Abstract
Description
B. Li⁺, Na⁺, K⁺, Ca2+, Mg2+, Cl⁻, HCO₃⁻, NH₃, usw.) Sauerstoff, Kohlendioxid oder metabolische Faktoren, wie Glucose, Lactat, Harnstoff (BUN), Harnsäure, Kreatinin, Pyruvat, Ascorbin, Phosphat, Cholesterol, Triglyceride, Phenylalannin, Bilirubin, Thyrosin, Protein sowie für die Enzyme, wie Lactatdehydrogenase, Creatinkinase (CK), Aspartat- Amino-Transferase (ASAT), alkalische und saure Phosphatase, Lipase, Amylase, Cholinesterase, etc., mit Hilfe eines speziellen, akkubetriebenen Handgerätes vermessen wird. The invention relates to a particularly simple and contamination-free sampling and automated, portable measuring device, preferably of body fluids, in particular of blood, by means of a method which includes integrated sample preparation, sensor calibration, validation and quality assurance. The blood is transferred in the shortest possible way with the exclusion of air into a sampling and measuring cartridge according to the invention with integrated chemo and biosensors and calibration standards between an inexpensive, commercially available hypodermic needle, or arterial catheter and syringe, where it is used for the most common blood parameters, such as Electrolytes (e.g. Li⁺, Na⁺, K⁺, Ca 2+ , Mg 2+ , Cl⁻, HCO₃⁻, NH₃, etc.) oxygen, carbon dioxide or metabolic factors such as glucose, lactate, urea (BUN) , Uric acid, creatinine, pyruvate, ascorbin, phosphate, cholesterol, triglycerides, phenylalannine, bilirubin, thyrosine, protein and for the enzymes, such as lactate dehydrogenase, creatine kinase (CK), aspartate amino transferase (ASAT), alkaline and acidic phosphatase, lip , Amylase, cholinesterase, etc., is measured using a special, battery-operated handheld device.
Die Erfindung löst die Probleme aller herkömmlichen invasiven oder minimalinvasiven Blut-Analysen, die darin bestehen, daßThe invention solves the problems of all conventional invasive or minimally invasive Blood analyzes that consist of that

The state of the art in the field of blood analysis is venous or arterial Acceptance and a transfer into prepared special containers, whereby for Coagulation inhibition is usually still a small one in the syringe or in the sample container Amount of heparin is added. Because this sampling method is not under Exclusion of air is carried out, the content of dissolved gases here especially oxygen and carbon dioxide in the process or during transport to the laboratory change. Again, this may be due to related, chemical Balances for changes in other important parameters, e.g. B. the pH Value that lead to protein ion binding capacity. Can also be metabolic Processes in the sampling vessels change the parameters to be measured, e.g. B. Breakdown of glucose etc. - The clinical chemical devices are often so large, complex and expensive that only larger companies can afford such central laboratories can.
In contrast, the general practitioner very often needs a quick and safe information about the clinical-chemical parameters, if necessary serious danger of being able to intervene in time.
Especially in the field of emergency medicine have been great in recent years Progress made. The acceleration of the blood analysis was particularly important here through the electrochemical methods of electrolyte determination with the help of corresponding table machines that contain ion-selective electrodes. Although such devices are always immediately ready for operation, the ones to be analyzed Blood samples will still be sent to an (emergency) laboratory because these devices only can be operated stationary and the effort for the necessary Quality assurance by qualified personnel and a corresponding amount of work required.
Target the latest trends in clinical chemistry and laboratory medicine aims to bring the so-called points of care closer to the sick, d. H. to the bedside bring. In this context, the so-called bedside monitoring wins. H. the Determining important parameters right next to the patient, very important. The great demand for cost reduction, higher sample throughput, larger Decentralization, and have reduced demands on the training of staff worldwide research and development in this field easily motivated using, inexpensive, reliable and portable devices for bedside measurements develop.
