Description

The present invention relates to an indwelling device for a capsule medical device for using a capsule endoscope for a digestive tract such as a capsule endoscope for a small intestine as an indwelling capsule, and such a general-purpose capsule endoscope or indwelling dedicated The present invention relates to a capsule indwelling medical device using the in-body cavity indwelling capsule endoscope.

In recent years, swallowable capsule endoscopes have been developed in the field of endoscopes. This capsule endoscope has an imaging observation function and a wireless function, and after being swallowed from the patient's mouth for observation inside the body cavity, until it is naturally discharged from the human body, for example, the esophagus, stomach, small intestine It has a function of moving inside the organ according to the peristaltic movement and sequentially imaging (see, for example, Patent Document 1).

While moving inside the body cavity, image data captured inside the body by the capsule endoscope is sequentially transmitted outside the body by wireless communication, and is stored in a memory provided in a receiver outside the body. The doctor or nurse can make a diagnosis based on the image displayed on the display based on the image data stored in the memory.

On the other hand, along with the development of endoscopic techniques, endoscopic operations such as endoscopic mucosal removal (EMR) and endoscopic submucosal dissection (ESD) are possible. After endoscopic surgery, the surgical site is stopped, but there is a possibility of bleeding at night or the like, so it is necessary to monitor the presence or absence of bleeding. Therefore, such a body cavity is monitored by using a capsule endoscope that is openable and closable and includes a plurality of fasteners that are locked to tissue in the body cavity and can be placed at a desired position in the body cavity. A method has also been proposed (see, for example, Patent Document 2).

In addition, for example, Patent Document 3 discloses a technique for fixing a medical capsule for detecting pH in a body cavity to a target site in the body cavity.

In the technique disclosed in Patent Document 2, the capsule endoscope includes a plurality of fasteners and the like, and the capsule endoscope is locked to the body cavity tissue at a plurality of locations. However, the locked indwelling state is simply a hanging lock with respect to the tissue in the body cavity, and in the locked indwelling state, a moment acts on the distal end of the capsule endoscope and the like, and it is easy to swing. Here, in the capsule endoscope having the observation optical system, the direction of the observation visual field or the observation axis is shifted due to the swing, and the continuous and stable monitoring function of the monitoring target portion is impaired.

Moreover, in what is shown by patent document 3, the medical capsule is provided with the some needle-shaped member with a sharp part, and by inserting in a tissue in a body cavity so that a needle-shaped member may cross | intersect, a medical capsule is in several places. I try to lock it. However, it requires complicated changes to the internal structure of the medical capsule and is difficult to apply to a capsule endoscope having an observation optical system.

The present invention has been made in view of the above, and it is possible to place a capsule medical device with a simple structure in a state where there is no deviation in the direction of an observation field, and a body cavity in a continuously stable state. And a locking member having a plurality of locking portions to be attached.

The capsule indwelling medical device according to claim 4 is the above-described invention, wherein the capsule medical device includes an installation part for installing the capsule medical device in a body cavity tissue at the lowest part thereof. A first locking member having a first locking portion provided at a position spaced upward from the installation portion in the capsule medical device and attached to the tissue in the body cavity by the mounting member; A second locking member having a second locking portion that is spaced apart from the installation portion and is disposed at a position displaced from the first locking portion and is attached to the tissue in the body cavity by the mounting member; It is comprised from these.

According to the capsule medical device indwelling apparatus and the capsule indwelling medical device according to the present invention, the capsule medical device or the capsule for the tissue in the body cavity is provided by being connected to the holding member for holding the capsule medical device. There is an effect that it can be placed in a state where it does not occur, and can be continuously used for monitoring in a body cavity in a stable state.

Hereinafter, an indwelling device for a capsule medical device and a capsule indwelling medical device according to an embodiment of the present invention will be described with reference to the accompanying drawings. In addition, this invention is not limited by this Embodiment. In the description of the drawings, the same or corresponding parts are denoted by the same reference numerals.

(Embodiment 1)
Embodiment 1 of the present invention will be described. FIG. 1 is a schematic perspective view illustrating a configuration example of the capsule indwelling medical device according to the first embodiment, and FIG. 2 is a cross-sectional structure diagram illustrating a configuration example of the capsule indwelling medical device. The capsule indwelling medical device 100 includes a capsule medical device indwelling device 200 and a capsule endoscope 300 as a capsule medical device mounted and held on the capsule medical device indwelling device 200.

The capsule endoscope 300 is basically swallowed from the oral cavity and introduced into the body cavity of the subject, and images of the body cavity are captured as body cavity information, and data transmission such as the body cavity image is transmitted wirelessly. It is the existing and general-purpose one for the digestive tract to output.

