一点固定式の自動ロック式保持方法
本発明は、請求項1の上位概念に記載の保持方法に関する。このような保持方
法は様々な分野で使用され得る。
1つの可能な使用分野は医学分野であり、例えば脳神経外科における生検に用
いられる。生検は脳神経外科において、一般的には定位的(ステレオタキシー)
脳手術として行われる。この定位的脳手術では、外科医によって選定された穿頭
孔点および目標点に基づき、生検経路が規定される。
従来のステレオタキシーのために、定位リング(Stereotaxiering)および目
標アーチ(Zielbogen)が使用された(例えば、「Kelly System(ケリーシステ
ム)」、Riechert&Mundinger社、CRW、BRW;また、:「Neurosurgery(脳
神経外科、R.H.WilkinsおよびS.S.Rengachary著、1996年第2版発行、第3
巻、第4067頁以下参照、これらの頁から当該背景技術に関する適切な概説が
得られる)。定位リングおよび目標アーチは、頭蓋冠の外板(頭蓋骨の外層)に
おける多点固定を前提としている。多点固定式のこのような定位リングは、長時
間にわたって患者に装着されねばならず、このことは患者にとって大きな負担と

る。さらに、このステレオタキシーシステムによって、外科医のための施術行動
半径が制限されてしまう。
本発明は、例えば脳神経外科の分野における課題、すなわち、核スピントモグ
ラフイ(Kernspintomograph)または磁気共鳴断層像撮影装置(MRI)をさら
に発展させることによって介入式の(Interventionell)磁気共鳴断層像撮影装
置(IMR)を提供する、という課題を解決する。このIMRによって、リアル
タイムに人体の高画質な断層像が得られ、かつ、画期的な開発として、断層像作
成中に患者に手術することができる。これに対して慣用のステレオタキシーでは
、術前に得られた個々の断層像が手術プランのための基礎データとして使用され
る。このような方法は、断層像を作成してから手術するまでの間に生じた変化を
考慮することができない。
従来のステレオタキシー法によって提供される必須の固定式の補助照会システ
ム(Hilfsreferenzsystem)は、IMRではもはや不要である。なぜならば、患
者は全施術中において、IMRに接続された補助装置「フラッシュポイントシス
テム」(FPS)によって規定される照会システムを備えたIMR内にあるから
である。
このFPSは2つまたは3つのLEDのガイドであり、このガイドの位置は三
次元的に3つのカメラによって検出される。FPSは、軸線を規定することによ
って手術のプランニングを可能にする。この軸線は、IMRによって形成される
断層像内に記入されて、モニターに同時に表示される。
FPSによって生ぜしめられる仮想の軸線は、生検経路に合致させられる。従
って本発明の課題は、規定された生検・軸線をFPSの三次元的な固定によって
維持することである。FPSの製造者によって、FPSは頚部に固定するために
考案されている。従来公知の頚部固定式の装置は、確実な固定を保証できない。
本発明による保持装置は、該保持装置が図示の実施例では患者の頭蓋冠に固定
されるので、三次元的に完全な固定を保証する。さらに本発明による方法ならび
に装置は、わずかなスペースしか必要とせず、かつ、手術に起因する患者への負
担および不都合な点を最小限に減少させる。
前記課題は、本発明によれば、請求項1の特徴部に記載の構成により解決され
る。
第1図〜第18図は、医学分野における本発明の実施例を示している。例とし
て、脳神経外科における生検のための、MRに適合する構成を選んである。この

以下に、本発明による実施の形態を、脳神経外科による実施例に基づき示す。
このような実施分野にとって有意義な点は、IMR内の、障害となるアーチファ
クトの形成(Artefaktbildung)を回避するために、本発明による装置をプラス
チック(PEEK、POM)から製造することである。
本発明は、脳手術のために必要不可欠な穿頭孔に自動ロック式の保持装置を嵌
め込む、という思想に基づいている。保持装置は、このような実施例では、第3
の固定点に頼ることなしに保持されるようにするために、1つの円筒形の穿頭孔
の形状を利用している。
前記穿頭孔が形成されるべき箇所を正確に規定するために、FPS10を位置
決め装置の目標ピン9(第4図)に被せ嵌める。この位置決め装置は、第3図に
示したように組み立てられる。この際、目標ピンのボールジョイント9bが、固
定輪1内および位置決め台架8内に設けられた上方および下方のジョイントソケ
ット1a,8cによってガイドされる。目標ピン9の
旋回半径は固定輪1の円錐形の内周面によって、後にガイドスリーブが固定輪1
内で制限されるように、制限される。固定輪1を回して締めることによって、目
標ピン9を位置固定することができる。
次いで位置決め装置を脚部8aで以て患者の頭部に対して保持し、かつ、FP
Sによって算出された軸線、もしくは目標ピン9が、手術すべき場所を示すまで
、この位置決め装置を移動させる。フレキシブルな脚部8aによって、位置決め
装置を押すことによりセンターマーカ8bを頭皮に押圧することができ、従って
、穿孔すべき箇所にマークすることができる。薄片4bの外周と穿頭孔とを等し
い大きさで選択する。
円筒形の穿頭孔(通常は直径15mm)を頭蓋冠7に形成した後、脳膜剥離装
置12で、脳膜を頭蓋冠7の内側から剥離する(第6図)。次いで、脳膜剥離装
置12に設けられた測定目盛り12aを用いて、頭蓋冠7の厚さを測定し(第7
図)、ひいては挿入片4の最良なサイズを選定する。挿入片4を前記穿頭孔へ貫
通案内する際に、フレキシブルな薄片4bは、突出する小フック4cによって一
層小さい半径に圧縮される。挿入片4の小フック4cが頭蓋内面に達した後、フ
レキシブルな薄片4bはその出発位置に弾発して戻り、これにより小フック4c
は穿頭孔の縁部を越えて押圧されて(第10図および第11図)、穿頭孔からの
保持装置の抜出しを防止する。挿入片4に環状に配置
されたフレキシブルな薄片4bは、これらの薄片4bが穿頭孔を貫通して案内さ
れた後に、この穿頭孔に密に接触する。さらに、押突棒11を用いて、ロックス
リーブ3を挿入片4に挿入する(第12図)。ロックスリーブ3は、このロック
スリーブ3がフレキシブルな薄片4cの内周面と嵌合することによって、薄片が
再び弾発して戻るのを防止している。次いで、プレロード輪5を回して締めるこ
とによって、頭蓋冠7を小フック4cとプレロード輪5との間で締め付ける。こ
のプレロード輪5によって、頭蓋冠7の種々異なる厚さを考慮することができる

、ガイドスリーブ2を位置固定することができる(第13図および第14図)。
次いで、生検針6をガイドスリーブ2内へ導入し、サンプルを採取することがで
きる(第14図)。