An example of this new and optimal medical diagnosis is in this Connected the development of the first portable measuring device to the fast Determination of blood electrolytes and other substances by the US company I-Stat. The The handheld meter in question represents the top of the developments that the above try to meet the requirements mentioned. It is mostly based on electrochemical measuring methods with miniaturized measuring electrodes and on the US Patents 5,063,081, 5,096,669 and 5,112,455. U.S. Patent 5,096,669 describes a disposable device for real-time liquid analysis that is integrated into a Handheld device for reading the data must be inserted. A disadvantage of this flat built-up measuring cartridge is the sample task. For a blood analysis, one must certain amount of blood are placed in a small receiving trough, of which the Sample is then transported into the measuring section by capillary forces after a small one Snap lid has been closed. With a little awkwardness, blood can be splashed what u. U. with infectious blood at risk to Operator leads. It also leads to an undesirable one Equilibration with the ambient air, which means the oxygen, bicarbonate and pH Value determination makes inaccurate. With the I-Stat system, the sample cartridge then plugged into the handheld meter, where the corresponding ones are automatically Cartridge calibration solutions are released by external pressure. Around let the blood sample and the calibration solutions flow past the sensors, The next step from the handheld device must also be in the cartridge Existing compressed air chamber can be opened, the solutions by means of overpressure presses through the measuring channels. After the actual measurement, d. H. Reading the sensor Signals, the cartridge is thrown away. The structure of the cartridge is because of this complex construction not easy and the manufacture not inexpensive, which is why cannot be offered under a few dollars. This is the cost of this On-site clinical analysis significantly higher than for traditional laboratory analysis using corresponding large machines. I-Stat are also responsible for the design analysis of various materials to dispose of what extra money can cost. For cost reasons alone, this system can only be extremely extreme Enforce emergencies. Technologically, the cartridge with the integrated chemical and biosensors constructed far too complex and inconsistent, so that system-related measurement errors cannot be excluded.
Auch ist bekannt, daß derartig miniaturisierte Dünnfilm-Sensoren sehr anfällig gegenüber Temperaturschwankungen, Verschmutzungen etc. reagieren.US Patent 5,063,081 to I-Stat describes the manufacture of some of the im The above-mentioned handheld meter used sensors using the techniques of Microfabrication. The thin-film technology used with photo-lithography and vapor deposition processes working under vacuum is not cheap and is only worthwhile very large quantities, which are not yet available in the case of bedside measurement. It is also known that such miniaturized thin film sensors are very susceptible react to temperature fluctuations, contamination, etc.
US Patent 5,112,455 describes those in the above-mentioned handheld meter applied method of measured value evaluation, which in the present case the Development of the company I-Stat absolutely different than usual (= wait until more stable Measured value has to be set). The reason is that in the above mentioned sample cartridge micro sensors are used, there dry, d. H. are installed without liquid contact and are thus stored and delivered. At the first contact of sample or calibration liquid with the dry sensors the latter penetrates the sensor membrane to the inner discharge electrode and builds the measuring signal slowly increases. This soaking process takes until the setting all thermodynamic equilibriums for quite a long time (a few minutes). Therefore the patent describes a chemometric extrapolation method from which the final analyte-proportional measurement signal emerges after only a few seconds. However, this method is less reliable than the real equilibrium data used for evaluation because a different, sample matrix-dependent Diffusion behavior of the potential determining substances is not taken into account. Also the response time of an ion-selective electrode is known to depend on the number and Concentration of interfering ions. This cannot be standardized. Therefore, in practice all electrochemical measurements usually only under equilibrium or Steady state conditions performed. When analyzing blood, it can also depending on the individual coagulation behavior, to protein deposits on the Sensor surfaces come using the chemometric extrapolation method cannot be recorded, which leads to incorrect measurements. Also in the evaluation facility a complicated time-controlled process of data acquisition and evaluation to perform, which further increases the price. For all of these reasons, this could Development of the company I-Stat on the market has not yet been implemented. That too is Disposal of the measuring cartridge due to the variety of different materials not easy.
An alternative electrochemical measuring method is known from US Pat. No. 5,114,859 known, in which the sensor is in a Calibration solution is located and to measure this chamber through a close leaves the sealing opening to come into contact with the sample. Kick here of course the above-mentioned undesirable drift phenomena when used dry sensors. Here is a precise mechanical movement of the measuring Sensors in or through the individual calibration solutions required. Big problem this version results in the necessary tight seal between the individual liquid compartments that are "scanned" with the sensor. There the analyte-sensitive zone of the sensors must be arranged laterally, that rubs Sealing material over the sensitive measuring zone and leads to unwanted Artifacts. For example, the finest grinding marks lead to the surface ion-selective membranes for a correspondingly longer response time and / or destruction of an important one required for the correct measurement Surface layer. Another feature of this patent is that with Calibration solution compartments complicated, sensor-carrying blood collection Syringe is placed in a tabletop device for evaluation. This takes over mechanical relative transport of sensor and measuring compartment. The patent describes only sensors of conventional, too expensive technology for a one-time Items. Disposal is not easy.
