The present invention relates to a surgical Instrument, especially rectus or trocar with one Entry area, on which there may be a ring collar connects.
Surgical instruments of this type, in particular trocars or rectuscopes are in a wide variety of shapes and designs known and in use on the market. They serve in primarily to intervene in a human Allow body. For example, a trocar is made by introduced an interface into the human body. A rectus is used for anal, especially rectal Introducing into the human rectum. Because of this surgical instrument can then, for example Hemorrhoid surgery, punctures or other such Treatments are made.
A disadvantage of such surgical instruments is that on the one hand an obturator for rectal insertion must be used, for example, for the rectum or the inner area of the intestine or body is not get hurt.
Another disadvantage is that the elongated and in Cross-section of a constantly formed sleeve of the trocar Operation in the side areas is only possible to a limited extent is.
The present invention is based on the object surgical instrument, especially rectus or To create trocar with which to easily Operate especially in lateral areas is relieved.
Furthermore, the manageability of such surgical instrument, especially the insertion be relieved in the human body.
The insertion area leads to the solution of this task is at least partially expandable.
In the present invention, the sleeve is preferred a plurality of slats movable, in particular hinged assigned, which after insertion of the Trocares or rectus in the human body can create an extension.
The individual slats are articulated with the sleeve connected. At least one is preferred within the sleeve Thrust element, in particular associated with a thrust sleeve, which is connected to the Sleeve and thus also connected to the slats.
By moving the thrust element relative to the sleeve or the sleeve opposite the thrust element can Slats outwards from a tubular starting position fold or swivel. Moving the thrust element, especially the thrust sleeve relative to the sleeve happens by rotating an annular disc, which is axially in the Ring collar of the sleeve is fixed by means of an annular groove. It preferably engages within the ring collar lying ring groove of the circular washer a pin element of Ring collar and axially fixes the circular washer.
A thread is provided inside the circular washer, which fits into a suitable thread of the Thrust element or the thrust sleeve engages. By The thrust element can be rotated move back and forth against the sleeve. About the Connection element to which articulated between the thrust sleeve and the sleeve arranged at the end a plurality of lamellae and on which the slats are fixed the slats move and swivel. Because of this The connecting element can be pivoted the individual slats attached to it open or close.
It is particularly advantageous when operating if the lamellae of the surgical instrument in the body Let it open so that the surgeon in the body more Space, also by expanding the side areas, especially in rectal areas.
In this way, larger interiors can be accessed Extending the sleeve through the lamellae produce what when Operation is an advantage.
It can also be advantageous to use individual areas arranged slats to pivot outwards design, so that, for example, an expansion to only one certain direction, for example quarter or semicircular or three-quarter circle is possible. This is also intended to be encompassed by the present inventive concept be.
Furthermore, should also be within the scope of the present invention that the slats are hemispherical in one Insertion area are arched for insertion to lighten in a human body. Also supposed to be remembered the slats in different Number and in different lengths of the sleeve assign.
Overall, the present invention is a surgical instrument, especially trocar or Rectuscope created with which an insertion easier and which is easily expandable in the body and can be closed after the procedure. It can be done by the corresponding arrangement of thrust element and sleeve each any setting of the extension using the Fix the circular washer. Folding in the slats without It is not possible to turn the circular disc.

Fig. 8 shows a partial longitudinal section at least partially illustrated in accordance with the inventive surgical instrument Fig. 1 taken along line VIII-VIII.
As shown in FIG. 1 has an inventive surgical instrument R a sleeve 1, which extends at one end an annular collar 2 and at the other to connect a plurality of lamellae 3. The lamellae 3 are connected to the sleeve 1 in a joint-like manner, in particular foldable outwards, via a plurality of joints 4 .
The sleeve 1 is also assigned a holding element 5, preferably close to the ring collar 2 . It can be gripped by a human hand. Connected to the ring collar 2 is an annular disk 6 , which is mounted within the ring collar 2 .
In FIG. 2 a position of use of the surgical instrument R is shown, in which the fins 3 to the outside are opened.
In Fig. 3, the sleeve 1 is shown with subsequent lamella 3 . The lamella 3 is preferably slightly curved, as can be seen in particular in FIG. 2, so that overall a sleeve-like insertion region 7 is created by a plurality of lamellae 3 and the sleeve 1 . In FIG. 1, the insertion area 7 is closed and, in the position of use in FIG. 2, is expanded or opened accordingly by the opened slats 3 .
The joints 4 connect, as shown in particular in FIGS. 3 and 4, the lamella 3 with the connecting element 8 . The connecting element 8 connects movable sleeve 1 with the pusher member. 9 The thrust element 9 is preferably designed as a thrust sleeve 10 and can be displaced therein relative to the sleeve 1 , as shown in particular in FIG. 5. Via individual web elements 11 , which are placed on the thrust element 9 , in particular on the thrust sleeve 10 , the connecting element 8 can be moved up and down accordingly in the double arrow direction Y shown, which correspondingly pivots the slat 3 around the joint 4 of the double arrow direction Z shown.
By moving the thrust sleeve 10 downward relative to the sleeve 1 , the at least one lamella 3 can be pivoted inwards. The connecting element 8 is preferably oval. However, it can take other forms. The lamella 3 of the sleeve 1 preferably adjoins at the end. Due to the fact that the lamella 3 is slightly curved, a sleeve 1 can be produced over a plurality of lamellae 3 distributed over the circumference of the sleeve 1 .
In a further exemplary embodiment of the present invention according to FIG. 7, a surgical instrument R 1 is shown which essentially has the components corresponding to the surgical instrument R according to FIGS. 1 and 2. The difference here is that the lamellae 3 are curved in a hemispherical manner in an end insertion region 7 . As a result, an obturator can be omitted when inserting the surgical instrument rectally or, for example, when operating through an interface in the human body for inserting the rectus scope. It is then important that the lamellae 3 are rounded, in particular at the end thereof, so as not to cause any injuries in the body when opened into a position of use by rotating the annular disc 6 .
The functioning of the displacement of the thrust sleeve 10, which is preferably arranged on the inside, with respect to the outer sleeve 1, takes place by radial rotation of the annular disk 6 , which is held within the ring collar 2 via an annular groove 11 assigned to the annular disk 6 on the sleeve 1 or on the annular collar 2 . The holding takes place by means of pin elements 12 which engage in the annular groove 11 of the annular disc 6 and which can be designed, for example, as grub screws. The pin elements 12 allow the annular disk 6 to be rotated radially. On the inside, the circular disk 6 is provided with a thread 13.1 , which engages in a suitable thread 13.2 of the thrust sleeve 10 . By radially rotating the annular disc 6 , which is axially fixed in the annular collar 2 , the thrust sleeve 10 or the thrust element 9 can be moved back and forth in the double arrow direction Y shown. By moving it back and forth, the individual slats 3 can be opened and closed.
From the present inventive idea should also be included that the sleeve is slidably arranged with respect to a push sleeve 10 1, for example by rotation of the annular disc. 6
It should also be within the scope of the present invention that only partially lamellae 3 are assigned to the sleeve 1 . In this way, for example, only a quarter of the cylinder element of the sleeve 1 or of the insertion area 7 can be expanded to the outside. This is an advantage if an operation in the body only needs to be carried out in this area. The remaining lamellae 3 can then be designed like sleeves and rigid, not pivotable. Several variants are conceivable here. It is also conceivable that different opening widths of individual lamellae 3 result from each other due to different mounting of the lamellae in order to make certain areas in the body more accessible. This is beneficial for certain operations. The individual lamellae 3 can also be formed in a wide variety of lengths, even among one another, if this is desired.
