Abstract

【課題】 圧縮力のみを必要とし、同時に、移動および滑りを防止する優れたトラクションが得られるステープル／クリップを提供することにある。
【解決手段】 クリップコンポーネント（２１０、２０５）および固定部材（２２０）を備えた、広範囲の手術に使用できる外科用ステープル／クリップ。本発明のステープル／クリップは、非組立て状態で、小さいポートまたはトロカールを通して手術部位まで導入できる。クリップコンポーネントおよび固定部材を位置決めしかつ適用するための、１対の対向ジョー状チャネルを備えたステープル／クリップアプライヤも提供される。クリップコンポーネントは目標組織の周囲に位置決めされ、かつ閉塞、結紮または固定等の特定手術に必要な力のみを用いて組織上に圧縮されかつクランプされる。クリップコンポーネントが適正に適用されたならば、固定部材がクリップコンポーネント上で前方に押されて、ステープル／クリップを固定する。

Description

発明の詳細な説明
本願は、「外科用ステープル／クリップおよびアプライヤ（Surgical Staple-Clip and Applier）」の名称に係る２００２年１２月１７日付米国仮特許出願第６０／４３４，３４４号の優先権を主張する非仮特許出願であり、本願には、前記米国仮特許出願の全部を援用する。
本発明は、広くは医療装置に関し、特に、手術に使用できるステープル／クリップおよびアプライヤに関する。
クリップおよびステープルは、種々の組織および脈管の閉塞、結紮および固定等の多くの手術に広く使用されている。クリップは、目標組織の周囲に配置されかつ一体にクランプされて組織を絞窄または閉塞する、ほぼＵ型の開端ワイヤである。クリップアプライヤは、一般に、クリップを適用するのに使用される。クリップアプライヤは、切開手術および最小侵襲手術すなわち腹腔鏡手術の両手術において、単一クリップまたは多クリップを適用するように構成できる。個々のクリップは、クリップアプライヤのローディングカートリッジまたはラックに入れて提供される。最小侵襲手術では、クリップアプライヤは、約１０〜１２ｍｍの直径をもつ小さいアクセスポートまたはトロカールを介してクリップを供給できなくてはならない。従って、供給可能な腹腔鏡クリップのサイズは、該クリップが導入されるトロカールの内径より小さくなくてはならない。一般に、１２ｍｍクリップアプライヤが供給できるのは７〜８ｍｍ以下のクリップである。腹腔鏡を用いる外科医にとって、入手できる最大クリップでも特定手術にはサイズが小さ過ぎることを見出すことは珍しくない。また、クリップは、ときとして、クリップが適用される組織の回りで移動しまたは滑り落ちることがある。或る場合には、外科医は、クリップの移動または滑りを最小にするため、クリップおよび組織に過大圧縮力を加えることがある。この過大圧縮により、組織への養分補給が妨げられまたは遮断されるため、組織の壊死が引起こされることもある。
またステープルは、身体組織または脈管を絞窄または閉塞する多くの手術に広く使用されている。外科用ステープルは一般に、ベース部分により連結された１対の穿刺脚を有している。外科用ステープルはステープラを用いて適用される。ステープラは、穿刺脚が身体組織を通って前進するときに穿刺脚を圧縮し、かつステープラの対向ジョーに当接して曲げられ、ステープルを身体組織に固定する。ステープルの１つの特徴は、ステープルが曲げられるときでも、組織に養分を供給する開部分を形成することである。外科用ステープルは有効であることが証明されているが、ステープルの適用に使用されるステープラは嵩張るものが多く、非常に強い閉じ力すなわち圧縮力を必要とする。これは、最小侵襲手術すなわち腹腔鏡手術にとって理想的ではない。
従って、ステープル／クリップを閉じる力すなわち圧縮する力のみを必要とすると同時に、移動および滑りを防止する優れたトラクションが得られるステープル／クリップを創出することが望まれている。特に、身体組織を絞窄または閉塞するのに必要な力と、ステープル／クリップを所定位置に固定しかつ維持するのに必要な力とが異なるステープル／クリップを得ることが望まれている。このようなステープル／クリップならば、身体組織への適正な養分補給を維持できると同時に、優れたトラクションが得られるであろう。このようなステープル／クリップは、例えば、ドナー腎摘出術において腎脈管を固定するステープル／クリップとして使用するのに有利である。
