本発明は、動脈血中のヘモグロビンの濃度を測定する血液測定装置に関する。

【従来の技術】
貧血検査は、スポーツ選手のみならず、一般の人でも健康管理する指標として重要である。貧血検査は、動脈血中のヘモグロビンの濃度を測定する検査であり従来、採血による方法が採られている。採血は、伝染病等の感染の恐れがあったり、病院や保健所等に出向く必要があったり、有資格者にしか行えなかったり、あるいは、検査結果がでるまでに時間を要したりと、様々な問題がある。

そこで、採血による方法でなく、光学的な方法による血液測定装置が開発されている。このような血液測定装置の中には、動脈血中のヘモグロビンを測定する装置もあり、例えば、非侵襲的にヘモグロビンを測定するパルスオキシメータがある。パルスオキシメータは、酸化ヘモグロビンと還元ヘモグロビンとにおける赤色光と赤外光とに対する吸光度の違いを利用してヘモグロビンの酸素飽和度を測定する装置である。このパルスオキシメータの一般的な構成は、例えば、特許文献1に開示されている。この他にも、光学的な血液測定装置について開示している文献がある(例えば、特許文献2〜4参照)。

【発明が解決しようとする課題】
しかしながら、従来のパルスオキシメータでは、CW(Continuous Wave:連続波)光を用いているので、動脈の脈動による動脈血管径の変化を検出することができない。そのため、従来のパルスオキシメータでは、ヘモグロビンの濃度を測定することができなかった。

そこで、本発明は、ヘモグロビンの濃度を測定することができる血液測定装置を提供することを課題とする。

【課題を解決するための手段】
本発明に係る血液測定装置は、動脈血中のヘモグロビンを光学的に測定する血液測定装置であって、第1波長の第1検査光を変調した第1入射光を生体内の動脈に向けて入射する第1光入射手段と、第1波長とは異なる第2波長の第2検査光を変調した第2入射光を生体内の動脈に向けて入射する第2光入射手段と、生体から出た第1波長の第1検出光を検出する第1検出手段と、生体から出た第2波長の第2検出光を検出する第2検出手段と、第1検出光及び第2検出光に基づいてヘモグロビンの酸素飽和度を算出するとともに第1検査光又は第2検査光の動脈拡張時と動脈収縮時との変化に基づいて動脈拡張時と動脈収縮時との光路長の変化を算出し、酸素飽和度及び光路長の変化に基づいてヘモグロビンの濃度を算出する処理手段とを備えることを特徴とする。

【発明の実施の形態】
以下、図面を参照して、本発明に係る血液測定装置の実施の形態を説明する。

本発明は、非侵襲的にヘモグロビンの酸素飽和度のみならずヘモグロビンの濃度も測定するために、ヘモグロビンの濃度の算出に必要な動脈収縮時の動脈血管径と動脈拡張時の動脈血管径との差に相当する動脈収縮時と動脈拡張時との光路長差を求める構成を有する。そのために、本発明では、2つ異なる波長の検査光を強度変調して生体内の動脈に向かって入射させ、動脈を通過した光の動脈収縮時と動脈拡張時との位相変化から光路長差を求める。また、本発明では、2つ異なる波長の検査光をパルス駆動して生体内の動脈に向かって入射させ、動脈を通過した光の動脈収縮時と動脈拡張時との時間変化から光路長差を求める。

本実施の形態では、本発明に係る血液測定装置を、動脈血液中のヘモグロビンの酸素飽和度及びヘモグロビン濃度を非侵襲的に測定する2つの形態のパルスオキシメータに適用する。第1の実施の形態に係るパルスオキシメータは、赤色光及び赤外光を強度変調して指に入射し、指から出た光の動脈収縮時と動脈拡張時との位相変化を求めるための手段を備えている。一方、第2の実施の形態に係るパルスオキシメータは、赤色光及び赤外光をパルス駆動して指に入射し、指から出た光の動脈収縮時と動脈拡張時との時間変化を求めるための手段を備えている。

本実施の形態に係るパルスオキシメータについて説明する前に、図7乃至図10を参照して、パルスオキシメータの原理について説明しておく。図7は、酸化ヘモグロビンと還元ヘモグロビンの吸光スペクトルである。図8は、パルスオキシメータにおける吸光度を示す図である。図9は、パルスオキシメータにおけるヘモグロビンの酸素飽和度に対する赤色光及び赤外光の波形を示す図である。図10は、パルスオキシメータにおける入射光の光強度と検出光の光強度との関係を示す図であり、(a)が動脈収縮時であり、(b)が動脈拡張時である。

血液は、ヘモグロビンの作用によって、酸素を多く含むほど明るい赤色となり、酸素が少ないほど暗い赤色になる。明るい赤色の血液は、赤色光をよく通し、赤外光を通し難い。図7は、横軸が光の波長、縦軸が吸光度であり、明るい赤色である酸化ヘモグロビン(破線)と暗い赤色である還元ヘモグロビン(実線)の吸光度を示している。酸化ヘモグロビンは、赤外光(940nm付近)に対する吸光度が大きく、赤外光を通し難く、赤色光を通し易い。一方、還元ヘモグロビンは、赤色光(660nm付近)に対する吸光度が大きく、赤色光を通し難く、赤外光を通し易い。

