本発明は、血球成分の変形を抑える機能を有する気泡除去チャンバー(ドリップチャンバーともいう。)に関するもので、更に詳しくは本発明は、体外循環回路用サイドキャップの内壁に形成された平面部に体液(血液)を意図的に沿わせ、体液自身の流れをスムーズにし、チャンバー内の泡立ちを極力防止して、赤血球を壊れ難くして体外循環回路内におけるコアグレーション・フィブリン(以下、コアグラ・フィブリンという)等の凝血乃至血栓を極力防止することができる優れた機能を有する気泡除去チャンバーに関するものである。

【従来の技術】
従来、体外循環回路システムを使用して血液などの人工透析する方法は知られており、近年では、前記体外循環回路システムの気泡除去チャンバー内の側壁に体液を旋回させ、沿わせることにより極力消泡することが行われている。従来技術では、この体液中の気泡除去装置においては、被処理血液を容器本体の中心軸まわりに旋回する旋回流として上部空間に導入する液入口を有している。更に旋回流となった被処理血液は、容器本体の上部空間において旋回しながら外周部側から中央部側に移行する。ここで、旋回流の流速が大なる外周部では、比重の異なる血液と気泡に大なる遠心力が作用することから、血液中の気泡が積極的に遠心分離される。このように遠心分離された血液及び気泡が次第に流速を低下して中央部側に移行するに従い、大きな浮力上昇作用が作用し、気泡は上方に移行し、血液のみが濾過体を通過して下部空間に流入し、液出口から流出する。他方、上部空間において上方に移行した気泡は、上部空間の気泡だまりに一定量集まる毎に開かれる弁を経て空気出口から除去される(例えば、特許文献1参照)。また人工透析システムにおいて、血液の体外循環システムを円滑に作動する貯血用具が開発されており、この貯血用具を構成する円筒状の貯血部にその接線方向に臨むように血液導入用管体を接続し、前記管体を介して血液を貯血部の内周壁面に周回するように構成し、これによって前記血液内に気泡を極力発生させることなく当該貯血部に貯留される血液を容易且つ正確に視認出来ることが記載されている(例えば、特許文献2参照)

一方、現在実用化されている気泡除去チャンバーにおいて、図6に示されるタイプでは、気泡除去チャンバー2の上面に設けられた落し込みチューブ22から血液中へ流入する形式からなり、この形式では、落し込みチューブ22が血液中に導入されているので、血液の流入方向は、血液中を下部方向23と上部方向24の二方向へ流れながら濾過網6を通過して下部から流出する。また図7に示されるタイプでは、気泡除去チャンバー2の上面に設けられた落し込みチューブ22がチャンバー内の血液面の上方に設置され、血液を貯留血液の上面に落とす形式からなる。この形式では、血液を貯留血液の上面に落とすので、その際、血液面にぶつかり、泡が発生するばかりでなく血球成分の破壊される恐れが大きいという問題がある。更に図8に示されるタイプでは、気泡除去チャンバーの側壁部から血液が導入される形式からなり、この形式では、導入血液の速度が遅い場合は、血液の流れ方向27の如く、血液上面にぶつかる形で導入される。また導入血液の速度が速い場合には、チャンバー側壁21に血液が強くぶつかり(26)、さらに血液の流れ方向が26a及び26bへと一部は、壁21に沿って流れ、また直接血液中へ入る。

【発明が解決しようとする課題】
しかしながら、前述のごとき、従来の気泡除去チャンバー乃至貯血用具では、体外循環回路用血流は、標準では、180ml/min〜200ml/minで設定され、また遅い場合には、120ml/min前後であり、更に早い場合では、250ml/min前後に設定されるが、特に、血流が遅い場合、即ち血流が少ない場合には、側壁に沿わないで落下し直接血液レベル面に落ちる場合があり、このことにより気泡が発生する原因となる。また血流が速い場合には、壁に当たる時、赤血球が壊れる恐れがないか、赤血球の溶血はどうなのかなどを考慮する必要がある。また図6に示されるタイプでは、泡が出難く、血球成分が壊れないという性能を有する点で好ましいが、血液中に溶け込んでいる泡が落し込みチューブ22の周囲に付着して残血ができるという問題がある。図7に示されるタイプでは、血液を貯留血液の上面に落とすので、その際、血液面にぶつかり、泡が発生するばかりでなく血球成分の破壊される恐れが大きいという問題がある。更に図8のに示されるタイプでは、導入血液の速度が遅い場合は、血液の流れ方向27の如く、血液上面にぶつかって導入されるので、泡が発生するという問題があり、また導入血液の速度が速い場合には、チャンバー側壁21に血液が強くぶつかり(26)、さらに血液の流れ方向が26a及び26bへと一部は、壁21に沿って流れ、また直接血液中へ入るので、特に血液がチャンバー側壁21にぶつかった時に、血球成分が壊れる恐れが大きいという問題がある。