本発明は、広範囲の手術に使用する外科用ステープル／クリップに関する。本発明のステープル／クリップは、クリップコンポーネントと、複合ステープル／クリップを形成すべくクリップコンポーネントに連結される固定部材とを備えた複数の個々の要素を有している。複合ステープル／クリップは、切開手術および最小侵襲手術すなわち腹腔鏡手術の両方に使用できるように構成できる。ステープル／クリップは、小さいポートまたはトロカールを介して、非組立て状態で手術部位まで導入できる。クリップコンポーネントおよび固定部材の位置決めおよび適用を行うのに、ジョーのような１対の対向チャネルを備えたステープル／クリップアプライヤが提供される。クリップコンポーネントは、例えば閉塞、結紮または固定等の特定手術に必要な力のみを用いて、目標組織の周囲に位置決めされかつ目標組織上に圧縮すなわちクランプされる。クリップコンポーネントが適正に適用されかつ所望の効果が観察されたならば、固定部材がクリップコンポーネント上で前方に向かって押され、ステープル／クリップを固定する。
本発明の一態様では、クリップコンポーネントは、表面凹凸、突出部、谷部、隆起部等を含むトラクション増強手段を有している。本発明の他の態様では、クリップコンポーネントは、ステープルの組織穿刺手段と同様な組織穿刺手段を有している。本発明のステープル／クリップによれば、組織を絞窄または閉塞するのに要する力と、ステープル／クリップを所定位置に固定しかつ維持するのに要する力とは異なるものであることが理解されよう。すなわち、閉塞、結紮または固定等の特定手段の遂行に必要な圧縮力のみが身体組織に加えられ、従来技術で通常必要とされている、クリップを固定しかつ維持する力は加えられない。なぜならば、トラクションおよび固定力は、ステープル／クリップのクリップコンポーネントおよび固定部材により付与されるからである。従って、身体組織が過大圧縮されることはなく、身体組織への養分補給が維持される。
本発明の他の態様は、ハンドアシスト型腹腔鏡（hand assisted laparoscopy：ＨＡＬ）に使用する親指操作型クリップアプライヤである。一実施形態では、本発明のクリップアプライヤはハンドルおよび親指操作機構を有し、該親指操作機構は、片手のみを用いて親指操作機構を前後に摺動させることにより、クリップを身体組織または脈管上に滑り込ませるように解放する。この設計は、身体組織または脈管の周囲で両ジョーを閉じ、閉じられたクリップを所定位置に滑り移動させるものである。他の実施形態では、第一アームと、該第一アーム上に重ね合わされる第二アームとを備えたクリップが提供される。第一アームの遠位端には、内方に曲げられた部分すなわちフックのようなラッチ機構が設けられており、該ラッチ機構は、両アームが一体にクランプされたときに第二アームの遠位端と相互ロックすなわち係合するように構成されている。ラッチ機構は、親指の圧力に応答して、ヘアクリップと同様に作動する。オペレータはクリップに片手でアクセスして、必要に応じて身体組織または脈管上にクリップを滑り込ませる。クリップの両アームの対向内面には、組織穿刺要素を設けることもできる。
本発明のもう１つの態様では、クリップコンポーネントと、クリップが適正に位置決めされた後にクリップを固定するステープルコンポーネントとを有する２段階クリップを開示する。このクリップコンポーネントは第一材料ピースから形成されかつ対向アームを有している。該対向アームの各々には、ステープルコンポーネントの脚を通すことができる開口が設けられている。ステープルコンポーネントは、第二材料ピースから形成されかつ身体組織または脈管を穿刺しかつクリップコンポーネントを相互ロックするのに使用される。クリップコンポーネントの遠位端には、両アームが閉じられまたは一体にクランプされたときに互いに係合するラッチ機構を設けることができる。使用中の第一段階では、クリップの両アームが閉じられる。第一段階後では、クリップは依然として安全に取外すことができる。第二段階では、ステープルコンポーネントをクリップコンポーネントに適用することにより、クリップが身体組織上に永久的に固定される。他の実施形態では、２段階クリップは、その全体が単一の材料ピースから形成されかつ第一アームとこれに対向する第二アームとを有している。第二アームは更に固定要素を有し、該固定要素の全体が一体ワンピース構造として形成される。この構造によれば、両アームが身体組織または脈管上にクランプされたときに、固定要素は、第一段階後に依然として開いた状態を維持する。次に、第二作用により、固定要素が身体組織または脈管内に押込まれる。
