Descrizione dell’invenzione avente per titolo: Description of the invention entitled:
PROTESI PER CRANIOPLASTICA E METODO DI REALIZZAZIONE CRANIOPLASTIC PROSTHESES AND METHOD OF CONSTRUCTION
La presente invenzione concerne una protesi per cranioplastica, in materiale biocompatibile, atta ad essere impiantata in sostituzione di parte della ossatura mancante della calotta cranica, nonché il metodo per la sua realizzazione. The present invention relates to a cranioplasty prosthesis, made of biocompatible material, suitable for being implanted to replace part of the missing skeleton of the skull cap, as well as the method for its realization.
Le evenienze più frequenti che richiedono un intervento di cranioplastica sono sostanzialmente tre, cioà ̈ la craniectomia post traumatica, la craniectomia demolitiva e le lacune ossee secondarie a sindromi di tipo malformativo. The most frequent occurrences that require a cranioplasty intervention are substantially three, namely post traumatic craniectomy, demolition craniectomy and bone gaps secondary to malformative syndromes.
Come tutti i materiali che vengono impiantati nel corpo umano, una protesi cranica deve essere biocompatibile. Inoltre deve ricostruire fedelmente il profilo osseo originario ed essere idonea a svolgere la funzione protettiva. Deve inoltre garantire una procedura chirurgica semplice e non deve alterare gli esami strumentali che si rendessero eventualmente necessari, quali RM, angiografia, EEG, ecc.. Like all materials that are implanted in the human body, a cranial prosthesis must be biocompatible. La tecnica nota prevede l’impiego di protesi su misura per cranioplastica in idrossiapatite porosa. Dette protesi, oltre a rispondere a questi requisiti, hanno qualcosa in più, in quanto alla biocompatibilità aggiungono la capacità osteoinduttiva dell’idrossiapatite porosa, che à ̈ un materiale biomimetico. In pratica una cranioplastica in cui la giustapposizione dei margini craniectomici con quelli dell’opercolo in idrossiapatite porosa sia eseguita a regola d’arte, consente un’ottima ricostruzione dell’osso mancante. Questa metodica presenta tuttavia alcuni inconvenienti. However, this method has some drawbacks.
Un primo inconveniente à ̈ la fragilità iniziale, che perdura comunque per molti mesi. Questo fatto obbliga il paziente ad indossare un caschetto protettivo ed a porre particolare attenzione in tutte le sua azioni, in quanto un eventuale trauma potrebbe provocare la frattura della protesi. Un secondo inconveniente à ̈ il rilevante costo di produzione. A second drawback is the significant cost of production.
Un terzo inconveniente consiste nel fatto che, per l’esecuzione della craniectomia e la conseguente realizzazione del bordo craniectomico, viene utilizzato un modello in resina di opercolo derivato dalla ricostruzione 3D del cranio. In pratica, il chirurgo deve trovare la migliore allocazione possibile dell’opercolo di prova e, una volta appoggiatolo sulla teca, ne disegnerà il perimetro e, quindi, procederà alla demolizione ossea per realizzare la preparazione dei bordi del cratere osseo al fine di accogliere la cranioplastica. Questa procedura à ̈ particolarmente delicata perché spostamenti di pochi millimetri sono assai difficili da controllare quando si lavora con tessuti molli e con posizionamenti degli organi non certo paragonabili a quanto possibile in un’officina meccanica. Si possono quindi commettere errori di posizionamento che, pur essendo ininfluenti sulla demolizione della lesione, possono condizionare pesantemente il risultato estetico, specie se collocati sulle bozze parietali o frontali e queste zone non sono coperte dai capelli. Per ovviare a questo inconveniente si realizza un’apertura di dimensioni inferiori a quelle dell’opercolo da accogliere e, successivamente, si conformano i margini con una fresa. Questa operazione tuttavia allunga i tempi dell’intervento e, soprattutto, à ̈ particolarmente critica in quanto viene effettuata a mano libera ed eventuali imprecisioni nella giustapposizione dei margini craniectomici con quelli dell’opercolo in idrossiapatite condiziona l’attecchimento della protesi stessa e la sua colonizzazione da parte degli osteoblasti ed osteoclasti. To obviate this drawback, an opening of dimensions smaller than that of the operculum to be accommodated is made and, subsequently, the margins are conformed with a cutter. However, this operation lengthens the time of the intervention and, above all, is particularly critical as it is carried out freehand and any inaccuracies in the juxtaposition of the craniectomy margins with those of the hydroxyapatite operculum conditions the engraftment of the prosthesis itself and its colonization by osteoblasts and osteoclasts.
