角膜挿入物、角膜挿入物の挿入器具、および眼の屈折力を矯正する方法
本発明は、請求項1の前文に記載の角膜挿入物、特に眼の角膜の周辺内にリン
グ様チャネルをカッティングするための用具、および眼の屈折力(refractive p
ower)を矯正するための方法、または眼の屈折率誤差を補償するための方法に関
する。
眼の光学画像形成システムにおいて、角膜は、最も有効な部分を代表する。角
膜表面は、屈折率が低い空気と屈折率が高い角膜組織との間の境界表面を形成す
る。従って、角膜の湾曲半径の小さい変化でさえも、眼の全屈折力の実質的な変
化をもたらす。従来では眼鏡が必要とされた視力の欠損を、角膜の湾曲を変化さ
せることを意図した外科手術により治癒することに対して努力がなされてきた理
由は、この事実のためである。ラジカル角膜切開(radical keratotomy)または
光屈折手術のような従来使用されてきた方法は全て、眼に不可逆的な変化を生じ
るという重大な欠点を有し、かつ、それらの方法は多くの場合、永久的に成功す
るわけではない。アプローチの別の方針は、角膜インレー(corneal inlay)ま
たは角膜挿入物とも呼ばれることがある、いわゆる角膜リング(corneal ring)
を用いて、眼の湾曲を変化させることである。
角膜屈折外科手術の技術および角膜リングインレーの先行技術の要旨は、国際
出願(公報番号WO 94/06534)の導入部に記載されている。別の要旨は、米国特
許第5,391,201号に記載されている。より詳細には、米国特許第5,391,201号は、
眼の角膜の中心光学領域の湾曲を変化させる方法を考慮しており、この特許は、
以下の工程を包含する:
a)角膜、光学領域の周辺において、円形の周辺カットを作製する工程;
b)Bowmann膜を用いて、支質(stroma)を円形に掘る(undermine)工程;
c)該支質の掘られた部分に、生体適合性の、角膜の湾曲を変化させるのに有
用なジオメトリーを有する材料の連続的リングからなるインレーリング装置を配
置する工程;
d)該組織の周辺を密封する工程。
この米国特許の開示によれば、このリングは、「タイヤ着脱用てこ(tyre-ron
)」アプローチを用いて挿入される。このアプローチは、角膜の端部を、リング
の周辺を囲むリングから持ち上げる手段であると理解されている。
角膜リング自体は、多くの特許出願に記載されている。例えば、番号WO94/031
29号の下で発行された国際出願は、支質に導入するのに適切であり、そして少な
くとも1つの、低弾性率の物理的に適合可能なポリマーを含む、スプリット(sp
rit)ポリマー製リングの形態であるハイブリッド支質内リングを開示する。該
リングの端部を接続するための種々の技術が開示される。
請求項1の前文は、WO 94/03129から公知の従来技術を反映している。ここで
開示されている支質角膜リングは、水と接触する際に膨潤する低弾性率の物理的
に適合可能なポリマーの外部層と、支質内に導入された際に膨潤しないポリマー
の内部とを有する。この内部は、ゲルまたは硬化可能なポリマーで満たされ、そ
して、薬物または生物学的試薬を含み得る。
さらに、公開番号WO94/06381を有する国際特許出願は、スプリットリングの端
部の種々の異なる形状を記載しており、これにより、端部が互いにかみ合って(
intermesh)、滑らかで連続的な外部輪郭を有するリングが提供される。
公開番号第WO95/03755を有するさらなる国際特許出願は、セグメント化された
予備形成支質内角膜挿入物に関する。この場合、種々の実施例が提供されている
挿入物は、角膜の視界の角膜外側の前方を包囲するリングまたは「弧」の部分の
みに対する。この出願はまた、このデバイスを角膜に挿入する手順を記載する。
公開番号WO95/03747号を有する国際特許出願もまた、支質内角膜挿入物を考慮
している。この挿入物は、角膜の円周の360°未満に対する柔軟なポリマー挿入
物を含む。この出願はまた、1つ以上のこのような挿入物を角膜内に挿入するた
めの、侵襲性が最少である手順を記載する。別の角膜リングは、米国特許第5,32