本発明の上記および他の特徴および長所は、添付図面を参照して述べる好ましい実施形態についての以下の説明から一層明瞭になるであろう。
図１および図２は、第一脚１５、第二脚２０、および該第一脚１５と第二脚２０とを連結するベース３０を備えた、全体的に開いた形状を有する従来技術の外科用クリップ１０を示すものである。クリップ１０は、図５Ａに示すようなクリップアプライヤ１５０を用いて、例えば血管のような身体導管すなわち通路の周囲に配置されかつ圧縮される。クリップアプライヤ１５０は、概略的に、外科用トロカールのポートに挿通できるようにサイズが定められかつ構成された細長シャフト１５５と、１対の対向ジョー１６５、１６７を備えた遠位端１６０と、両ジョー１６５、１６７を開閉させるハンドル１８０を備えた近位端１７０とを有している。クリップ１０は、開状態に保持され、自動または手動によりジョー１６５、１６７に供給される。クリップ１０は、所望部位に前進され、次に閉じられかつ圧縮される。第一および第二脚１５、２０は適正に形成されており、両ジョー１６５、１６７は、組織の周囲にクランプされたクリップ１０を残して、開かれかつ組織から離れる方向に移動される。クリップの強度は、クリップが作られる材料で定まる。選択される材料は、クリップが、図１Ｂに示すように圧縮できると同時に、圧縮力を加えた後の材料のスプリングバックを防止する充分な展性を有するものでなくてはならない。
外科用クリップを身体組織または脈管に適用する上で考慮しなければならない多くのファクタがある。第一に、クリップは、組織を完全に包囲できる充分な幅をもたなくてはならない。図１Ａに示すように、クリップ１０は、目標組織を包囲するのに充分幅広である開領域５０をもたなくてはならない。第二に、クリップは、リーズナブルな適用力、すなわちオペレータが身体組織への快適な適用を感じ得る力で圧縮されなくてはならない。第三に、クリップは、一旦適用されたならば適用位置から移動してはならない。図２に示すように、クリップ１０は、身体脈管６０の軸線８０Ａ、８０Ｂに沿って滑ることまたは脈管６０から滑り落ちてはならない。第四のファクタは、クリップ１０が、組織の養分補給を絞窄し、妨げまたは遮断するまでに身体組織を圧縮してはならないことである。理解されようが、クリップによる結紮または固定の上記ファクタは、単一ワイヤで形成されたクリップを用いて達成することは困難である。また、クリップの確実な配置を維持すると同時に組織の適正な養分補給を維持すべき場合には、圧縮およびトラクションの問題点を分離して考えなくてはならない。
外科用ステープルは、身体組織および脈管の閉塞、結紮および固定に一般的に使用されるもう１つの医療装置である。図３Ａに示すように、一般的な外科用ステープル１００は、ベース部分１３０により一体に連結されたほぼ真直な穿刺脚部１１０、１２０を有している。穿刺脚部１１０、１２０が身体組織または脈管に適用されると、脚部は組織を通って前進しかつステープラの対向ジョーに当接して、図３Ｂに示すように互いに向かい合う方向に曲げられる。ステープル１００が曲げられると、組織自体の圧縮とは独立的に、身体組織の大部分が閉じ込められる。図５Ｂには腹腔鏡ステープラ１９０が示されており、該ステープラ１９０は、クリップアプライヤ１５０と非常に良く似た態様で作動する。使用に際し、ステープラ１９０は身体組織の一部上を前進されかつ圧縮されて、少なくとも１つの（多くの場合には幾つかの）外科用穿刺ステープルを供給する。例えば図４Ａおよび図４Ｂを参照されたい。圧縮された外科用ステープル１００は、全体的にアルファベットの大文字「Ｂ」に似た形状になり、このため、折曲げられたステープル１００の開部分１４０を介して組織の流体養分補給が行われる。