Description

SURGICAL PLIERS FOR TENSIONING
OF A LIGAMENT OF OSTEOSYNTHESIS
The present invention relates to surgical forceps for tensioning an osteosynthesis ligament, in particular vertebral osteosynthesis.

It is known to intertwine a ligament around the spinous processes, the vertebrae located on either side of the joint to be treated, and to sew the ends of the ligament to each other or to the ligament itself, after placing in tension of it. This technique helps stabilize the joint while retaining a certain functional play.

In certain cases, an intervertebral wedge can be inserted between the spinous processes of at least two successive vertebrae, to prevent any mutual contact of the vertebrae at the level of these processes, which can cause the crushing of the spinal nerves.

To facilitate tensioning and maintaining tension in the ligament while these seams are being made, French patent application No. 93 05742 proposes a device secured to one of the ends of the ligament, comprising means for engaging and retention through which the other end of the ligament can be engaged after having bypassed the spinous processes of the two vertebrae, and through which this end of the ligament can slide in its direction of engagement, but not in the opposite direction.

The present invention aims to provide a clamp for tensioning the ligament equipped with such a device, capable of making it possible to perfectly control this tension and to maintain it until the seams of the ends of the ligament are made, and this in the low space available.

To this end, the clamp which it concerns, of the type comprising two branches pivoting with respect to each other about an axis, is characterized in that it comprises a substantially straight branch, shaped to be immobilized by relative to the end of the ligament equipped with said engagement and retention means, and a bent branch equipped with a clamping jaw capable of grasping, without possibility of sliding, The other end of the ligament, after this end has bypassed the spinous processes of the vertebrae and has been engaged through said means, the proximal ends of the two branches can be brought into a position close to one another, in which they are substantially parallel, and be separated from one 'other by pivoting a branch relative to the other.

This clamp thus makes it possible to ensure perfect tensioning of the ligament, with control of the tension exerted and possibility of maintaining this tension, by immobilization of the branches relative to one another, the time that the seams of the ends to one another or the seams of these ends to the ligament itself.

Advantageously, the clamp is equipped with means making it possible to maintain these two branches in a plurality of determined relative positions, to make it possible to adjust, by reaching such or such determined position, the tension imparted to the ligament, and to keep the ligament under tension over time that said seams are made.

According to a preferred embodiment of this clamp
- The end of the rectilinear branch comprises a cavity allowing it, by engagement in a direction perpendicular to the ligament, to come to cap without possibility of play, the tubular cage which advantageously comprises one of the ends of the ligament to delimit the conduit of engagement of the other end of this ligament, and
the clamping jaw is formed, on the one hand, by the proximal part of the bent branch of the pliers and, on the other hand, by a finger mounted pivoting about an axis perpendicular to this proximal part, this finger being activated, on one side of this axis, by a spring which tends to move it away from the proximal end of the bent branch and, on the other side of this axis, by a threaded rod passing through a threaded bore arranged in the proximal part of the bent branch, which allows, when tightened, to bring the proximal end of the finger closer to the proximal end of the bent branch, the rod having a length such that its free end is easily accessible to the practitioner and being shaped at this free end to receive a tool, such as a wrench or a screwdriver, allowing it to maneuver in rotation.

The cavity of the proximal end of the straight branch makes it possible to immobilize this branch perfectly with respect to the ligament, and to present the clamping jaw of the other branch near the end of the ligament that this jaw is intended to grip.

Advantageously, means are provided for maintaining the proximal end of the straight branch in the engaged position on this cage. For example, the walls of the rectilinear branch delimiting the cavity may include ribs parallel to the axis of engagement and the side walls of the cage may include notches in which these ribs engage during the establishment of the proximal end of the straight branch on the cage.

The threaded rod allows, by its length, an easy operation of the closing of the jaw and a tightening of the ligament which eliminates any risk of slipping.

For its proper understanding, the invention will be described again below, with reference to the appended schematic drawing representing, by way of nonlimiting example, a preferred embodiment of the surgical forceps that it relates to.

In this drawing,
Figure 1 is a flat view of this clamp;
Figure 2 is a side view
Figure 3 is a partial view, similar to Figure 2, in a position other than that shown in this figure
FIG. 4 is a view similar to FIG. 1 in another position of this clamp than that shown by this figure, and
Figure 5 is a sectional view along line VV of Figure 4 of a vertebral osteosynthesis ligament equipped with means for connecting its ends, and the proximal ends of the branches of the forceps.

