【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明はリドカイン含有外用貼付
剤、さらに詳しくは薬効成分としてリドカインまたはそ
の塩を水溶性高分子に分散または溶解して含有し、該薬
効成分を安定的にかつ長時間にわたって経皮吸収させる
ことができる外用貼付剤に関する。
【０００２】
【従来技術と解決すべき課題】近年、老人に比較的多く
発症する帯状疱疹神経痛及び帯状疱疹後神経痛に対して
神経ブロック療法、針治療、イオントフォレーシス療
法、中枢性鎮痛薬または抗うつ薬投与などが行われてい
る。神経ブロック療法の薬物として、代表的なものにリ
ドカインがある。リドカインは局所麻酔剤として開発さ
れ、表面・浸潤・伝達麻酔作用を有し、主に歯科領域に
おける表面麻酔等に使用されている。また、期外収縮、
急性心筋梗塞や心臓外科手術に伴う心室性不整脈治療の
第一選択薬として広く用いられている薬物である。
【０００３】このリドカインを帯状疱疹神経痛または帯
状疱疹後神経痛に適用するに際しては、例えば神経ブロ
ック療法の場合、ブロック針を用いて硬膜外腔への薬液
注入等が考えられるが、このような方法では下記の如き
問題がある。 (１) 薬物は速やかに患部組織に浸透されるが、血圧、
心拍数及び全身状態をモニターしながら治療する必要が
ある。 (２) 高齢者、心疾患及び肝疾患を有している患者につ
いて全身的に副作用が発現する可能性が高くなる。 (３) 自宅で治療することが不可能なため、治療時ごと
に通院もしくは入院しなければならない。 (４) 一回の治療に長い時間を要する。
【０００４】また、経皮吸収を目的として治療する手段
として、軟膏を塗布することも考えられる。この方法で
は外用貼付剤では貼付しにくい部位(例えば頭部、顔面
等)に塗布できる利点もあるが、その反面、下記の如き
問題がある。 (１) 薬物の定量的投与性に欠け、また手、指、衣類な
どを汚すことが少なくない。これを防止するために、塗
布部をカーゼなどで覆っておく方法もあるが煩雑とな
る。 (２) 製剤中の溶媒が揮散する結果、薬物の結晶が析出
し経皮吸収性が低下する。これを防止するために、適当
なフィルムなどで覆う方法もあるが、この場合、皮膚角
質層の水分が過剰となり、湿疹、蕁麻疹などの皮膚アレ
ルギー症状が現われることが予測される。 (３) 帯状疱疹神経痛または帯状疱疹後神経痛の治療に
おいて、疾患部位に常に薬物が供給されていることが望
まれるが、リドカインは代謝が早いため軟膏では１日数
回の投与が必要である。
【０００５】また、最近注目を浴びているイオントフォ
レーシス療法についても、無痛的局所療法として利点も
多々あるがなお下記の如き問題点がある。 (１) 特殊な機械類を使用するため、治療時ごとに通院
しなければならない。また、治療時において手間と時間
を要する。 (２) 一回の治療範囲の大きさが限定され、また、個人
差により通電条件を変えなければいけない。 (３) ヒト皮膚に対して通電させるため、稀に熱傷など
二次的な副作用が起こる可能性がある。
【０００６】そこで、本発明者らは、経皮投与に適する
外用貼付剤としてリドカインを適用することを検討すべ
く鋭意研究を行なった結果、リドカインを水溶性高分子
に分散または溶解させることにより、粘着性の優れた、
また、薬物の優れた放出制御機能を発揮し、かつ、薬物
を長時間にわたって経皮吸収させることができ、これに
より経皮吸収型の外用貼付剤が得られることを見出し、
本発明を完成させるに至った。
【０００７】