また、吸光度は、動脈の脈動によって変化する。図8は、動脈が脈動している場合の生体内の組織、静脈血、動脈血での各吸光度を示している。組織での吸光度(X部分)及び静脈血での吸光度(Y部分)は、脈動に関係なく、一定である。しかし、動脈血での吸光度(Z部分)は、脈動に応じて変化する。これは、脈動に応じて動脈血管が収縮と拡張とを繰り返すため、収縮時には動脈血管における光路長が短くなり、血液量の減少により吸光度が小さくなり、一方、拡張時には動脈血管における光路長が長くなり、血液量の増加により吸光度が大きくなるからである。

さらに、図9には、動脈血に含まれるヘモグロビンの酸素飽和度(SaO2)が0%、85%、100%の場合の赤色光(660nm)と赤外光(940nm)との図8のX部分の脈動波形及びその赤色光の脈動波成分に対する赤外光の脈動波成分の比(R/IR比)を示している。図9から判るように、赤色光の脈動波の振幅が大きいほど(赤色光の吸光度が大きいほど)、酸素飽和度が小さく(すなわち、還元ヘモグロビンが多い)、赤外光の脈動波が大きいほど(赤外光の吸光度が大きいほど)、酸素飽和度が大きい(すなわち、酸化ヘモグロビンが多い)。つまり、パルスオキシメータでは、赤色光を多く検出できるほど酸素飽和度が大きく、赤外光を多く検出できるほど酸素飽和度が小さい。

パルスオキシメータでは、上記に示す酸化ヘモグロビンと還元ヘモグロビンとにおける赤色光と赤外光とに対する吸光度の違い及び動脈の脈動による吸光度の変化を利用して、ヘモグロビンの酸素飽和度を測定している。以下に、パルスオキシメータにおけるヘモグロビンの酸素飽和度の算出式について説明する。

ここで、動脈血管内を通る光は直進するとみなし、動脈血管内での光路長が動脈血管径に等しいとする。というのは、動脈血管内では血液濃度が非常に高いため、少しでも散乱された光は吸収され、パルスオキシメータの検出器には散乱光が到達しないと考えられるからである。したがって、散乱の影響を殆ど受けない直進した光のみを検出器に到達するとみなすことができる。そのため、動脈血管内での最大時の光路長と最小時の光路長との差は、拡張時の動脈血管径と収縮時の動脈血管径との差ΔDとみなすことができる。

ヘモグロビンの濃度Cは、式(8)から変形した式(10)又は式(11)によって算出することができる。式(10)、式(11)によってヘモグロビンの濃度Cを算出する場合、拡張時の動脈血管径と収縮時の動脈血管径との差ΔDが必要となる。本実施の形態に係るパルスオキシメータでは、ヘモグロビンの酸素飽和度Yを算出した後に、式(10)、式(11)中のΔDを動脈血管内での最大時の光路長と最小時の光路長との差としてヘモグロビンの濃度Cを算出する。そのため、本実施の形態に係るパルスオキシメータは、動脈血管内での最大時の光路長と最小時の光路長との差を求める手段を有している。

それでは、図1を参照して、本実施の形態に係るパルスオキシメータ1について説明する。図1は、パルスオキシメータの外観図である。

パルスオキシメータ1は、装置本体2とプローブ3とを備えており、装置本体2とプローブ3とがケーブル4で接続されている。パルスオキシメータ1では、プローブ3に挿入された指Fに向けて赤色光(660nm)と赤外光(940nm)とを入射させ、指Fを通過した赤色光(660nm)と赤外光(940nm)とを検出する。さらに、パルスオキシメータ1では、装置本体2において、その検出した赤色光(660nm)と赤外光(940nm)とに基づいてヘモグロビンの酸素飽和度及び濃度を求め、装置本体2の前面の表示部2aにヘモグロビンの酸素飽和度(SaO2:%)、ヘモグロビンの濃度(Hb:g/dl)を表示する。ちなみに、図1に示す表示部2aにはパルスレート(パルス/分)も表示しているが、これは第2の実施の形態に係るパルスオキシメータにおけるパルス駆動時の毎分当たりの発振パルス数である。なお、パルスオキシメータ1では、酸化ヘモグロビンの濃度及び還元ヘモグロビンの濃度を求め、表示するようにしてもよい。

装置本体2は、各種回路、データプロセッサ、ディスプレイドライバ、ディスプレイ等を備えている。装置本体2では、プローブ3の光源を駆動するとともに、プローブ3で検出した検出光に各種処理を施す。そして、装置本体2では、その検出光に基づくデータを用いてヘモグロビンの酸素飽和度及び濃度を算出し、ヘモグロビンの酸素飽和度及び濃度を表示する。
