Abstract

The device has a circular wire (1) whose lower edge is a cutting edge and integrated to a handle (3). The wire and handle slide in a guide tube (5). The wire is rolled in the tube during its insertion into the eye and is unrolled to its initial shape. The wire is applied on an anterior capsule of an intraocular lens by a simple pressure for repeatedly performing cutting operation. The handle has a stop (6) to limit its movement.

Description

La présente invention concerne un dispositif permettant leThe present invention relates to a device allowing the

This cutting is an essential initial time in this intervention, allowing to remove a part of the envelope of the lens (capsule, whose thickness is a few microns) in order to access its content (nucleus, cortex) which will then be extracted through this capsular orifice.

The cutting of this capsule, called capsulorhexis, is currently carried out in three ways, either by a needle whose pointed end and with sharp edges is bent at 90 relative to its main axis (cystitome), or using forceps with curved jaws and ends similar to those of the cystitome, or finally using a blade which performs the cutting by setting it in motion thanks to a motorized system. In the first two cases, the most widely used, two types of drawbacks exist: on the one hand cutting requires several dissection times of the capsule making it difficult to produce a perfectly regular edge with the risk of tearing towards the periphery of the capsule, on the other hand there is an absence of precision, however sought after, of the size and of the shape of this cut (the rigorous determination of the exact diameter of the capsular flap is impossible, and always approximate). In the third case, it is a cutting by complex motorized mechanism of a blade on the capsule, which does not allow a direct 360 cutting because of the blade outlet arrangement outside its handle, and the cutting action of which is made possible only by setting the said blade in motion on the capsule.

The device according to the invention overcomes these drawbacks. According to a first characteristic, it indeed comprises a deformable circular wire having an excellent shape memory, the lower edge of which is sharp while a portion of the upper edge is secured to a handle. The assembly (wire and handle) is completely introduced into a guide tube in which it can slide. The wire thus contained in this guide tube, its sharp cutting edge has no tissue aggressiveness during its introduction into the eye. The guide tube is thus introduced inside the eye to the central part (pupil). The operator then only has to push on the handle to take the wire out of the guide tube (sheath), the wire then regaining its initial circular shape, then simply apply the sharp underside of this wire on the capsule of the lens to cut it in a single time and with extreme qualitative precision (regularity of sphericity of the peripheral edge of the cut capsular flap) and quantitative (diameter and shape of cut capsule). As soon as the cut is made, a reverse traction movement on the handle allows the wire to be reintroduced into its sheath (guide tube 15) so that the assembly can be removed from the eye without risk of injury by the sharp edge of the wire. In addition, the fact of introducing the folded wire into the guide tube reduces the size of the entry incision of the instrument to the minimum, that is to say that of the width of the guide tube. The displacement of the handle in the guide tube is achieved by simple mechanical action on a handpiece secured to the distal end of the handle.

According to particular embodiments: the guide tube can have an oval, rectangular or any other shape.

-le manche peut être de forme rectangulaire ou de tout autre forme autorisant son coulissage dans le tube guide.  the handle can be rectangular or any other shape allowing it to slide in the guide tube.

-le fil circulaire peut être de coupe triangulaire (à pointe inférieure et base supérieure), ou polygonale (à bords inférieur tranchant), ou de toute autre forme.  the circular wire can be of triangular cut (with lower point and upper base), or polygonal (with lower sharp edges), or of any other shape.

-le fil peut être de dimensions variables.  -the wire can be of variable dimensions.

-the wire can be metallic, plastic or any other constitution.

-the handle can be secured by its distal, proximal end or any other segment to a handpiece, itself secured to the guide tube.

The accompanying drawings illustrate the invention: Figure 1 shows the device of the invention seen from its underside with the circular wire, the handle and the guide tube.

FIG. 2 shows the device of the invention with the wire introduced into the guide tube and the handle in the maximum retraction position [A], then with the wire completely removed from the guide tube and the handle in the maximum thrust position [C ], and in the intermediate position [B]. With reference to these drawings, the device comprises a circular wire (1), a portion of the upper face (2) of which is integral with a handle (3), and the lower edge (4) of which is sharp over its entire circumference. .

The wire-sleeve assembly slides inside a guide tube (5). The stroke of the handle (3) in the guide tube (5) is actuated by a handpiece (6) integral with the distal end of the handle (7). It is thanks to its deformability that the wire (1) can be inserted into the guide tube (5) when the handle (3) is pulled back. It is also thanks to its perfect shape memory that it (1) will resume its initial circular shape when it is removed from the guide tube (5) by pushing the handle (3) forward.

By way of nonlimiting example, the wire will be circular with a diameter of 8 mm and a triangular cut with an upper base. The thickness of the wire will be 300 microns and the width of its upper face 500 microns. The external width of the guide tube will be 2 mm, its height 1 mm and its length 20 mm. The length of the handle will be 30 mm and that of the handpiece 20 mm.

The device according to the invention is particularly intended for cutting the anterior capsule of the lens (capsulorhexis) during cataract surgery.
