本発明は、例えばインスリンの如き薬剤を定量的に患者に注入するための薬剤注入装置に関するものである。

【従来の技術】
糖尿病患者の治療方法としてインスリンを注入することが行われてきているが、生体の血糖調節機能が朝1回ないしは朝夕2回の注射で十分にコントロールできないことから、糖尿病患者の最大の問題点である細小血管合併症の発症、進展を十分に阻止することが困難であることが分かってきている。そこで、患者の状態の変化に応じて、投与すべきインスリンの量及びタイミングを長期に亘って制御するベッドサイド型人工膵島とよばれるインスリン注入装置が提供されるに至っている。

かかるベッドサイド型人工膵島は、患者の血糖値を連続的に測定するセンサと、該センサで測定された血糖値に基づきインスリン(ブドウ糖)注入率を測定する制御手段及び注入ポンプとを有して成り、所定の測定キーを操作すると血糖連続測定が行われ、所定の運転キーを操作すると注入ポンプが作動して血糖制御が始まるよう構成されている。

ここで、血糖連続測定及び血糖制御の前に予め、患者の血糖値を測定すべきセンサの調整を行う必要がある。かかる調整は、例えば特許文献1で開示されているように、濃度が既知の低濃度溶液と、同じく濃度が既知の高濃度溶液との2つの溶液を用意しておき、これらを適宜電磁弁で切り換えることによりセンサがそれぞれの濃度を計測して検量線を作成し、これに基づいて当該センサを校正して調整を行うものが一般的であった。

ところで、上記ベッドサイド型人工膵島は極めて大型のものであり患者の普段の行動に支障を来すことから、近時においては、患者がインスリン注入装置を常時携行し、適宜のタイミングにてインスリン注入を行うことができる携帯型のインスリン注入装置が提供されつつある。かかる携帯型のインスリン注入装置は、軽量且つ小型の箱形本体から主に成り、該本体内にインスリンを収容可能なシリンジ及び該シリンジ内のインスリンを定量的に導出する定量ポンプが配設されているとともに、導出すべきインスリンの量及び注入速度を適宜なものとすべく定量ポンプを制御する制御手段(コントローラ)が配設されていた。

そして、シリンジの吐出口からはチューブが延設されており、該チューブの先端には患者の皮下に穿刺可能な注入針が形成されている。かかる注入針を皮下に穿刺したまま箱形本体を携行するようにすれば、制御手段による制御にて適宜のタイミングにてインスリン注入を行うことができ、患者の行動にあまり支障を及ぼすことがなくなるのである。

然るに、上記携帯型のインスリン注入装置においても、ベッドサイド型人工膵島などと同様、患者の血糖値を連続的に測定し、その測定値に基づいてインスリン注入のタイミング及び注入量を制御するようになっている(例えば、特許文献2にて開示されている)。かかる特許文献2には、携帯型のインスリン注入装置において、患者の血糖値を計測するためのセンサを別途配設し、かかるセンサの測定値に基づいて制御手段(コントローラ)によるインスリンの注入制御を行うものが開示されている。

【発明が解決しようとする課題】
しかしながら、上記の如きインスリン注入装置(ベッドサイド型人工膵島のような大型のもの及び携帯可能な小型のものを含む)においては、患者の血糖値を計測するセンサを具備させた場合、予めそのセンサの調整(センサを使用する前の動作確認又は検出値の校正を行うことを含む)を行う必要があるが、従来の高濃度及び低濃度の2溶液を用いた調整方法を適用した場合、各濃度の溶液を収容する容器からセンサまで流す切り換え手段(電磁弁など)が必要となり、装置が大型化してしまうという問題があった。

本発明は、このような事情に鑑みてなされたもので、センサで計測した計測値に基づき患者の体内に薬剤を注入するものにおいて、当該センサの調整手段を小型化して装置全体を小型化し得る薬剤注入装置を提供することにある。

【課題を解決するための手段】
請求項1記載の発明は、患者の体内に注入すべき薬剤を収容した薬剤収容手段と、該薬剤収容手段内の薬剤を定量的に導出する定量導出手段と、患者に穿刺されて、前記定量導出手段によって導出された薬剤を患者の体内に注入可能な注入針と、患者の体内における所定物質の濃度を計測するセンサと、該センサで計測された濃度に基づき、前記定量導出手段によって導出されるべき薬剤の注入速度又は注入タイミングを制御する制御手段とを具備した薬剤注入装置において、前記センサで計測すべき所定物質と略同一物質であって既知の量及び濃度の試液に前記センサ及び注入針を浸し、前記定量導出手段にて注入針から定量の薬剤を注入して試液を希釈するとともに、その希釈過程の濃度を前記センサにて計測して当該センサの調整を行うことを特徴とする。

【発明の実施の形態】
以下、本発明の実施形態について図面を参照しながら具体的に説明する。
本実施形態に係る薬剤注入装置は、糖尿病患者に対してインスリンを注入するための携帯型インスリン注入装置に適用されたものであり、図1に示すように、略箱型のインスリン注入装置本体1と、薬剤収容手段としてのシリンジ2と、操作パネル3と、樹脂製チューブ4と、注入針7と、センサ8とからその外観が主に構成されている。

