Abstract

The catheter has a flexible and grooved collar (1) fixed to a deployed and lower end (5) when the probe is reinforced, where the catheter integrates a device for deploying the collar. A tube (2) has a distal end connected to the collar and another end connected to a circular pusher (3). The pusher is actuated outside the buccal cavity by a user after endotracheal intubation. The tube is stopped in a deployment position of the collar against stopping edges (4).

dispositif pour diminuer son risque de déplacement après intubation laryngée et auscultation des voies respiratoires. La sonde d'intubation, orotrachéale ou nasotrachéale, avec ballonnet ou sans ballonnet, est 5 traditionnellement fixée sur la face des patient(e)s par divers moyens qui peuvent se montrer parfois insuffisants pour prévenir un enfoncement ou un retrait accidentel de la dite sonde. Ces incidents surviennent la plupart du temps lors des changements de position des patient(e) s intubé(e) s (par exemple au cours des transports, de certaines chirurgies, ....), et peuvent entraîner des conséquences préjudiciables pour la perméabilité des voies respiratoires. 10 Le dispositif selon l'invention permet principalement de remédier aux risques d'intubation sélective par enfoncement accidentel de la sonde endotrachéale, mais ne dispense en aucun cas des actes de surveillances obligatoires. Ce dispositif à l'état déployé bloque la sonde endotrachéale de façon atraumatique, en lui évitant le franchissement des cordes vocales. Ce dispositif intègre la sonde endotrachéale, sa première caractéristique étant d'être 15 actionné hors de la cavité buccale en faisant coulisser son mécanisme d'ouverture sur une partie de la sonde. Il peut être détaillé en quatre parties, toutes reliées les unes aux autres : le pied de fixation d'une collerette, la collerette à ouverture parapluie , un tube coulissant et un poussoir circulaire. Ce poussoir, situé hors de la cavité buccale, fait coulisser le tube de quelques millimètres autour de la sonde et permet ainsi le déploiement de façon atraumatique 20 de la collerette devant les cordes vocales. Le poussoir peut alors être bloqué dans une butée, ce qui permet de laisser la collerette en position déployée. Selon des modes particuliers de réalisation : - la collerette peut varier en forme et en structure pour gagner en efficacité. La matière de la collerette doit être la plus souple et la moins traumatique possible en cas de contact 25 prolongé avec les muqueuses de la cavité orotrachéale. le tube peut coulisser en avant ou en arrière selon la configuration de la collerette (découpage, présence de stries, ...). - Le poussoir doit pouvoir se bloquer et se débloquer facilement en cas de repositionnement de la sonde ou pour toute autre manoeuvre. 30 - La sonde endotrachéale peut comporter différentes sortes de rebords de mise en butée. Les dessins annexés illustrent l'invention : La figure 1 représente le dispositif au repos. La figure 2 représente le dispositif déployé. En référence à ces dessins, le dispositif comporte une collerette souple et striée (1) 35 épousant la forme de la sonde à l'état de repos, fixée à son extrémité inférieure (pied de -2- fixation) (5) et déployée quand le dispositif est armé. Le tube (2), relié à son extrémité distale à la collerette et à son extrémité proximale au poussoir circulaire, coulisse jusqu'au pied de fixation de la collerette et permet ainsi son déploiement. This device incorporates the endotracheal tube, its first characteristic being to be actuated out of the oral cavity by sliding its opening mechanism on a part of the probe. It can be detailed in four parts, all connected to each other: the foot of attachment of a collar, the collar with umbrella opening, a sliding tube and a circular pusher. This pusher, located outside the oral cavity, slides the tube a few millimeters around the probe and thus allows atraumatic deployment 20 of the collar in front of the vocal cords. The pusher can then be locked in a stop, which allows to leave the collar in the deployed position. According to particular embodiments: - the collar can vary in shape and structure to gain efficiency. The material of the collar should be the most flexible and the least traumatic possible in case of prolonged contact with the mucous membranes of the orotracheal cavity. the tube can slide forward or backward depending on the configuration of the collar (cutting, presence of streaks, ...). - The pusher must be able to lock and unlock easily when repositioning the probe or for any other maneuver. The endotracheal tube may have different kinds of abutment flanges. The accompanying drawings illustrate the invention: FIG. 1 represents the device at rest. Figure 2 shows the deployed device. With reference to these drawings, the device comprises a flexible fluted collar (1) conforming to the shape of the probe in the rest state, fixed at its lower end (fixing foot) (5) and deployed when the device is armed. The tube (2), connected at its distal end to the collar and at its proximal end to the circular pusher, slides to the fixing foot of the collar and thus allows its deployment. This tube should slide easily around the endotracheal tube, without increasing the outer diameter of the probe.