Here, the capsule endoscope 300 will be described with reference to FIG. The capsule endoscope 300 supplies power to a plurality of illumination units 301 such as LEDs that illuminate the inside of a body cavity of a subject and an imaging element 302 such as a CCD or CMOS that captures an image inside the body cavity. A button-type battery 303 and a capsule-type housing 304 are provided together. As the battery 303, a silver oxide battery, a rechargeable battery, a power generation battery, or the like can be used.

The capsule-type casing 304 covers the illumination unit 301 and the like, and is a transparent hemispherical dome-shaped tip cover casing 304a, and a cylinder provided with the tip cover casing 304a in a watertight state and having a battery 303 and the like disposed therein. Formed into a size that can be swallowed from the oral cavity of the subject. The trunk housing 304b is formed of a colored material that does not transmit visible light.

The imaging element 302 is mounted on an imaging substrate 305, and an optical system 306 such as an imaging lens is disposed on the front surface. An observation optical system 307 having an observation axis O at the center in the longitudinal direction of the capsule housing 304 and a predetermined observation field of view is configured by the illumination unit 301, the image pickup element 302, the optical system 306, and the like. The imaging substrate 305 is mounted with a controller 308 for processing or controlling each unit on the back side.

The capsule endoscope 300 includes a reed switch 309 that is turned on and off by an external magnetic field in order to control the driving of the capsule endoscope 300. In the storage state of the capsule endoscope 300, the capsule endoscope 300 is housed in a package including a permanent magnet that supplies an external magnetic field, and maintains an off state in an environment where a magnetic field of a certain intensity or more is applied. It has a structure that turns on when the strength of the magnetic field decreases. For this reason, in the state accommodated in the package, the capsule endoscope 300 is not driven.

Furthermore, the capsule endoscope 300 includes a transmission device 311 with an antenna 310 that wirelessly outputs image information captured by the image sensor 302 to the outside on the back side of the battery 303.

On the other hand, the capsule medical device indwelling device 200 is provided with a holding member 201 for mounting and holding the capsule endoscope 300, and the capsule endoscope 300 provided in connection with the holding member 201 in the body cavity at a desired site. And a locking member 202 for locking and fixing to the tissue.

Here, the holding member 201 has a structure that integrally holds the outer surface of the capsule endoscope 300 in a surface contact state, and integrally holds the fitted capsule endoscope 300. . More specifically, the holding portion 201 is formed in a shape including a substantially cylindrical portion having substantially the same radius as that of the capsule endoscope 300, and further on the distal end cover housing 304a side of the capsule endoscope 300. It is formed in a cap shape that covers the whole area except for the portion. In addition, the holding unit 201 includes a protruding gripping portion 201a that is gripped by gripping forceps described below at an end thereof.

In addition, the capsule endoscope 300 is mounted and held by the holding unit 201, such as a press-fitting method, a method in which the capsule endoscope 300 is temporarily mounted using a heat-shrinkable tube or the like, and then contracted by applying heat, and an adhesive or the like. It may be a method of fixing and holding using. In short, the existing capsule endoscope 300 can be attached, and the holding state in which the attached capsule endoscope 300 is not easily pulled out can be maintained in the body cavity for the monitoring time, and Any system and structure that can ensure the same swallowability (introduction into the body cavity) as the capsule endoscope 300 may be used. The holding unit 201 may be any material that does not impair the imaging observation function, the transmission function, and the like of the capsule endoscope 300 and that does not interfere with the living body even if the holding unit 201 is introduced and placed in the body cavity. Any of a hard member and a soft member (elastic member) may be sufficient, and any of a transparent member and an opaque member may be sufficient.

Further, the locking member 202 has a grip shape (a holding member 201 at a position away from the observation axis direction) that is symmetrical about the observation axis O on both sides of the cap-shaped holding member 201 on a plane including the observation axis O. The front end portion is configured as a locking portion 203. That is, the pair of locking members 202 on both sides are located outside the capsule endoscope projection plane with respect to the tissue in the body cavity and sandwich the capsule endoscope 300, more specifically, the observation axis O of the observation optical system 307. Two locking portions 203a and 203b that are locked to the tissue in the body cavity at a position in the direction of sandwiching.

In other words, when the installation portion 204 for installing the lowest part busbar L portion of the holding member 201 (capsule endoscope 300) to the tissue in the body cavity is used, the locking member 202 of the present embodiment holds A first locking portion 203a provided at a position spaced upward from the installation portion 204 with respect to the member 201 (capsule endoscope 300) and attached to the tissue in the body cavity by an endoscopic fixture described below is provided. The first locking member 202a and the holding member 201 (capsule endoscope 300) are provided at positions spaced upward from the installation portion 204 and are displaced from the first locking portion 203a. And a second locking member 202b having a second locking portion 203b attached to the tissue in the body cavity by an endoscopic fixture described later. Here, “lowest part” and “upward direction” refer to the position or direction of the holding member 201 (capsule endoscope 300) close to the body cavity tissue in the positional relationship with the body cavity tissue. Direction, and the direction away from the body cavity tissue is the upward direction, and does not mean the vertical direction. Note that the two locking members 202a and 202b and the locking portions 203a and 203b are appropriately described and described as the locking member 202 and the locking portion 203 unless it is necessary to distinguish between them. The same applies to a pair of locking members and locking portions in the embodiments and modifications described later, but the distinction by the subscripts a and b is omitted for the sake of simplicity.