Infine, in qualche raro caso si à ̈ verificato uno spostamento della protesi, con la conseguente necessità di un nuovo intervento chirurgico per il riposizionamento. Infine viene realizzata una protesi in polimetilmetacrilato (PMMA) che viene impiantata nella sua sede mediante delle clip realizzate in metallo a memoria di forma. Questa tecnica consente di ottenere una protesi perfettamente combaciante con i bordi dell’apertura sulla teca cranica, in quanto vengono prima realizzati a mano detti bordi e poi, rilevandone la geometria, viene realizzata di precisione la protesi. Inoltre il fatto di lavorare con un polimero compatto, permette di superare i problemi di fragilità delle protesi in idrossiapatite porosa. Le clip in lega metallica a memoria di forma sono realizzate in modo da serrare sia la protesi che l’osso cranico sul quale detta protesi viene impiantata. Le alette di dette clip vengono raddrizzate a freddo in modo da inserire l’opercolo nella sua sede. La protesi cranica secondo l’invenzione à ̈ del tipo atto ad essere impiantato in modo permanente in una cavità ricavata nella calotta cranica, ricalcandone con precisione i contorni e riproducendo sostanzialmente la geometria dell’osso originale che detta protesi sostituisce ed à ̈ caratterizzata dal fatto di prevedere primi mezzi atti a posizionarla correttamente in detta cavità e secondi mezzi atti a fissare in modo permanente detta protesi alle ossa del cranio, detto fissaggio essendo immediatamente idoneo a garantire la protezione al tessuto cerebrale sottostante, senza ulteriori mezzi aggiuntivi. Detti primi mezzi, atti a posizionare correttamente la protesi nella cavità della calotta cranica, comprendono un bordo sporgente lungo il contorno di detta protesi, detto bordo sporgente andando in battuta contro la superficie esterna della calotta cranica ed essendo fissato ad essa tramite viti. Il bordo sporgente che va in battuta contro la calotta cranica, consente di posizionare, con grande precisione e rapidità , la protesi nella sua sede, con ovvii vantaggi in termini di riduzione dei tempi dell’intervento e di miglioramento del risultato, soprattutto sotto il profilo estetico. Il fissaggio tramite viti consente poi di disporre fin dall’inizio della resistenza massima, con riduzione drastica dei tempi di recupero. Infatti à ̈ possibile premere in tutta sicurezza la protesi (3) contro la sua sede (2) in quanto la presenza del bordo (4) impedisce alla protesi stessa di premere contro il tessuto cerebrale. Il successivo bloccaggio con le viti (5) fisserà stabilmente la protesi (3) alla calotta cranica (1), la resistenza meccanica di detta giunzione essendo ampiamente sufficiente a mantenere in sede la protesi stessa. Inoltre eventuali traumi cui dovesse essere accidentalmente sottoposto il paziente, sarebbero efficacemente sopportati dalla protesi stessa, sia per l’elevata resistenza del materiale con cui à ̈ realizzata, sia per il fatto che distribuirebbe uniformemente alle ossa circostanti il carico dovuto al trauma, grazie al bordo sporgente (4) che poggia efficacemente su una superficie, relativamente estesa, che circonda il bordo craniectomico.
