た。また、日本看護協会では創傷ケアの認定看護師を育成し、褥瘡対策に力を入れている。
褥瘡は、骨突出部位上の皮膚が、長期臥床により圧迫壊死により生じるものであるが、治癒が著しく遅い原因として、体動時の骨と皮膚の横ずれが大きな問題と考えられている。
現在体圧分散マットなど、褥瘡に対する介護用品が数多く販売されるようになってきた。しかし、それらでは、治療において横ずれ予防効果は得にくく、また褥瘡そのものを対象とする治療用具、製品はあまり開発されていないのが現状である。

しかし、従来の陰圧閉鎖療法には、以下のような問題点があった。
1)ドレーンは潰瘍外からの刺入を要する。

これら従来法は、潰瘍以外の部位からのドレーン挿入を要するうえ、フィルム材を利用しているため十分な陰圧が加わりにくいという問題点があった。
また、空気漏れにより持続的な陰圧が加わりにくいうえ、専用の装置が無いため臨床的に使用しにくかった。
また、特許文献としては、特表2002-524109号公報に、傷をカバーし更に傷の周辺付近にシールを形成すると共に傷上に空洞を形成するように包帯を具備し、流体供給管によって薬物を供給し流体ドレナジー管によって流体を取り除く傷治療装置が開示されている。

この吸引操作を繰り返したところ、図2(b)に示すように、創傷の底部が盛り上が って、創傷を完治することができた。
以下の本発明の陰圧閉鎖装置を適用した臨床結果に示すように、本発明により、褥瘡などの皮膚潰瘍や組織欠損による創傷を効果的に治療することができることが確認された

Therefore, using the negative pressure closing device of the present invention, first, cleaning water was supplied to the wound from a cleaning water supply pipe provided inside the drain pipe and cleaned.
In the present invention, the washing water supply method is not limited, but as shown in FIG. 2 (a), there is a method of injecting the washing water into the washing water supply pipe provided inside the drain pipe using, for example, a syringe. preferable.
Next, when the fluid in the wound was sucked using the suction pump (negative pressure generating device) 3, the cover 1 was in close contact with the surface of the wound, and a space was created between the cover 1 and the surface of the wound. I was able to suck it well.
When this suction operation was repeated, as shown in FIG. 2 (b), the bottom of the wound was raised, and the wound could be completely cured.
As shown in the clinical results of applying the negative pressure closure device of the present invention below, it was confirmed that the present invention can effectively treat a skin ulcer such as a pressure ulcer and a wound caused by a tissue defect.

<症例1> 54才女性 仙骨部褥瘡
胃癌に対する胃全摘手術を受けるも、術後臥位の継続を余儀なくされ術後3日後に仙骨部に褥瘡を形成した。褥瘡は骨の露出を伴い、皮下に広いポケットを形成し、壊死組織が付着し、感染を伴っていた。
約2か月間軟膏塗布にて治療を継続されていたが、創の縮小が全く認められないため、本発明者に紹介された。
入院後外科的に壊死組織を切除、創を洗浄後、本装置にて治療を開始した。本装置にて便による創汚染も予防でき、陰圧の漏れも生じず、良好に装着を継続できた。術後2週で潰瘍面積は1/2に縮小し、その後は外来通院にて本装置を装着して治療を行った。約40日にて潰瘍は治癒した。
<症例2> 67才女性 仙骨部熱傷潰瘍
約60%の広範囲全身熱傷を受傷。数回の手術(皮膚移植術)により、救命を行った。当初仙骨上の熱傷は浅い潰瘍であったが、臥位の継続により、褥瘡と同様の機序で徐々に深度が進行し、骨露出を伴い、皮下に広いポケットを形成し、壊死組織が付着する状態となった。そこで、外科的に壊死組織を切除後、本装置の装着を開始した。本装置にて便による創汚染も予防でき、陰圧の漏れも生じず、良好に装着を継続できた。約1週間で総面積は1/2に縮小し、2週間で創は治癒した。熱傷による創を全て閉鎖できたため、リハビリテーション目的に他院に転院した。
<症例3> 78才男性 仙骨部熱傷潰瘍
仙骨部、臀部にお湯による熱傷を受傷し、保存的に治療を受けた。臀部の創は約3週間で治癒したが、仙骨部の創は、臥位の継続により、褥瘡と同様の機序で徐々に深度が進行し、骨露出を伴い、皮下に広いポケットを形成した状態となり、本発明者に紹介された。創を洗浄後、本装置にて治療を開始した。本装置にて陰圧の漏れも生じず、良好に装着を継続できた。術後2週で潰瘍面積は1/2に縮小し、その後は外来通院にて本装置を装着して治療を行った。約50日にて潰瘍は治癒した。

With this device, wound contamination due to feces could be prevented, and no negative pressure leaked. Two weeks after the operation, the ulcer area was reduced to ½, and then the patient was treated at the outpatient clinic. The ulcer healed in about 40 days.
<Case 2> A 67-year-old female. Sacral burn ulcer. Lifesaving was performed by several operations (skin transplantation). Initially, the burn on the sacrum was a shallow ulcer, but with continued recumbence, the depth gradually increased with the same mechanism as pressure ulcer, with bone exposure, forming a large pocket under the skin, and necrotic tissue attached It became a state to do. Therefore, after the necrotic tissue was surgically excised, mounting of this device was started. With this device, wound contamination due to feces could be prevented, and no negative pressure leaked. In about a week, the total area was reduced to 1/2, and in 2 weeks the wound was healed. Since all wounds due to burns were closed, he was transferred to another hospital for rehabilitation.
<Case 3> 78-year-old male sacral burn ulcer The sacrum and buttock were burned with hot water and treated conservatively. The wound of the buttocks healed in about 3 weeks, but the wound of the sacrum gradually progressed in depth by the same mechanism as the pressure ulcer due to the continued position of the decubitus, forming a large pocket under the skin with bone exposure. It became a state and was introduced to the present inventors. After cleaning the wound, treatment was started with this device. With this device, no negative pressure leakage occurred, and it was possible to continue wearing the device well. Two weeks after the operation, the ulcer area was reduced to ½, and then the patient was treated at the outpatient clinic. The ulcer healed in about 50 days.

前記ドレーンパイプの反創傷側が負圧生成装置に連結されることにより、