外科用ステープルは有効であることが証明されており、手術における標準となっている。しかしながら、ステープルの適用に使用されるステープラはしばしば大型であり、非常に強い閉じ力すなわち圧縮力を必要とする。これは、最小侵襲手術すなわち腹腔鏡手術にとって理想的ではない。従って当業界では、ステープルおよびクリップの両方の特徴および長所を備えた医療装置が要望されている。本発明のステープル／クリップは、医療装置が身体組織から移動しまたは外れてしまうことを防止すると同時に、組織への適正な養分補給を維持できる良好なトラクションが得られる。また、身体組織の絞窄または閉塞に要する力は、医療装置を所定位置に固定しかつ維持するのに要する力から分離される。より詳しくは、治療すべき組織の部分は、所望結果の達成に必要とされる以上に圧縮されることはない。
図６Ａは、本発明の第一実施形態による外科用ステープル／クリップ２００を示すものである。ステープル／クリップ２００は、第一組織係合部材２０５と、該第一組織係合部材２０５に対向する第二組織係合部材２１０と、第一および第二組織係合部材２０５、２１０を固定する固定部材２２０とを有している。第一組織係合部材２０５は、全体的に対向する壁２３０、２３２と、これらの対向壁２３０、２３２と協働して細長チャネル２３６を形成する連結壁２３４とを有している。第二組織係合部材２１０も第一組織係合部材２０５と同様であり、全体的に対向する壁２４０、２４２と、これらの対向壁２４０、２４２と協働して細長チャネル２４６を形成する連結壁２４４とを有している。両組織係合部材２０５、２１０は、通常、連結壁２３４、２４４の前面が互いに対向するようにして保持される。両連結壁２３４、２４４の対向前面には、それぞれ、複数の組織穿刺要素２３８、２４８を設けることができる。
本発明のステープル／クリップ２００の長所は、該ステープル／クリップに過大圧縮力を加える必要なく、優れたトラクションが得られることである。特に、最小の圧縮力で組織係合部材２０５、２１０を身体組織または脈管の周囲に確実にクランプするため、固定部材２２０は、細長チャネル２３６、２４６内に滑り込むことができるようにサイズが定められかつ構成されている。より詳しくは、（充分なトラクションを得るため）ステープル／クリップを固定しかつ維持するのに必要な力は、身体組織または脈管を絞窄または閉塞するのに必要な力から独立している。本発明のステープル／クリップでは、身体組織には、閉塞、結紮または固定等の特定手術の遂行に要する圧縮力のみが身体組織に加えられ、従来技術のクリップを固定しかつ維持するのに通常必要とされる力は加えられない。なぜならば、組織係合部材２０５、２１０によりトラクションおよび固定が行われるからである。換言すれば、本発明のステープル／クリップは、該ステープル／クリップが移動しまたは緩まないように維持すべく過大圧縮力を要することなく、必要なトラクションが得られる。この結果、軽く圧縮された組織の養分補給が維持され、過大圧縮により組織が壊死することがなくなる。
本発明の他の態様では、組織係合部材は、図１３Ａおよび図１３Ｂに全体的に示すように、突出部、隆起部、スロット、孔等のトラクション増強手段を有している。トラクション増強手段は、組織をグリップできるようにサイズが定められかつ構成され、かつ従来技術の一般的なクリップの過大圧縮により達成されるトラクションおよび固定力を超えるトラクションおよび固定力が得られる。固定部材２２０は、図７、図９Ａ〜図９Ｈおよび図１４に示すように、保持部材として作用しかつ閉塞された組織または脈管を横切る均一圧力を付与して、時間の経過とともにステープル／クリップ２００が緩むことを防止するスプリングクリップまたは変形可能クリップとして構成できる。組織穿刺要素２３８、２４８は、図１１Ａ〜図１１Ｄ、図１６および図１７に示すようにクランプされたときに、組織を穿刺して、組織が移動または滑ることを防止する。
連結壁２３４、２４４およびそれぞれの組織穿刺要素２３８、２４８は、一体ワンピース構造として形成できることは理解されよう。また、組織穿刺要素の列の数および１列当りの組織穿刺要素の数は、各適用、クリップの形状およびサイズ、および身体組織に従って変えることができることも理解されよう。更に、各列の組織穿刺要素は、所望に応じて、整合させることも、互い違いに配置することも可能であることに留意されたい。組織係合部材２０５、２１０および固定部材２２０は、多角形、円形および楕円形を含む任意の断面形状にすることができる。
