動ノブ５０の長手軸方向の動きは、作動チャネル４４の対応する長手軸方向の動きと、スレッド２８の作動とをもたらす。
図３および６を参照すると、保持チャネル１４は、底部分６０と、二つの直立した平行の壁６２および６４とを備える。接着剤の線の長さを示すために、数値が、保持チャネル１４の壁６２、６４上に刷り込まれている。ディスポーザブル充填ユニット２０に提供された対応する構造体と係合するために、保持構造体が、壁６２、６４の遠位端に提供される。具体的には、対応する隆起（例えば、隆起６７）と係合するために、ノッチ６６ａおよび６６ｂが提供され、そして、対応する移動止め（例えば、移動止め６９）と係合するために、スロット６８ａおよび６８ｂが提供される。これらの構造体は、保持チャネル１４内でのカートリッジ２２（およびディスポーザブル充填ユニット２０）の、側方、長手軸方向、および垂直方向の移動を防止する。外科用装置１０内に充填された場合に、一対の対向する隆起７２ａおよび７２ｂ（図３および４を参照のこと）と相互作用して、ディスポーザブル充填ユニット２０を保持チャネル１４内へと導くために、傾斜係合スロット７０ａおよび７０ｂもまた、保持チャネル１４の対向する壁に規定される。
引き続き図３を参照すると、外科用装置１０はさらに、ほぼＵ字形状の断面構成を有する、細長のアンビル支持梁８０を備える。支持梁８０の近位端部分８２は、一対の対応する移動止め８６（一方のみが示される）と係合するための、切れ目の入った領域８４を有し、この移動止め８６は、その近位端に隣接する本体部分１２の空洞１５内に延びる。移動止め８６は、カートリッジまたは第一の半セクション１１ａと、アンビルまたは第二の半セクション１１ａとが、互いに嵌合するときに、係合される。アンビル支持梁８０の遠位端部分８８は、外科手順の間に、内部に針２４の一部分を受容するために、それぞれの針２４と並列して配置された、複数の開口部１４１（図９を参照のこと）を有する、成形プレート９０を支持するように構成される。
プレート９０は、好ましくは、単一片の金属から形成され、そして、複数の針受容開口部１４１（図５を参照のこと）を規定するように、冷間成形され、そして、印を付けられる。各針受容開口部１４１は、カートリッジ２２内に収容される特定の針２４と整列する。プレート９０には、図３に示されるように、製造および組立ての間に、アンビル支持梁８０内の相補的な係合スロット９３ｂと係合するために内向きに延びる、二つの対向するこみ９２ａおよび９２ｂ（一方のみが示される）が提供される。プレート９０の断面構成は、支持梁８０の断面形状と相補的になるような寸法である（図５を参照のこと）。より具体的には、プレート９０の長さに沿って延びる空洞９３は、支持梁８０内に形成された同様のチャネルと対応する。これらの領域は、ナイフブレード３６が身体組織内に切開を形成するように遠位に平行移動する際に、シャンク３５およびナイフブレード３６に適応し、そして、この切開は、外科的な接着剤適用作業の間に、接着剤によってつながれる。
一対の矩形の開口部９５ａおよび９５ｂが、一対の対応するように位置するフランジまたは突出部９６ａおよび９６ｂを受容するために、その近位端と隣接して、プレート９０内に形成される。これらの突出部９６ａおよび９６ｂは、組織接触面９７から離れて下向きに突出する（図３および４を参照のこと）。開口部９５ａ、９５ｂと、フランジ９６ａ、９６ｂとの間の相互作用は、カートリッジ２２およびプレート９０が、外科的な接着剤適用手順の間に、互いに適切に整列することを確実にする。
好ましくは、上部レザバ４ｂの底面１６０は、エラストマー材料または他の適切な材料１６１から製造される。底面１６０は、アンビル支持梁８０に取り付けられる。外科的な接着剤適用手順の間、針２４は、レザバ４ｂのエラストマー材料１６１を穿通し、そして、加圧された接着剤が、針２４の内部へと流れる（図９を参照のこと）。エラストマー材料１６１は、内部に形成され、針２４と位置合わせされた一連の弱い線（ｗｅａｋｅｎｅｄ ｌｉｎｅ）を備え得ることが想定される。この様式において、針２４が、エラストマー材料１６１を穿通するためには、弱められた力が必要である。
