Description
COMPOSANT FÉMORAL POUR GENOU DROIT OU GAUCHEFEMORAL COMPONENT FOR RIGHT OR LEFT KNEE
La présente invention se rapporte à un composant fémoral d'une prothèse de genou ainsi qu'à une prothèse de genou comportant un 5 composant fémoral de ce genre. Il est connu dans le domaine du genou de nombreux types de composants fémoraux. Ces composants fémoraux comportent généralement deux condyles du côté antérieur et une partie de trochlée du côté postérieur, à la surface extérieure de laquelle est défini un trajet pour la trochlée. Compte 10 tenu du fait que le genou naturel présente entre l'axe anatomique et l'axe du tibia une petite différence angulaire, les chemins de trochlée formés dans la partie de trochlée sont disposés de manière à prendre en compte cet écart angulaire et sont inclinés par rapport au plan antéropostérieur. Il en résulte qu'il est nécessaire de prévoir des composants fémoraux avec des trochlées 15 inclinées à droite pour être adaptées à des genoux droits et des composants fémoraux ayant des trajets de trochlée inclinés à gauche pour s'adapter à des genoux gauches. La présente invention vise à surmonter ces inconvénients en proposant un composant fémoral qui peut être utilisé aussi bien pour un genou 20 droit que pour un genou gauche, sans aucune modification constructive. Suivant l'invention, un composant fémoral d'une prothèse de genou, comportant au moins un, de préférence deux condyles du côté antérieur ; une partie de trochlée du côté postérieur dans la surface extérieure de laquelle est défini un trajet de trochlée par un canal ouvert formé à la surface extérieure de 25 la partie de trochlée et s'étendant de la partie d'extrémité postérieure de cette partie de trochlée en direction de la partie antérieure de la prothèse, le canal ayant, en coupe transversale dans un plan axial de la prothèse (plan parallèle à l'axe antéropostérieur et à l'axe médio-latéral), la forme d'une courbe concave à concavité tournée vers l'extérieur de la prothèse, chaque courbe 30 ayant un rayon de courbure correspondant mesuré au niveau de son intersection avec le plan antéropostérieur, est caractérisé en ce que ledit rayon de courbure diminue de l'extrémité postérieure de la partie de trochlée en direction de l'extrémité antérieure de la prothèse, au moins le long d'un tronçon du canal. 35 Ainsi, au niveau de l'extrémité postérieure, la largeur du canal est plus grande qu'au niveau de l'extrémité antérieure de la partie de trochlée, de sorte que la trochlée peut entrer en coopération avec ce chemin de trochlée, soit en venant de la gauche (genou gauche), soit en venant de la droite (genou droit) et en étant ensuite prise dans le chemin de trochlée qui, petit à petit, a une largeur qui diminue à la manière d'un entonnoir pour se terminer en un chemin de trochlée au niveau de la partie antérieure de largeur sensiblement constante. Ainsi, contrairement au système de l'art antérieur, le chemin de trochlée est d'abord large, puis se rétrécit pour pouvoir accepter des trochlées venant soit de la gauche, soit de la droite au départ du chemin. Suivant un mode de réalisation préféré de l'invention, le composant est symétrique par rapport à un plan antéropostérieur. Ainsi, on obtient une prothèse qui peut être utilisée aussi bien pour le genou droit que pour le genou gauche et qui est très facile à fabriquer. Suivant un mode préféré de l'invention, le canal ouvert a la forme d'un entonnoir en ayant une section de passage à l'extrémité postérieure supérieure à la section de passage à l'extrémité antérieure. Suivant un mode de réalisation préféré, le canal ouvert comporte un tronçon postérieur ayant une section de passage qui diminue jusqu'à un tronçon antérieur ayant une section de passage constante. Suivant un perfectionnement de l'invention, le canal est creusé dans la surface extérieure du composant. La présente invention se rapporte également à une prothèse de genou ayant un composant fémoral suivant l'invention. Thus, at the posterior end, the width of the channel is greater than at the anterior end of the trochlea portion, so that the trochlea can enter into cooperation with this trochlear path, either coming from the left (left knee), or coming from the right (right knee) and then being taken in the trochlea way which, little by little, has a width that decreases in the manner of a funnel to end in a trochlear path at the anterior portion of substantially constant width. Thus, unlike the system of the prior art, the trochlea path is first wide, then narrows to accept trochlées coming from the left or right from the path. According to a preferred embodiment of the invention, the component is symmetrical with respect to an anteroposterior plane. Thus, a prosthesis is obtained that can be used for both the right knee and the left knee and is very easy to manufacture. Aux figures 2a et 2b, il est représenté une vue en coupe d'une partie de la prothèse de la figure 2, suivant les lignes 11A-IIA et 11B-IIB respectivement. Aux figures, il est représenté un composant fémoral d'une prothèse de genou. Elle est constituée d'une partie 1 de trochlée, d'une part, et de deux condyles 2 et 3, d'autre part. A l'intérieur du composant, il est formé une cage 4 définie par plusieurs faces, à savoir une face 5 d'extrémité postérieure, une face 6 intermédiaire et une face 7 d'extrémité antérieure. II est également possible de prévoir des faces supplémentaires. Des plots 8 d'ancrage sont issus de la face 6 intermédiaire pour un ancrage du composant fémoral dans le fémur. A la surface extérieure 9 du composant de la prothèse il est défini un trajet de trochlée. Ce trajet de trochlée est défini par un canal 10 ouvert en forme d'entonnoir. Ce canal 10, dans des plans de section axiale, c'est-à-dire des plans successifs correspondant au plan de la figure 1, a la forme de courbes ayant leur concavité tournée vers l'extérieur. On définit le rayon de courbure d'une courbe comme étant le rayon de courbure de la courbe, au niveau du centre de symétrie du composant (c'est-à-dire à l'intersection de la courbe avec le plan antéropostérieur), c'est-à-dire au niveau du fond 11 du canal définissant le trajet de trochlée. Au niveau de l'extrémité 12 postérieure du trajet de trochlée, ce rayon de courbure est égal à RI. Au niveau de l'extrémité extérieure de cette partie de trochlée, il est défini par R2. Le rayon de courbure R1 est supérieur au rayon de courbure R2, cela signifie que le canal est plus étroit du côté de son extrémité antérieure que de son extrémité postérieure. Lorsque la trochlée commence à coopérer avec le trajet, elle peut être "happée" par le canal en entonnoir en venant aussi bien de la gauche de l'axe 15 qu'en venant de la droite. Ainsi, comme dans le cas des prothèses de l'art antérieur, la trochlée va bien suivre un chemin qui est incliné, en fait, au départ par rapport à l'axe 15, que ce soit incliné vers la gauche ou vers la droite en fonction du genou. Cependant, on peut utiliser la même prothèse dans les deux cas. Le canal comporte un tronçon 16 postérieur dont la section de passage (surface comprise en section perpendiculairement au plan de la figure 2 entre la paroi 17 définissant le canal et une droite 18 imaginaire reliant les deux points les plus hauts du canal au niveau de la section) diminue jusqu'à un tronçon 19 de section constante. La paroi 17 définissant le canal comporte une paroi latérale 21 gauche et une paroi latérale 22 droite.