3,788号に記載される。この角膜リングは、ヒトの眼に挿入し得るサイズおよび
形状を有する、可撓性の、好ましくは円形の本体を有し、かつ、ヒトの眼組織と
適合性である材料から構成される。この円形本体の端部は、互いに重なり、そし
ていくつかの異なる形状または態様に形成される。この端部は、好ましくは、一
緒になったときにかみ合って、滑らかでかつ連続的な外部輪郭を有するリングを
提供する。
米国特許第5,405,384号は、寸法が均一でない支質内角膜リングに関する。これ
は、代表的には、リング上で互いに間隔をあけて離れた2つ以上の盛り上がった
領域(raised area)(または、さらなるバルクの領域)を有する。このリング
の設計は、角膜中に導入されてそこで正確に調整された場合、眼の乱視を少なく
とも部分的に矯正する。この特許中に記載されている調整は、リング周辺の方向
の調整に関し、その結果、このリングの乱視矯正部分は、眼に対して正しく位置
する。
公開番号WO88/10096を有する国際特許出願は、プラスチックのスプリット端部
調整リングを眼の角膜に挿入するための外科手術装置であって、該調整リングが
、その一部として、支質を分離して、リングが回転する際にリングを調整するた
めの経路を提供するための切開ヘッド48を含む外科手術装置を記載している。こ
の出願では、調整リングの端部は動かされて、角膜の形状を、所望の視覚矯正に
従って変化させ、その後、調整リングの端部は強固に接合して所望の形状を維持
する。
角膜リングを挿入するための別の技術は、米国特許第4,671,276号に記載され
ている。
ここに記載されている、眼の角膜の湾曲を調節するためのシステムは、プラス
チックのスプリットを挿入する工程と、角膜リングの約8ミリメートルコード上
にある角膜の支質内で、リングを調整する工程と、次いで、金属スプリット端部
切開リングの一方の端を、角膜の外側に残存する切除リングのトレーリング端(
trailing end)を有する切り目内に挿入する工程とを包含する。切開リングの一
方の端は、その先端の近傍に、横断穴(transverse hole)を有する。切開リン
グは、磁気保持部、および金属切開リングを受容するためおよびこれを円形に保
持するための円形溝および回転デバイスを有する。保持部がこの金属リングの挿
入された端を回転させ、この金属リングがこの保持部を磁気的に回転させるに
つれて、切開リングは、支質内の円形経路内に挿入される。この切開リングのそ
り型(sled shaped)端部は、、切開リングが回転可能に支質に入るにつれて、
切開リングの挿入された端を動かして支質の前方に向かって上部に偏らせる。切
開リングの挿入された端が、第1の切開部に到達したとき、その回転は停止し、
そして垂直方向でかつ第1の切開部の一端を横断する切開リングにおいて、水平
の穴に第2の切開部が作製される。プラスチック調整部材はまた、第1の切開部
に隣接するそり型端部の近傍に、水平の穴を有する。接続している結合部材は、
穴を介してそれぞれの切開部の端部に挿入され、そして2つのリングが解放可能
に接合するようにリングを調整する。次いで、保持器具の回転方向は逆転して、
切開されたリングを「バックアウト(back out)」させ、そして同時に、リング
をプラスチック調整リングの後ろに引き戻す。2つのリングのここで接合された
端部が、挿入点の周囲を回転して戻されるとき、ここで引っ込められた切開リン
グは、調整リングから解放される。角膜トポグラフィーの角膜スコープ(cornea
scope)型画像を、視覚的表面に表示し、そして所望の標的の画像と比較する。
次いで、調整リングの端の相対的位置を調整し、角膜の形状を変化させて、現在
の形状を所望の形状に一致させる。この際に、調整リングの2つの端は、強固に
接合して、所望の形状を維持する。
この種類の別のシステムはまた、米国特許第4,766,895号に記載されている。
最初に述べた、公開番号WO94/06504を有する国際特許出願はまた、特に関心が