【課題を解決するための手段】本発明は、水溶性高分子
物質、水及び保水剤を必須成分とする粘着性ゲル基剤に
リドカインまたはその塩を１〜１０％(重量％、以下同
様)配合してなる薬物保持層を支持体上に設けたことを
特徴とするリドカイン含有外用貼付剤を提供するもので
ある。
【０００８】本発明で用いる粘着性ゲル基剤は、水溶性
高分子物質、水及び保水剤を必須成分とする。上記水溶
性高分子物質としては、ゼラチン、デンプン、寒天、マ
ンナン、アルギン酸、ポリアクリル酸、ポリアクリル酸
塩、デキストリン、メチルセルロース、メチルセルロー
スナトリウム、カルボキシメチルセルロース、カルボキ
シメチルセルロースナトリウム、ポリビニルアルコー
ル、ポリビニルピロリドン、メチルビニルエーテル−無
水マレイン酸共重合体、アラビアガム、トラガントガ
ム、カラヤガム、ローカストビーンガムなどが挙げら
れ、また、これらの金属塩、並びに有機、無機の架橋剤
により架橋されたものも用いることができる。これらの
水溶性高分子物質は、粘着性ゲル基剤中に使用される他
の原料の物性及び所望の物性を出現させるために用いら
れる。実際に使用するに当たっては、一種または二種以
上の組み合わせにより用いられ、粘着性ゲル基剤中の配
合量としては０．５〜５０％(重量％、以下同じ)が好ま
しく、より好ましくは５〜２５％の範囲で選定すれば良
い。
【０００９】上記粘着性ゲル基剤中の水は、皮膚角質層
の膨潤及び薬物の透過性を向上させるもので、その水分
量は１０〜７０％が好ましく、より好ましくは２０〜５
０％の範囲で選定すればよい。
【００１０】上記保水剤は、粘着性ゲル基剤中の水分量
を一定に保持せしめ、目的貼付剤の保存中あるいは使用
中の水分揮散による皮膚への薬物放出率に対する悪影響
を抑えるもので、例えばエチレングリコール、ジエチレ
ングリコール、ポリエチレングリコール、グリセリン、
ソルビトール、マルチトール、プロピレングリコール、
１,３−ブチレングリコールなどのグリコール類、糖類
などが挙げられ、これらの一種または二種以上組み合わ
せて用いてよい。粘着性ゲル基剤中の配合量としては１
〜７０％が好ましく、より好ましくは１０〜６０％の範
囲で選定すれば良い。
【００１１】なお、保水性を一層向上させるために、超
高吸水性高分子物質を配合することができる。具体例と
してはデンプン−アクリロニトリルグラフト体、デンプ
ン−アクリル酸グラフト体、デンプン−スチレンスルホ
ン酸グラフト体、デンプン−ビニルスルホン酸グラフト
体、ポリビニルアルコール架橋体、アクリル酸−酢酸ビ
ニルケン化物、ポリエチレングリコールジアクリレート
架橋物等が挙げられる。これらの超高吸水性高分子物質
の配合量は、０〜２０％が好ましく、より好ましくは
０．０１〜１０％の範囲で選定すればよい。
【００１２】さらに必要に応じて通常の吸収助剤(例え
ばサリチル酸、ヒアルロン酸、オレイン酸、Ｎ,Ｎ−ジ
エチル−m−トルアミド、ステアリン酸n−ブチル、ベン
ジルアルコール、ミリスチン酸イソプロピル、パルミチ
ン酸イソプロピル、ポリプロピレングリコール、クロタ
ミトン、ジエチルセバケート、Ｎ−メチルピロリドン、
Ｎ−エチルピロリドン、ラウリルアルコール等)及び該
吸収助剤をゲル基剤中の水に乳化させるための界面活性
剤(例えばポリオキシエチレンソルビタンモノオレー
ト、ポリオキシエチレンソルビタンモノステアレート、
ソルビタンモノオレート、ソルビタンモノパルミテート
等)が使用できる。その他、防腐剤、酸化防止剤等を適
量配合してもよい。なお、酸化防止剤や防腐剤を用いる