図１０を参照すると、ここには、本発明の他の態様によるモノリシックステープル／クリップ５００が示されている。モノリシックステープル／クリップ５００は、アルファベットの大文字「Ｕ」に似た形状を有している。ステープル／クリップ５００は、第一組織係合部分すなわち脚５０５と、これに対向する第二組織係合部分すなわち脚５１０と、これらの第一および第二組織係合部分５０５、５１０を連結する変形可能な連結部分５２０とを有している。両組織係合部分５０５、５１０の各対向面は、それぞれ、複数の組織穿刺要素すなわち突出部５２５、５３０を有している。これらの突出部５２５、５３０は、両組織係合部分５０５、５１０の間に捕捉される組織を穿刺できるようにサイズが定められかつ構成され、かつ従来技術の一般的なクリップの過大圧縮により達成されるトラクションおよび固定力を超えるトラクションおよび固定力が得られる。本発明の他の態様と同様に、閉塞、結紮または固定等の特定手術の遂行に要する力のみを身体組織に加えればよく、クリップの固定および維持に従来必要とされた力はもはや加える必要はない。かくして、軽く圧縮された組織の養分補給が維持され、過大圧縮による組織の壊死が生じることはない。
モノリシックステープル／クリップ５００は、ノッチまたは歯が形成された第一部分と、滑らかな連結部分と、ノッチまたは歯が形成された第二部分とをダイカットし、スタンピングしまたはエッチング形成することにより、平らな金属シートから作ることができる。次に、第一および第二部分のノッチまたは歯が同一方向または同一平面内に延びかつノッチまたは歯内にチャネルが形成されるようにして、ノッチまたは歯が曲げられる。次に、両組織係合部分が対向するように連結部分を曲げることにより、ステープル／クリップがＵ型に形成される。本発明のモノリシックステープル／クリップの好ましい実施形態は、チタンまたはステンレス鋼等の展性材料からなる。他の材料として、延性、展性または変形可能性を有する医用金属またはプラスチックがある。
本発明のステープル／クリップは、図１２Ａおよび図１２Ｂに示すようなアプライヤ６００を用いて身体組織または脈管に適用できることは理解されよう。ステープル／クリップアプライヤ６００は、概略的に、外科用トロカールポートに挿通できるようにサイズが定められかつ構成された細長シャフト６０５と、１対の対向ジョー６１５、６２０を備えた遠位端６１０と、両ジョー６１５、６２０を開閉させるハンドルを備えた近位端（図示せず）とを有している。ステープル／クリップアプライヤ６００は更に摺動部材６２５を有し、該摺動部材６２５は、より詳細に後述するように、両ジョー６１５、６２０が閉じられた後に組織係合部材２０５、２１０上で固定部材２２０を前進させる。両ジョー６１５、６２０は、目標身体組織または脈管の周囲に、それぞれ組織係合部材２０５、２１０を適用すべく作動する。組織係合部材は、手動または自動によりジョーに供給される。組織係合部材２０５、２１０が適正に配置されたならば、閉塞、結紮または固定等の特定手術に必要な力のみを用いて両ジョー６１５、６２０を圧縮することができる。組織係合部材２０５、２１０が適正に適用されたならば、次に摺動部材６２５により固定部材２２０を組織係合部材２０５、２１０上で前方に押しやり、図９Ａ〜図９Ｈ、図１１Ａ〜図１１Ｄおよび図１４に示すようにステープル／クリップ２００を固定する。組織係合部材２０５、２１０および固定部材２２０は、非組立て状態で、小さいポートまたはトロカールを通って手術部位に導入できる。また、図８および図１５Ａ〜図１５Ｃには、最小侵襲手術すなわち腹腔鏡手術に使用できるようにサイズが定められかつ構成されたステープル／クリップおよびアプライヤが示されている。
ステープル／クリップアプライヤには多数のステープル／クリップを装填し、個々にまたは同時に、両ジョー間に前進させることができる。ステープル／クリップを同時に適用する場合には、アプライヤの対向ジョーには、多数のステープル／クリップを受入れるための複数のスロットを設けなくてはならない。アプライヤにはブレードのような切断部材を設けることができる。切断部材は、図１７および図１８に示すように、ステープル／クリップが適用された後に両ステープル／クリップ間に前進されて、身体組織を離断する。
