【発明の属する技術分野】
この発明は、栄養の補給、輸血、点滴時等に人体に装着されるチューブを患者等が引き抜くのを遅延もしくは防止するチューブの引き抜き防止装置に関する。

【従来の技術】
栄養分を補給するためのチューブは、自力では食事をとることのできない、または何らかの理由により口から物を飲み込めない患者等に使用され、患者の鼻腔から挿入して液体状の栄養分を胃に直接供給できるようにしたものである。

【発明が解決しようとする課題】
しかしながら、このようなチューブは、装着に慣れないと不快感を感じるため、介護者が目を離した隙等に患者が鼻腔から引き抜いてしまうという問題があった。また、患者によっては、食事のたびにチューブを引き抜いてしまうため、その度に挿入し直さなければならず、患者および介護者双方の負担が増大してしまうという問題があった。

さらに、食事中にチューブが中途半端に抜けてしまうと、該チューブが気管に入ってしまい、摂取中の栄養剤が肺に入ってしまうという危険もあった。また、輸血時や点滴時に使用されるチューブも同様に、患者が引き抜いてしまうという危険性があった。

そこで、以上のような問題を解決するために、従来は栄養を補給するためのチューブに関しては、引き抜くことによって危険が生じる可能性が高い食事中に限り、患者の手を縛ったり、特別な手袋を装着させることによってチューブを直接把持できないようにするという措置を取っていた。

しかしながら、このような方法は患者に対して肉体的、精神的に不快を感じさせるだけでなく、人権侵害にあたるとされ、問題視されている。また、輸血時や点滴時に用いるチューブの場合にあっては、有効な対策がないのが実情であった。

この発明は、上記に鑑みてなされたものであって、患者等のチューブ装着者に精神的な負担を与えることなく、チューブの引き抜きを遅延もしくは防止できるチューブの引き抜き防止装置を得ることを目的とする。

また、この発明は、チューブの引き抜き動作時に患者や介護者等に警告できるチューブの引き抜き防止装置を得ることを目的とする。

【課題を解決するための手段】
上述した課題を解決し、目的を達成するために、この発明にかかるチューブの引き抜き防止装置は、人体に装着されるチューブの一部を伸縮自在に形成したカバー部材で覆い、当該カバー部材を当該チューブに対して相対移動可能に形成したものである。

チューブの引き抜き防止装置を装着した患者が不快感により意識的または無意識的にチューブを引き抜こうとする状態を仮定する。この場合、チューブはカバー部材に覆われているので、患者はチューブに直接触れることができず、当該カバー部材をつかむこととなる。このため、チューブを人体から引き抜こうとしても、カバー部材がチューブに対して相対移動して伸長するだけなので、人体から抜けることはない。これにより、チューブの引き抜きを効果的に遅延もしくは防止できる。

つぎの発明にかかるチューブの引き抜き防止装置は、少なくともカバー部材の一部に設置され所定以上の圧力が加わったことを検知して検知信号を出力する圧力検知手段と、前記圧力検知手段の検知信号に基づいて警告を発する警告手段とを備えたものである。

【発明の実施の形態】
以下、この発明にかかるチューブの引き抜き防止装置の実施の形態を図面に基づいて詳細に説明する。なお、この実施の形態によりこの発明が限定されるものではない。

実施の形態1.
図1は、この発明の実施の形態1にかかるチューブの引き抜き防止装置を示す説明図である。なお、以下の説明においては、自力で食事をとるのが困難な患者に対する栄養補給を例にして説明する。チューブ1の一端は、栄養セット3に接続されており、他端は患者の鼻腔から体内に挿入されている。ここで、栄養セット3は、患者が摂取する栄養剤を入れるための容器である。なお、チューブ1は、栄養セット3からの栄養の補給量を調整する図示しない活栓が設けられる。

コイル状かつ伸縮自在に形成された伸縮カバー2は、上記チューブ1を覆うものであり、後述するように一端をチューブ1に固定されている。スイッチ部4は、チューブ1に設置され、たとえばマイクロスイッチ等のセンサにて構成されており、伸縮カバー2の伸長によってその端部が触ると、当該スイッチがONとなるように形成されている。また、スイッチがONになると、警報装置5に信号を送信するように形成されている。警報装置5は、たとえばスピーカにて構成され、スイッチ部4から信号が入力すると、ブザー等の所定の警告音や音声等を出力するようになっている。

つぎに、伸縮カバー2およびスイッチ部4について図2を用いてさらに詳細に説明する。ここで、図2は、伸縮カバーおよびスイッチ部の設置状態を示す説明図である。伸縮カバー2は、樹脂等の素材をコイル状に巻いて形成したものであり、その端部aを患者の鼻腔近傍のチューブ1に固定してある。なお、この端部aのチューブ1への固定手段は、テープ等によって固定したり、あるいはチューブ1および端部aの両者に凹凸状の係合部分を設けて固定してもよい。

一方、チューブ1の端部bは自由端としてあり、伸縮カバー2は図中のA方向に伸長できるようになっている。スイッチ部4のチューブ1への設置位置Pは、伸縮カバー2が所定の長さ伸長すると、その端部bがスイッチ部4をONにする位置に設定してある。ここで、上記所定の長さとは、伸縮カバー2の端部aに加わる力がチューブ1を鼻腔から引き抜く力よりも小さくなるような長さである。