針２４が創傷治療物質「Ｗ」を分配した後、針２４は、ディスポーザブル充填ユニット２０内に引き戻され、そして、エラストマー材料１６１が、レザバ４ｂを再度シールして、組織１２０、１２２を針２４から解放し、そして、レザバ４ｂ内に収容される残りの接着剤の流出を防止する。一実施形態において、針２４は、レザバ４ｂからの外科的な圧力の増加によってレザバ４ｂから出され、針２４はディスポーザブル充填ユニット２０内へと引き戻され得る。別の実施形態において、ばね９８（図７を参照のこと）は、各針２４の周りに適切に配置され、そして／または、適切に位置決めされて、作動スレッド２８が引き戻された際に、針２４を、配置されていない状態へと戻し得る。具体的には、使用時に、作動スレッド２８が、プッシャー２６を動かすように進められたとき、ばね９８は、作動チャネル４４の梁４６、４８によって、圧縮および維持されて、プッシャー２６と接触したままになる。作動スレッド２８が、近位方向に引き抜かれたとき、創傷治療物質「Ｗ」を適用した後、ばね９８が伸び、それによって、針２４をディスポーザブル充填ユニット２０内へと動かすか、引き戻す。
図１〜３を参照すると、装置１０のアンビルまたは第二の半セクション１１ｂは、さらに、プレート９０の表面と、カートリッジ２２の組織接触面９７との間に組織をしっかりと挟むために使用されるクランピングハンドル１００を備える。クランピングハンドル１００は、横方向のピボットピン（図示せず）の周りで、アンビル支持梁８０に旋回可能に設置される。本体部分１２内に規定される内部空洞内に支持されるＵ字形状のクランピング梁１０４と相互作用するために、一対のクランピングフック１０２ａおよび１０２ｂが、クランピングハンドル１００から吊り下がっている。
装置１０が、使用前に組み立てられるとき、アンビル支持梁８０の近位端８２にある切れ目の入った領域８４は、本体部分１２の内部空洞１５内の協働する移動止め８６と係合する。その際、アンビルまたは第二の半セクション１１ｂは、カートリッジまたは第一の半セクション１１ａと嵌合し、そして、クランピングハンドル１００が、図１に示される直立した制動されていない（ｕｎｄａｍｐｅｄ）位置に配置されている。その後、身体組織が、プレート９０の表面と、カートリッジ２２の組織接触面９７との間に適切に配置されると、アンビルまたは第二の半セクション１１ｂが、本体部分１２内の移動止めの周りで、カートリッジまたは第一の半セクション１１ａに向けて旋回され、その結果、クランピングフック１０２ａおよび１０２ｂの遠位端が、Ｕ字形状のクランピング梁１０４の底部の近位端の直ぐ近くに位置決めされる。同時に、フランジ９６ａおよび９６ｂが、プレート９０内の開口部９５ａおよび９５ｂと係合して、アンビルとカートリッジとの適切な位置合わせを確実にする。
次いで、捕捉された身体組織をしっかりと挟むために、クランピングハンドル１００が、図１に図示される位置から、図２に示される位置に向けて旋回される。この時点で、クランピングフック１０２ａおよび１０２ｂは、クランピング梁１０４の底部と係合し、そして、装置１０を挟まれた状態にロックする。挟んでいる間に、捕捉された身体組織は、カートリッジ２２の組織接触面９７およびプレート９０の締付け形成面に対して対向力を及ぼし、そして、２つの構造体の間隔を空けるようにする。これらの力を克服し、そして、アンビル支持梁８０の近位部分８２が曲がらないようにするために、保持チャネル１４内にベアリング面が規定され、挟んでいる間に生成される圧縮力を支持する。具体的には、図３に図示されるように、対向するベアリングシェルフ１１０ａおよび１１０ｂは、保持チャネル１４の対向する壁６２および６４内に形成される印である。ベアリングシェルフ１１０ａ、１１０ｂは、アンビル支持梁８０の内側セクションが、クランピングハンドルのピボットピンの近くに沿うように位置決めされる。装置が起動された後、装置の再起動を防止するために、ロッキング機構を提供することが望ましい。
ここで、図７〜９を参照すると、外科用の創傷治療物質「Ｗ」（ここでは、接着剤）を適用する方法が図示される。この手順によれば、複数の針２４が、作動スレッド２８によってか、または、カムウェッジもしくは表面３２ａ、３４ａ（図４を参照のこと）によって、カートリッジ２２から配置または押し出され、針の中を通って穴１２２を出て、プレート９０の針受容開口部１４１によって受容される前、間、または、その後に、組織１２０内へと駆動される。その間に、ナイフブレード３６（図４を参照のこと）が組織１２０を切開し、スロット２２ｃに沿ってナイフ切断線を形成する。作動時に、外科用装置１０を始動させる前、作動スレッド２８は、最近位の位置にある。この時点で、ナイフブレード３６は、ディスポーザブル充填ユニット２０の近位端に隣接して形成された、保護ハウジング２５（図３および４を参照のこと）内に囲まれる。