These locking portions 203a and 203b are locked to the body cavity tissue by the endoscopic fixing tool, that is, the hemostatic clip 410 for endoscope in the first embodiment. The locking member 202 is made of at least an elastic material, and can be folded around the holding member 201 (and therefore around the capsule endoscope 300) when introduced into the body cavity. It can be restored to a fixed grip shape. Unlike the case of the capsule endoscope 300, there are few restrictions on adding the locking member 202 to the holding member 201 that does not include a built-in object, and this can be easily realized. Note that the locking member 202 is not necessarily integral with the holding member 201, and may be a separate body. In short, it is only necessary to be integrally coupled.

The capsule indwelling medical device 100 including the capsule endoscope 300 attached and held in such an indwelling device 200 for a capsule medical device is placed in a desired position in the subject 400 and is received and fixed. A capsule indwelling medical system is configured by combining with the above. FIG. 3 is a schematic diagram illustrating a schematic configuration example of a wireless capsule indwelling medical system. As shown in FIG. 3, the wireless capsule indwelling medical system is introduced into a subject 400 and placed and fixed at a desired site in a body cavity such as the stomach 401 to capture an image of the stomach 401 in color. The capsule indwelling medical device 100 including the capsule endoscope 300 that wirelessly transmits data such as video signals to the receiving device 402 and the color image data wirelessly transmitted from the capsule endoscope 300 are received. A portable receiving device 402 and a display device 403 such as a portable viewer that displays a color image based on a video signal received by the receiving device 402 are provided. The receiving apparatus 402 includes a receiving antenna 404 attached to a part corresponding to the indwelling / fixed part of the capsule endoscope 300 on the external surface of the subject 400, for example, near the stomach 401.

Thus, while using the general-purpose capsule endoscope 300 for the digestive tract, the capsule endoscope 300 is placed and fixed at a desired site in the body cavity as the capsule indwelling medical device 100, so that the capsule endoscope By observing and observing a desired site with 300 and observing the captured in-vivo image with the display device 403, it is suitable for monitoring the affected area after surgery.

Here, with reference to FIG. 4 to FIG. 6, a procedure for placing the capsule endoscope 300 in the body cavity will be described in order. The introduction and placement of the capsule endoscope 300 into the body cavity is for monitoring the presence or absence of bleeding in the affected area after the endoscopic operation, and the endoscopic operation of the subject 400 to be processed. Done later. It should be noted that the capsule endoscope 300 may be introduced into the body cavity and placed before the endoscopic surgery, and the placement work may not be performed after the endoscopic surgery. In addition, the receiving antenna 404 is attached to the external surface of the subject 400 at an appropriate timing before and after the capsule endoscope 300 is introduced into the body cavity.

First, a general-purpose capsule endoscope 300 is prepared for the digestive tract, and this capsule endoscope 300 is fitted into the holding member 201 to be integrated with the holding member 201. Then, as shown in FIG. 4, the gripping portion 201 a at the end of the holding member 201 is gripped by the gripping forceps 408 that has passed through the forceps channel 407 of the endoscope 406, and the cylindrical conveyance at the distal end of the endoscope 406 is performed. The capsule indwelling medical device 100 is pulled into the member 409. At this time, the locking member 202 is bent so as to be wound around the holding member 201, folded, and drawn into the conveying member 409, so that no extra protruding portion is generated around the conveying member 409.

Then, as shown in FIG. 5A, the distal end side of the endoscope 406 is introduced into the body cavity while the capsule indwelling medical device 100 is drawn into the transport member 409. At this time, since the locking member 202 and the like are not in a protruding state, the introduction into the body cavity is not hindered and can be introduced into the body cavity as in the case of the capsule endoscope 300 alone.

When the distal end side of the endoscope 406 is introduced to a desired site in the body cavity, for example, near the post-surgical monitoring site in the stomach 401 or the like, as shown in FIG. 5B, the capsule forceps 408 is opened to release the capsule placement type. The medical device 100 is released from the conveying member 409 into the body cavity. With the opening operation in the body cavity, the latching member 202 folded in the capsule indwelling medical device 100 is restored and developed to the grip shape which is the original fixed shape by the elastic restoring force.

Therefore, this time, as shown in FIGS. 5-2 and 6, an endoscopic treatment tool such as a grasping forceps 408 is used for the capsule indwelling medical device 100 released into the body cavity. By the treatment, the two locking portions 203a and 203b at the tip of the locking member 202 are locked and fixed to the body cavity tissue 411 by two endoscopic fixing tools, here, the hemostatic clip 410 for endoscope. At this time, the locking portions 202a and 203b are engaged with the tissue 411 in the body cavity by the hemostatic clip 410 for endoscope as if it is pulled outward so that the locking member 202 is in a state of being stretched with respect to the holding member 201. Fix it.