場合、薬物の放出制御機能や皮膚刺激の影響に留意すべ
きである。
【００１３】かかる成分からなる粘着性ゲル基剤のpＨ
は皮膚刺激性及び薬物の安定性を考慮してpＨ５〜９に
調整するのが好ましく、この場合アルカリ性物質(例え
ば水酸化ナトリウムまたはトリエタノールアミン、ジイ
ソプロパノールアミンなどのアミン類)などでpＨ調整し
てもよい。
【００１４】本発明にかかるリドカイン含有外用貼付剤
は、上述の粘着性ゲル基剤に有効量の薬物すなわちリド
カインまたはその塩(塩酸リドカインなど)を配合して、
これを適当な支持体に塗工して薬物保持層(薬物含有量
０．５〜１５．０％、好ましくは２．５〜１０．０％)
を含有することにより得られる。該薬物含有基剤の塗布
量は、通常、５００〜２０００g／m2の範囲である。
【００１５】必要に応じ、薬物保持層の水分揮発を防止
して該層を保護する目的で、その表面に適当な材質のラ
イナーを添着してもよい。上記支持体としては、人体の
動作にある程度追随し易い柔軟なものが好ましく、例え
ば各種の不織布、織布、スパンデックス、ネル、または
これらにポリエチレンフィルム、ポリエチレンテレフタ
レートフィルム、ポリ塩化ビニルフィルム、エチレンビ
ニルアセテートフィルム、ポリウレタンフィルム等をラ
ミネート加工したものが挙げられる。
【００１６】
【発明の効果】本発明の貼付剤によれば薬物を定量的に
放出させることができ、また使用方法が簡便で、かつ長
時間にわたる貼付が可能であり、さらに具体的には以下
の利点が奏せられる。 (１) 肝臓における初回通過効果を回避することがで
き、生物学的利用率を高くすることができる。 (２) 貼付剤化により薬物の長時間の持続放出ができ常
に安定な薬効を期待できる。また投与回数を少なくする
ことができ、患者のコンプライアンスの改善につなが
る。 (３) 帯状疱疹後神経痛は時として風、衣服の摩擦によ
っても痛みを生じる場合があり、この場合、皮膚の表面
を貼付剤で覆うことにより痛みを軽減できる。 (４) 患者の年齢、健康状態、胃腸等の消化器官あるい
は肝機能の状態、食物の影響に関係なく投与が可能であ
る。

【実施例】次に実施例及び比較例を挙げて本発明をより
具体的に説明するが、本発明はこれらに限定されるもの
ではない。なお、実施例または比較例中「部」は特に断わ
らない限り「重量部」を意味する。
【００１８】実施例１ リドカイン ５部 Ｄ−ソルビトール １５部 グリセリン ２０部 プロピレングリコール １０部 ポリアクリル酸ナトリウム ４部 カルボキシメチルセルロースナトリウム ５部 ポリアクリル酸 ２部 パラオキシ安息香酸メチル ０．１部 パラオキシ安息香酸プロピル ０．０５部 水酸化アルミニウム ０．３部 精製水 残量 全量 １００部 精製水にＤ−ソルビトール、ポリアクリル酸を加え混合
する。更にプロピレングリコール、リドカインを混合溶
解した液を加えて混合する。次いでグリセリンにポリア
クリル酸ナトリウム、カルボキシメチルセルロースナト
リウム、水酸化アルミニウム、パラオキシ安息香酸、パ
ラオキシ安息香酸プロピルを分散させた液を加えて均一
になるまで十分に混合する。得られた基剤を不織布上に
１０００g／m2で展延し、シリコン加工したポリエチレ
ンテレフタレート製のライナーを添着し、所望の大きさ
に裁断する。これによりpＨ７でリドカインを５mg／cm2
含有するリドカイン含有外用貼付剤を得る。
