Description
The invention relates to an excision scalpel for obtaining tissue samples for histological examinations according to the preamble of claim 1 and to an excision unit having an excision scalpel according to the preamble of claim 19.
The invention relates to a field of medicine, in particular the field of medical instruments or tools for biopsy specimens, that is to say for obtaining tissue samples of the skin, mucous membrane, cartilage or other relevant tissue. Such tissue samples are usually obtained from small areas of skin or other tissues showing abnormalities whose nature is to be clarified by histological examination. In the tissue samples, of course, healthy areas can also be detected. Biopsies of the skin or other tissues in humans or animals are routine operations in dermatological practice and consist in the excision and removal of small areas of a patient's skin.
In
The use of simple scalpels to obtain biopsies is not common as the surgeon must make several cuts in sequence to expose both sides and the substrate of the tissue sample. This is time consuming and often results in irregularly shaped preparations.
Finally, skin biopsies are a major reason for delays in dermatological consultations. A biopsy takes approximately 15 to 30 minutes, during which all needed material is provided and used to perform the procedure. Therefore, this diagnostic option is often omitted, although it would serve the cause. Such failures are a common cause of legal action in countries such as the United States.
The invention is therefore an object of the invention to provide a biopsy tool of the type mentioned, with a quick and inexpensive biopsy to obtain tissue samples of the skin is possible. Another object of the invention is to provide an excision unit which, in conjunction with the biopsy tool, enables easy handling of the tissue samples obtained.
The solution of this object is achieved according to the characterizing part of claim 1, characterized in that the cutting part has at least two interconnected, flat leg having at least partially a respective cutting edge at a leading edge and that only one of the legs is firmly connected to the handle part.
In a preferred embodiment of the excision scalpel according to the invention, two legs of different length are arranged at an angle to one another, the longer leg being fixed in the grip part. and the legs thereby form a V-shape in the distal region of the excision scalpel, wherein the two cutting edges are formed in the tip region of the V and there seamlessly merge into one another. One or both legs of the cutting part may be kinked in the implementation of the invention, so that a "V" is formed, the upper end portions are extended by preferably parallel parts of the legs.
A cut through a skin area performed with such an excision scalpel thus results in an elongated, thread-like tissue sample with a triangular cross-section, the length of which can be determined by the surgeon at will. With a single instrument, seamless tissue samples can be obtained over the entire extent of a skin area to be examined. This possibility is not given in a conventional biopsy punch because the size of the punched-out area is determined by the diameter of the cutting part.
In a further, particularly preferred embodiment, the cutting part has a U-shape or teardrop shape with two substantially straight legs and a curved region or leg connecting the legs. The two straight legs of the "U" are of different lengths. The longer of the two legs is firmly anchored in the handle, while the other leg is exposed. In this design, not only the curved portion is provided with a cutting edge, but also adjoining him straight portions of the two legs of the cutting part of Excisionsskalpells are formed as cutting.
This embodiment of the excision scalpel results in a tissue sample with a cross-section in U-shape, with also freely selectable length. The width of the generally filamentary tissue sample is determined by the distance between the two legs, which is preferably in the range of 0.5 to 5 mm, with the range of 1 to 2 mm being particularly preferred.
In an advantageous modification of the above-described designs of the excision scalpel according to the invention, the region of the cutting part provided with cutting edges can also be arranged at an angle to the grip part, so that an arrangement in the form of a hockey stick results. This makes it possible to obtain tissue samples even below a skin layer.
An excision scalpel according to the invention can also be designed in such a way that the cutting part has a straight leg and a bent leg of different length, the cutting edges of which merge seamlessly with one another, wherein only the, longer leg is connected to the grip part. This design thus results in a J-shaped arrangement of the cutting edges of the cutting part, which facilitates the removal of the obtained tissue sample. Alternatively, only the bent leg of the cutting part can be connected to the handle part, while the straight leg is exposed.
A simplified removal of the sample is possible in all embodiments of the invention, when the legs or bent portions of the cutting part widen in a funnel shape counter to the cutting direction. The cutting direction is of course predetermined by the arrangement of the cutting on the cutting part.
In a particular embodiment of the excision scalpel according to the invention, the cutting part has a central straight leg, which is firmly connected to the grip part. At the distal end of this leg, ie at the tip region, a further leg, each with a cutting edge, is arranged towards each side. These two legs are bent or have a bend in their course, so that the cutting part has the overall shape of a ship's anchor. The two additional legs preferably have the same dimensions and shapes, but not is mandatory. Of course, they can also be of different sizes and shapes. With such a design of the cutting part, two separate tissue samples from the same region of a tissue can be obtained simultaneously with one cutting operation.
In a further advantageous embodiment of the invention, a lateral attachment part is arranged or formed on the handle part or on the cutting part, wherein the attachment part has an opening or a through hole in its free end region. This approach extends at right angles to the cutting direction and to the longitudinal axis of the preferably rod-shaped handle part. The hole formed in the attachment part serves to fix the excision scalp on a skin area by means of a needle. The excision scalpel can thus be guided in the manner of a compass around an axis of rotation defined by the needle, as a result of which a circular-arc-shaped tissue sample can be achieved.