At this time, the operation part to be monitored is positioned in the observation field of view of the observation optical system 307 of the capsule endoscope 300 by monitoring the captured image of the capsule endoscope 300 through the receiving device 402 and the display device 403. The locking parts 203a and 203b are locked and fixed to the tissue 411 in the body cavity by operating the hemostatic clip 410 for the endoscope with the grasping forceps 408 and the like while determining the placement direction. In this case, in the first embodiment, two locking portions 203a and 203b are prepared, and after one of the locking portions 203a and 203b is locked in the body cavity by the endoscopic hemostatic clip 410, the capsule endoscope 300 is provided. The captured position of the capsule endoscope 300 can be adjusted by monitoring the captured image on the display device 403 and adjusting the locking position of the remaining locking portion 203b (or 203a) by the hemostatic clip 410 for the endoscope. It is.

Thereafter, by pulling the endoscope 406 out of the body cavity, as shown in FIG. 6, the capsule endoscope 300 is placed and fixed at a desired site in the body cavity through the holding member 201 and the locking member 202 that are attached and held. The indwelling observation using the capsule endoscope 300 becomes possible.

After the end of monitoring, the hemostatic clip 410 for endoscope falls into the body cavity together with the holding member 201 and the capsule endoscope 300 due to necrosis of the tissue 411 in the body cavity where the hemostatic clip 410 for endoscope is locked. Therefore, the dropped capsule endoscope 300 may be collected endoscopically with a collection net or the like while being integrated with the holding member 201, or may be discharged out of the body cavity as it is.

Here, the indwelling fixed state of the capsule endoscope 300 (holding member 201) will be described with reference to FIGS. FIG. 7 is a schematic plan view showing the indwelling fixed state, and FIG. 8 is a schematic cross-sectional view showing the indwelling fixed state. In the first embodiment, the locking member 202 is set at a position outside the projection surface of the capsule endoscope with respect to the body cavity tissue 411 and sandwiching the capsule endoscope 300 in a direction perpendicular to the observation axis O. The two locking portions 203a and 203b of the capsule endoscope 300 are locked by the endoscopic hemostatic clip 410 with the tension in the body cavity tissue 411, so that the bus of the capsule endoscope 300 parallel to the observation axis O L is in an indwelling fixed state in which the line L stably contacts the body cavity tissue 411 in a desired direction, and an indwelling state in which the direction of the observation visual field of the observation optical system 307 or the observation axis O is not displaced can be secured.

In other words, in the first embodiment, the first locking having the first locking portion 203a provided at a position spaced upward from the installation portion 204 in the holding member 201 (capsule endoscope 300). A member 202a and a second locking member 202b that has a second locking portion 203b that is spaced apart from the first locking portion 203a while being spaced upward from the installation portion 204 and having a second locking portion 203b. Since the locking member 202 is used and the two locking portions 203a and 203b are locked by the endoscopic hemostatic clip 410 with the tension in the body cavity tissue 411, the holding member 201 parallel to the observation axis O is used. The placement section 204 of the (capsule endoscope 300) is placed and fixed so as to be in stable contact with the tissue 411 in the body cavity in a desired direction, and the orientation of the observation visual field of the observation optical system 307 or the observation axis O is not shifted. State can be ensured.

More specifically, both in a plan view as shown in FIG. 7 and in an observation axis direction as shown in FIG. 8, the inside of the capsule mold is centered on the locking position by the respective locking portions 203a and 203b. The tissue 411 in the body cavity is set by the hemostatic clip 410 for the endoscope in a state where the locking positions by the locking portions 203a and 203b are set so as to cancel each other as shown by arrows acting on the endoscope 300 and the tension is held. Therefore, the observation in the body cavity can be continuously monitored in a stable state without causing the swinging of the observation field of view.

(Modification 1)
Modification 1 will be described with reference to FIGS. 9-1 and 9-2. FIG. 9A is a schematic perspective view illustrating a configuration example of Modification 1. FIG. 9B is a schematic perspective view illustrating a state during the collection and removal. In Modification 1, as shown in FIG. 9A, instead of the locking member 202, a locking member 205 having an approximately 8 or hook shape is used in plan view, and the outer front end portion is engaged. In addition to the stop portion 206, the boundary portion is provided with a cut 207 that facilitates cutting.

According to this, after the end of the monitoring operation, as shown in FIG. 9B, the endoscope 406 is introduced, the inner part of the locking member 205 is gripped with the grasping forceps 408 and pulled, and the break 207 is By cutting the capsule endoscope 300, the capsule endoscope 300 can be collected while leaving the engaging portion 206 of the endoscope hemostatic clip 410 left untouched.