外科用装置１０を始動させるためには、組織１２０が、第一の半セクションの遠位端と第二の半セクションの遠位端との間に挟まれた後、始動ノブ５０が、遠位方向に動かされる。従って、図８に図示されるように、始動ノブ５０を遠位方向に動かすと、作動チャネル４４が、作動スレッド２８を、カートリッジ２２内へ、そして、カートリッジ２２を通して遠位に駆動する。その遠位への移動の間、カムウェッジ３２および３４の角度のついた前面（ｌｅａｄｉｎｇ ｓｕｒｆａｃｅ）は、連続してプッシャー２６と接触し、プッシャー２６を、作動スレッド２８の移動方向に対して横断する方向へと押し進める。結果として、プッシャー２６は、その個々の脱出孔１２２から針２４を押し出し、針２４を組織１２０内へと駆動し、その後、針２４は、プレート９０内に提供された対応する針受容開口部１４１によって受容される（図９を参照のこと）。
作動スレッド２８が、カートリッジ２２の再遠端へと進められるまで、針２４の連続した発射が続けられ、この時点で、全ての針２４が、一旦カートリッジ２２内に収容され、かつ、ディスポーザブル充填ユニット２２から外へと押し出され、そして、ナイフ切断線が、ナイフブレード３６によって組織１２０内に形成される。所望される場合、バルブ１２６を開くことによって、ナイフ切断線、ならびに、組織１２０の外側領域、内側領域および／または中間領域に、接着剤が供給され、そして、各キャニスタ６内に収容される加圧された流体が、レザバ４ａ、４ｂ内へと解放され、それによって、創傷治療物質「Ｗ」（ここでは、例えば、接着剤「Ａ」として、または、接着剤の各成分「Ａ １ 、Ａ ２ 」として示される）を、針２４の中を通して、そして、穴１２１を通して組織１２０の間へとならびに、スロット２２ｃの中を通してナイフ切断線へと、押し出す。バルブ１２６は、十分な接着剤「Ａ」が分配されるか、全ての接着剤「Ａ」が分配されるか、そして／または、キャニスタ６内の全圧力が使い果たされるまで、開いたままにされる。
その後、始動ノブ５０は、その元の位置まで引き戻され、例えば、針プッシャー２６と、ディスポーザブル充填ユニット２０の組織接触面９７の内面との間に配置される圧縮ばねまたは他の付勢手段９８によって、針２４が、ディスポーザブル充填ユニット２０内へと引き戻され、第一の半セクションと第二の半セクションとが分離され、そして、ディスポーザブル充填ユニット２０ならびに片方もしくは両方のレザバ４ａ、４ｂが、保持チャネル１４から除去され得る。好ましくは、付勢手段９８は、各プッシャー２６の各針２４の周りに配置される。その後、新しい、完全に充填されたディスポーサブル充填ユニット２０が、保持チャネル１４内に位置決めされ得、その結果、作動スレッド２８のスロット４０が、作動チャネル４４のフランジ４２と係合して、装置１０を再使用可能にする。
キャニスタ６はまた、必要に応じて交換され得ることが想定される。あるいは、レザバ４ａ、４ｂは、遠隔圧力源（図示されず）に流体係合され得、それによって、キャニスタ６に対する必要性を取り除くことが想定される。なお別の実施形態において、レザバ４ａ、４ｂは、手で起動される圧力源（例えば、シリンジ、手動ポンプなど）に流体係合され得ることが想定される。
本発明の装置は、好ましい実施形態に関して記載されてきたが、変更および改変が、本発明の装置の精神または範囲から逸脱することなく、本発明の装置に対してなされ得ることは、本発明の装置が属する分野の当業者に容易に明らかである。例えば、本発明は、内視鏡外科手術にも開胸型外科手術にも適用され得る（例えば、吻合装置）。本発明は、手で保持され手動で操作されるか、または、遠隔的（例えば、ロボットにより）操作され、組織処置部位に創傷治療物質を同時に、または連続して適用するために、例えば、一対のジョー、例えば、アンビルとカートリッジを使用する、内視鏡的な直線状のステープラーに容易に適用され得る。