From US Pat. No. 5,145,565, another blood collection cartridge is known also between a needle and a syringe body using standardized Luer Couplings can be installed. This cartridge contains in separate Compartments also all calibration solutions that are used for the later measurement suitable sensors are required, all solutions using absorbent, inert Carrier materials are solidified in these chambers. This is important for the measurement. In There are no sensors in this blood sampling cartridge. The latter are of them separated and can also be used multiple times. The individual sensors are in a suitable, further arrangement, which is then later on the one side opened cartridge is moved. They are in electrical contact with you larger tabletop unit that contains the electronics. So this is not a automatic hand-held measuring device for bedside measurements. The disadvantage here is that the individual cartridges from the patient on their way to electrochemical readers can be interchanged. Here, too Sensor surface by means of a "cleaning" zone, which is a transfer of sample in to prevent the individual calibration solutions and vice versa. One thing is certain also that potential adulterating protein deposits are not removed in this way can. Electrode poisons can thus be transferred from one sample to the next. Another disadvantage is the complicated structure of the individual cartridge versions stands in the way of inexpensive mass production. In no case can Device operated in only one hand.
From the German patent DE 30 46 016 C2 is a complete automatic Known analyzer with a pipette. Corresponding microsensors are used here Suction channel of a pipette housed so that when pipetting out of a Storage vessel at the same time the sensor analysis can be carried out. The sensor-equipped pipette suction tip is open at the bottom and must of course for the purpose the calibration can be immersed in appropriate solutions. That is why this machine is very complicated and complex. He can hardly become a handheld device be miniaturized. He also needs one in a suitable container collected sample. It does not combine sampling with a measurement.
From Austrian Patent No. 376 117 is a liquid and / or Gas test device is known that a blood analysis using sensors performs that in the sampling syringe itself at different locations are installed. This usually increases the cost of production very inexpensive syringes considerably. They must be delivered sterile. Also contains this patent is not a preferred method of measurement, which involves calibration and measurement would simplify. The disposal of these complicated syringes with integrated electrical connections are also a problem. You cannot do them as with Clinic rubbish usual to burn. The cable connection between the syringe and the measuring device requires laborious handling.
From a European patent application 0 317 847 A1, which has since been withdrawn a sensor-equipped cartridge is known, which is also between the syringe and the needle can be positioned and which is connected to an electronic measuring and evaluation device is in electrical contact. This is also missing here Description of a suitable and reliable measuring method, which the The likelihood of measurement errors is greatly reduced and unskilled personnel enables. For example, there are no indications in the disclosure that how to calibrate the sensors. Any calibration with additional standard solutions to be sucked in and or other additional operations complicates the on-site analysis, is prone to errors and also requires evaluations Arithmetic operations. It is also disturbing that the electrical contact of this cartridge the freedom of movement of the Operating personnel are restricted. The safe positioning of the entire arrangement during the measuring process is also not described. An exposed syringe is dangerous and can be dangerous if the material is infectious. The sensor manufacturing technology according to the invention with miniaturized versions of the traditional, hand-made macro electrodes also do not provide inexpensive ones Device that is thrown away after each measurement for safety reasons can. The return according to the invention is also one with this adapter Blood sample measured in the body is extremely problematic because of the blood in it Contact coming sensor membranes contain materials that are highly toxic.
European patent application 0 399 227 A1 describes an almost identical one Arrangement as before with the difference that here in a measuring assembly retractable, planar, electrochemical measuring sensors based on the High temperature thick film technology (ceramics) are used and this after each Discarded use. Here the multi-analyte calibration solution is either in separate, gas-tight bottles included or only one is already there blood sampling in the syringe. Since the usual medical syringes are not these syringes must consist of a material that is impermeable to gas in the long term get a special, gastight cover. So you can't get cheap ones Use routine syringes, which makes the analysis very expensive. Also lets through the one-point calibration provided here is not a sensor malfunction determine. Fiddling with this device is therefore missing automatic calibration only possible for an expert. The handling is also very complicated; the operator has it in one hand Handheld meter and in the other the syringe with the sensor assembly, which with one thick, shielded cables are connected to each other. There cannot be any more Operations such as calibration, rinsing, measuring, ect. do more. Besides, is known that micro-reference electrodes are not very reliable. One out Functional control necessary for safety reasons is given in this European Patent application not disclosed. The sensor arrays are ceramic-based because of their metal content for the conductor tracks, not together with the other Dispose of hospital waste by incineration.