つぎに動作について説明する。患者が不快感により意識的または無意識的にチューブ1を引き抜こうとする状態を仮定する。この場合、チューブ1は伸縮カバー2に覆われているので、患者はチューブ1に直接触れることができず、伸縮カバー2をつかむこととなる。このため、チューブ1を鼻腔から引き抜こうとしても、上記所定の力が加わるまでは伸縮カバー2が伸長するだけで鼻腔から抜けることはない。

伸縮カバー2がスイッチ部4の設置位置Pまで伸長すると、端部bが当接してスイッチ部4がONになるので、スイッチ部4は警報装置5に信号を送信し、その信号を受信した警報装置5は警告音(たとえば、ブザー音)を発する。この警告音は、チューブ1を意識的に引き抜こうとする患者に対しては、その動作を中止するように促すことができるとともに、無意識的に引き抜こうとしている患者に対しても同様に、注意を喚起することができる。

さらに、患者がチューブ1の引き抜き動作をしていることを介護者等に知らせることもできるので、チューブ1が引き抜かれるのを未然に防止することができる。また、仮にチューブ1が引き抜かれてしまった場合であっても、その後の処置に迅速に対応することができる。

なお、本装置を病院等で用いる場合には、警報装置5を遠隔のナースセンタに設置するようにしてもよいし、患者の近隣とナースセンタのような遠隔の場所との両方に設置するようにしてもよい。これらは、無線または有線の通信手段を介して実現できる。

本実施の形態2は、スイッチ部4を、接点たるリード部4a,4bを備えたリードスイッチとして構成し、伸縮カバー2の伸長によりリード部4a,4bが開くことで、チューブ1の引き抜き動作を検知できるようにしたものである。このスイッチ部4は、図4および図5に示すように、伸縮カバー2の端部aの近傍に設置されている。

また、図5および図6に示すように、リード部4a,4bは、伸縮カバー2の1組の隣接するコイル部に設けられており、通常は、接した状態で警報装置5との間でループ回路を構成し、当該警報装置5によって微弱電流が流されている。

つぎに動作について説明する。患者がチューブ1を鼻腔から引き抜こうとしても、チューブ1が伸縮カバー2に覆われているので、患者はチューブ1に直接触れることができず、伸縮カバー2をつかむこととなる。このため、チューブ1を鼻腔から引き抜こうとしても、所定の力が加わるまでは伸縮カバー2が図4中のB方向に伸長するだけで鼻腔から抜けることはない。

伸縮カバー2に所定以上の力が付与され、伸縮カバー2が伸長されると、前記隣接するコイル部が離れるので、リード部4a,4bが離れて接点が開く。すると、上記ループ回路に電流が流れなくなり、これを警報装置5が検知して所定の警告音またはメッセージ等を出力する。なお、誤検知や失検知を減少させるために、リード部4a,4bが設置されるコイルのそれぞれにいわゆるマジックテープや磁石を設けて、当該リード部4a,4bが接点を開く力を調節するようにしてもよい。

ここで、図8を用いて接点カバー10について説明する。接点カバー10はカバー部11、導体部12、不導体部13、スペーサ14から構成され、カバー部11の上には図8aに示すように導体部12、不導体部13、スペーサ14が設置される。カバー部11はガーゼ等で構成されている。導体部12は金属等の電気を通すものであれば何であってもよい。不導体部13は電気を通さない伸縮性のある物質で構成され、本実施例では網状のポリウレタンを用いている。スペーサ14も不導体部13と同様に電気を通さない伸縮性のある物質で構成され、本実施例ではスポンジを用い、図8aに示すように各導体部12の間に設置される。図8bは接点カバー10を側面から見た図であり、スペーサ14は導体部12よりも高さがあり、不導体部13を導体部12より浮かせるように構成している。図8cは接点カバー10を伸縮カバー2に巻きつけたときの断面図である。この図に示すように、導体部12は不導体部13とスペーサ14によって伸縮カバー2には接触しないようになっている。

つぎに図9を用いて伸縮カバー2に設置されたリード線20a、20bについて説明する。リード線20a、20bはそれぞれ、伸縮カバー2のコイルに所定の距離をおいて設置される。ここで所定の距離とは、リード線20aが図8aにおける導体部12の上端a、リード線20bが導体部12の下端bに合う位置である。また、図7、図9に示すようにリード線20a、20bは警報装置5との間でループ回路を構成し、当該警報装置5によって微弱電流が流されている。

つぎに接点カバー10とリード線20a、20bの関係について説明する。接点カバー10は図7に示すように伸縮カバー2に設置されるが、その位置は各導体部12の上端aにリード線20aが、下端bにリード線20bが合うように巻きつける。この状態の断面を示すのが図8cであり、前述したとおり、通常はリード線20a、20bと各導体部12は不導体部13およびスペーサ14によって接触しないようになっている。ところが、接点カバー10に一定以上の圧力が加わると、不導体部13およびスペーサ14が押しつぶされ、リード線20a、20bと各導体部12が接触するようになる。このとき、ループ回路を構成するリード線20a、20bに電流が流れるようになる。

つぎに動作について説明する。警報装置5はリード線20a、20bのループ回路に微弱電流を流そうとするが、通常はリード線20a、20bが離れているため電流は流れない。ここで、患者がチューブ1を鼻腔から引き抜こうとしたとする。このとき患者は、まずチューブ1を掴む動作を行う。そこで、患者が接点カバー10を掴むと、当該接点カバー10の不導体部13およびスペーサ14がその圧力によって押しつぶされ、導体部12とリード線20a、20bが接触する。すると、上述したループ回路に電流が流れるようになるので、これを警報装置5が検知して所定の警告音またはメッセージ等を出力する。
