本発明は、医師により患者を診療する現場などで診療録などの医療情報を伝達するのに用いられる医療情報伝達システムに関する。

従来、医師により作成される患者の診療録であるカルテは、手書きによる記入が主であった。コンピュータの発達に伴い、コンピュータ端末を用いたキーボードやマウスによる入力データを電子カルテとして利用する動きが予想される(特許文献1参照)。

しかしながら、コンピュータ端末を用いた入力による場合、医師が端末に相対することが必要となり、患者と相対して患者から得られる情報量が少なくなる。

本発明は上記課題を解決するためになされたもので、その目的とするところは、医師及び患者の双方にとって効率的かつ確実な診療行為を可能にする医療情報伝達システムを提供することにある。

【課題を解決するための手段】
この発明の一の観点によれば、ペン先端部に位置情報を発信する発信機を備えたデジタルペン本体と、前記発信機からの位置情報を検出する受信機を備え、前記デジタルペン本体により書き込まれる診療録用紙を固定するベースユニットと、前記ベースユニットで検出された位置情報を医師端末に伝送する伝送媒体とを具備し、前記医師端末は、前記一情報に基づき前記デジタルペン本体により前記診療録用紙に書き込まれた文字情報をイメージとして有するイメージデータを生成する手書情報読取手段と、前記イメージデータを電子カルテ識別番号に関連づけて通信ネットワークを介して医療情報管理サーバに送信する医療情報処理手段とを具備し、前記医療情報管理サーバは、前記医師端末から送信されたイメージデータを前記電子カルテ識別番号に関連づけて格納するとともに、該イメージデータの問い合わせに応答して関連部門端末に該イメージデータを送信し、かつ前記イメージデータに電子署名を付加して診療録データベースに格納し、前記関連部門端末は、前記医療情報管理サーバから受信したイメージデータを表示装置に表示する送受信データ管理手段と、患者に対する作業を実施したことを示す実施記録を入力する入力装置、前記入力された実施記録を前記医療情報管理サーバに送信し、前記医療情報管理サーバは、前記実施記録を格納するとともに、該実施記録の前記医師端末からの問い合わせに応答して前記医師端末に実施記録を送信し、該医師端末の表示装置に表示させることを特徴とする医療情報伝達システムが提供される。

【発明の実施の形態】
以下、図面を参照しながら本発明の実施形態を説明する。

図1は本発明の一実施形態に係る医療情報伝達システムの全体構成を示す図である。図1に示すように、医師端末1と、医療情報管理サーバ2と、関連部門端末3が通信ネットワーク4を介してデータを送受可能に接続されている。通信ネットワーク4は無線、有線の別を問わない。医療情報管理サーバ2には、診療録データベース(DB)21及び指示管理データベース(DB)22が接続されている。診療録情報データベース21は、医師や関連部門担当者により手書きやデータ入力により得られたデータを電子カルテとして登録するためのデータベースで、例えばCDやDVDなどの光ディスクや磁気ディスクなどの記録媒体と、その記録媒体に対するデータの読取/書込みを行う記録媒体読取/書込装置などにより構成される。さらに望ましくは、記録媒体は書込み後の書換ができない追記型光ディスクが望ましい。これにより、診療録の改ざんを防止するとともに、診療録を半永久的に保存することができる。

医師端末1の詳細な構成の一例を図2に示す。図2に示すように、通信インタフェース11と、この通信インタフェース11に接続されたプロセッサ12と、プロセッサ12に接続された表示装置13,入力装置14,記憶装置15及び印刷装置18からなる。プロセッサ12には、外部入力端子16が接続されている。この外部入力端子16には、信号線17dを介してデジタルペンシステム17に接続されている。デジタルペンシステム17は、デジタルペン本体17aと、このデジタルペン本体17aの先端部に設けられた超音波発信機17bと、信号線17dを介してプロセッサ12に接続されるベースユニット17cからなる。

デジタルペンシステム17は、医師が手書きにより記入した文字情報をプロセッサ12に送信し、医師端末1の表示装置13に表示させることのできる装置である。具体的には、診療録記録紙17fをベースユニット17cに固定する。診療録記録紙17fはいかなる用紙を用いることもできるが、ベースユニット17cの例えばクリップ式の挟み込み固定位置に対して用紙の角度が一定となるように変形しない台紙などとともにベースユニット17cに固定するのが望ましい。超音波発信機17b付のデジタルペン本体17aにより、診療録記録紙17fに診療情報を手書きにより書き込む。ベースユニット17cには超音波受信機が設けられている。この超音波受信機は、デジタルペン本体17aに設けられた超音波発信機17bからの超音波17eを受信し、超音波発信機17bの位置を例えば角度及び距離などにより解析する。得られた位置データは信号線17dを介してプロセッサ12に出力される。プロセッサ12では、入力された位置データに基づき手書情報処理手段12aにより位置データに基づき入力情報を解析し、イメージデータを生成する。以上がデジタルペンシステム17による自動イメージ生成機能である。