The German patent application describes a further development in this area DE 44 27 725. This saves the injection of the blood sample to be measured into one suitable handheld measuring device in that all necessary for the blood analysis Measuring electrodes are placed in the sampling syringe. Before the blood test the measuring sensors are located behind or between two Syringe plunger in a suitable calibration and conditioning solution. At the Drawing the final blood sample into the syringe will displace the syringe or syringes. Flask the calibration solution and replace the calibration solution with the blood, being a soft piston rim frees the sensor surfaces from the calibration solution. Disadvantageous This innovation involves the integration of the individual, mostly planar sensors in the usually cylindrical piston wall of the syringe, which is the necessary vacuum-tight piston guide difficult. Also before using this Device for drawing blood from the space behind the syringe plunger filled with the calibration solution, which requires a special production. In addition must have the necessary heparin units before the blood is drawn before the Syringe plunger should be given, avoiding air bubbles. This structurally significant problems that u. U. only by a rectangular Syringe assembly are to be solved. Such syringes have so far been used in medical technology Not known, will likely have sealing problems and because of the Special production can only be too expensive for this application. The problem of destruction of potential-determining (measurement-influencing) surface layers through the Displacement of the plunger remains unresolved here too.
The present invention solves all of the prior art problems mentioned above especially those of operational safety, reliability, correctness, handling, Constriction due to cable connections, costs and disposal because all miniaturized sensors required for blood analysis in a suitable Device (measuring cartridge or module) between commercially available syringe bodies and Needle can be used that according to the invention the cheapest planar sensor type, which is printed on paper, can be used for calibration and quality assurance function control is done automatically, so that you really from can speak a process that runs in one hand. The inventive sensor-equipped, pre-pluggable hand analysis arrangement, preferably for blood two standardized Luer plug connection ports, one for a tight connection of the typical, uncritical and therefore inexpensive blood collection syringe and one second opposite for attaching the hypodermic needle or Connection to an arterial catheter. The measuring cartridge that is so easy to attach between needle and syringe can be used in the sense of an ergonomic handling in the outer shape to be adjusted to the diameter of the syringe body, because the external shape of the blood measuring and collecting device is irrelevant. The individual chemical or biochemical sensors for the most important Blood parameters are easy to dispose of, miniaturized and are directly electrical Connection to contacts on the outer surface of the measuring module. The corresponding counter contacts for electrical connection, for operation amperometric and impedimetric sensors and for reading the According to the invention, the relevant sensor signal is in an associated one special handheld meter. Through a suitable, controlled insertion technique, for example by a nipple in the measuring module and a corresponding recess The sensor-specific sensors can be located in the appropriate opening in the handheld measuring device Close the contacts of the individual sensors precisely and reliably for the measurement.
The type of chemical and biosensors integrated in the measuring cartridge is irrelevant. In principle, they can also be optical, especially if they are based on the integrated optics. Preferably, however, electrochemical microsensors are used uniformly, which are "printed" on paper-like carriers using a particularly inexpensive technology (double-matrix membrane type). The pH value, pCO₂ value and the electrolytes Li⁺, Na⁺, K⁺, Ca 2+ , Mg 2+ , Cl⁻, and others are detected potentiometrically with suitable ion-selective electrodes, oxygen, glucose, lactate, etc. are preferred determined with amperometric biosensors.
The individual measuring sensors are according to the invention in the measuring cartridge from one injection-moldable or otherwise mass-producible plastic type, preferably in Series positions and are inferior with their analyte-sensitive measuring surface appropriate calibration in direct contact with the one drawn by means of the syringe Sample. The measuring channel between the two Luer connectors can be of any shape and Have dimensions. It can be straight or with a large number of sensors meandering. A particularly preferred embodiment results if the circular cross section at the entrance and exit is in the area of the Sensor measuring zone is rectangular and linear, because then it is production-related only a precisely fitting multi-sensor strip from a suitable carrier (e.g. Paper or plastic film) must be inserted and glued in such a way that the individual measuring sensors in this strip in the later by joining the Module parts emerging measurement channel come to rest, while also in this simple double matrix membrane technology manufactured planar sensor leads (= Signal line) this automatically leads to the edge of the Measurement module are performed. There you can find counter contacts in the handheld measuring device be contacted. This connection to the contact exists for disposal reasons According to the invention from a non-metallic electron conductor such. B. graphite or be made of a conductive polymer.
In terms of production technology, it comes about because of the sealing problem that is not necessary here on the exact fit of the planar sensors in the sample (blood) channel not what the manufacture is significantly simplified. The individual sensors work potentiometric, impedimetric (alternating current at different frequencies) or amperometric according to the known basic structures. A preferred and Particularly inexpensive sensor production form is that of screen printing technology (here Low-temperature thick film technology) or that of automatic printing of flow or filter paper or materials with similar properties using a dispenser (see DE 41 37 261). Here it could be shown that the batch scatter is so small are that you can get by with a single point calibration.