持たれている。なぜなら、この出願において、眼に挿入した後にリング厚みが変
えられる角膜リングを、眼に挿入しているからである。このような角膜リングの
種々の例が、この出願中に記載されている。これらの全ては、リングの機構的調
整装置(例えば、糸で共に接続された2つの同中心部分に製造することによる)
を含む。
公開番号WO93/11724を有する国際特許出願は、外科手術用カッティング用具の
カッティング動作を容易にするための音響振動(acoustic ocsillations)の使
用を記載している。外科手術用カッティング器具は、米国特許第5,403,335号に
記載されている。
本発明の目的は、屈折力を矯正するための方法、あるいはデバイス(このデバ
イスは、好ましくは、取り外し可能である)を用いて眼の屈折誤差を補償するた
めの方法、ならびに、可逆的で、かつ、小さい程度の努力で、いかなる時におい
ても、屈折力の変化を微小かつ制御可能に矯正することが可能な方法であって、
かつ、比較的小さい寸法を有する機構的に調整可能なリングの設計に関する不都
合がない方法を提供することである。
本発明のさらなる目的は、デバイスの挿入を可能にする(すなわち、この方法
の実行を可能にする)器具を提供することである。
この目的を満足させるために、眼の屈折力を矯正するためのリングまたはリン
グセグメントの形態の角膜挿入物であって、金属またはプラスチックの少なくと
も1つの伸長部からなり、該伸長部は、少なくとも1回転されてリングに形成さ
れるかまたはリングセグメントに形成され、該リングまたはリングセグメントは
、角膜の直径に適合したリング直径を有し、該リングまたはリングセグメントは
、例えば、6〜12ミリメートルに達するリング直径を有し、該リングまたはセグ
メントのスプリング特性(該リング直径の膨脹または収縮の少なくとも1つとい
う意味)が、照射(特にレーザー光のような電磁照射)の作用によるか、または
磁気の処理によるか、または誘導的処理により、挿入後に調節可能または良好に
調節可能であるような材料から作製されることを特徴とする、角膜挿入物が提供
される。
眼の屈折力を矯正するため、または屈折誤差を補償するための、本発明の方法
は、以下の工程を包含する:リング様チャネルを、角膜の周辺内にカットする工
程;該角膜の該周辺の直径に適合された直径を有する少なくとも1回転を有する
リングのセグメントに形成され得る金属またはプラスチックの細長い部材を、該
チャネル内に導入する、工程;および反射誤差の微矯正を、該セグメントまたは
リングの挿入後に実行する、工程。この方法は、該リングが、放射(特に、レー
ザー光のような電磁照射、熱照射、電磁誘導、または磁性)の作用による挿入の
後に膨脹および/または緊縮し得る材料からなり、そして、該微矯正が、放射(
特に、レーザー光のような電磁照射、熱照射、電磁誘導、または磁性)のような
照射の使用により、該眼中でインサイチュで実行されることを特徴とする。
言い換えれば、リング様チャネルは、まず、角膜の周辺に設けられる。このチ
ャネルは、正確に中心に位置し、かつ、支質中で、角膜の深さのほぼ半分の深さ
でなければならない。
この器具を引き戻す前に、設けられた、チャネルとほぼ同様の直径を有する薄
いリングを、器具の先端に固定させ、そしてこれを引っ込める際には、この器具
をチャネル内に引く。
同時に最初の状態に回復することが望ましいなら、単にこのリングを取り除く
だけで十分である。
一般的に言うと、リングの特性、特に取り付け状態でのそのバネ張力は、眼の
屈折力(refractive power)の粗い変化をもたらすように選択
される。例えば、バネ張力の増加は、角膜の平坦性を増し、そして逆もある。本
発明によれば、計画的な、および正確にメーターで計る適用(例えば、リングへ
のレーザーエネルギー、これは、その体積およびまたその長さの両方、ならびに
一定の限界以内の張力を変えることを可能にする)によって、細かい調整が、次
いで、起こる。つまり、角膜の屈曲を変えることによる欠陥のある視覚の矯正は
、便宜上、2つのステップで起こる:
1.あらかじめ計算された直径またはバネ張力を有する角膜リングの挿入による