本発明の他の態様において、図１９は、特にドナー腎摘出術用に構成された伝統的な腹腔鏡ステープラ８００を示すものである。ステープラ８００は、患者側の複数のステープル列８０５と、腎臓側の一般的なステープル組８１５に置換される一時的クリップ８１０とを有している。ステープラ８００は、一時的クリップ（単一または複数）８１０が腎臓側のステープル組８１５に置換される点を除き、既存の配置／切断ステープラと同様に作動する。一時的クリップ８１０は、ドナー腎摘出術用に構成されたステープルカートリッジで構成できる。この構成の長所は、移植手術のための脈管の大きい部分を治療できることにある。
本発明の他の態様は、ハンドアシスト型腹腔鏡（ＨＡＬ）に関するものであり、ＨＡＬが広範囲に受入れられていることは、片手手術を利用する外科的進歩に多くの機会を創出している。図２０Ａ〜図２０Ｅには、本発明の教示による親指操作型クリップアプライヤ８５０、８７５、８９５が示されている。クリップアプライヤ８５０は、ハンドル８５５と、親指操作機構８６０とを有している。オペレータは、図２０Ａおよび図２０Ｂに示すように、親指操作機構８６０を前方８７０Ａおよび後方８７０ｂに摺動させることにより、クリップ８６５を摺動解放することができる。この設計は、脈管の周囲で両ジョーを閉じて、閉じられたクリップを所定位置に摺動できるようにするものである。ハンドル８５５はまた、付加クリップ８６５のリザーバとしても機能する。クリップアプライヤ８７５もＨＡＬ用として設計されており、第一アーム８８０と、該第一アーム８８０上に重ね合わされる第二アーム８８５とを有している。第一アーム８８０はラッチ機構８９０を有し、これにより、両アーム８８０、８８５が一体にクランプされたときに、第一アーム８８０の遠位端に設けられた内方湾曲部すなわちフックが、第二アーム８８５の遠位端と相互ロックすなわち係合するように構成されている。ラッチ機構８９０はヘアクリップと同様に作動し、親指の圧力に応答する。オペレータは、クリップ８７５に片手でアクセスし、必要に応じて、クリップを身体組織または脈管上に摺動させることができる。
複数のクリップ８７５は、開ポートを通して懸架されるクリップスリーブ８９２内で、端と端とを突合せて一列に並べることができる。クリップ８７５の長所は、配置を間違えることが全くないことである。クリップスリーブ８９２は、任意の開ポートまたはトロカールを通して引出しまたは供給することもできる。クリップスリーブ８９２はまた、より小さいポートまたはトロカールを使用できるように、クリップ８７５の一部を閉じた状態に維持するように設計できる。図２０Ｅに示すクリップ８９５はクリップ８７５と同様であるが、アーム８９６、８９７の内面上に組織穿刺要素８９８が更に設けられている。クリップ８９５はまた、両アーム８９６、８９７を連結する第三アーム８９９を有している。
図２１に示す本発明の他の態様では、クリップおよびステープルの所定位置に身体組織または脈管を縫合するのにスプリング状のコイル９００が使用される。スプリング状コイル９００は、溝付きジョー９１０、９１５を備えたステープラ９１０を使用することにより身体組織または脈管９０５上に適用される。スプリング状コイルは一方の溝付きジョー９１０、９１５内に装填すなわちねじ込み、次に、他方の溝付きジョーに当接させて圧縮すなわちクランプして脈管９０５を絞窄または閉塞するのが好ましい。この設計の長所は、コイル９００を閉じることにより、均一に適用された縫合糸に似たステッチのような性質が得られることにある。一形態では、各縫合糸の代わりに１つのコイルが挿入される。各特定の手術または閉塞に対して、ステープラの溝付きジョー内に単一または複数のコイルを装填できることは理解されよう。
