Abstract

The invention relates to a device allowing, through the vein, a wrongly positioned filter of the Greenfield type to be detached from the internal wall and to be moved. The device is composed of an extractor situated at one end of the catheter tube, a side adaptor allowing injections being situated at the other end. It is characterised by a hook situated at the end of a semi-rigid central rod, which makes it possible to secure the cap of the filter and to cause it to return by a relative reversed movement into the case of the extractor after having detached it from the vein.

Description

The present invention relates to a Greenfield or related filter receptor extractor. For many years, thromboembolic disease has in some cases been a partial interruption of the vena cava. The aim of these partial vena cava interruptions is to avoid embolic recurrence, particularly when there is a contraindication to conventional treatment or anticoagulant treatment. Over the past ten years, techniques have evolved a lot. This involves under local anesthesia to surgically strip a large peripheral vein # either the internal jugular vein or the femoral vein. The internal jugular route is the most used currently. When this vein is exposed, a large caliber catheter is inserted containing an endovascular prosthesis at its distal end.

It is either a MOBIN-UDDIN type umbrella or a GREENFIELD or related filter.

These prostheses are lowered into the inferior vena cava under the renal veins and released there. These endoprostheses have the role of filtering the blood flow and of retaining possible trombis, cause of recurrences of the disease # thromboembolism. Unfortunately in a number of cases Greenfield type filters can be incorrectly positioned, too low, tilted, insertion into a renal vein, or into the right atrium. Some of these malpositions are likely to cause a serious repercussion in particular when it is about an insertion in the auricle, or in the renal vein. It is then a question of extracting this filter and until now only the surgical way, under general anesthesia was possible.

This situation, although rare, poses many problems because the intervention is not always well supported by an already weakened patient. The description of the different elements will facilitate good understanding.

Figure 1 is a view of the extractor assembly 1 in section, the inner part 2 serves as a store for the Greenfield filter 3. The flare or internal cone 4 allows the head 5 of the Greenfield filter 3 to easily enter the interior of the magazine 2 pulled by the hook 6 in the form of a hook located at the end of the rod 7 of semi-rigid steel which passes through the extractor 1 at point 8 and slides in the catheter tube 9. The external shape of the extractor 1 is characterized by its oval-shaped distal termination 10, connected to the base of the inner cone 4 by a rounded 11. The hook 6 is slightly open and its termination is rounded 12 to make it atraumatizing and facilitate attachment of the conical cap 5 surmounting the Greenfield filter 3. The catheter tube 9 is fixed to the nipple 13 located at the upper end of the extractor 1 is permanently fixed by a ferrule 14 whose external angles 15 are rounded.

FIG. 2 represents the other end # of the catheter tube 9 mounted with a ferrule 14 on an adapter 16 of the Luer Lock type with lateral bypass 17. The steel rod 7 passes through it and receives, in its extension, a system 1E of diametric locking by tightening a conical pliers 19 tightened by a knurled conical nut 20 which ensures by its outside diameter a good grip of the operation - causing the rod 7 to move throughout.

FIG. 3 represents the extractor in the vein, the hook sor and moored to the cap 5 of the filter 3.

FIG. 4 schematically represents a recovery moment the recovery operation of the filter 3. A downward movement of the extractor 1 which the operator pushes through the catheter tube 9 in the direction of the plug and a reverse movement of the rod 7 makes it possible to unhook the points of the filter 3 from the vein. By continuing to pull the rod 7 and lowering the extractor 1, the diameter of the filter 3 is reduced and it is introduced entirely into the magazine 2 of the extractor.

The object of the present invention is to remedy the malposition acc lace of the Greenfield filter 3 in the vein which is in the form of a flared parasol, the very flexible threads of which end in dots to hang inside the veins. These wires are grouped at a central sum by a small conical cap 5. The introduction into the right internal jugu lar area of a catheter assembly 9, provided at its distal part with an extractor 1 and at the other end of the catheter 9 is fixed an adapter 16 to lateral channel 17 of the Luer-Lock type . A thin central rod 7 of steel passes through the adapter assembly 17, catheter tube 9, extractor 1, and is mine by a hook 6 in the form of a hook. The adapter 16 Luer-Lock, allows by the lateral route l injection of different liquids which circulates in catheter tube 7 arrives in the vein through the extractor 1. The adapter 17 does not allow the injected liquid to escape other than by the tractor 1.

The opposite end of the central steel rod 7 protrudes from the atomizer 17 and receives a locking device 18 with pliers 19 which by rage with knurled nut 21 becomes integral with the toga 7. By its external dimensions this system of locking allows the operator a good grip of the central rod so as to maneuver it, to direct the hook 6 the vein towards the Greenfield filter 3. The assembly catheter tube 9 extrac 1 and adapter 16 Luer-Lock allows the operator to pull, push or tou: the central rod even if the assembly is bent.

The extractor 1 has the peculiarity inside of having a circular magazine 2 ending at its distal part by a cone 4 or flare which allows the open umbrella forming the filter 3 in the vein to be able to penetrate therein. diametrically constricting.

The main characteristic of the intervention resides in the maneuver of total penetration of the filter 3 into the store by a first movement of mooring of the cap 5 of the filter by the hook 6 taken out of the store 2 in a second time the operator descends the extractor above the filter 3 in malposition and the outer cone 4 comes by this downward movement, fitting onto the umbrella of the filter 3, continuing to descend the ramp 4 restricts it, its diameter gradually decreases and finally the filter 3 is fully retracted with the tip completely hidden, in the magazine of extractor 1.

Thereafter two maneuvers are possible:
A. Removal of the extractor containing the filter by traditional methods.

B. Either with the extractor reposition the filter and release it in the right place.

The external distal part of the extractor has an ovoid shape which facilitates introduction into the jugular vein and which is connected with the internal cone 4 by a rounded 11.