粗い矯正。
2.眼の場所のリングによる外科手術の傷の治療後、および何らかの最初に存在
する組織引っ張り力の消失後の細かい矯正。この細かい矯正は、例えば、眼全体
の光学パラメーターを、適切な屈折計の補助により検査しながらのレーザーの使
用によって起こる。レーザーを、調整し、操縦するために、データプロッセシン
グ装置、またはコンピューターの使用が、有利である。
しかし、本発明の角膜挿入を伴う最初の実験は、達成され得る調整の範囲は、
実質的に、挿入は、最初、粗い調整および細かい調整の両方を伴って挿入され、
次いで、眼の中で、なされるものであり、すなわち、リングの挿入前の分けた粗
い調整は、必要でないことを示した。このことは、リングが、挿入物の挿入を容
易にするリングチャネルのように同じ直径で作られ得るという利点を有する。

図1および2は、ヒトの眼の本発明にとって特に重要である少なくとも角膜の
領域のレイアウトの一般的な説明を提供する。この図中で、角膜は、参照数字1
0によって特徴付けられ、そして瞳孔の領域で空気との境界表面を形成する。角
膜は、眼の屈折力全体に明白な影響を有する。角膜は、眼のいわゆる強膜(sc
lera)12に併合され、そして、角膜の周辺領域14は、この転移の前部に
、直接、位置する。角膜の周辺は、レンズ18と瞳孔19と、大体、同軸である
。角膜の周辺領域内に虹彩16が、そしてそれの下にレンズ18が、図2でのレ
ンズの左右の繊毛体(ciliary body)20と共に、位置する。
図2では、リング様チャネル22が示され、これの中へ、角膜リングは、本発
明によって、挿入されねばならない。
このチャネルは、リングの挿入の前に、最初に切断されねばならない。これは
、
本発明によって、図3〜6の道具を用いて行われる。これらの図から解り得るよ
うに、装置は、作用端32を有し、これは、少なくとも1つのターン33の少な
くとも実質的にらせん状のワイヤからなり、そして該ワイヤは、1つのターンの
ねじ山(thread)に似ており、そしてその一端34に、カッティング先端

作動部またはハンドルに通じるワイヤ部分38は、実質的にらせんターンと同軸
に伸びている。
作用端の特別な形は、瞳孔の周辺の位置で、先端を角膜中に挿入し、その後、
それを角膜内で円形の軌道で移動する(このことは、瞳孔の周辺全体の眼の表面
で角膜を切断することなしにチャネルを形成するのに必要である)ために必要で
ある。
ハンドル40は、振動発生器として形成され、そして装置またはハンドルの中
心の縦軸39の周辺に、カッティング先端がリング形状のチャネルの方向のあち
こちに揺れるような仕方で、振動を起こす(すなわち、振動は、縦軸39周辺の
角度振動である)。ここで、振動は、ハンドルまたは眼の自然振動数よりも、実
質的に高い振動数、そして、実際は、好ましくは、カッティングの道具の自然振
動数と実質的に同じ振動数を有する。この大変重要な実施態様の意識は、カッテ
ィング抵抗を減少し、それによって、装置の案内、および外科医による外科的手
術の作業の実行を、容易にする。
可能なら、光学的拡大化、または画像システムを用いて、装置を手によって案
内することが好ましいけれども、しかし、作動を機械的に移動すること(例えば
、対応してプログラムされたロボットの手(必要に応じて、コンピューター、ま
たは外科医により、コントロールする)による)がまた、考えられる。
装置のカッティング先端は、リングチャネルを、角膜の半分の深さの所で、切
断し、そして、角膜をカッティングワイヤの側部全体に置換する。しかし、好ま
しくは、何ら、物質は、切り取られない(すなわち、作動先端は、単なる分割機
能を有する)。
リングチャネルの切断後、カッティング先端は、再び、挿入の位置に戻り、そ
して、そこで、角膜リング(例えば、図7の角膜リング44)を形成する伸長部
の一端と接続され得る。この接続の性質は、本明細書では示さない。結合は、種
々の方法により行われ得る。それは、例えば、作動先端が、スリーブ様台を経て
、リングを形成する伸長部の一端に結合されるような仕方で、成し遂げられ得る