ここで、図１０を参照すると、外科用装置１０の代替的な実施形態が示され、そして、記載される。外科用装置１０は、第一の半セクション１１ａの遠位端を通って延びる第一の流体送達チャネル１６２と、第二の半セクション１１ｂの遠位端を通って延びる第二の流体送達チャネル１６４とを備え得る。望ましくは、第一の流体送達チャネル１６２は、好ましくは、少なくともいくらか、第一の半セクション１１ａの遠位端の組織接触面に形成されたステープル受容／形成ポケット１６６の各々と流体連絡している。さらに、第二の流体送達チャネル１６４は、好ましくは、少なくともいくらか、第二の半セクション１１ｂの遠位端の組織接触面に提供されたステープル保持スロット１６８の各々と流体連絡している。外科用ステープル１７０は、望ましくは、ステープル保持スロット１６８の各々の内部に配置される。従来どおり、ステープル保持スロット１６８の各々は、ステープル受容／形成ポケット１６６の各々と並列して整列される。
チャネル１６２、１６４の各々は、例えば、上記のように、同じか、または、個々の流体供給源（図示されず）に対して流体接続される。この様式において、創傷治療物質「Ｗ」の形態の流体が、それぞれのチャネル１６２、１６４を介して、ステープル保持スロット１６８、および／またはステープル保持／形成ポケット１６６へと送達され得る。
従って、使用時に、外科用装置１０を始動させると、ステープル１７０は、ステープル保持スロット１６８から配置されて、組織を通して穿通し、そして、ステープラー受容／形成ポケット１６６内に形成される。それと同時に、創傷治療物質「Ｗ」が、上記のように、ステープル保持スロット１６８および／またはステープル受容／形成ポケット１６６から分配される。
本明細書中に開示される外科用装置の実施形態に対して、種々の改変がなされ得ることが理解される。従って、上記の説明は、制限するものとしてみなされるべきではなく、単に、実施形態を例示するものとしてみなされるべきである。当業者は、本開示の範囲内で他の改変を想定する。

Claims (21)

  1. 外科用装置であって、
    その遠位端にディスポーザブル充填ユニットを受容するように適合された、第一の半セクション；
    該第一の半セクションと並列した関係の、第二の半セクション；
    該第一の半セクションの遠位端と、選択的かつ作動可能に係合できるディスポーザブル充填ユニットであって、該ディスポーザブル充填ユニットは、以下：
    カートリッジと；
    該カートリッジ内に支持される複数の配置可能な針であって、該針の各々は、内部を通って延びる管腔を備え、そして、該針の外周には、少なくとも１つの穴が形成されている、針と；
    該カートリッジから、標的組織内へと該針の各々を配置するために、該針の各々と作動可能に関連した針プッシャーと；
    該針プッシャーの各々に駆動力を送達して該カートリッジから該針を配置するために、該カートリッジ内に並進可能に配置された作動部材と；
    を備える、ディスポーザブル充填ユニット；ならびに
    該標的外科手術部位へと創傷治療物質を送達するための、創傷治療物質適用器アセンブリであって、該適用器アセンブリは、以下：
    該第一および該第二の半セクションのそれぞれの遠位端上に支持された、第一および第二のレザバと；
    該レザバの各々と流体連絡した、創傷治療物質の供給源と
    を備える、適用器アセンブリを備え、
    ここで、該創傷治療物質適用器アセンブリの第一および第二のレザバが該ディスポーザブル充填ユニットの該針を通って延びる管腔と流体連絡している、外科用装置。
  2. 前記創傷治療物質の供給源が、少なくとも一つの創傷治療物質の加圧型キャニスタである、請求項１に記載の外科用装置。
  3. 前記キャニスタの各々が、前記第一の半セクションおよび前記第二の半セクションのうちの一方の上に支持されている、請求項２に記載の外科用装置。
  4. 前記針プッシャーの各々が、前記第一および第二の流体レザバが前記針のそれぞれの管腔と流体連絡するために、内部に形成された開口部を備える、請求項３に記載の外科用装置。
  5. 前記レザバから前記カートリッジへの創傷治療物質の伝送を可能にするために、前記第一の半セクションの遠位端が、該レザバと該レザバ上に支持された該カートリッジとの間に提供される複数の開口部を備える、請求項４に記載の外科用装置。
  6. 前記第二の半セクションの遠位端の上に、複数の針受容開口部を備えるプレートが支持されている、請求項５に記載の外科用装置。
  7. 請求項６に記載の外科用装置であって、該外科用装置の作動中に、前記針の各々の遠位端が、前記第二の半セクションのそれぞれの針受容開口部に入る、外科用装置。
  8. 請求項７に記載の外科用装置であって、エラストマー材料が、前記第二の半セクションの遠位端上に支持された前記レザバ内に前記創傷治療物質を閉じ込める、外科用装置。
