Description
本発明は、甲状腺の自己免疫疾患、詳細には、グレーブス病で生じるTSH受容体自己抗体(TSHR-Auto-Ab)を測定する、改善された受容体結合アッセイに関する。

TSH = 甲状腺刺激ホルモン。略語TSHが、別段の付加なしに用いられる場合、それは特定の産物に関するものではなく、このホルモンの結合または機能の一般的な形態を論じるものである。
(1 グレーブス病患者の血清からのアフィニティー精製ポリクローナルヒト自己抗体試料の産生)
(1.1.結合TSHRを有するアフィニティーカラムの調製)
アフィニティー精製形態にある公知のモノクローナル立体構造抗体BA8(Sabine Costagliolaら、「Second Generation Assay for Thyrotropin Receptor Antibodies Has Superior Diagnostic Sensitivity for Graves' Disease」、J Clin Endocrinol Metab、第84巻、90〜97頁、1999年;91頁「Selection of moAb by FACS」;およびS. Costagliolaら、「Genetic Immunization Against the Human Thyrotropic Receptor Causes Thyroiditis and Allows Production of Monoclonal Antibodies Recognizing the Native Receptor」、The Journal of Immunology、1998年、第160巻、1458〜1465頁)20mgを2mg/mlで含むリン酸緩衝食塩水溶液(PBS)と、NaIO4 50mgとを混合し、20分間、室温で、インキュベートし、PBSで平衡化されたNAP25カラム(Pharmacia社)を用いて脱塩した。得られたタンパク質画分(14ml)を、PBSで洗浄し、Carbolink(商標)物質(5mlゲル;Pierce社)と混合し、穏やかに振盪させながら4℃で20時間インキュベートした。
1.1に従って得られた洗浄済み固相物質を、グレーブス病患者の血清プール400ml(平均濃度190U/l、全75U)に懸濁し、穏やかに振盪させながら、4℃で10時間インキュベートした。

1.2に記載したアフィニティー精製自己抗体30μl(100μg)を、1M NaPO4(pH7.2) 70μlで中和し、NHSアクリジニウムエステル(Behringwerke AG社Marburg;欧州特許第0257541号参照;アセトニトリル中1mg/ml) 10μlと混合し、室温で、10分間インキュベートした。その後、過剰のアクリジニウム活性エステルを、0.1グリシン/NaOH(pH7.2) 10μlで飽和させ、結合していないアクリジニウムエステルを、SW300カラム(Millipore社)を用いてゲル濾過HPLCによって沈降させた。280nmおよび368nmにUV吸収を有するカラム流出液の全画分を収集した。プールされたタンパク質画分は、受容体結合アッセイでさらに使用する必要が生じるまで-80℃で保存した。
1.3.1の別法として、アフィニティー精製自己抗体を、アクリジニウムエステルで標識されたヤギ抗ヒトIgGと、1:1のモル比で反応させた(グレード2、Scantibodies社)(1% BSA、PBS 10ml中に自己抗体100μgおよびヤギ抗ヒトアクリジニウムエステル100μg)。2時間インキュベートした後、この物質を、PBS、1% BSA、および10%ヒト血清からなる緩衝液中に、100μlあたり20ng(約0.25mU抗体)となるように溶出した。
使用された標準物質は、本出願人のLUMItest(登録商標)TRAKの標準物質であった。
グレーブス病患者の血清から得たアフィニティー精製自己抗体に基づく標識された新規のトレーサー試料を評価するために、以下の順序のピペット操作およびインキュベーションステップを用いて測定を行った。
2.そこに、直接標識(1.3.1)または間接標識(1.3.2)されたアフィニティー精製自己抗体を、いずれの場合でも100μlピペットで入れる。
3.Heidolph社製オービタルシェーカー(300rpm)で、室温で、2時間インキュベートする。
4.洗浄溶液(PBS)1mlをピペットで入れる。
5.デカント。
6.ステップ4および5をそれぞれ3回反復する。
7.発光反応を引き起こし、化学発光光を発生させ、適当なルミノメータ、例えば、Berthold社製LB952T/16を用いることによって、試験管の壁に残存するアクリジニウムエステルを測定する。
8.患者血清中のTSHR-Auto-Abを測定する。
組換えヒトTSHRは、記載の通り、2%トリトン、10%グリセロール、10mM EDTA、0.5% BSA、pH7.4を含むHEPESで、K562細胞から抽出し、抽出物として22℃で保存した。
、これらの結果は、公知のアッセイにおける場合と比べて、測定精度が時間に影響される程度が非常に小さくなっていることを意味する。
グレーブス病血清から得られた標識されたアフィニティー精製ポリクローナル自己抗体試料を競合物質として用いたアッセイは、それが相同アッセイの特性によるものなので、グレーブス病自己抗体と実際に競合する動物抗体、詳細にはモノクローナル抗体の選択を可能にするものである。そのような動物抗体は、これまで、いまだに記載のない、刺激性作用を有する抗体である可能性もある。
Claims (5)
- 生化学的に均質な状態にまで精製され、少なくとも1IU/mgのタンパク質(ヒト免疫グロブリン)比活性を有し、かつアフィニティークロマトグラフィーによる精製で得られ、前記精製中に、グレーブス病患者の血清プールから得られた自己抗体が、本質的に完全に機能的なヒト組換えTSH受容体を結合した親和性物質に結合され、タンパク質を含まなくなるまで洗浄され、その後、前記親和性物質から溶出される、患者試料中の測定されるべき抗ヒトTSH受容体自己抗体に対する競合物質としての、免疫学的競合アッセイ方法における、直接標識または間接標識された形態にある、抗TSH受容体アフィニティー精製ポリクローナルヒト自己抗体(TSHR-Auto-Ab)の使用。
- 前記免疫学的アッセイ方法が競合的方法であり、前記競合的方法において、固相に結合した形態、または可溶化された形態にある機能的な組換えヒトTSH受容体試料が、測定されるべき自己抗体、および、グレーブス病患者の血清から得られ、かつ競合物質として使用される前記抗TSH受容体アフィニティー精製ポリクローナルヒト自己抗体(TSHR-Auto-Ab)の特異的結合パートナーとして用いられる、請求項1に記載の使用。
- グレーブス病患者の血清から得られた前記抗TSH受容体アフィニティー精製ポリクローナルヒト自己抗体(TSHR-Auto-Ab)が、ラジオ同位元素、化学発光標識、または蛍光標識で標識された形態で用いられるか、または同様に標識された動物抗ヒトIgGの補助のもとに用いられることを特徴とする、請求項1または2に記載の使用。
- 患者血清中にある抗TSH受容体自己抗体を免疫学的に測定する臨床診断用試薬キットであって、
希釈および/または緩衝溶液、標準物質及びゼロ標準物質に加えて、
2つの追加構成物として、可溶化されているか、または固相に結合されているヒトTSH受容体と、グレーブス病患者の血清から得られた抗TSH受容体アフィニティー精製ポリクローナルヒト自己抗体(TSHR-Auto-Ab)であって、本質的に完全に機能的なヒト組換えTSH受容体を結合した親和性物質を利用して生化学的に均質な状態にまでアフィニティー精製され、標識された形態で、少なくとも1IU/mgタンパク質(ヒト免疫グロブリン)の比活性を有する抗体とを含む試薬キット。 - 対照血清をさらに含む、請求項4に記載の試薬キット。