。
カッティング先端に横断穴(示さない)を与え、そして、細かいピンを、この
横断穴および(スプリット)リングの一端に通すことがまた、考えられる。ばね
リングまた、スプリットリングの一端を、カッティング先端に固定するために、
使用され得、そしてカッティング先端は、角膜リングのそれに信頼性のある結合
を可能にするために、カッティング先端の後3に、隣接部、またはリング肩部を
有し得る。代わりに、実際のカッティング先端は、そのあとに、分割角膜リング
の一端が挿入され、次いで、何らかの方法(例えば、小さな横断ピンによる)で
、しっかり留められるソケットを残して、除去可能であり得る。
装置に戻ると、好ましくは、開放リング44内に前もって形成される伸長部は
、リングチャネル22内に引き込まれ得る。
角膜リング44は、それが、眼への挿入の後に、少なくとも、角膜の正確な屈
曲、従って眼の屈折力を定めるために、微調整を受けさせ得るように形成される
。このような微調整の実現のために、多くの考えられる可能性がある。
1つの特別な興味ある可能性は、それによって、リングの特性の制御可能な変
化を達成するために、レーザー照射で小さな空孔が生成され得るプラスチック製
のリングを使用することにある。
この可能性は、ここで、図8、9、10および11に関して、より詳細に説明
される。
最初に、図8に戻ると、この図は、角膜10の周辺領域中の支質中に挿入され
た角膜リング44を有する図2に類似の眼の中間断面の概略図を示す。このリン
グは、最初に、閉鎖リングである寸法であり、そして、患者が被っている近視を
、部分的に、完全ではなく、矯正するよう選択された。リングを用いた矯正視覚
は、眼が、入射光に、網膜の前の点F3で、焦点を合わせることをもたらす。
角膜は有効半径Rcを、角膜リング44は半径Rrを有することが、注意され
る。
残余の(residual)近視を矯正するために、図9に示すように、Rc
をRc+ΔRcに変えるように、角膜をいくらか平坦にする必要があり、これは
、入射光を網膜上のF4に焦点を合わさせることをもたらす。
この角膜の平坦化を達成するために、図9の左で示すように、リング44をい
くらか拡大させる必要がある。つまり、リングは、その半径をRrからRr+Δ
Rrへ変化するように作動しなければならず、これは、また、必然的に、スプリ
ットリングの2つの端部間の距離が増加することをもたらす(このことは、また
図9で示される)。図9から、リングの半径および関連する角膜の部分平坦化の
変化によって、近視は、ここで、完全に矯正され、そして、入射光は、正確に、
眼の網膜に焦点を合わせることに留意すべきである。
図10および11は、遠視を被っている患者を処置する場合の類似の状況を示
す。この場合、図10に示すように、最初の矯正は、眼に入る光が、網膜の後ろ
の概略の点F3で、焦点を合わすように、遠視の程度を減じる角膜リング44を
選択することによって、なされる。従って、患者は、まだ、遠視を被っているが
、しかし、程度はより少ない。この場合、角膜の半径はまた、Rcで示され、そ
してリングの半径はまた、Rrであるが、しかし、この場合、リングは、その二
つの端部間に、一定の間隔を有することが注意される。
リングは、ここで、例えば、あとで記載される仕方で、その半径をRr−ΔR
に減ずるよう処理され、このことは、部分的に、または完全に、リング44の2
つの端部間の間隔を近付けることをもたらし(図11に示す)、そして、角膜の
半径を、前の値RcからRc−ΔRcへ減ずる。これは、入射光の焦点をF4に
変え、これはまた、眼の網膜上であり、患者は、もう一度、矯正視覚を有する。
角膜リング44の半径Rrを変えるための種々の特定の可能性が、ここで、図
12および13を参照して、記載される。これらの図は、2Rrに等しい角膜リ
ングのわずかの直径Dを示す。
図には、この場合、半径方向の内側部60および半径方向の外側部62と考え
られるべきスプリットリングの形で、角膜リング44を示す。最初の種々の単一
の材料リングは、1つの材料(例えば、PMMA(ポリメチルメタクリレート)