(Modification 2)
Modification 2 will be described with reference to FIG. FIG. 10 is a schematic perspective view showing a configuration example of the second modification. In the second modification, instead of the simple grip-shaped locking member 202, the locking members 210 positioned on both sides of the holding member 201 overlap each other with a plurality of locking portions 211a, 211b, 211c,... Are formed in a multi-stage shape. According to such a configuration, in the locking operation of the endoscopic hemostatic clip 410 to the body cavity tissue 411, a plurality of locking portions 211a on one side are selected according to the situation of the body cavity tissue 411 at the locking site. , 211b, 211c,... Can be selected and performed, and the locking operation by the endoscopic treatment can be easily performed.

(Modification 3)
Modification 3 will be described with reference to FIGS. 11 and 12. FIG. 11 is a schematic perspective view illustrating a configuration example of Modification Example 3, and FIG. 12 is a schematic front view illustrating a locked state thereof. In Modification 3, instead of the locking members 202 provided on both sides of the holding member 201 on the plane including the observation axis O, the holding member 201 (capsule endoscope 300) is tangentially connected to one side surface. A locking member 215 that is integrally formed by being offset is provided. That is, the locking member 215 has a continuous planar shape on both sides of the holding member 201.

According to such a configuration, when the capsule indwelling clothing apparatus 100 is left in the body cavity from the conveying member 409 described with reference to FIG. 5B, as shown in FIG. 12, the locking member 215 having a continuous planar shape is formed. Is in surface contact with the body cavity tissue 411, and the cylindrical holding member 201 does not roll and is more stable than the case where it directly contacts the body cavity tissue 411. The locking operation of the hemostatic clip 410 for endoscope is easy and reliable.

Further, in the configuration shown in FIG. 11, as shown in FIG. 13, the connecting portion of the holding member 201 and the locking member 215 is disposed so as to be located on the opposite side of the body cavity tissue 411 to be locked. The locking portion 203 may be locked to the body cavity tissue 411 with the hemostatic clip 410 for endoscope. According to this, as shown by an arrow in FIG. 13, the force for pressing the capsule endoscope 300 toward the body cavity tissue 411 side is strongly acted by the locking member 215 that is locked with tension, and the bus L Can be more reliably obtained in a line contact with the tissue 411 in the body cavity in a desired direction.

(Modification 4)
Modification 4 will be described with reference to FIG. FIG. 14 is a schematic perspective view illustrating a configuration example of the fourth modification. In the fourth modification, instead of the grip-shaped locking member 202, a rod-shaped locking member 220 having a large locking portion 221 at the tip is provided integrally with the holding member 201. The locking member 220 is formed in a direction orthogonal to the observation axis O, and the locking portion 221 is disposed at a position sandwiching the observation axis O. As in the case of the locking member 202, the locking member 220 can be folded around the holding member 201 (capsule endoscope 300) when introduced into the body cavity, and has an original fixed shape when released into the body cavity. It is made of an elastic material that restores and develops into a rod shape. The locking member 220 is formed integrally with the holding member 201 on a plane including the observation axis O. However, as in the case described with reference to FIG. It may be offset and connected so as to be.

Also with the configuration of the modified example 4, the same effect as in the case described above can be obtained. Further, the bar-shaped locking member 220 is not limited to one on one side, and for example, as shown in FIG. 15, a plurality of locking members 220a to 220c each having locking portions 221a to 221c are provided. May be. According to this, similarly to the case shown in FIG. 10, in the locking operation to the body cavity tissue 411 by the endoscopic hemostatic clip 410, depending on the situation of the body cavity tissue 411 at the locking site, one side Of the plurality of locking portions 221a to 221c, it is possible to select and perform a location where clipping is easy, and it is easy to perform locking work by endoscopic treatment.

(Modification 5)
Modification 5 will be described with reference to FIGS. 16 and 17. FIG. 16 is a schematic perspective view showing a configuration example of Modification Example 5, and FIG. 17 is a schematic perspective view showing a folded state of the mesh member. In the modified example 5, instead of the locking members 202 and 220 having the locking portions 203 and 221 at specific locations, a locking member 225 made of a mesh member having the locking portions 226 at arbitrary positions is provided to the holding member 201. The connecting portion 227 is connected by an adhesive or the like and provided in a direction orthogonal to the observation axis O. The locking member 225 has a width corresponding to the length of the holding member 201, and the connecting portion 227 is set in the axial direction on the outer peripheral surface of the holding member 201 over the entire width thereof. As shown in FIG. 17, the locking member 225 is made of an elastic material that can be folded around the holding member 201 and can be restored and deployed to the original net-like developed shape in the open state.