本発明の更に別の態様として、図２２には、現在のステープラカートリッジに付随する高い製造コストを低減させるため、単一の材料ピース９３０から形成された多ステープル９２５が示されている。このようなコスト削減は、利ざやを増大させまたは製造コストを低減させる。各ステープル９２５は、ステープラが容易に持上げかつ閉じることができるように、傾斜脚９３５および傾斜レール９４０を有している。ステープル９２５は共通部分９４５を有し、該共通部分９４５は、ステープルの列を一体ワンピース構造として形成することを可能にする。ステープル９２５はまた、互いにオフセットして多列を構成するように形成することもできる。本発明のこの態様の長所は、ドナー腎摘出術に適したコンパクトなカートリッジを提供できることである。
図２３は、本発明の他の態様による、ドナー腎摘出術に使用する一時的ＨＡＬクランプ９５０を示すものである。クランプ９５０は、近位端９６５から遠位端９７０まで延びている開口９６０を形成している管状セクション９５５と、近位端９６５に操作可能に取付けられた導入ワイヤ９７５とを有している。導入ワイヤ９７５は、開位置と閉位置との間で移動できる。閉じられたとき、導入ワイヤ９７５は、管状セクション９５５の開口９６０内に摺動して受入れられかつ固定される。使用中に、オペレータは、導入ワイヤ９７５の周囲に脈管（単一または複数。例えば、腎動脈および腎静脈をクランプで一体に締付けることができる）を手で巻付け、ワイヤ９７５を管状セクション９５５の開口９６０内に固定できる。導入ワイヤ９７５は管状セクション９５５の壁に当てて曲げ、腎臓の摘出および搬送のために脈管（単一または複数）を更に固定することができる。
図２４、図２５および図２６には、それぞれ、本発明の付加実施形態による２段階クリップ１０１０、１０２０、１０３０が示されている。クリップ１０１０、１０２０、１０３０は、これらを身体組織または脈管上に取付けるためのアプライヤ（図示せず）を必要とする。クリップ１０１０は、クリップコンポーネント１０１１と、クリップが適正に位置決めされた後にクリップを固定するステープル１０１２とを有している。クリップコンポーネント１０１１は単一ワイヤで形成されており、対向アーム１０１３、１０１４を有している。各アーム１０１３、１０１４はそれぞれ開口１０１５、１０１６を有しており、これらの開口には、ステープル１０１２の脚１０１７、１０１８が通ることができる。ステープル１０１２は第二ワイヤで形成され、かつ身体組織または脈管を穿刺しかつクリップコンポーネント１０１１を相互ロックするのに使用される。アーム１０１３、１０１４の遠位端には、両アームが閉じられすなわち一体にクランプされたときに互いに係合できるラッチ機構を設けることができる。使用の第一段階では、クリップのアーム１０１３、１０１４が閉じられる。第一段階の後では、クリップは依然として安全に取外すことができる。第二段階では、ステープル１０１２をクリップコンポーネント１０１１に取付けることにより、クリップが身体組織または脈管上に永久的に固定される。
図２５に示す２段階クリップ１０２０は、その全体が単一の材料ピースから形成されており、第一アーム１０２１と、これに対向する第二アーム１０２２とを有している。第二アーム１０２２は更に固定要素１０２３、１０２４を有し、これらの全ては一体ワンピース構造として形成されている。この構造により、固定要素１０２３、１０２４は、第一段階（アーム１０２１、１０２２が身体組織または脈管上にクランプされたとき）の後に依然として或る角度に維持される。次に、第二作用により、固定要素１０２３、１０２４が身体組織または脈管内に押込まれる。図２６に示すクリップ１０３０はクリップ１０１０に似ており、クリップコンポーネント１０３１と、クリップが適正に位置決めされた後にクリップを固定するステープルコンポーネント１０３２とを有している。クリップコンポーネント１０３１は対向アーム１０１３、１０１４を有し、該アームは、それぞれ開口１０３５、１０３６を形成している。ステープルコンポーネント１０３２は対向アーム１０３７、１０３８を有し、該アーム１０３７、１０３８は、それぞれ組織固定要素１０３９、１０４０を備えている。組織固定要素１０３９、１０４０は、クリップを、開口１０３５、１０３６を通る身体組織または脈管上に固定すべく作用する。クリップ１０３０は、シート材料で作ることができる。
