Abstract
Description
Apparatur zum Absaugen von Linsenresten bei augen-Apparatus for suctioning off lens residues from ocular
Cataract Surgical Operations The invention relates to an apparatus for suctioning off lens residues during cataract ophthalmic operations, in which a double-lumen cannula of a handpiece through an incision on the edge the cornea of an eye to be operated on is introduced into its anterior chamber and the latter Rinsing liquid is supplied to the kantle via a lumen and then together with the Lens residues are sucked off again via the other lumen, with a source of irrigation fluid, which can be released with the one lumen of the cannula of the handpiece via a hose line connected is, and with one having a rotating rotary piston Pump that with the other, sucking off the rinsing liquid with the remnants of the lens serving lumen of the double cannula also releasably connected via a hose line wherein a portion of tubing extending around the orbit of the rotary piston squeezed off per piston revolution and thus one that corresponds to the direction of rotation of the piston Pumping effect is generated.
In extracapsular cataract operations, for example, must be opened the anterior lens capsule and, after the lens has been smashed, the resulting lens fragments are aspirated, including here all the lens particles that occur during such operations are to be understood. The operative measures to open the anterior lens capsule and about smashing the lens are not of interest here.
In operations of this type, it is known to make an incision on the At the edge of the cornea of an eye to be operated on, a double-lumen cannula of a handpiece to be introduced into the anterior chamber of the eye and to supply irrigation fluid via a lumen and at the same time via the other lumen of the cannula, the one with debris sucking off charged rinsing liquid again. In the case of an apparatus known to that extent the rinsing liquid is transferred from an infusion surface via a hose to the a lumen fed to a double cannula of a handpiece, while by means of a motor-driven peristaltic pump over the rinsing liquid with the lens residues the other lumen of one inserted into the anterior chamber of the eye to be operated on Double cannula is sucked off with the said pump via a hose line is releasably connected.
Apart from shortcomings that are not of further interest here of the handpiece that became known in connection with the previously known apparatus does the previously known apparatus not adequately meet the requirements of practice, as an adaptation to the individual needs of the respective test person is possible. The present invention is intended to remedy this deficiency be that in an apparatus of the type specified between the pump and ir the latter with the hose line connecting one lumen of the double cannula #a the suction effect of the pump on a predetermined one adjustable Pressure level limiting device is arranged. By such a negative Pressure limitation ensures that when sucking off rinsing liquid with the in this contained lens particles in the anterior chamber of the eye to be operated on a predetermined vacuum is not exceeded.
According to an embodiment of the invention, the device to limit the pressure of the pump in stages to different negative pressures be adjustable pressure cell and in a further embodiment according to the invention, the device for negative pressure limitation can be activated if the value falls below the set value Vacuum must be assigned to the earth switch that switches off the pump.
With such a design of the apparatus according to the invention occurs if the set vacuum is exceeded, the Pump on.
The reason for exceeding the set vacuum is shown in usually a blockage of the aspiration opening of the double-lumen cannula, which can be caused, for example, by lens residues that affect the cross-section of the Suction opening exceed and therefore cannot be extracted.
Since in such operations the surgeon often has no direct visual contact having to work with the operating field, it has proven advantageous to use after Another feature of the invention of the pump is one of its suction strokes to be assigned to the acoustic signal generator indicating the mode of operation of the pump and emits a signal tone with each stroke of the hose pump so that the surgeon is continuously informed about the operation of the apparatus.
Another feature of the invention is such designed start-up delay of the drive motor for the rotary piston of the pump characterized in that the pump only after a predetermined period of time has elapsed the start of operation reaches a predetermined pumping capacity.
The suction effect does not occur suddenly when the pump is switched on at full strength, but the pump gradually runs with a slow build-up the suction power to after a predetermined period of time of, for example, five up to eight seconds to achieve their full suction power.
It has been found that the suction power of the pump can be adjusted proven to be useful, regardless of the negative pressure limit, so that the delivery rate per unit of time to the individual circumstances of the respective operation can be customized. The suction power of the pump can be changed in a simple manner the speed of the rotary piston can be adjusted.
This is a particularly important design feature of the invention characterized in that the direction of rotation of the rotary piston of the pump can be reversed is.
When aspirating the double cannula through a lumen into the anterior chamber Rinsing liquid introduced into an eye may be given in the suction stream lens fragments carried along easily to blockages of the aspiration opening of the in The double cannula of the handpiece inserted into the anterior chamber of the eye to be operated on come. Naturally, in the event of such blockages, it can only be operated be reached again when the obstruction is cleared. To that extent, the Reversibility of the pump by reversing the direction of rotation of the rotary piston is advantageous proven because of it a corresponding one on the actual suction side Overpressure can be built up, leading to the reflux of flushing liquid and thus leads to flushing of the aspiration opening.
It goes without saying that such a flushing of the aspiration opening must not happen in the anterior chamber of an eye to be operated on, but that for this purpose the double-lumen cannula from the anterior chamber of such an eye removed and must be reinserted after flushing.
Another advantageous embodiment of the invention is through a central control unit for controlling various device functions, which is preferably a foot switch for switching on the pump motor and to reverse the direction of rotation of the rotary piston of the pump.
In addition, in the apparatus according to the invention, the feed line for supplying irrigation fluid to an operating field with an actuatable Shut-off valve be provided by means of the said central Control unit can be actuated between its closed and open position.