In such a configuration, when the latch member 225 is introduced into the body cavity in a folded state as shown in FIG. 17 and released from the transport device 409 into the body cavity, the locking member 225 is restored and deployed into a net-like deployment shape. Therefore, for example, as shown in FIG. 16, the locking member 225 is disposed on the upper side of the holding member 201 (capsule endoscope 300), and the observation visual field of the observation optical system 307 is in a desired direction. After the adjustment, the locking portion 226 is locked to the body cavity tissue 411 with the endoscope hemostatic clip 410 in a state where the locking member 225 is stretched with respect to the holding member 201. At this time, the locking member 225 is made of a net-like member, and the hemostatic clip 410 for endoscope may be locked at any position. That is, since the position of the locking portion 226 is arbitrary, it is easy to lock. The location can be selected and locked, and the locking workability is improved. Also, it is optional to lock the holding member 201 (capsule endoscope 300) at a plurality of positions on each side.

Even when the locking member 225 is locked by such a net-like member, the body cavity tissue 411 is positioned outside the projection surface of the capsule endoscope with respect to the body cavity tissue 411 and sandwiched between the capsule endoscope 300. By locking the locking member 225 with tension, the directionality of the connecting portion 227 is fixed, so that the generatrix L of the holding member 201 (capsule endoscope 300) is in the body cavity tissue in a desired direction. It can be locked so that the direction of the observation visual field or the observation axis O does not shift by making line contact with 411. In particular, as shown in FIG. 16, when the connecting portion 227 is disposed so as to be opposite to the body cavity tissue 411 and the locking member 225 is locked, the capsule type is the same as in the case of FIG. 13. A pressing force toward the body cavity tissue 411 side can also be applied to the endoscope 300, and a stable locking state is obtained. Similarly to the case of FIG. 12, the locking member 225 may be locked by arranging the connecting portion 227 so as to be positioned at the body cavity tissue 411 side.

Further, the same effect can be obtained by using a locking member 230 made of a fibrous sheet-like member as shown in FIG. Reference numeral 231 denotes an arbitrary locking portion that is specified at a location where a hole is formed by inserting and locking the hemostatic clip 410 for endoscope. Further, the locking member 230 and the holding member 201 are connected so as to be integrated by a connecting portion (not shown) similar to the case of the connecting portion 227.

(Embodiment 2)
A second embodiment of the present invention will be described with reference to FIGS. 19 to 20-2. 19 is a schematic perspective view showing a configuration example of the second embodiment of the present invention, FIG. 20-1 is a schematic perspective view showing a state of a one-side locking state, and FIG. 20-2 is a both-side locking. It is a schematic perspective view which shows the mode of a state. In the second embodiment, the elongated locking member 236 having the locking portions 235 at both ends is connected to the outer peripheral surface of the holding member 201 by the rotation fulcrum 237, whereby the locking member 236 and the holding member 201 are provided so as to be rotatable relative to each other. The locking member 236 is made of an elastic material, and can be bent and folded around the holding member 201 (and therefore around the capsule endoscope 300) when introduced into the body cavity. It can be restored and restored to its original fixed shape.

In such a configuration, after the capsule indwelling medical device 100 is introduced and released at a desired position in the body cavity, the locking member 236 is placed so that the rotation fulcrum 237 is positioned on the side opposite to the body cavity tissue 411. With the capsule endoscope 300 placed, the indwelling position and visual field direction of the capsule endoscope 300 are roughly determined. As shown in FIG. 20A, the locking portion 235 on one side is placed in the body cavity tissue with the hemostatic clip 410 for the endoscope. 411 is locked. In this state, the indwelling position and the visual field direction of the capsule endoscope 300 are finely adjusted while monitoring the display device 403 as indicated by a virtual line in FIG. Then, in the state where the desired indwelling position and visual field direction are obtained, as shown in FIG. 20B, the remaining locking portion 235 side of the locking member 236 is pulled outward and is stretched against the holding member 201. The locking portion 235 is locked to the body cavity tissue 411 with the hemostatic clip 410 for endoscope. As a result, the holding member 201 (capsule endoscope 300) enters the locked state in which the pressing force by the locking member 236 also acts on the body cavity tissue 411 side, and the holding member 201 (capsule endoscope 300) is in the locked state. The generatrix L is maintained in a state in line contact with the tissue 411 in the body cavity without rotating with respect to the locking member 236, and an accurate visual field can be obtained.

In addition, if it is set as the structure which requires the external force by an endoscopic treatment tool for the rotation between the holding member 201 and the latching member 236, the latching member 236 so that the rotation fulcrum 237 may become the body cavity tissue 41 side. May be disposed and locked.