  9. 請求項８に記載の外科用装置であって、該外科用装置の起動時に、前記針の各々の遠位端が、前記エラストマー材料を貫通し、そして、該針の各々の管腔が、前記第二の半セクションの遠位端上に支持された前記レザバと流体連絡する、外科用装置。
  10. 前記創傷治療物質が、接着剤、シーラント、止血剤、および医薬品のうちの少なくとも一つである、請求項９に記載の外科用装置。
  11. 前記ディスポーザブル充填ユニットが、前記針を引き戻された状態に付勢するために、該針の少なくとも一つと作動可能に関連した付勢部材を備える、請求項１０に記載の外科用装置。
  12. 請求項１１に記載の外科用装置であって、前記第一の半セクションの遠位端と前記第二の半セクションの遠位端との間に挟まれた組織を切断するための、作動スレッドの上に支持されたナイフブレードをさらに備え、該ナイフブレードは、前記カートリッジ内に提供された長手軸方向のスロットに沿って移動する、外科用装置。
  13. 前記針のうち少なくともいくつかは、前記カートリッジの長手軸方向のスロットのいずれかの側に配置される、請求項１２に記載の外科用装置。
  14. 前記適用器アセンブリは、前記創傷治療物質の供給源と、前記レザバとの間に流体的に挿入されたバルブを備える、請求項１に記載の外科用装置。
  15. 標的の外科手術部位において組織を手術するためのシステムであって、該システムは、以下：
    以下：
    第一の半セクションの遠位端内に選択的かつ作動可能に支持されることが可能なディスポーザブル充填ユニットであって、該ディスポーザブル充填ユニットは、複数の配置可能な針を作動可能に支持するカートリッジと、該針の各々に駆動力を送達して該カートリッジから該針を配置するために、該カートリッジ内に並進可能に配置された作動部材と、を有し、該針の各々は、内部を通って延びる管腔を備え、そして、その外周には、少なくとも一つの穴が形成されている、ディスポーザブル充填ユニットと；
    該標的の外科手術部位に創傷治療物質を送達するための、創傷治療物質適用器アセンブリであって、該適用器アセンブリは、該第一の半セクションおよび該第二の半セクションのうちの一方の遠位端の上に支持された少なくとも一つのレザバと、該レザバの各々と流体連絡する創傷治療物質の供給源とを有する、適用器アセンブリと；を備える、外科用装置と；
    該外科用装置の第一の半セクションの遠位端と第二の半セクションの遠位端との間に、組織の隣接する層を挟むための手段と；
    該外科用装置を始動させるための手段であって、該外科用装置を始動させた際に、該作動部材が遠位方向に進められ、該挟まれた組織に複数の針を打ち込み、それによって、該複数の針を通して、該創傷治療物質の供給源から、該挟まれた組織へと創傷治療物質を分配するように構成されている、手段と、を備え、
    ここで、該創傷治療物質適用器アセンブリの第一および第二のレザバの少なくとも一つが該ディスポーザブル充填ユニットの該針を通って延びる管腔と流体連絡している、システム。
  16. 前記創傷治療物質の供給源が加圧され、そして、前記外科用装置を始動させた際に、該創傷治療物質が、該供給源から排出される、請求項１５に記載のシステム。
  17. 前記外科用装置を始動させた際に、前記針の各々の遠位端が、前記第二の半セクションの遠位端に入る、請求項１６に記載のシステム。
  18. 前記外科用装置が、前記第一の半セクションおよび前記第二の半セクションの各々の遠位端の上に支持されたレザバを備える、請求項１７に記載のシステム。
  19. 前記創傷治療物質が、前記レザバの各々から、前記針の各々の管腔へと送達される、請求項１８に記載のシステム。
  20. 前記外科用装置がさらに、前記第一の半セクションの遠位端と前記第二の半セクションの遠位端との間に挟まれた組織を切断するために、該第一の半セクションおよび該第二の半セクションの遠位端に沿って並進移動可能なナイフブレードを備え、該外科用装置を始動させた際に、該ナイフブレードが、該第一の半セクションおよび該第二の半セクションの遠位端に沿って遠位に進められ、該第一の半セクションの遠位端と該第二の半セクションの遠位端との間に挟まれた組織を切断するように構成される、請求項１９に記載のシステム。
  21. 前記創傷治療物質が、接着剤、シーラント、止血剤および医薬品のうちの少なくとも一つである、請求項１５に記載のシステム。