そこで、本発明者等は、これらの点について種々検討したところ、サイドキャップの円筒内面の一部を平面に形成すると共に、メインチューブ接続部の先端部を片面テーパー状に形成して、メインチューブ接続部からドリップチャンバー内へ流入した血液が前記平面に当たるように形成したことにより、低流量であってもメインチューブ接続部の側面の平面を流れ、血液は直接チャンバー内の血液レベル面に直接落下することがないこと、及び流速が速い場合でも円筒内面に形成された平面に流れるように当たるため赤血球が壊れる恐れが少ないという格別顕著な効果を奏することを見出し、本発明に至った。したがって、本発明が解決しようとする課題は、気泡除去チャンバー内の血流により血液に泡が発生するのを極力防止し、また赤血球を壊れ難くすることにより、体外循環回路内におけるコアグラ・フィブリン等の凝血・血栓を極めて良好に防ぐことができるドリップチャンバーを提供することにある。

本発明の上記課題は、以下の各発明によってそれぞれ達成される。

【発明の実施の形態】
以下に、本発明の実施の形態を図面を用いて説明するが、本発明は、これらの形態のみに限定されるものではない。

図1は、本発明のドリップチャンバーを示す断面図である。図2は、本発明のサイドキャップを示す正面図である。図3は、図2のサイドキャップのA−A′で切断した断面図である。図1乃至図3において、気泡除去チャンバー2は、円筒部5の上端にサイドキャップ1を有し、また下端には下部キャップ3を有し、更に円筒部の底部には濾過網6を有している。ここでサイドキャップ1は、円筒部本体4からなり、該本体の頂部に面調節ライン接続部7及び圧力モニターライン接続部8を有すると共に、外周側面にはメインチューブ接続部9を有している。また図3から明らかなように、サイドキャップ1は、円筒部本体4からなり、また該円筒部本体4の内面41は、メインチューブ接続部9の該接続部延長方向に対しサイドキャップ内面のいずれかの内面に、平面部42が形成されており、この平面部42の平面と平行かつ並列してメインチューブ接続部9が設けられている。このメインチューブ接続部9の先端10は、円筒部本体4の平面部42と平行かつ並列した先端内面43(平面部42に近位側の貫通孔内面部)を有しており、言い換えれば、サイドキャップ1の円筒部本体4の内壁の平面部42とメインチューブ接続部9の先端10の内面43とメインチューブ接続部9の内面は、それぞれ平行面を形成している。前記の内壁の平面部42と内面43とは、並列でもまた段差を有していてもよい。

更にメインチューブ接続部9の先端10の先端内面傾斜部44は、サイドキャップ1の円筒部本体4の内面の平面部42の方向に傾斜した内壁を有しており、このように形成することにより、気泡除去チャンバー内へ流入した血液を意図的にサイドキャップの円筒内面の平面部42に沿わせ、体液(血液)自身の流れをスムーズにする。その結果、血流(体液流)に泡の発生を極力抑えるという優れた効果を奏するものである。メインチューブ接続部9の先端10の内面43に対する内面44の角度は、あまり急峻でなければよく、一応の目安としては、1°〜20°が好ましく、更には、5°〜10°が好ましい。また、メインチューブ接続部9の上下方向への角度は、円筒部5に対して80°〜120°が好ましく、更に好ましくは85°〜120°である。この角度範囲で血流に泡を発生させることなく、また血球成分を破壊することなくドリップチャンバーを通過させることができる。