The shut-off valve for the supply line for the supply of rinsing liquid can also be a dependence on switching the pump on and off in its opening or. Be in the closed position actuated solenoid valve. To that extent it has proven to be particularly easy Valve training proved when a section of tubing is used to feed the Rinsing liquid by means of a between a blocking position and an unlocking position actuatable organ can be squeezed.
In the accompanying drawing is an embodiment of the invention Apparatus shown schematically, which will be explained below.
The suction apparatus designated in its entirety by 10 of lens residues during cataract operations is known with a so-called hose pump 11 Type equipped, in a manner to be explained in more detail below to generate a for the suction of lens residues from the anterior chamber of an eye to be operated on necessary vacuum is used. This hose coupling has a motor-driven one Rotary piston 12 with two in the circumferential direction by i800 against each other offset arranged cams 13, which extend over a circumferential area of approximately 1800 around the piston extending hose section 14 when rotating by pressing Squeeze it against a radial peripheral limit 15 of the pump and do so in the direction of rotation of the piston located in front of the respective pinch point volume of this hose section convey further in the circumferential direction. The drive motor for the rotary piston 12 of the Pump sit in a known manner by means of a potentiometer 16 at different speeds adjustable, which is equivalent to adjusting the flow rate of the pump is.
The one around the rotary piston 12 of the pump 11 over a circumferential area hose section 14 extending around 1,800 leads on the one hand over a line 17 to a collecting container 18 for receiving the pumped liquid and is on the other hand via a pressure cell 20 with a further hose line 21 in connection with a handpiece which is not of particular interest here 22 with one through an incision at the edge of the cornea of an eye to be operated on double cannula 23 which can be inserted into the anterior chamber thereof.
A lumen of this double cannula 23 is connected to the hose line 21, which leads to said pressure cell 20. The pressure cell is by means of an im individual adjusting means 25-auf, which are not of interest here with regard to its structure different negative pressures can be set and the pressure cell is assigned a limit switch that responds when the vacuum that has been set is exceeded 26 assigned to the drive motor of the pump 11 via a control line 97 acts and stops it when the occurring vacuum is greater than the set one WeXt will.
Furthermore, srzh extends from a rinsing liquid 30 in which it is around a conventional infusion bottle attached at a predetermined height acts, a hose line 31 to the handpiece 22, which with the second lumen the double cannula 23 of the handpiece is connected and for supplying rinsing liquid serves to the surgical field. This hose line 31 is via a between one Closed and an open position actuatable solenoid valve 32 out.
A foot switch 35 designed as a foot switch is used to operate the apparatus central control unit.
If the apparatus 10 is activated by pressing a key which is of no further interest here Main switch is ready for operation, is activated by pressing several switching stages equipped foot switch in a first switching stage initially that in the supply line 31 between the irrigation liquid source 30 and the handpiece 22 arranged solenoid valve 32, which is in the closed position when the device is switched off is switched to its open position via a control line 36. In order to the supply of rinsing liquid to the double cannula 23 of the handpiece 22 is released.
The actuation of the foot switch 35 sets in a second switching stage by the drive motor for the pump 11 via a control line 37 and a delay element 38 in operation. The delay element causes the rotary piston to start up gradually 12 and ensures that the set speed only after a predetermined Time span of, for example, five to eight seconds is reached.
As a result, it builds up over the lumen intended for suction the double cannula 23 of the handpiece leading hose line 21 that the suction process causing Vacuum gradually on. The full suction power is only achieved after the predetermined Duration reached. By pressing the foot switch 35 in a further switch level, the direction of rotation of the pump motor is reversible with the result that on the actual suction side of the pump now a corresponding to the pump output Overpressure occurs.
If the surgeon uses the double cannula 23 of the Handpiece 22 in a manner not of further interest here through an incision on The edge of the cornea of an eye to be operated on is inserted into its anterior chamber has, after the apparatus 10 has been made ready for operation beforehand was by the aforementioned pressing of the foot switch 35 in its first switching stage the supply of rinsing liquid to the handpiece 22 through the solenoid valve 32 is released. As a result of pressing the foot switch to the next switching stage, in which the solenoid valve 32 remains in its open position, the pump 11 starts up and it is gradually Vacuum built up. Accordingly, that of the irrigation fluid source 30 becomes the anterior chamber of rinsing liquid supplied to the eye to be operated on together with Lens debris through an aspiration opening in the corresponding lumen of the double cannula 23 sucked off. The vacuum that occurs is the same as that set on the pressure cell 32 Limited negative pressure. Falling below this negative pressure, as it is for example can occur in the case of blockages at the aspiration opening of the double cannula a shutdown of the pump 11 by the limit switch 26. So much for the shutdown the pump and a slight reflux of the aspirated rinsing liquid that occurs in the process the blockage cannot be removed, the double cannula must be removed from the anterior chamber of the eye to be operated on are removed, whereupon by appropriate actuation of the foot switch the direction of rotation of the pump and accordingly its suction and Can be reversed. With such an actuation it builds up the aspiration opening of the double cannula a corresponding overpressure, which in practically all possible use cases for flushing the clogged Aspiration opening leads. The double cannula can then be returned to the anterior chamber of the eye to be operated on and the suction of those charged with lens debris Rinsing liquid continued in the manner explained above.