【００１９】実施例２ リドカイン １０部 Ｄ−ソルビトール １０部 グリセリン ２０部 プロピレングリコール １０部 ポリアクリル酸ナトリウム ４部 カルボキシメチルセルロースナトリウム ５部 ポリアクリル酸 ３部 パラオキシ安息香酸メチル ０．１部 パラオキシ安息香酸プロピル ０．０５部 水酸化アルミニウム ０．３部 精製水 残量 全量 １００部 実施例１と同様の方法にて混合し基剤を得て、不織布上
に１０００g／m2で展延し、シリコン加工したポリエチ
レンテレフタレート製のライナーを添着し、所望の大き
さに裁断する。これによりpＨ７でリドカインを１０mg
／cm2含有するリドカイン含有外用貼付剤を得る。
【００２０】実施例３ リドカイン ５部 ゼラチン １部 カオリン １部 Ｄ−ソルビトール １５部 グリセリン ２０部 プロピレングリコール １０部 ポリアクリル酸ナトリウム ５部 カルボキシメチルセルロースナトリウム ５部 ポリアクリル酸 ２部 尿素 １部 パラオキシ安息香酸メチル ０．１部 パラオキシ安息香酸プロピル ０．０５部 水酸化アルミニウム ０．３部 精製水 残量 全量 １００部 精製水にゼラチン、ポリアクリル酸を加え溶解した液
に、カオリン、Ｄ−ソルビトール、尿素を加え、十分に
混合する。更にプロピレングリコール、リドカインを混
合溶解した液を加えて混合する。次いでグリセリンにポ
リアクリル酸ナトリウム、カルボキシメチルセルロース
ナトリウム、水酸化アルミニウム、パラオキシ安息香酸
メチル、パラオキシ安息香酸プロピルを分散させた液を
加えて均一になるまで十分に混合する。得られた基剤を
不織布上に１０００g／m2で展延し、シリコン加工した
ポリエチレンテレフタレート製のライナーを添着し、所
望の大きさに裁断する。これによりpＨ６でリドカイン
を５mg／cm2含有するリドカイン含有外用貼付剤を得
る。
【００２１】比較例１ リドカイン ５部 白色ワセリン ２５部 ステアリルアルコール ２５部 プロピレングリコール １０部 ポリアクリル酸 ２部 ポリオキシエチレン硬化ヒマシ油 ４部 パラオキシ安息香酸メチル ０．１部 パラオキシ安息香酸プロピル ０．０５部 精製水 残量 全量 １００部 白色ワセリン及びステアリルアルコールを水浴上で溶か
しよくかき混ぜ、約７５゜に保ち、これに精製水、ポリ
アクリル酸、ポリオキシエチレン硬化ヒマシ油及びプロ
ピレングリコールに溶解させたリドカイン溶液、パラオ
キシ安息香酸メチル、パラオキシ安息香酸プロピルを加
え、室温にて固まるまでかき混ぜ製する。これによりp
Ｈ７でリドカインを５０mg／g含有するリドカイン含有
軟膏を得る。
【００２２】比較例２ リドカイン ５部 絆創膏基剤 ９０部 プロピレングリコール ５部 全量 １００部 絆創膏を基剤とし、プロピレングリコールに溶解させた
リドカイン溶液を加え混合し、絆創膏の製法に準じて不
織布上に１０００g／m2で展延し製する。これにより、
リドカインを５mg／cm2含有するリドカイン含有硬膏剤
を得る。
【００２３】比較例３ リドカイン １０部 プロピレングリコール １０部 スチレンイソプレンスチレンブロックコポリマー(ＳＩＳ) ３０部 脂環族飽和炭化水素石油樹脂 ４５部 流動パラフィン ４部 ジブチルヒドロキシトルエン １部 全量 １００部 ＳＩＳ以下の各成分を加熱下において溶解し、プロピレ
ングリコールに溶解させたリドカイン溶液を加え混合す
る。次に、適当な不織布上に５００g／m2になるように
展延し、冷却した。この基剤にポリエチレンテレフタレ
ートフィルムをラミネートし、所望の大きさに切断し
た。これによりリドカインを５mg／cm2含有するリドカ