The cutting part is preferably made in the embodiment of the invention of a metallic or ceramic material which is suitable for forming a particularly thin and sharp cutting edge. Particularly suitable for this purpose are stainless steels or ceramics, such as zirconium or aluminum oxide, which are used, inter alia, for the production of surgical instruments or household cutlery. Particularly preferably, the cutting part is made of a flexible, bendable band, of which at least one side edge is formed as a cutting edge. Such flexible, bendable bands made of stainless steel are known, for example, from commercial razors for wet shaving with a plurality of staggered blades.
The invention also includes embodiments in which the distance between two legs is variable. Here, for example, an adjusting screw used, which is mounted in a leg and can exert pressure against an opposite leg. It is understood that the material of the cutting part for this must be flexible enough to allow bending.
The grip part of an excision scalpel according to the invention is generally rod-shaped and, in a preferred design, at least partially flattened and / or structured on the surface. The structuring may, for example, in diamond-shaped elevations and depressions or in a corrugation. The two shapes serve to increase the grip and thus the safe guidance of the Scalp by the surgeon. The material used for the handle part are plastics or metals, such as stainless steel, or metal alloys, possibly with a chromium coating suitable.
An advantageous embodiment of the excision scalpel may consist in that the handle part has means for releasably fixing the longer a leg of the cutting part. This makes it possible to use as desired differently shaped cutting parts with a handle part. The fixation of the cutting part can be done for example by pressing the legs of the cutting part between jaws or by screwing.
Alternatively, the cutting part can be fixedly connected to the grip part, which is particularly advantageous in one embodiment as a disposable instrument. The connection of the two components is preferably carried out by pouring, for which plastics are particularly suitable as a material for the handle part. For secure fixation of the cutting part in the handle part, it is advantageous if the einzugießenden areas of the cutting part are provided with openings through which the sprue can pass.
In a particularly preferred embodiment, the handle part in the region adjacent to the cutting part on a predetermined breaking point, which may be formed for example by a notch. As a result, the material of the handle part is so weakened that can be done by bending a fraction of the handle part. Such a notch is preferably arranged so that the cutting part can be separated with a relatively short part of the grip part of a longer part. This makes it possible, after biopsy to transfer the cutting part with an attached tissue sample and the short piece of the handle part in a container of small volume.
In an alternative embodiment, the handle part is formed in two parts, wherein the two parts are releasably connected to each other. The connection of the two parts of the handle part can be done for example by nesting, for which the ends to be joined of the two parts are complementary to each other.
In the embodiment of the invention, the handle part can be equipped at will with additional functions, including, for example, a lighting device for the engagement area.
To achieve the object mentioned above, with regard to the excision unit according to one of claims 19 to 21, it is provided that the excision unit has a container part which can be connected to the excision scalpel according to the invention in a detachable manner.
In a preferred embodiment of the excision unit, the container part is tubular with a closed bottom and with an opening opposite the bottom, in which the excision scalpel is insertable and fixable. For this purpose, the gripping part of the excision scalpel is preferably designed such that it closes the opening of the container part as a lid. In a preferred embodiment, the grip part has a thickened region whose shape and dimensions are adapted to the shape and size of the opening of the container part such that the grip part with a tissue sample adhering to it can be inserted into the container part in the manner of a sealing stopper. This makes it possible to insert or remove the tissue sample into the formalin-filled container part without the need for another tool. At the same time, the removal of the tissue sample from the container is facilitated in the same way.
In a particularly preferred embodiment of the invention, the handle part is formed in two parts, wherein a first, relatively short part is fixedly connected to the cutting part and a longer, the actual handle forming second part is releasably connected to the first part. This design makes it possible, after biopsy, to separate the first part with the cutting part and the tissue sample attached to it from the second part and transfer it to a commercially available, preferably formalin-filled, and closable transport container.
The excision scalpel according to the invention largely eliminates the disadvantages of the excision tools according to the prior art described at the outset. It allows the rapid and cost-effective extraction of tissue samples of the skin with a low expenditure on equipment already at the first visit of the patient in dermatological practice. This saves time and reduces delays in diagnosis. In addition, it allows easy determination of the thickness of the detached skin layer in the biopsy, since the penetration depth can be determined by the pressure exerted on the instrument by the surgeon. As a result, epidermal, dermal, subcutaneous or deeper tissue samples are readily obtained. The cutting direction, i. the feed direction when performing the biopsy is determined by the position of the cutting edges of the excision scalpel, with a pulling cut being preferred, since it allows better control of the penetration depth and thus the thickness of the tissue sample. Due to the open design of the excision scalpel according to the invention, the area of intervention for the surgeon is easier to overlook than is possible with curettes or excision punches.
The wounds caused by the excision scalpel according to the invention have an elongate strip shape, the edges of which are easily contractible with a staple plaster and which can be supplied with a compression dressing for a short time. The scar formed afterwards is barely visible and thus much more aesthetic than a result caused by curettes or excision punches. This is particularly advantageous when the procedure has been performed on the face.
