Abstract
Nach dem Stand der Technik werden Bronchusverschlüsse zur Abdichtung aufwändig mittels Ballonkatheter oder Tamponaden angestrebt, aber häufig nicht sicher erreicht. Die Erfindung ist ein selbst-expandierender Verschlussmechanismus, realisiert durch eine mittels Formschraube (6) befestigte Stanzbiegefeder als Spannelement (5), dessen 8 Spannarme sich an der Bronchuswand (1) verspannen und in Verbindung mit der an ihnen befestigten Polymer-Dichtungsfolie (4) einen, den Querschnitt eines Bronchus verschließenden Schirm bilden. AufgabenstellungenTasks
Zur Überwindung dieser Probleme beim mechanischen Verschließen von Bronchien, vorzugsweise zum Stillen von Blutungen, muss die Erfindung folgenden grundlegenden Aufgabenstellungen gerecht werden:
- • Materialauswahl, Formgestaltung und Anordnung der Funktionselemente des Verschlusses müssen unter Nutzung von Federvorspannung und Zwangsführung ein sicheres endobronchiales Festsetzen des Verschlussmechanismus durch selbsttätiges Verspannen zwischen den Bronchuswänden ermöglichen.
- • Das korrekte Platzieren des Verschlusses soll weitgehend frei vom Geschick des Operateurs, insbesondere bei schwierigen endobronchialen Sichtverhältnissen infolge von Blutmengen, mittels eines flexiblen Bronchoskopes und ohne Notwendigkeit einer Intubationsnarkose ablaufen.
- • Der Verschluss muss am Lokalisationsort im Ergebnis einer Federexpansion und gleichzeitiger Reduzierung deren Vorspannung unter Nutzung der Bronchialwand als Widerlager einen festen Sitz im Bronchus erlangen und dabei mit seiner schirmartig ausgebildeten Sperrschicht quer zur Längsachse des Bronchus ausgerichtet sein, um insbesondere unter den beengten und verzweigten Raumverhältnissen im Bronchus eine möglichst geringe ringförmige Kontaktfläche mit dem Bronchus zu beanspruchen.
- • Die Anwendung des endobronchialen Verschlusses ist zeitlich begrenzt. Damit gewinnt dessen schnelle und sichere Rückgewinnung eine besondere Bedeutung, insbesondere bei eingeschränkter endobronchialer Sicht.

- - einer Befestigungschraube für das Spannelement mit speziell halbrund geformtem Kopf als Lehre für die Biegung der Spannarme des Spannelementes zur Initiierung deren Vorspannung,
- - einem in ausreichender Länge gespeicherten Rückholfaden, zentrisch befestigt am Greifansatz.

9 ) außerhalb des menschlichen Körpers ohne Zuhilfenahme eines Bronchoskopes. Die erfindungsgemäßen Eigenschaften der Bauelemente (5 ) und (7), insbesondere die nach distal ausgerichtete Schirmbildung des Verschlussmechanismus im entfalteten Zustand sowie die konische Formschräge der proximalen Kante des Führungskolbens (7 ) inklusive der zentrierten Befestigung des Rückholfadens (9 ) am Greifansatz (8 ) des Führungskolbens (7 ), erlauben diese Möglichkeit.

- - Platzsparender Verschlussmechanismus mit universeller und selbständiger Anpassungsfähigkeit an die engen und verzweigten Räumlichkeiten des menschlichen Bronchialsystems (z.B. in unmittelbarer Karina-Nähe), bedingt durch seine geringe Raumtiefe in axialer Richtung infolge der schmalen ringförmigen Auflagefläche an der Bronchuswand.
- - Festsitzender, sicherer Verschlussmechanismus, im Ergebnis eines festen Andruckes der Spannarme des Spannelementes an die Bronchuswand, bedingt durch die selbstexpandierende Eigenschaft der Spannarme als Folge ihrer Vorspannung und der gewählten Materialeigenschaft als Stanzbiegefeder, der sowohl eine Dislokation weitgehend vermeidet, als auch seine Entfernung aus den Atemwegen unkompliziert ermöglicht.
- - Schnelle und unverzügliche Implantierbarkeit im Rahmen einer flexiblen Bronchoskpie ohne weiteren Geräteaufwand, auch bei einer limitierten Atmungssituation im Rahmen einer respiratorischen Notlage (z.B. bedingt durch endobronchiale Blutmengen).
- - Schnelles und sicheres Wiederfinden sowie zügiges Entfernen des Bronchusverschlusses (Okklusionsschirmes) ohne besondere Hilfsmittel, auch bei schwierigen endobronchialen Sichtverhältnissen infolge blutgefüllter Bronchien beziehungsweise bei endobronchialer Dislokation des Okklusionsschirmes.