The simply in the place of later by merging the two partial cartridges The resulting multi-sensor test strips to be glued in are provided by the construction requires contact lines to the outer edge of the measuring module. But it can according to the invention also redox systems and / or cavities filled with electrolyte gel can be switched in between. The latter are shaped accordingly Wells in the measuring channel can also be easily manufactured using injection molding technology. By the inclusion of a traditional electrode structure with inner potential Leakage electrolytes are particularly temperature-insensitive according to the invention Sensors created. The measuring cartridge according to the invention is expediently produced in two matching halves. Then the accessibility to the Given measurement channel. According to the invention, there is also the possibility of both halves to be provided with sensors so that the individual measuring sensors later on after Join the halves together and also glue opposite. Let it through easily accommodate 4 to 10 of these inexpensive sensors per cm.
According to the invention, however, they are also similar for micro measuring modules for micro blood samples simple and inexpensive, based on silicon technology (e.g. containment sensors DE 41 15 414) mass-producible sensors that are in corresponding recesses the planar measuring channels can be pressed in and glued. With appropriate In this case, micro sensors can also be used if the number of items is high Measuring channel can be designed in Si technology. With this version you can accommodate significantly more sensors per measuring channel. The single ones Microsensors are connected to the relevant connection on the back External contacts brought into contact. With minimally invasive sampling (e.g. in infants) the Si technological solution is preferred because only here a blood volume of far below 1 mL is sufficient for reliable measurement.
According to the invention, all potentiometric measuring electrodes can have one and the same micro reference electrode can be measured, or each with its own own, while for the amperometric sensors other, less polarizable be used. Alternatively, however, the Syringe measuring cartridges arrangement according to the invention in the handheld meter Pierce a septum and behind it into a gel-solidified or Unconsolidated electricity key electrolytes from a handheld device Macro reference electrode penetrate, which also makes a particularly stable, even easily current-carrying reference potential arises, which means that the Sensors integrated reference electrodes checked for their proper function what amounts to a quality assurance measure.
According to the invention, the measurement process in the blood analysis is thereby automatic started that when closing this electrolytic connection between the measuring channel in the cartridge and external reference electrode a resistance measuring circuit with a Contact zone in the sensor area is closed (drastic reduction in resistance). According to the offset voltage between the micro reference electrodes in the Measuring module that do not have a large current key due to the design can, and the macro reference electrode in the handheld meter to a few microvolts exactly known. This is taken into account in the microprocessor evaluation.
According to the invention, instead of potentiometric or amperometric Sensors (chemo and biosensors) can also be used impedimetric. At the latter uses the complex AC resistance level to determine the salary evaluated higher frequencies. You need next to a very thin one analyte-selective membrane only two electron-conducting micro-lead electrodes and no potential constant reference electrodes and are also extremely inexpensive planar producible.
According to the invention, all of the above can be described in the description of the prior art mentioned calibration, handling and function control problems be solved that in the measuring cartridge next to the measuring chamber (measuring channel) more interconnected chambers or compartments and at least one Transport channel for the sample to be taken, the contents of which are automatically moves past the sensors according to the inventive method. The chambers can be manufactured in one or both cartridges Installation halves must be embedded. One of these chambers, which is a much larger one Volume than the measuring channel (e.g. 3 mL versus 300 µL) according to the invention at least two pistons which are easily displaceable by pressure and which Separate defined multi-analyte calibration solutions from one another by a outer syringe plunger caused air pressure (for example) or by the sample one after the other (one after the other) in contact with the sensors, which is a results in much more reliable two-point calibration. The latter demands from the Biosensors much simpler design conditions since the slope of the Calibration lines need not be known, which in turn makes it easier to use inexpensive, amperometric, single-use sensors on paper basis. For the Potentiometric chemical sensors result from a two-point calibration a much higher accuracy with longer durability, because the Nernst Steepness is also determined.
In a further embodiment of the invention, it is also possible Calibration solution in two different, separate chambers in one or insert both cartridge halves, which are sealed by gas-tight films tear corresponding pressure, are sealed. Another, not mandatory necessary chamber can contain an adsorber material and as a collecting or Serve transport containers for the measured solutions.
The measuring cartridge according to the invention has in front of the two standardized Luer plug-in Connection ports for tight connection of an inexpensive, commercially available syringe and hypodermic needle a simple, flexible fluid control (Slide valves) through which the individual chambers with the measuring channel or according to the requirements of the process concerned, with one another or with the needle and the syringe can be connected.
