1 La présente invention concerne un dispositif appelé trocart, utiliséThe present invention relates to a device called trocar, used
Cette gaine maintient donc une ouverture permanente pendant l'intervention chirurgicale et constitue un passage pour différents instruments nécessaires à l'intervention. in endoscopic surgical procedures which consists mainly of a tubular sheath constituting a crossing of the external wall of a patient to reach organs, especially in the abdomen. This sheath thus maintains a permanent opening during the surgical procedure and constitutes a passage for various instruments necessary for the intervention.
In the case of the invention, the device is primarily intended for the establishment of an endoscope through which we illuminate and view the operative field. BACKGROUND OF THE INVENTION The target organ of the operation is generally separated from the patient's abdominal wall, for example, by a pocket of gas injected at the rear of the wall and kept under pressure for this purpose. During the entire procedure, the pressure of the pocket is maintained and each trocar intended for the passage of the instruments has a system of valves that allows to introduce or remove each instrument without significantly reducing the pressure inside the aforementioned pocket . The endoscopic device, which illuminates the operative field and receives images from it, is subjected during the procedure to soiling resulting from several splashing of body fluid, contact with the instruments inside. of the operating space ....: It is therefore necessary for the surgeon to clean the optics of this endoscopic apparatus and all the more frequently that the "gaseous" space in which it is placed is weak dimensions. To do this he is forced to extract it from the trocar and, after having cleaned it, to reintroduce it. These manipulations have several drawbacks that essentially stem from the fact that a surgical procedure is always a battle against time and therefore the shorter it is, the better and also that a surgical intervention fully mobilizes the attention of the surgeon who necessarily relaxes the cleaning time, during which nothing happens. OBJECT OF THE INVENTION The present invention intends to overcome this disadvantage by means of a device that allows the cleaning of the optics of an endoscopic device without the need to take it outside the body of the patient. SUMMARY OF THE INVENTION For this purpose, therefore, the subject of the invention is an endoscopic surgical intervention device designated trocar, comprising a tubular body open at one of its ends, and equipped at another end with an arrangement sealant for the passage and guidance of a vision equipment terminated by an optic having. a substantially front surface displaceable in the tubular body so that the front surface can project out of this first end. The wall of the tubular body has in the vicinity of this first end one or more nozzles for ejecting a cleaning agent from the front surface of the optics, oriented towards this surface and. connected to a source of this agent external to the body. With this ejection nozzle of this agent which can be liquid, it is possible to rinse the front surface of the optics within the field of operation, the flushing liquid can be a biological fluid ( for example a physiological saline) with or without added products for example surfactants, which is assimilable. This cleaning agent may also be gaseous (a neutral gas) by the blowing of which it is possible to drive a particle depositing on this front surface. Finally, it is possible in the spirit of the invention to project through the nozzle, on the front surface of the optics first a liquid and then a drying gas of the cleaned optics, in the same ejection operation. . Preferably, the ejection nozzle is connected to the source of the cleaning agent by a channel formed in the same wall of the tubular body of the trocar from the nozzle to a connection port of the source in question. . Advantageously, the connection orifice is provided with a pierceable sealing plug restored after perforation, while the liquid source is a syringe. This syringe may also contain a dose such that its total injection by action on the syringe is sufficient to achieve a perfect cleaning of the front surface of the optics exposed to soiling. It becomes a disposable syringe (or adapted to cooperate with a single-use dose). In an advantageous embodiment, the single dose may also comprise, after the liquid, a cushion of gas that can be propelled after the liquid towards the optics. To carry out this cleaning, it is necessary to bring the front surface of the optics into a determined position inside the tube, therefore to the nozzle, this position being reached by retraction of the front surface of the optics to the inside of the tubular body until a mark provided on the inner surface of the tubular body above the nozzle is visible by this optic. The retraction is then stopped and the spraying or the ejection of liquid and / or gas is carried out. It has been found that, by the effect of the surface tension forces, a drop of liquid could remain attached to the surface of the lens and in particular to the lower point thereof in the angle which it forms with the tubular wall of the trocar. To easily remove this drop, the inner surface of the tubular body has a draining area extending from the position taken by the optics for cleaning to the end of the tubular body. This draining zone may be made by a particular coating of this surface (water repellent) or by micro-reliefs such as for example a grooving advantageously comprising a groove for collecting this liquid which may be partial and at least one groove between the groove in which it de-mouths and the end of the tubular body. It has been realized that with this type of means, a simple and effective drainage of the residual liquid at the bottom of the lens in the angle with the inner surface of the lens can be achieved. tubular body. Other features and advantages of the invention will emerge from the description given below of one of its embodiments. BRIEF DESCRIPTION OF THE DRAWINGS Reference is made to the appended drawings, in which: FIG. 1 is a general sectional view of a trocar according to the invention, FIG. 2 is a detailed view on a larger scale of the provisions of FIG. trocar according to the invention. DETAILED DESCRIPTION OF THE INVENTION Reference is made to these two figures. A trocar 1 is formed of a substantially tubular body 2 at the end 3 of which it comprises a sealing arrangement, that is to say, in known manner, a set of shutters and buttons which can to be retracted when passing an instrument while sealing around this instrument and closing the upper opening of the trocar in the absence of an instrument. This sealing arrangement can be contained in a chamber in one piece with the body or attached to the body. At its other end, the tubular body 2 has a lower opening 4 through which an optic 5 having a terminal and front surface 6, belonging to a movable vision material in the trocar, can project out of the tubular body. It is understood that the opening 4 of the trocar is that which is located above the target organ of the operative field, that is to say below the wall, for example abdominal of a patient. In the vicinity of this opening 4, the trocar has an ejection nozzle 7 of a fluid agent, liquid or gaseous, turned towards the inside of the tubular body and thus towards the front surface 6 of the optics 5 when the latter It is retracted into a defined position inside the tubular body 2. In the embodiment shown, this nozzle 7 facing upwards and towards the axis of the trocar is connected to a feed duct 8 formed in the same wall of the tubular body 2 which opens into a chamber 9 open on the outside, close to the sealing arrangement 3 and closed by a plug 10 in an elastomeric material such that the plug is perforable by the needle d a syringe 11 around which it forms a seal and after removal of the needle, continues sealingly sealing the chamber 9.
The syringe 11 can contain a fixed dose of liquid to effect an effective rinsing of the front surface 6 of the optical lens 5. Thus, during an operation, can the surgeon use several of these syringes? unique. Of course, any other source of liquid may be suitable and the chamber 3 may include any suitable means of connection to this other source. Similarly, instead of a liquid source can propel a neutral gas (carbon dioxide) sterile. We can also conceive the propulsion of a dose first liquid then gaseous. Before doing this injection, however, the surgeon will place the front surface 6 of the optics in a specific position to obtain a good rinsing. This determined position is that which will be indicated to it, during the retraction of the optics inside the trocar, by a marking index 12 formed on the inner surface of the tubular body 2, that it will appear in the field of optics when the front surface 6 will arrive at this level. It will thus be certain that the optic will be in a correct position vis-à-vis the ejection nozzle 7. This marking index can be of any nature to be clearly visible by the endoscope optics. It is located above the nozzle 7. In this same zone, the inner surface of the body 2 is hollowed out with a groove here in part 13 which communicates with axial grooves 14 putting it in communication with the opening 4. This throat and these grooves are means for draining a drop of cleaning liquid which could be located, by the play of the surface tension forces, between the front surface and the inner surface of the tubular body 2 at the lowest point thereof. this. Indeed, the use of such devices is always in an inclined deflection, which defines a high point and a low point for the front surface 6.