インスリン注入装置本体1は、患者が携行し易いように、軽量且つ小型化されたものであり、上面には操作パネル3が形成されるとともに、その近傍には長手方向に延びる収容凹部1aが形成されている。かかる収容凹部1aは、シリンジ2が収容され得る形状及び大きさに設定されており、当該収容凹部1aに収容された状態のシリンジ2の吐出口2bは、インスリン注入装置本体1から微少寸法突出するよう形成されている。

シリンジ2は、患者の体内に注入すべきインスリンを所定量収容し得るものから成り、当該インスリンを収容した内部空間内に挿入可能なピストン部2aを有して構成されている。即ち、一般のシリンジと同様、ピストン部2aをスライドさせて内部空間内に挿入すれば、その分、収容されたインスリンが吐出口2bから導出するよう構成されているのである。

シリンジ2の吐出口2bには、樹脂製チューブ4の基端が接続されており、該樹脂製チューブ4の先端には注入針7が接続されている。かかる注入針7は、患者の皮下に穿刺されて、シリンジ2の吐出口2bから導出されたインスリンを患者の体内に注入可能なものである。尚、図1に示すように、注入針7の基端側から側方に延設された2枚の板状部材は、樹脂製の接着板6を示しており、注入針7を患者に穿刺した後、かかる接着板6を患者の皮膚表面に接着固定させるべきものである。

また、注入針7の側面には、患者の体内における血糖値(グルコース濃度)を計測するためのセンサ8が固定されており、インスリン注入時には、注入針7とともにセンサ8が患者の皮下に穿刺され得るようになっている。かかるセンサ8からは、その計測値を示す電気信号をインスリン注入装置本体1側に送るための配線5が延設されている。

このように、注入針7とともにセンサ8も患者の皮下に穿刺された状態とされるため、患者の血糖値(グルコース濃度)を常時計測して監視することができ、かかる計測値に応じて自動的にインスリンを注入することが可能なように構成されている。尚、本実施形態においては、センサ8が注入針7の側面に固定されたものとされているが、これに代えて、注入針7内にセンサ8が一体的に形成されたもの、或いは注入針7とセンサ8とが完全に別体とされたもの(勿論、別体のセンサ8も患者の皮下に穿刺される必要はあるが)であってもよい。

一方、インスリン注入装置本体1内には、図2に示すように、定量導出手段としてのモータ11とこれを制御する制御手段としての電子回路基板10(より具体的には当該電子回路基板10上に形成された不図示のマイコンなど)とが配設されている。このうちモータ11は、減速機部12を介して送りねじ軸14を回転可能とされたものであり、この送りねじ軸14が回転することにより、ピストン部2aが同図中左方向にスライドして、シリンジ2の内部空間内に挿入し得るよう構成されている。

また、モータ11は、任意の速度で精度良く回転駆動し得るものから成るため、シリンジ2内のインスリンを、その吐出口2bから定量的に導出することが可能となっている。尚、同図中符号9、13は、送りねじ軸14を手動にて回転させるための手送りダイアル、及び送りねじ軸14の回転量を検出するエンコーダを示しており、エンコーダ13にて送りねじ軸14の回転数を検出し、吐出口2bから導出されたインスリンの量を認識し得るよう構成されている。

また、電子回路基板10は、センサ8で計測された濃度(グルコース濃度)に基づき、モータ11によってシリンジ2から導出されるべきインスリンの注入速度又は注入タイミングを制御するものである。即ち、センサ8による測定値は、電子回路基板10にフィードバックされ、血糖値が高い(グルコース濃度が高い)と判断された場合に、モータ11を駆動させてインスリンを患者の体内に注入させるように構成されているのである。

勿論、電子回路基板10には予め医師などが設定した注入タイミングになると、モータ11を所定速度で所定量駆動させ、インスリンを設定速度にて任意量だけ注入するよう制御することも可能である。ところで、図1に示すように、操作パネル3には、注入状態を表示する液晶表示パネル3a、装置の入出力スイッチ3b、インスリンの流量を調整する流量設定スイッチ3c及びワンショット注入時に使用するワンショット用スイッチ3dが形成されている。尚、ここでいう「ワンショット注入」とは、持続注入する流量の何倍或いは何倍の薬液(インスリン)を短時間に注入する作業のことをいう。

ここで、樹脂製チューブ4及びその先端に接続された注入針7やセンサ8などは、シリンジ2内のインスリンを全て使い果たして1回のインスリン注入動作が終了した際には、インスリン注入装置本体1から取り外されて、新しいものに交換される。即ち、樹脂製チューブ4及びその先端の部品は使い捨てであるため、新たに交換したものの最初の使用前には、センサ8の調整を行う必要がある。

本実施形態におけるセンサ8の調整は、以下の作業手順にて行われる。まず、インスリンが充填されたシリンジ2の吐出口2bに新たな樹脂製チューブ4を接続する。かかる樹脂製チューブ4の先端には、図1に示したように、注入針7、センサ8及び接着板6が形成されており、既述の如く、センサ8からはインスリン注入装置本体1まで配線5が延設され、計測した濃度が電気信号として電子回路基板10に形成されたマイコン(不図示)などに送信されるようになっている。