イン含有外用貼付剤を得る。
【００２４】試験例１ フランツ拡散セルを用いてラット腹部摘出皮膚を透過
するリドカインの量をＨＰＬＣにより定量した。実施例
１〜３、比較例２、３について直径１．７cmの円形(リ
ドカイン１１．３５mg、または２２．７mg含有)に打ち
抜き、拡散セルのラット皮膚上に貼付した。レセプター
側にpＨ６．８−リン酸緩衝液を用い、経時的にサンプ
ルを採取し、薬物透過量を測定した。なお、比較例１に
ついては直径１．７cmの円内に塗布し、以下同様に試験
を行なった。 (結果)結果を図１に示す。(n＝３) 図１から明らかなように、実施例１〜３及び比較例１に
ついて薬物は経時的にラット皮膚を透過し、その透過性
は比較例２、３に比べて優れていた。
【００２５】試験例２ ラットの背部を除毛した後、これに実施例１〜３、比
較例２、３について４cm×４cm(リドカイン８０．０mg
または１６０．０mg含有)の大きさに裁断したものを貼
付した。この状態で経時的に血液を０．５mlずつ採取
し、蛍光偏光イムノアッセイ法にて血清中のリドカイン
量を測定した。なお、比較例１については４cm×４cmの
大きさのガーゼに１．６g(リドカイン８０．０mg含有)
塗布し、以下同様に試験を行なった。 (結果)結果を図２に示す。(n＝４) 図２から明らかなように、実施例１〜３について薬剤投
与後６〜８時間で血中濃度は一定状態に達した。以後２
４時間まで持続し、比較例に比べて高い血中濃度を示し
た。また、薬剤除去後は速やかに消失した。比較例１に
ついては薬剤投与後速やかに血中濃度は上昇するが、持
続性がなく、薬剤投与後４〜５時間でピークとなり、以
後減少した。比較例２、３については経皮吸収性が悪
く、血中濃度は低かった。
【００２６】試験例３ モルモットの背部を除毛した後、マンドリン線で一定
強度の刺激を与え、表皮に正常な収縮反応の起こること
を確認後、実施例１〜３、比較例２、３について４cm×
４cm(リドカイン８０．０mgまたは１６０．０mg含有)の
大きさに裁断したものを貼付した。貼付後経時的に薬剤
を除去し貼付部位をマンドリン線で６回刺激し、３回以
上皮膚収縮反応のなかったものを局所麻酔作用陽性とし
た。なお、比較例１については４cm×４cmの大きさのガ
ーゼに１．６g(リドカイン８０．０mg含有)塗布し、以
下同様に試験を行なった。 (結果)結果を表１に示す(n＝５) 表１から明らかなように、実施例１〜３について薬剤投
与後４時間で効果が認められ、以後薬剤を除去するまで
持続し、高い効果率が得られた。また、薬剤除去後速や
かにその効果は消失した。比較例１について薬剤投与後
４〜８時間で高い効果率が得られたものの、持続性がな
かった。比較例２については薬剤投与後４時間で効果が
認められたものの、低い効果率であり、また比較例３に
ついては全く効果が認められなかった。

    (57)【特許請求の範囲】
  1. 【請求項１】 酢酸ビニル重合体を除く水溶性高分子物
    質、水及び保水剤を必須成分とする粘着性ゲル基剤に、
    リドカインまたはその塩を１〜１０重量％配合してなる
    薬物保持層を支持体上に設けたことを特徴とするリドカ
    イン含有外用貼付剤。
  2. 【請求項２】 粘着性ゲル基剤が薬物の放出制御機能を
    有し、そのｐH値が５〜９である請求項第１項記載の外
    用貼付剤。
  3. 【請求項３】 粘着性ゲル基剤中の水分量が２０〜７０
    重量％である請求項第１項または請求項第２項記載の外
    用貼付剤。