FIG. 5 represents a vertebral osteosynthesis ligament as described in French patent application No. 93 05742.

This ligament 2 comprises a plate 3 of resistant and malleable material, capable of being fixed to one 2a of the ends 2a, 2b of the ligament 2, by seams, and a tubular element 4 forming a cage, integral with the plate 3, delimiting a conduit 5 in which a cam 6 is arranged transversely.

This cam 6 can pivot on itself between an erased position and an active position. In the retracted position, it allows the engagement and sliding of the end 2b of the ligament 2 between it and the wall of the cage 4, after the ligament 2 has been engaged around the two spinous processes 7 of the vertebrae delimiting the joint to be treated. In the active position, it wedges said end 2b between it and this wall, to prevent any movement of the end 2b in the opposite direction from its direction of engagement.

The clamp 10, shown in Figures 1 to 4, is intended to tension the ligament 2, allowing to perfectly control the tension exerted, and to maintain this tension while the seams of the ends 2a, 2b one to the other or the seams of these ends 2a, 2b to the ligament 2 itself, and this, in the small space available.

This clamp 10 comprises two branches 11, 12 which can pivot relative to each other about an axis constituted by a screw 13.

The branch 11 is rectilinear and its proximal end comprises a cavity allowing it to be engaged on the cage 4, transversely with respect to the ligament 2, coming to cap this cage 4 without play.

As shown in FIG. 5, the side walls of the branch II delimiting this cavity comprise two ribs 14 parallel to the axis of engagement of the branch II on the cage 4, while the side walls of the cage 4 include notches in which these ribs 14 are capable of being engaged and of sliding.

At its distal end, the branch 11 has an axis 15 around which is engaged a pin spring 16 and can pivot a rack 17 provided with teeth 18.

The branch 12 is bent near the axis 13, on the distal side, and is equipped with a clamping jaw 20 capable of grasping, without possibility of sliding, the end 2b of the ligament 2 after this end 2b has bypassed the spinous processes 7 and has been engaged through the cage 4.

The clamping jaw 20 is constituted, on the one hand, by the proximal part of the bent branch 12 and, on the other hand, by a finger 21 mounted pivoting about an axis 22 perpendicular to this proximal part, this axis 22 being carried by at least one tab 23.

The finger 21 and the proximal part of the branch 12 comprise, on the proximal side of the clamp relative to the axis 22, two recesses located opposite one another, receiving the ends of a helical spring 24 On the other side of this axis 22, the finger 21 is chamfered and the branch 12 is pierced with a tapped bore in which is engaged a screw 25. The proximal end of the screw 25 bears against the chamfered part of the finger 21, while the distal end of the rod 25 is shaped to receive a tool allowing its operation in rotation, such as a wrench or a screwdriver.

At its distal end, the bent branch 12 comprises a tooth 26 capable of coming to bear against one or the other of the teeth 18 of the rack 17.

In practice, the cavity of the proximal end of the straight branch 11, once engaged on the cage 4, makes it possible to immobilize this branch il relatively to the ligament 2, and to present the clamping jaw 20 of the other branch 12 near the end 2b of the ligament 2 that this jaw 20 is intended to grip.

The engagement of the ribs 14 in the notches of the lateral walls of the cage 4 makes it possible to maintain the proximal end of the branch 11 in the engaged position on this cage 4.

The threaded rod 25 makes it possible to maneuver the finger 21 in the direction of closing the jaw 20, by opposing the action of the return spring 24 which, in turn, tends to move said finger 21 away from the proximal end of the branch II when the rod 25 is maneuvered in the other direction.

This rod 25 has a length such that its free end is easily accessible to the practitioner. By its length, it allows an easy operation of the closure of the jaw 20 and a tightening of the ligament 2 which eliminates any risk of slipping.

When the branches 11, 12 of the clamp 10 are pivoted in the direction of approximation of their distal parts, that is to say when they are brought from the position they occupy in Figure 1 to that they occupy in Figure 4, there is provided a tensioning of the ligament 2 by pulling its end 2b. The tooth 26 of the branch 12 is hooked behind one or the other of the teeth 18 of the rack 17, to maintain the branches 11, 12 in a determined relative position. Thus, it is possible to adjust, by reaching such or such position determined by the hooking of the tooth 26 behind one or the other of the teeth 18, the tension imparted to the ligament 2, and to keep the ligament under tension for the time that the aforementioned seams are made.

It goes without saying that the invention is not limited to the particular embodiment of this clamp which has just been described by way of example but that it embraces, on the contrary, all the variant embodiments.