The inventive method of reliable bedside measurement of the most important Blood parameters using an automatically measuring hand-held device are done in an execution version by sucking the blood into the syringe, what by corresponding, delivered position of the fluidic control is reached. To do that drawing blood volume - matched to the measuring cartridge - can keep constant the syringe plunger according to the invention up to a stop or the maximum Syringe volumes are moved. Thereafter, according to the invention, the complete Syringe arrangement with needle and measuring cartridge in an opening provided of the handheld measuring device, wherein according to the invention the fluidics control automatic fitting pressure is switched so that the two first Calibration solutions can come into contact with the sensors when the syringe plunger is moved in the direction of emptying. In addition, one for the external reference electrode necessary electrolytic connection between Measuring chamber and needle created. The automatic measuring process is thereby started when a perfect electrolytic connection has been established. The double matrix membrane sensors according to the invention usually supply it Equilibrium potential in a few seconds. The syringe plunger will according to the invention by means of an integrated, preferably spring, Clamping mechanism, pressed into the syringe under constant pressure so that enough time for the formation and acquisition of the measurement signals under the influence of Calibration solutions remain. Instead of this energy-saving spring drive when using rechargeable power batteries, as known from drilling machines are, also an actuator the push rod of the syringe plunger into the desired Move direction. According to the various calibration and conditioning solutions the sample (preferably blood) reaches the sensors and can be analyzed accordingly will. Then, like the calibration solutions, it is placed in the collecting or Transport chamber of the measuring cartridge directed. You can because of the well known and minimal dilution with the known calibration solutions also for determination other parameters can be transported to a laboratory.
In a further preferred embodiment according to the invention, the Measuring cartridge already manufactured by the manufacturer with a suitable one for all sensors common, as sterile calibration solution as possible. These are the inputs and Output ports with a gas-tight, only immediately before taking blood sealing foil and the entire measuring module is also located still sealed in a gas-tight, metallized plastic film. Before the According to the invention, the actual blood withdrawal is performed for a few seconds placed in the associated handheld measuring device, where the data for an accurate Single point calibration can be saved for later blood measurement. The technical The maturity of the planar double matrix membrane sensors is due to the Degree of automation so high that the specimen spreading areas are well known and Allow one-point calibrations (constant calibration curve slope). Once a Sensor leaves these areas, the handheld device can indicate which parameters u. U. could be less reliable. This results in an automatic Quality control. After that, the gas-tight films at the two Luer ends the cartridge removed, the needle and syringe connected and the sample up to a certain volume that is large compared to the volume of the measuring channel, drawn. The calibration solution is taken from the small measuring channel (approx. 0.3 mL) displaces and mixes with the sample in the syringe plunger. Then the whole Arrangement, as in the first version of the invention more or less with the Place the needle pointing downwards (air bubble on top) in the handheld meter. Here the sample-filled needle pierces a septum behind which the Current key electrolyte is an external macro reference electrode. At the same time is a fluidic control behind the needle Luer port so by adjusting pressure changed that a diaphragm the electrolytic contact of the measuring channel with the maintains an external reference electrode, but the sample at one Syringe plunger movement in the emptying direction in a collecting or Transport chamber is transported. Software housed in the handheld meter then compares the reference electrode potentials of the micro sensors with that of the external macro reference electrode where the prescribed tolerance limits are not may be exceeded. Then the sample measurement signals are acquired and the measured values are calculated in accordance with the previous calibration. After that the syringe plunger automatically moves in the direction of emptying and the sample / Standard mixture from the syringe displaces the pure sample in the narrow Measuring channel. This mixture is also measured with the sensors and by the Software in the handheld device according to the well-known method of standard addition evaluated. By the automated second measurement according to the invention after Standard addition can suspect a faulty or unreliable measurement be confirmed or invalidated. Within given tolerance limits, can an average of both measured values is formed. Since the standard addition Evaluation of systematic measurement errors can be recognized immediately, this method provides with redundant measured values a drastically increased reliability. It is one quality assurance measure.
The total volume of the isotonic, physiological multi-analyte calibration solution, which is permanently included in the measuring module by the manufacturer is usually less than a milliliter. This calibration solution expediently contains according to the invention also the necessary amount of heparin, which clots blood prevents as well as substances that stabilize the reference electrode potential. The External reference electrode is designed to be used for amperometric measurements in the Micro amp range is current-carrying, this allows more complicated three electrodes Measuring arrangements with the associated potentiostats are omitted.
After a blood analysis has been carried out in accordance with the invention, the syringe and the syringe are located Needle in the same way as it was previously used for disposal. The measuring cartridge with the Blood sample can be disposed of separately, or for testing for more Parameters taking into account the dilution by the exact content known calibration solutions can be sent to the laboratory. At a incineration must be taken into account that this module at Use of non-metallic electron conductors cannot contain metal. With some Amperometric sensors may fall within the range of gold and silver a millionth of a gram. The membrane chemicals for the potentiometric micro Electrodes are in the microgram range and do not contain CFCs, so one of them possible combustion and no dioxin formation must be feared.