図２７には、手術中に組織を傷付ける可能性がある例えば組織穿刺要素のような尖鋭部を備えたステープル／クリップに使用するための組織ホルダ１１００が示されている。より詳しくは、組織ホルダ１１００は、身体組織を平らに押さえて、ステープル／クリップの取付け中にクリアランスを付与するのに使用される。

Claims (43)

  1. 身体組織又は脈管を絞窄し、または、閉塞する医療装置であって、
    対向する壁２３０、２３２と、第一連結壁２３４とを備えた第一組織係合部材２０５を有し、これらの壁２３０、２３２、２３４が第一細長チャネル２３６を構成し、
    前記第一組織係合部材２０５に対向する第二組織係合部材２１０を有し、該第二組織係合部材２１０が、対向する壁２４０、２４２と、第二連結壁２４４とを備え、これらの壁２４０、２４２、２４４が第二細長チャネル２４６を構成し、
    第一連結壁２３４の前面と第二連結壁２４４の前面とが互いに対向するように、第一組織係合部材２０５と第二組織係合部材２１０とを固定する固定部材２２０を有する、
    医療装置。
  2. 前記第一連結壁２３４及び第二連結壁２４４の各前面が複数の組織穿刺要素２３８、２４８を有する、請求項１記載の医療装置。

  31. 身体組織又は脈管を絞窄し、または、閉塞するクリップを使用するためのクリップアプライヤ８５０であって、
    前記クリップが、遠位端にラッチ機構８９０を備えた第一アーム８８０と、遠位端で第一アーム８８０上に重ね合わせられ、第一アーム８８０のラッチ機構８９０と相互係止又は係合するように構成された第二アーム８８５とを有し、
    前記クリップを貯蔵するためのハンドル８８５と、
    親指操作型機構８６０を前方８７０ａ、後方８７０ｂに摺動させて前記クリップを解放することによって前記ハンドル８８５に沿って作動摺動される親指操作式機構８６０とを有する、
    クリップアプライヤ。
  32. 前記ハンドル８５５は、複数のクリップ８６５を貯蔵するためのリザーバを更に有する、請求項３１記載のクリップアプライヤ。
  33. 複数のクリップ８６５が、端ポートを通して懸架されたクリップスリーブ８９２内に端と端とを突合せて一列に並べられる、請求項３２記載のクリップアプライヤ。
  34. 複数のクリップ８６５は単一の材料ピースから形成されている、請求項３３記載のクリップアプライヤ。
  35. 身体組織または脈管を絞窄しまたは閉塞するクリップ８７５において、
    遠位端にラッチ機構８９０が設けられた第一アーム８８０と、
    遠位端で第一アーム８８０上に重ね合わされて、第一アーム８８０のラッチ機構８９０と相互ロックすなわち係合するように構成された第二アーム８８５とを有する、
    クリップ。

  38. 前記第一アーム８８０と第二アーム８８５とを連結する第三アーム８９９を更に有する、請求項３５記載のクリップ。
  39. 身体組織または脈管を絞窄しまたは閉塞するクランプ９５０において、
    近位端９６５から遠位端９７０まで延びている開口９６０を備えた管状セクション９５５と、
    近位端９６５に操作可能に取付けられかつ開位置と閉位置との間で移動できる導入ワイヤ９７５とを有し、
    閉位置において、前記導入ワイヤは、管状セクション９５５の開口９６０内に摺動可能に受け入れられかつ固定される、
    クランプ。
  40. 前記導入ワイヤ９７５は管状セクション９５５の壁に当接して曲げられ、身体組織または脈管を更に固定する、請求項３９記載のクリップ。
  41. 前記クランプはドナー腎摘出術に使用される、請求項３９記載のクリップ。
  42. 身体組織または脈管を絞窄しまたは閉塞する医療装置１０１０において、
    第一ワイヤで形成されかつ対向アーム１０１３、１０１４および開口１０１５、１０１６を備えているクリップ１０１１と、
    該クリップ１０１１を固定するステープル１０１２とを有し、該ステープル１０１２は、身体組織または脈管を穿刺しかつ開口１０１５、１０１６を通ってクリップ１０１１を相互ロックする脚１０１７、１０１８を備えた第二ワイヤから形成されている、
    医療装置。
  43. 前記アーム１０１３、１０１４の遠位端には更に、両アーム１０１３、１０１４が閉じられすなわち一体にクランプされたときに互いに係合するラッチ機構が設けられている、請求項４２記載の医療装置。