そして、図4に示すように、容器Yに、既知の量及び濃度のグルコースAを入れておき、その容器Y内に注入針7及びセンサ8を挿入するのであるが、このとき、注入針7の先端は勿論、センサ8が必ずグルコースAに浸るように、当該グルコースAの液面αを設定しておく。その状態から、モータ11を駆動させてシリンジ2のピストン部2aを所定速度でスライドさせると、当該シリンジ2内におけるインスリンBが定量づつ容器Y内に注入されることとなり、グルコースAが希釈されつつ容器Y内の溶液(グルコースAとインスリンBとの混合液)の液面が、図5で示すβの位置までとなる。

上記希釈過程において、センサ8により容器Y内の溶液の濃度を計測し、図6に示すような検量線L1を得る。具体的には、注入針7からインスリンBを注入する前の濃度(グルコースAのみの濃度)を計測して同図P1を記録する。その後、注入針7からインスリンBを注入していく過程において、所定タイミングでセンサ8による濃度計測を行い、P3からP5の如き記録を行い、最後に容器Yの液面がβとなった時点での濃度をP2として記録しておく。

上記P1〜P5に最も近似する直線がセンサ8における検量線L1とされるのであるが、容器Y内のグルコースAにおける濃度及び量が既知であるとともに、希釈のためのインスリンBの注入速度(ある時間における注入量)も既知であるため、インスリンBの注入量とセンサ8による計測値との関係を示す理論直線L2は、図6に示すように、計算上既に明らかとなっている。

このような理論直線L2と検量線L1との差Kにより、センサ8による計測値に対する補正値が決定され、かかる補正値により当該センサ8の調整が行われることとなる。従って、従来の如く高濃度及び低濃度の2つの溶液を使用してセンサ8の調整を行うものに比べ、一つの試液(グルコースA)にてセンサ8の調整を行うことができ、当該センサ8の調整作業を容易とすることができる。

ここで、上記の如きグルコースAの希釈からその過程における濃度検出、検量線の作成、及びセンサの補正値の決定までの一連の作業を電子回路基板10に形成されたマイコンにより行わせ、センサの調整を自動的に行わせるようにすることもできる。これにより、当該センサの調整作業を著しく簡単にすることができる。

また、グルコースAを希釈するための希釈液がシリンジ2内のインスリンとされているため、調整作業後に残ったインスリンをそのまま患者に注入することができ、インスリン注入装置構成をより簡素化することができる。即ち、調整作業時に、インスリンとは異なる他の溶液をシリンジ2内に収容しておき、これを希釈液として使用した場合、その後のインスリン注入時には、シリンジ2内の希釈液をインスリンに変更しなければならないのに対し、本実施形態の如く希釈液をインスリンとすれば、上記変更が不要となり、作業性をも向上することができるのである。

また更に、調整作業時における希釈過程の濃度をセンサ8にて任意個数(3個以上が好ましい)記録することができるので、従来の如く高濃度及び低濃度の溶液の濃度を計測して検量線を作成するものに比べ、より正確な検量線を作成することができ、センサ8の調整を更に正確に行うことができる。

次に、センサの調整作業における実験結果について説明する。
まず、容器Yに収容しておくグルコースの濃度は、200(mg/dL)程度のものが好ましい。かかる濃度のグルコースにおける比重は、約1.001(23℃条件下)となり、使用されるインスリンの比重である約0.996(23℃条件下)と略等しくなるため、混合し易くなるのである。

即ち、グルコースは、200(mg/dL)以下の濃度のものを使用すれば、インスリンの濃度に近似し、これらの混合がより良好となるのである。反対に、200(mg/dL)より濃い濃度のグルコースを使用した場合、当該グルコースの比重及び粘性が高くなってしまい、インスリン注入速度を高くしても、該インスリンとグルコースとが混ざり難くなってしまう不具合が生じてしまう。

また、容器Yに収容されたグルコース(上述の如く濃度が200(mg/dL)のもの)にインスリンを注入する際、注入速度が速いほど容器Y内で良好に撹拌し、十分に混合させることができるのであるが、実験の結果、以下の注入速度の範囲が好ましいことが分かった。例えば、インスリンの注入速度は、0.6〜0.8(mL/min)が最も好ましいが、0.4〜1.0(mL/min)であっても撹拌効果として良好な結果が得られる。尚、撹拌効果はそれほど十分ではないものの、0.2〜1.2(mL/min)の速度でインスリンを注入するようにしてもよい。

より具体的には、例えば、予め容器Yに収容されたグルコースの体積を40μL、インスリンの全注入量を40μLとし、希釈後の全量の10%を0.6秒程度で注入できる速度での希釈作業が好ましく、その際に使用される注入針は27G(内径が約0.2mm、外径が約0.4mm程度のもの)を使用するのが好ましい。また、インスリンの注入は、連続的であっても断続的であってもよい。