This simple procedure according to the invention thus solves all of the above Problems. The danger that the patient's blood to be measured will be released in an uncontrolled manner is or is irreversibly changed by contact with the air for too long is at the Invention excluded. It enables a safe, quasi-automatic bedside Monitoring with integrated quality assurance in the simplest way without certain additional calibration and control operations by the operating personnel severely restrict the use of a handheld device.
According to the measuring module is marked accordingly, so that the Handheld measuring device the respective sensor equipment and the necessary values of the Calibration solution when first inserted. This can be done by mechanical or electrical devices, but also by means of a simple one Barcode reader happen.
It goes without saying that a person skilled in the art will develop further versions of suitable fluidics Circuits to use the effect of simple and quasi-automatic two- Point calibration or standard addition.
Fig. 1 shows schematically a preferred embodiment of the measuring cartridge for blood sampling and analysis and the simple structure of two mounting halves.
FIG. 2 schematically outlines the individual process steps in calibration and measurement using the two-point calibration method.
FIG. 3 shows schematically an embodiment in which the multi-analyte calibration is already manufacturer is also stored solution in the measuring channel and is used as a standard addition solution.
Fig. 4 schematically shows a further embodiment for the method of the two-point calibration on calibration solutions which are separated by means of movable piston.
The two halves of the measuring cartridge ( 1 a) and ( 1 b) in Fig. 1 can be inexpensively manufactured in this shape with sufficient production tolerance from a suitable, easily disposed plastic material. For handling reasons, they should not be significantly larger than the syringe body when put together. The advantage here is that, because of the known design of the double matrix membrane sensors, it is not necessary to insert a metallic conductor that is difficult to dispose of into the plastic material. The strip ( 4 ) with any number of laminated paper strips ( 4 a... 4 z) preferably has a shape such that the contact surfaces extend into the recess ( 7 ) in the cover ( 1 b) when inserted into the correspondingly shaped housing and thereby form the contact surfaces ( 5 ).
In one half or in both (not shown), at least one, but preferably 3, storage chambers ( 8 , 9 , 10 ) are introduced in addition to the measuring channel ( 6 ) by injection molding or other shaping mass production processes, the latter having a volume of less than 10% of the of the chambers ( 8 , 9 , 10 ), while at least one further chamber, not shown, below the sensor level ( 4 ) is so large ( FIG. 1 not to scale) that it can accommodate all chamber volumes and the sample volume together.
The chambers ( 8 ) and ( 9 ) contain defined multi-analyte calibration solutions corresponding to the sensor analytes in a lower and upper concentration range, so that the important slope of the calibration line and its position can be reliably determined by a two-point calibration . If certain minimum criteria installed by software in the handheld measuring device are not met, an unreliability display is signaled for this parameter. For the purpose of conditioning and preservation, the calibration solutions also contain other substances and possibly also a sparingly soluble salt that remains on the walls during transport through the measuring channel and therefore also the sample with a constant ion concentration of an ion corresponding to the solubility product, that is never included in the sample. As a result, an additional micro-reference electrode can be built up without an interfering diffusion potential by means of a further ion-selective sensor for one of these ions (cation or anion). An example is the addition of a lanthanum fluoride suspension. The measuring electrode here is a fluoride-selective electrode. The storage chambers are sealed gas-tight after filling, which can be done for example with the help of a metallized film, which then breaks for use at a defined pressure. When storing calibration solutions containing gas standards, the entire measuring module is welded into a gas-tight film and some calibration solution is bound or introduced into this film to compensate for the partial pressure of the measuring gases in question.
The compartment ( 10 ) serves to temporarily store part of the sample. If necessary, a small amount of the sparingly soluble salt is stored here together with some heparin (in the case of a blood test).
The chamber ( 10 ) and the measuring channel ( 6 ) are left open. They are closed or opened by the fluidic control introduced in the recess ( 3 ). The fluidic control is adjusted by mechanical counterpressure when the measuring module (or cartridge) is inserted into the handheld measuring device in accordance with the requirements of the respective method.
The two halves of the measuring cartridge are brought together and glued using a sealing plastic spacer. (2 2 a, b) This is followed by the gas-tight sealing of the two Luer ports, wherein (2 a) side of the needle and (2 b) represents the syringe side.