手書情報処理手段12aは、受信した位置情報に基づき例えばJPEG形式のイメージデータを生成する。生成されたイメージデータは、表示装置13にイメージとして表示される(s3)。図6は診療録記入時の表示装置13の表示画面の一例を示す図である。図6に示すように、医師により手書きで記入された診療録がイメージとしてリアルタイムで医師端末1の表示装置13に表示される。

次に、医師は、入力装置14を用いて診療録の手書き記入作業の終了を示す手書き記入終了データを入力する(s4、s5)。入力された記入終了データはプロセッサ12に出力される。具体的には、例えばマウスなどを用いて表示装置13の表示画面上で所定のボタンなどを指定することで手書き記入終了データの入力が実行される。図6の例では、61で示されるオブジェクトを入力装置14を用いて選択することにより手書き記入終了データの入力がなされる。手書き記入終了データには、この診療録を電子カルテとして保存する際の識別情報としての電子カルテ識別番号と、診療録を記入した医師を識別するための医師識別番号が自動的に関連づけられる。電子カルテ識別番号は、手書き記入作業がされる毎に配番される番号としてもよいし、日付や時刻に基づき配番される番号でもよい。医師識別番号は、例えば医師端末1にアクセスする際のIDやパスワードに基づき、これらIDやパスワードに予め関連づけられてデータベース化されていた識別番号などを用いればよい。手書情報処理手段12aは、手書き記入終了データを取得すると、医療情報処理手段12bに手書イメージデータに基づく電子カルテの生成や関連部門への通知等の処理を指示する(s6)。この指示を受け、医療情報処理手段12bは、手書イメージデータを所定のデータ形式の画像ファイルに変換した後その画像ファイルに電子署名を付加する(s7)。その後、医療情報処理手段12bは、この電子署名付のイメージデータと電子署名を付加していないイメージデータを医療情報管理サーバ2に送信する(s8)。医療情報管理サーバ2は、受信したイメージデータを電子カルテとして保存管理すべく、診療録情報データベース21に格納する。電子署名付、署名無しのイメージデータには、医師識別番号と電子カルテ識別番号が関連づけられる。これにより、医師識別番号や電子カルテ識別番号により検索することが可能となる。この格納処理は、複数の診療録電子データを受け取ったタイミングで一括して実行してもよい。

また、医療情報処理手段12bは、イメージデータの宛先として、関連通知先情報をイメージデータとともに医療情報管理サーバ2に送信する(s9)。医療情報管理サーバ2は、この関連通知先情報を指示管理データベース22に格納する。この関連通知先情報は、医師識別番号や電子カルテ識別番号に関連づけられて指示管理情報として格納される。

一方、関連部門端末3は、定期的に(例えば数秒毎や数分毎)医療情報管理サーバ2の指示管理データベース22に問い合わせを行い、その関連部門宛の指示があるか否かを確認する。この問い合わせに応答して、医療情報管理サーバ2は問い合わせ元の関連部門の識別情報を参照し、指示管理データベース22を検索する。関連通知先情報がその関連部門の識別情報に一致する場合、その関連部門への指示があると判定し、指示着信通知を関連部門端末3に送信する(s10)。関連部門端末3の医療情報管理手段32aは、指示着信通知を受けると指示着信があったことを示すデータを表示装置33に表示し、関連部門担当者に指示着信を報知する(s11)。関連部門担当者は、この指示着信表示に基づき入力装置34を操作するなどして指示内容取得指示入力を行う(s12)。その指示内容取得指示は、関連部門端末3から医療情報管理サーバ2に送信される(s13)。医療情報管理サーバ2は、指示内容取得指示に基づき、対応する指示管理情報を指示管理データベース22から検索する(s14)。そして、検索された指示管理情報に含まれる電子カルテ識別番号に基づき診療録情報データベース21に検索指示を行う(s15)。診療録情報データベース21からは、その電子カルテ識別番号が関連づけられたイメージデータが読み出され、指示管理情報に関連づけられる(s16)。そして、このイメージデータは指示管理情報とともに関連部門端末3に送信される(s17)。関連部門端末3は、受信したイメージデータを表示装置33に表示させる(s18)。このイメージデータの表示を関連部門担当者が見て(s19)、その指示に基づく必要な処置やサービスを実施する(s20)。必要な処置やサービスは、関連部門が看護部門であれば例えば点滴、介助など、患者を看護する行為が該当する。また、関連部門が会計部門であれば、例えばその診療に対応する診療会計処理などが該当する。