(Modification 6)
Modification 6 will be described with reference to FIG. FIG. 21 is a schematic plan view showing a configuration example of the sixth modification. The modified example 6 includes two locking members 243 and 244 having locking portions 241 and 242 at their ends instead of the locking member 236, and one locking member 243 is on the outer peripheral surface of the holding member 201. The other locking member 244 extends so as to be opposite to the locking member 243 with respect to the holding member 201 so that the outer peripheral surface of the holding member 201 is rotated. It is fixedly connected to the top. In particular, the connecting portion of the locking member 244 with respect to the holding member 201 has a bifurcated shape, and has two connecting portions separated in the longitudinal direction of the holding member 201. Each of the locking members 243 and 244 is made of an elastic material, and can be bent and folded around the holding member 201 (and therefore around the capsule endoscope 300) when introduced into the body cavity. It can be restored and restored to its original fixed shape by opening at.

In such a configuration, after the capsule indwelling medical device 100 is introduced and opened at a desired position in the body cavity, the rotation fulcrum 244 and the connecting portion are locked so as to be located on the side opposite to the body cavity tissue 411. With the members 243 and 244 disposed, the indwelling position and visual field direction of the capsule endoscope 300 are roughly determined. First, the movable side locking portion 241 is inserted into the body cavity with the hemostatic clip 410 for endoscope. Lock to tissue 411. In this state, since the holding member 201 can be rotated with respect to the locking member 243, the placement position and the visual field direction of the capsule endoscope 300 are finely adjusted while monitoring the display device 403. Then, in a state where the desired indwelling position and viewing direction are obtained, the retaining member 244 side of the stationary-side retaining member 244 is pulled outward, and the retaining member 201 is stretched to provide a retaining portion. 242 is locked to the body cavity tissue 411 with the hemostatic clip 410 for endoscope. Thereby, the movement of the holding member 201 with respect to the locking member 244 is also eliminated, and the pressing force by the locking members 243 and 244 also acts on the body cavity tissue 411 side with respect to the holding member 201 (capsule endoscope 300). The holding member 201 (capsule endoscope 300) does not rotate with respect to the locking members 243 and 244 so that the generatrix L is in line contact with the body cavity tissue 411 while maintaining the desired direction. Maintained and accurate field of view can be obtained.

(Modification 7)
Modification 7 will be described with reference to FIG. FIG. 22 is a schematic perspective view illustrating a configuration example of Modification 7. In the modified example 7, the locking member 252 that has the locking portions 250 and 251 at both ends and is formed in an elongated shape is connected to the outer peripheral surface of the holding member 201 by the rotation fulcrum 253, whereby the locking member 252 and the holding member 201 are provided in a state in which they can rotate relative to each other. Here, the mutual rotation between the locking member 252 and the holding member 201 requires a forced external force using an endoscopic treatment instrument such as an endoscope or forceps. It has a semi-fixed connection structure so as not to move.

Further, one of the locking portions 250 and 251 is locked to the body cavity tissue 411 by the endoscopic hemostatic clip 410 as in the case of the various embodiments and modifications described above. Although it is configured as a part, the other locking part 251 has a locking function with respect to the body cavity tissue 411 and is configured as a locking part that is directly locked to the body cavity tissue 411 by endoscopic treatment. Has been. In the modified example 7, the two locking portions 251 are formed in an outward spike shape, but may have an anchor structure.

The locking member 252 is made of an elastic material, and can be bent and folded around the holding member 201 (and therefore around the capsule endoscope 300) when introduced into the body cavity, and can be folded at a desired position in the body cavity. It can be restored and restored to its original fixed shape.

In such a configuration, after the capsule indwelling medical device 100 is introduced to a desired position in the body cavity and opened, the locking member 252 is disposed so that the rotation fulcrum 253 is positioned on the body cavity tissue 411 side. In this state, the indwelling position and the visual field direction of the capsule endoscope 300 are roughly determined. First, the locking portion 250 is locked to the body cavity tissue 411 with the hemostatic clip 410 for endoscope. In this state, the indwelling position and the visual field direction of the capsule endoscope 300 are finely adjusted by applying a compulsory external force using an endoscopic treatment tool while monitoring the display device 403. Then, in the state where the desired indwelling position and the visual field direction are obtained, the holding member 201 is stretched with the spike-shaped locking portion 251 side pulled outward, and the locking portion 251 is endoscope-removed. It is locked directly by being inserted into the body cavity tissue 411 by a surgical treatment tool. In such a state, unless a forced external force is applied, the holding member 201 does not move relative to the locking member 252, and the holding member 201 (capsule endoscope 300) moves against the locking member 252. Thus, the generatrix L is maintained in a state where it is in line contact with the tissue 411 in the body cavity without rotating, and an accurate visual field can be obtained. Further, the clipping treatment is only required at one place on the side of the locking portion 250, which is simple.

(Modification 8)
Modification 8 will be described with reference to FIG. FIG. 23 is a schematic front view illustrating a configuration example of Modification 8. In the modified example 8, a belt-like locking member 256 having locking portions 255 at both ends is connected to the outer peripheral surface of the holding member 201 by a connecting portion 257 (which may be a rotation fulcrum). A binding member 258 having a check structure is interposed in a part of the locking member 256. The locking member 256 including the bundling member 258 is made of an elastic material and is foldable so as to be wound around the holding member 201 as in the case described above.