The automated sequence of the measuring method according to the invention is schematically outlined in FIG. 2. In the first step (a) and (b), the syringe only conveys a defined amount of air into the measuring cartridge, whereby a first calibration solution ( 11 ) is transported into the measuring channel ( 6 ) via a corresponding valve control ( 3 ) and fills it, which is achieved by the same hatching is shown. In the second step (b) and (c), the sample (blood) ( 13 ) is removed via the chamber (compartment) ( 10 ) by means of the corresponding fluidics control. Part of the blood is also in the syringe. Now the measuring cartridge together with the syringe and needle is inserted into a corresponding opening of the handheld measuring device, whereby the fluidics control is switched over simultaneously and automatically and the individual chemical and biosensors are contacted with the corresponding counter-contacts of the handheld measuring device via a snap-in mechanism on their contacts ( 5 ) . At the same time, the needle tip is pushed through the septum into the current key electrolyte of the external reference electrode. The automatic measuring process is started by establishing a perfect electrolytic connection.
The double matrix membrane sensors used have already reached their stable measured value after these minimal operating operations. A software logic built into the hand-held measuring device then compares all sensor signals that occur in contact with the first calibration solution with the known, small variations in the manufacturing technology and, if necessary, then signals any deviations. After a certain waiting time of a few seconds, the syringe plunger automatically moves in the direction of emptying through the mechanism integrated in the hand-held measuring device and thus presses the sample (blood) ( 13 ) under constant pressure into the measuring cartridge. The sample flows back from the fluid control through the chamber ( 10 ) ( Fig. 2 cd) and is deflected by the fluid control at the needle end into the chamber ( 9 ), where it is protected from mixing by an air bubble displaced second calibration solution ( 12 ), which in turn flows past the sensors ( 4 ) through the fluid control at the syringe end into the measuring channel ( 6 ) and in turn displaces the first calibration solution ( 11 ) previously introduced into the chamber ( 8 ) before it passes through again the same fluid control at the needle outlet also enters the chamber ( 8 ). Fig. 2e then shows the final state. The sample liquid has completely displaced the second calibration solution from the measuring channel and can be measured.
In an advantageous micro version, a flow sensor construction with a sensor made entirely of silicon, with contacts on the rear, is pressed and glued into a corresponding recess in both measuring cartridge halves ( 1 a) and ( 1 b). It is only necessary to ensure that the rear contacts are conductively connected to the corresponding measuring cartridge bushings. Of course, all process versions can also be carried out with the micro version.
FIG. 3 shows an advantageous device for carrying out the alternative method by means of the safeguarding of measured values using the standard addition evaluation method. Here, the fluidic control can be dispensed with if the needle tip receives a flow resistance after penetration through the septum through a solidified gel, so that a simple slot pressure valve in the measuring cartridge is opened when the sample is reinserted, so that the calibration solution is introduced into the chamber ( 14 ) flows. In this version of the method and the device, the calibration solution is already in contact with the sensors ( 4 a... Z) in the measuring channel ( 6 ) by the manufacturer. This is where the measuring sequence takes place, as described above, so that the measuring cartridge is first measured in the measuring device alone. Then, after removing the gas-tight closures via the Luer connections ( 2 a) and ( 2 b), they are fitted with a needle and syringe and the sample is taken. The rest, the installation of the complete arrangements in the hand-held measuring device, is done analogously to the first exemplary embodiment. Because of the very narrow and small measuring channel ( 6 ), the sample located there does not mix very quickly with the syringe content. Therefore, the sample can be measured first and then the sample mixed with the standard solution. If the volume of the measuring channel and thus that of the standard solution (calibration solution) can be neglected compared to the amount of sample taken, the evaluation is simplified. A further preferred embodiment on this basis arises when the sample is drawn into the syringe via a second valve-controlled by-pass. Then the handling is exactly the same as in the first example. After inserting it into the handheld device with the corresponding automatic valve displacements, the multi-analyte calibration solution, which should be in the middle of the expected concentrations, is first measured. A logic software then checks the plausibility of these values on the basis of the known sample variations. Only then will the device give the green light for the syringe emptying mechanism. After displacement of the calibration solution from the measuring channel, the sample and standard mix in a collection chamber. After measuring the sample signals, the syringe plunger moves back towards the filling and brings the sample-standard mixture from the chamber back into the measuring channel, where another measurement takes place. Because of the small volume of the measuring channel compared to the sample volume, the unmixed remaining amount of sample can be neglected.
Fig. 4 shows the functional sequence (a, b, c, d) in a cartridge version which separates several calibration solutions ( 11 and 12 ) in a chamber ( 15 ) via easily movable, sealing pistons ( 16 ). The blood sample is drawn into the syringe via a channel in the other part of the cartridge.
The plunger is moved past an opening ( 17 ) by the syringe pressure during the emptying of the same after sampling, whereby the calibration solutions and the blood sample are passed through the sensor measuring chamber one after the other. FIG. 4 is largely self-explanatory.