関連部門担当者は、必要な処置やサービスが終了すると、その処置やサービスの実施内容を電子カルテに登録すべく、実施記録登録処理を行う(s21)。この実施記録登録処理は、例えばイメージデータの表示の際、このイメージデータの表示ウィンドウとともに実施記録入力ウィンドウを表示させ、そのウィンドウに表示される指示に従った関連部門担当者による入力処理により実行される。この実施記録入力ウィンドウを作業の種別に応じて類型化しておおくことにより、規定のフォーマットに従った入力を行うことで入力作業の効率化を図ることが出来る。この場合、関連部門端末3側の看護婦などの担当者は、イメージデータの表示に基づき類型化された入力画面のうちのいずれかを選択指定する入力装置34を用いた入力が必要となる。類型としては、例えば患者からの臨床検査値を得る作業、患者に対して薬剤を投与する作業などが該当する。

図7は実施記録入力画面の一例を示す。図7は、患者の体温などのデータを得る作業の入力画面の一例である。関連部門の例えば看護婦などの担当者は、イメージデータとして表示された診療録に記載された指示に基づき、患者に対して、あるいは患者とは別の場所で指定された作業を実施し、その実施内容を実施記録として入力装置34を用いて入力する。実施記録としては、例えば患者から得た体温などの実績値や、その他患者の症状、投与した薬剤の種類や量などが該当する。また、これら実施記録には、通知先の医師の情報が関連づけられる。この通知先の医師に関する情報は、指示元の医師に自動的に通知を行う処理の場合には指示管理情報に含まれる医師識別番号を自動的に関連づけてもよいし、指示元の医師とは別の医師へ通知を行う処理の場合は関連部門担当者により入力された情報を用いてもよい。

関連部門端末3は、入力された実施記録に基づき実施登録処理の要求を医療情報管理サーバ2の診療録情報データベース21に行う(s22a)とともに、指示管理データベース22に通知先医師情報を送信する(s22b)。医療情報管理サーバ2は、この実施登録処理要求を受け、受診した実施記録データをその実施記録データに関連づけられた医師識別番号や電子カルテ識別番号に関連づけて診療録情報データベース21に格納する。これにより、電子カルテ識別番号をキーとして、実施記録データやイメージデータなど、電子カルテとして保存すべきデータが関連づけられていく。従って、その電子カルテに対応する各種情報の検索が可能となる。また、医療情報管理サーバ2は、受診した通知先医師情報を医師識別番号や電子カルテ識別番号に関連づけて指示管理データベース22に格納する。

一方、医師端末1は、医療情報管理サーバ2に対して定期的に(例えば数秒毎、数分毎)指示管理データベース22の検索を行い、その医師宛の実施通知があるか否かを確認する。この検索は、医師端末1から送信される医師識別番号に基づいて処理される。この検索により、その医師識別番号に対応付けられた実施医師識別情報が指示管理データベース22に関連づけられている場合、医療情報管理サーバ2は実施通知を医師端末1に行う(s23)。医師端末1の医療情報処理手段12bは、この実施通知を表示装置13に表示させ、医師に実施の通知を報知する(s24)。医師は、この実施通知を受け、入力装置14を用いて記録内容取込指示を行う(s25)。この記録内容取込指示は、医療情報管理サーバ2に送信される。医療情報管理サーバ2は、指示管理データベース22を検索し(s26)、その記録内容取込指示の対象とする指示管理情報を読み出す。そして、その指示管理情報に関連づけられる電子カルテ識別番号を読み出し、診療録情報データベース21に実施記録内容取得要求を行う(s27)。この実施記録内容取得要求を受け、医療情報管理サーバ2は診療録情報データベース21から実施記録を読み出し(s28)、医師端末1に送信する(s29)。医師端末1の医療情報処理手段12bは、その実施記録を表示装置13に表示させる(s30)。医師は、その表示装置13の表示内容を確認し(s31)、医師の指示に基づく処置やサービスが適切に実施されたか、またその処置やサービスの実施の結果を確認する。

以上に示した診療録の生成、実施記録の入力、承認作業の後、記録された過去の診療録を取り出したい場合には、医師端末1や関連部門端末3から医療情報管理サーバ2に問い合わせを行う。この問い合わせは、電子カルテ識別番号等に基づいて検索する。この問い合わせに応答して医師端末1は記録媒体から署名付画像データを記録媒体読取/書取装置を用いて読み出し表示装置に表示させる。これにより、過去の診療録を容易に確認することができる。ファイルには電子署名が付加され、また追記式の記録媒体に書き込まれている場合には書換ができないため、改ざんのされていない診療録を確認することができる。

このように本実施形態によれば、医師は医師端末1に相対することなくこれまでと同様に診療録を記録しながら患者を問診することができる。そして、患者に対する問診が終わったところで入力装置14を用いて電子カルテを手書きで生成する。この電子カルテの生成も、記入が終了したことを示す旨の入力を例えば1クリックするだけでできるので、手間がかからず簡便に行える。また、関連部門には自動的に診療録を記録したイメージデータが電子メールのような形式で送信され端末に表示されるため、医師による診療録の記入から時間差なく診療録に記入された内容に基づく作業を行うことができる。
