Claims (1)
- 【特許請求の範囲】 1. 身体に移植可能な移植片であって: 対向する第1および第2開放端を有し、この第1および第2開放端の間に流体が 流れる内腔を形成し、身体組織の中にこの第1端をこの組織の第1位置 におよびこの第2端をこの組織の第2位置に選択的に配置して、これら の位置間の障害物をバイパスするためにこの第1位置と第2位置の間に 流体が流れる流路を形成するようにされた管状移植片壁;並びに 上記移植片壁と隣接する組織とを熱的に固着するために、上記配置後、少なくと も上記第1端近くで上記移植片壁を加熱するように作用する移植片固定 装置; を含む移植片。 2. 請求項1の移植片に於いて: 上記固定装置が更に上記第1端近くで上記移植片壁を加熱するように作用する移 植片 3. 請求項2の移植片に於いて: 上記固定装置が上記第1端近くで上記移植片壁に関して取付けられた第1導電加 熱素子、上記第2端近くで上記移植片壁に関して取付けられた第2導電 加熱素子、並びにこの第1および第2加熱素子に電流を供給するための 手段を含む移植片。 4. 請求項3の移植片であって、更に: それぞれ、上記第1および第2位置で上記移植片壁を機械的に固着するための第 1および第2保持形態を含む移植片。 5. 請求項1の移植片に於いて: 上記固定装置が上記第1端近くで上記移植片壁に関して取付けられた導電加熱素 子を含む移植片。 6. 請求項5の移植片に於いて: 上記加熱素子が上記移植片壁の周りに実質的に全円周距離に伸びる移植片。 7. 請求項6の移植片に於いて: 上記加熱素子が上記第1位置で上記移植片の上記第1端を機械的に固着するため の形態を取入れた環状部材である移植片。 8. 請求項1の移植片であって、更に: 上記内腔を通る一方向だけの流体流れを可能にするために、上記流体が流れる内 腔に沿って上記移植片壁に取付けられた弁を含む移植片。 9. 請求項1の移植片であって、更に: 上記移植片壁に少なくとも一つのポケットを含む移植片。 10.請求項5の移植片に於いて: 上記加熱素子に電流を供給するための手段が高周波電流源、この電源と上記加熱 素子の間に結合した第1導線、上記加熱素子から離間した中立の電極、 並びにこの電源およびこの中立の電極に結合した第2導線を含み、それ によってこの隣接する組織の加熱がオーム加熱である移植片。 11.バイパス移植片を移植するためのシステムであって: その末端に、末端方向に収束する組織拡張先端を有する組織拡張部材; この拡張部材内に支持され、この拡張先端から末端方向に伸び、組織壁を穿刺し てこの拡張先端によって拡張可能なオリフィスを作るようにされた、組 織穿刺工具;並びに 実質的に流体不浸透性の移植片壁を含み、この壁の第1、第2および第3離間領 域がこの移植片壁を貫通する、それぞれ、第1、第2および第3開口を 有する移植片; を含み、そこでこの移植片がこの拡張部材上に取外し可能に取付けるようにされ 、この拡張部材がこの第1および第3開口を通って伸び、この第1開口 をこの拡張先端近くにおよびこの第3開口を第1開口の基部側に配置し て、この移植片の第1領域をこの組織壁の第1オリフィスに挿入してこ の第1領域をそこに固定するためにこの拡張部材を使用可能にし;並び に そこでこの移植片が更にこの拡張部材に対して摺動可能であって、上記固定後、 この拡張部材を第1領域から基部の方へ引込めることを可能にし、およ び更にこの移植片の第2領域をこの組織壁の第2オリフィス内に固着し 、それによってこの移植片がこの第1オリフィスと第2オリフィスの間 に 流体が流れる導管を形成するためにこの拡張部材を第2開口に挿入可能 にし;並びに この拡張部材を移植片から引込めてから、この第3開口を閉じるための閉鎖機構 を含むシステム。 12.請求項11のシステムであって、更に: それぞれ、上記第1および第2端近くの第1および第2固定素子を含むシステム 。 13.請求項12のシステムに於いて: 上記固定素子が上記移植片端を機械的に固着するための形態を取入れたシステム 。 14.請求項12のシステムに於いて: 上記固定素子の少なくとも一つが導電加熱素子を含むシステム。 15.請求項12のシステムであって、更に: 上記拡張部材に取付けた膨張可能バルーンを含むシステム。 16.請求項11のシステムに於いて: 上記閉鎖機構が上記第3開口の周りの上記移植片材料を通る細ひもまたは糸を含 むシステム。 17.請求項11のシステムに於いて: 上記穿刺工具が上記拡張部材内に摺動可能に取付けた長い針を含むシステム。 18.バイパス移植片を配置するためのシステムであって: 基端および末端を有し、この基端を体外に残したまゝ、この末端だけを体腔に沿 って選択した部位の方へ管内移動させるために挿入可能な長く且つ柔軟 な搬送体; この末端に隣接し、この選択した部位に近い第1位置に配置可能であり且つこの 第1位置で組織を貫通する第1開口を作るためにこの搬送体の基端から 操作可能であり、および更にこの選択した部位に近い第2位置に配置可 能であり且つこの第2位置で組織を貫通する第2開口を作るために操作 可能である組織穿孔機構; この搬送体によって支持され、上記末端近くに配置され、並びにこの第1位置か ら第1開口を通って第2位置へおよびこの第2開口を通って伸びる案内 位置へ動き得る、長い移植片案内; この搬送体に沿って動くためにこの搬送体に取付けられるようにされた管状移植 片;並びに そのように取付けたとき、この移植片を末端でこのカテーテルに沿ってこの移植 片案内の方へ、および次に、この移植片案内が案内位置にあるとき、末 端でこの案内に沿って、この移植片が第1位置から第2位置へおよび更 にこの第1および第2開口を通って伸びるバイパス位置へ動かすように 作用する移植片制御装置; を含むシステム。 