In such a configuration, after the capsule indwelling medical device 100 is introduced and released at a desired position in the body cavity, the locking member 256 is arranged so that the connecting portion 257 is positioned on the side opposite to the body cavity tissue 411. In the installed state, the locking portion 255 on one side is locked to the body cavity tissue 411 with the hemostatic clip 410 for endoscope. Next, the other locking portion 255 is also locked to the body cavity tissue 411 with the hemostatic clip 410 for endoscope. Here, when the locking member 256 is loose, the binding member 258 is moved in the tightening direction so that the locking member 256 is adjusted to be in a tension state with respect to the holding member 201. Since the binding member 258 has a check structure, it does not move freely in the longitudinal direction of the belt after adjustment. As a result, the holding member 201 (capsule endoscope 300) is brought into a locked state in which a strong pressing force by the locking member 256 also acts on the body cavity tissue 411 side, and the holding member 201 (capsule endoscope 300). ) Is maintained in a state in which the generatrix L is in line contact with the body cavity tissue 411 in a desired direction, and an accurate visual field can be obtained.

(Embodiment 3)
A third embodiment of the present invention will be described with reference to FIG. FIG. 24 is a schematic side view showing a configuration example of the third embodiment. The third embodiment is composed of a linear belt-shaped guide member formed longer than the capsule endoscope 300, and is aligned with the direction in which the capsule endoscope 300 is intended to be placed. The stop portion 260 includes a locking member 261 that is locked to the body cavity tissue 411 in a stretched state, and a part of the outer peripheral surface of the holding member 201 is movably connected to the locking member 261 by a connecting member 262. It has been made. Here, the locking member 261 has a serrated non-return structure over its entire length, and the connecting member 262 is connected and locked so as to be movable only in one direction.

According to the configuration of the third embodiment, the locking member 261 is positioned on the body cavity tissue 411 so as to coincide with the direction in which the capsule endoscope 300 is to be placed, and the locking portions 260 at both ends thereof are positioned. By locking the body cavity tissue 411 in a stretched state, the posture of the holding member 201 (and hence the capsule endoscope 300) connected to the locking member 261 by the connecting member 262 is also determined. In other words, the detention line L of the capsule endoscope 300 parallel to the observation axis O is stably in line contact with the tissue 411 in the body cavity in a desired direction through the connecting member 262 and the locking member 261. It is in a fixed state, and an indwelling state in which the direction of the observation visual field of the observation optical system 307 or the observation axis O is not displaced can be secured. Further, since the connecting member 262 is movably connected to the locking member 261, the capsule endoscope 300 can be moved in the observation axis direction even after the locking member 261 is locked to the body cavity tissue 411. It can be fixed by adjusting the position.

(Modification 9)
Modification 9 will be described with reference to FIG. FIG. 25 is a schematic perspective view illustrating a configuration example of Modification 9. The modification 9 uses a locking member in which a locking position is set at a position where the capsule endoscope 300 is sandwiched in the axial direction of the observation axis O as in the case of the third embodiment. This is different in that the locking member 265 is used. That is, the locking member 265 having the locking portion 266 at the end is provided by being connected to the top portion of the distal end cover housing 304a of the capsule endoscope 300 or the gripping portion 201a of the holding member 201, and the capsule type endoscope The mirror 300 is positioned so as to coincide with the direction in which the mirror 300 is to be placed, the locking member 265 is also aligned with the direction, and the locking portion 266 at the end is placed in a tensioned state so as to be hemostatic for the endoscope. The clip 410 is locked to the body cavity tissue 411.

According to the configuration of the modified example 9, the direction of the locking member 265 is determined in accordance with the direction in which the capsule endoscope 300 is to be placed, and the locking portion 266 at the end thereof is stretched on the tissue 411 in the body cavity. By locking in the installed state, the postures of the holding member 201 and the capsule endoscope 300 to which the locking member 265 is connected are also determined. In other words, the generatrix L of the capsule endoscope 300 parallel to the observation axis O is in an indwelling fixed state in which the generatrix L is stably in line contact with the body cavity tissue 411 in a desired direction, and the observation visual field direction or the observation axis of the observation optical system 307 is set. An indwelling state in which O does not shift can be secured.

The present invention is not limited to the above-described embodiment, and various modifications can be made without departing from the spirit of the present invention. For example, in the above description, the holding member 201 for mounting and holding the capsule endoscope 300 is provided and the locking member is connected to the holding member 201. However, the holding member 201 is omitted. Then, the capsule indwelling medical apparatus may be configured by providing a locking member directly connected to the capsule endoscope 300. In particular, in the case of the locking members 225 and 230 shown in FIGS. 16 and 18 and the like, the capsule endoscope 300 can be directly connected without any special treatment, which is preferable.