【実施例】
以下に実施例を示して本発明を更に具体的に説明するが、本発明は、この例によって限定されるものではない。

〔実施例〕透析装置の試装モデルとして、図5に示される試験装置を使用し、使用回路として、本発明の気泡除去チャンバーA,当社(ハナコメディカル株式会社製品)の従来の気泡除去チャンバーB,C及び他社の気泡除去チャンバーDを用いて試験した。図8において、手の代わりにサンプリングポート31、32を使用した。試験装置は、サンプリングポート31、V側チャンバー33、クランプ36、容器37、ポンプ35、サンプリングポート32、A側チャンバー34、サンプリングポート31の順に接続して循環回路を形成した。また容器37には、牛乳(那須山麓牛乳)38を入れ、液の流れが分かるようにサンプリングポート31、32から着色インキ(万年筆用水性インキ:青、クロス社製)を注入し得るように構成した。この試験装置において、A側チャンバー34とV側チャンバー33としては、上記の本発明の気泡除去チャンバーA(図4)、当社の従来の気泡除去チャンバーB(図6)と同C(図7)及び他社の気泡除去チャンバーD(図8)を使用した試験を行った。牛乳の注入速度は、ポンプ35で200ml/分及び100ml/分の2態様でA側チャンバー34からV側チャンバー33方向へ流し、A側チャンバー34の上流部のサンプリングポート32及びV側チャンバー33の上流部のサンプリングポート31に着色インキを注入してチャンバー内の牛乳の流れを観察し、かつ3分間経過後の牛乳の流れの状態を観察した。またV側圧力は、50、100及び150mmHgの3条件で試験を実施した。得られた結果を表1に示す。表1において、○は、インキの滞留がない。▲は、インキの滞留が若干量の残る。□は、インキの滞留層が発生。

表1から明らかなように、本発明の気泡除去チャンバーA(図4)では、サイドキャップ1の内壁の平面を流れ、ついで内筒壁に沿って液20が流れるので、流量を落とした状態でもチャンバー内にインキの滞留部分が発生しないことがわかった。これに対して、当社の従来の気泡除去チャンバーB(図6)では、上部の落し込みチューブ22から直接、液を落とし込むので、液23は下流し、また一部の液24、24は落し込みチューブ22の周囲に流れ、インキの層が発生し、その部分は全く動かない状態であった。また当社の従来の気泡除去チャンバーC(図7)及び他社の気泡除去チャンバーD(図8)では、速い流速の場合には、Cでは、液は液流23、25、25と3方向に流れ、Dでは,液流は26、26a,26bとなり、インキの滞留がないが、流量を遅くした状態では、Dでは、チャンバー内液面付近に若干のインキが残るのを観察した。以上の結果から、本発明の気泡除去チャンバーAは、この試験装置でチャンバー内の流れに滞留部分を生じることがないということは、実際の透析で血液を循環させる場合においても、チャンバー内の血液凝固を実質的に低減させることができることを意味する。

本発明のドリップチャンバーは、円筒部の上端にサイドキャップを有し、また前記円筒部の下端にフィルター部材を内包すると共に下部キャップを有する気泡除去チャンバーにおいて、前記サイドキャップは、円筒部本体と該円筒部本体の外周側面に設けられるメインチューブ接続部とからなり、前記メインチューブ接続部は、該円筒部本体の接線方向に開口部を有し、前記円筒部本体は、内壁の一部を平面とした平面部を有し、該平面部が上記メインチューブ接続部の軸芯の延長方向と平行になるように設けられ、かつ前記サイドキャップはメインチューブ接続位置からサイドキャップの内面へ通じる片面テーパー状の貫通孔を有することにより、ドリップチャンバー内の血流により血液に泡が極力発生するのを防止し、また赤血球を壊れ難くすることができ、かつ該貫通孔のテーパー形状は、前記平面部に近位側の貫通孔内面部が平行に形成され、かつ該貫通孔は、前記軸芯より平面部方向へ傾斜して形成されていることにより、ドリップチャンバー内へ流入した血液を意図的にサイドキャップの円筒内面の平面部に沿わせ、その後、スパイラル状に下方に向かって流下するので、血液自身の流れをスムーズにし、チャンバー内の泡立ちを極力防止し、また赤血球を壊れ難くすることにより体外循環回路内におけるコアグラ・フィブリン等の凝血・血栓を極めて良好に防ぐことができるという格別優れた効果を奏するものである。更に本発明のドリップチャンバーのメインチューブ接続部は、上下方向に角度を有していることで泡の発生を軽減することができるという優れた効果を奏するものである。