19.請求項18のシステムに於いて: 上記搬送体がカテーテル内腔の貫通するカテーテルを含むシステム。 20.請求項19のシステムに於いて: 上記移植片案内が、上記カテーテル内腔内に摺動するように含まれおよび先細り の末端を有する長い拡張器を含み、並びに上記組織穿孔機構が上記先端 およびこの拡張器内に摺動するように含まれる長い針を含むシステム。 21.請求項21のシステムに於いて: 上記カテーテル内腔および上記拡張器が、上記カテーテルから上記拡張器へのト ルクの伝達を可能にするために、実質的に整合する非円形輪郭で作られ ているシステム。 22.請求項21のシステムに於いて: 上記拡張器が、上記拡張器を回転することによって上記末端の選択的配置を容易 にするために、上記末端を含む末端領域で予備成形されているシステム 。 23.請求項20のシステムであって、更に: 上記針の組織の中への貫通程度を制限するためのストッパを含むシステム。 24.請求項20のシステムに於いて: 上記制御装置が上記カテーテル内に挿入できる長い、管状スタイレットを含むシ ステム。 25.請求項19のシステムに於いて: 上記移植片案内が上記カテーテルの末端領域を含み、並びに上記組織穿孔機構が (i)上記カテーテル内腔内に摺動するように含まれ、先細りの末端を 有する拡張器、および(ii)この拡張器内に摺動するように含まれる長 い針を含むシステム。 26.請求項25のシステムに於いて: 上記制御装置が上記カテーテル内に挿入できるスタイレットを含むシステム。 27.請求項19のシステムであって、更に: 上記移植片を少なくとも上記第2位置で固着するための固定素子を含むシステム 。 28.請求項19のシステムであって、更に: 上記移植片案内の末端近くで上記移植片案内に取付けた膨張可能バルーンを含む システム。 29.請求項19のシステムに於いて: 上記制御装置が更に、上記移植片案内および上記搬送体を上記選択した部位から 基部の方へ引込める間、上記移植片を上記バイパス位置に維持するよう にされたシステム。 30.バイパス移植片を径管配置するためのプロセスであって: 長いカテーテルを体腔に沿って選択した部位の方へ管内を前進させる工程; このカテーテルの末端をこの選択した部位に置いて、このカテーテルの末端近く に取付けた組織穿孔機構を使って、この体腔を形成する組織壁を貫通す る第1開口を作る工程; この組織穿孔機構をこの第1開口を通して、および次にこの第1開口から離間し た位置へ進め、次にこの機構を使って上記位置で組織を貫通する第2開 口を作る工程; 移植片案内をこの第1開口を通して、末端の方へ上記位置に、次にこの第2開口 を通して進める工程; この移植片案内をそのように配置して、管状移植片をこの案内に沿って、この移 植片が第1開口から第2開口へおよびこの第1および第2開口を通って 伸び、それによってこの体腔と流体連通したバイパス導管を作るバイパ ス位置へ進める工程;並びに この移植片を上記バイパス位置に維持しながら、このカテーテル、組織穿孔機構 および移植片案内を基部の方へ引込める工程; を含むプロセス。 31.請求項30のプロセスに於いて: 上記移植片案内が上記カテーテル内に摺動するように取付けられた拡張器を含み 、および上記移植片案内を進める上記工程がこの拡張器を上記カテーテ ルに対して末端の方へ進める工程を含むプロセス。 32.請求項30のプロセスに於いて: 上記移植片案内が上記カテーテルの末端領域を含み、上および上記移植片案内を 進める上記工程が上記カテーテルのこの末端領域が上記第1および第2 開口を通って伸びるまで上記カテーテルを末端の方へ進める工程を含む プロセス。 33.請求項30のプロセスであって、更に: 上記移植片を上記バイパス位置に置いて、上記移植片をその基端および末端部で 、それぞれ、上記第1および第2開口の組織に固着する工程を含むプロ セス。 34.請求項33のプロセスに於いて: 上記第1および第2開口の少なくとも一つを、体腔を形成する器官の組織を貫通 して作り、それによって固着したとき上記移植片がこの体腔に開いてい るプロセス。 35.請求項33のプロセスに於いて: 上記第1および第2開口を異なる血管の組織壁を貫通して作り、それによって固 着したとき上記移植片が二つの異なる血管を流体結合する導管を提供す るプロセス。 36.請求項30のプロセスに於いて: 上記開口の少なくとも一つがある器官からその器官の内腔へ通るプロセス。 37.請求項30のプロセスに於いて: 上記第1および第2開口を二つの異なる血管の組織壁を貫通して作るプロセス。