)からなり、そして、それは、例えば、図12aに示されるような四角形の
断面(角は丸い)、または図12bに示されるような丸い断面を有し得る。それ
はまた、任意の他の所望の断面(例えば、図12cに示すような、伸ばされたひ
し形を含む断面)を有し得、次いで、リングは、図cで、断面の斜めの位置によ
って示される浅い円錐断面のそれに似ている形を有する。
参照数字64は、リング44の材料を加熱するのに使用され得るレーザーの光
学システムを示す。該レーザーは、例えば、ネオジウム(Neodyn)YAG
レーザーであり得る。PMMA材料が、焦点を合わされたレーザービームで処理
される場合、次いで、小さい空孔がレーザービームの焦点においてその中で生成
し、これは、材料の局所的内部融解によると考えられ、そして、これらの空孔は
、材料中に永久的に残り、そして、材料の拡張をもたらす。
レーザービームが、角膜リング44の半径方向の内側位置60上に向けられた
場合、次いで、そこに形成される空孔は、この内側の層を、外側の層に対して拡
張させ、このことは、リングを全体として拡張させ、従って、リングの直径を増
加し、そして、図8および9に示すような近視を矯正する方法を達成する。
あるいは、処理が外側のリング部分62に制限される場合、今度は、内側のリ
ングに比較した、この外側のリングの拡張は、リング44の半径を、サイズを減
じるか、または、サイズを減ずるようにし、従って、図10および11による遠
視の矯正の方法を提供する。明らかに、リングが緊縮する場合、今度は、図12
に示されるよりも、より大きな間隔66が、最初のリングの2つの端部68と7
0間に、提供されねばならない。
レーザービームは、内部または外部リング部分60、62のいずれかを、それ
らの全長にわたって加熱する必要はない。逆に、リングの内部または外部部分の
分散した領域が、代表的にはレーザービームで処理され、ちょうどこれらの領域
の膨張を引き起こす。上記のように、内部部位のみでの膨張は、リングの開口を
導く(すなわちリングを開口させる試みでリング中の応力を増大させる)。リン
グの半径は増大し、そして角膜の曲率は平らになる。リングの膨張の程度は、内
部リング部分の処理の強度(すなわち生成される泡の数)に依存する。これは非
常に段階的に制御可能であり、好ましくはリングの内周全体にわたって泡の均一
な分布を達成し、その結果、調整の微制御が可能である。
明らかにこれは、外部リングの処理によってリングの半径が減少する場合にも
同様に可能である。
図13は、図12のリングと本質的に同一のリングを、リングの平面図および
可能な断面図に関して示す。これはやはり均一なリングであるが、しかし今度は
2つの異なる材料からなる複合リングである。より詳細には、内部リング部分6
0は前記のようなPMMAからなり、そして外部リング部分62は非晶性または結晶性
の成分を有するPVCからなる。
PVCは、加熱時に収縮する材料である。従って、この場合、PVCの外部リング部
分60の加熱はPVCの収縮を引き起こし、従ってリングを開口させてリングの半径
を増大させる意味でリングの応力を変化させる。従ってこのリングは、図8およ
び9に従う近視の矯正に適切である。
PVCの加熱はやはり、焦点化レーザービームによって達成され得る。
内部リング部分60はPMMAからなるので、この材料を同時に加熱処理してそこ
に泡を発生させ、内部リング部分の膨張を導き、従って外部リング部分の緊縮の
効果を増強し、そしてリング全体について可能である膨張の範囲を実質的に増大
させることもまた可能である。
代替的な実施態様において、内部リング部分62はPVCから形成され得、そし
てそれに結合した外部リング部分はPMMAから形成され得る。この場合、内部PVC
リングの加熱はその緊縮を引き起こし、従ってリングを緊縮させて半径のサイズ
を減少させる意味でリングの応力を変化させる。この種類のリングは、例えば、
図10および11に従う近視を矯正するために有用である。リングが緊縮する場
合、リングの2つの末端68および70の間のギャップ66は、図13において
