It is directed to an apparatus and method whereby the duration of treatment of a patient with the blood treatment device or the time of treatment termination can be individually changed by the operator of the blood treatment device, and the changes are patient-specifically logged over a number of treatments. The possibility of changing the duration of treatment of a patient with the blood treatment device or the end of the treatment end depends on the recorded changes in previous treatments.
Description
The invention relates to the field of control of medical devices, in particular in blood treatment devices such as dialysis machines.
Hintergrundbackground
Blood treatment devices are medical devices in which the blood of a patient is usually treated extracorporeally. For example, drugs may be added to the blood or blood components removed from it, it may be heated or cooled. For this purpose, the blood is often pumped outside the body with a blood pump, treated and returned to the patient's body. Such a blood circulation is called extracorporeal blood circulation.
In the following, the invention will be explained with reference to the example dialysis machine as an embodiment of a medical device.
Other blood treatment devices include, for example, devices for supporting cardiac pulmonary action, such as blood oxygenators, or devices for supporting liver function that remove blood toxins from the blood by adsorption. Often, liver function support devices combine dialysis and adsorption techniques in one device, such as Applicant's Prometheus device.
Dialysis is a procedure for purifying the blood of patients with acute or chronic renal insufficiency. Basically, a distinction is made here between methods with an extracorporeal blood circulation, such as hemodialysis, hemofiltration or hemodiafiltration and peritoneal dialysis, which has no extracorporeal blood circulation.
The blood is passed in hemodialysis in an extracorporeal circuit through the blood chamber of a dialyzer, which is separated by a semipermeable membrane of a dialysis fluid chamber. The dialysis fluid chamber is flowed through by a dialysis fluid containing the blood electrolytes in a specific concentration. The substance concentration of the blood electrolytes in the dialysis fluid corresponds to the concentration in the blood of a healthy person. During the treatment, the patient's blood and the dialysis fluid on both sides of the semipermeable membrane are generally passed countercurrently at a predetermined flow rate. The urinary substances diffuse through the membrane from the blood chamber into the dialysis fluid chamber, while at the same time electrolytes present in the blood and in the dialysis fluid diffuse from the chamber of higher concentration to the chamber of lower concentration. If a pressure gradient from the blood side to the dialysate side is established on the dialysis membrane, for example by a pump which removes dialysate from the dialysate circuit downstream of the dialysis filter on the dialysate side, water passes from the patient's blood via the dialysis membrane into the dialysate circuit. This process of ultrafiltration leads to a desired drainage of the patient's blood.
In hemofiltration, ultrafiltrate is withdrawn from the patient's blood by applying a transmembrane pressure in the dialyzer, without dialysis fluid being conducted past the side of the membrane of the dialyzer opposite the patient's blood. In addition, a sterile and pyrogen-free substituate solution can be added to the patient's blood. Depending on whether this Substituatslösung is added upstream of the dialyzer or downstream, it is called pre- or post-dilution. The mass transfer occurs convectively during hemofiltration.
Hemodiafiltration combines the methods of hemodialysis and hemofiltration. There is both a diffusive mass transfer between the patient's blood and dialysis fluid via the semipermeable membrane of a dialyzer, as well as a filtration of plasma water by a pressure gradient at the membrane of the dialyzer.
The methods of hemodialysis, hemofiltration and hemodiafiltration are usually performed with automatic hemodialysis machines, such as those marketed by the assignee under the name 5008.
Plasmapheresis is a blood treatment procedure in which the patient's blood is separated into the blood plasma and its corpuscular components (cells). The separated blood plasma is purified or replaced with a substitution solution, and the purified blood plasma or the substitution solution is returned to the patient.
Dialysis machines, as an example of complex medical devices, have extensive functions. To control these functions, medical devices such as dialysis machines are equipped with at least one control device. This can be implemented as a CPU (central processing unit) or microcontroller, which is programmed by software programs. The operation of such devices is often done via touchscreen displays. Such a touchscreen display combines in a common surface an input and an output device in which it provides a touch-sensitive surface with which operator inputs are detectable.
Patients requiring dialysis must regularly undergo dialysis for several hours. Here, the duration of dialysis is set individually by a doctor, with the parameters of the dialysis treatment and the physiological parameters of the patient being included in the calculation of the duration.
For example, it is taken into account with which overhydration the patient appears at the beginning of the treatment. Overhydration is a measure of the excess water volume or weight in the patient's blood and tissues. Patients who suffer from renal insufficiency can not, or only insufficiently, excrete the water absorbed with food. This water accumulates in the blood and tissues of the patient and must be withdrawn from the patient by ultrafiltration dialysis treatment. The overhydration will i. d. R. determined by weighing the patient and comparison with a reference weight, the so-called dry weight. The ultrafiltration of the blood in a dialysis treatment can be carried out at a constant ultrafiltration rate, intermittently or along an ultrafiltration profile. Ultrafiltration rate, ultrafiltration volume and ultrafiltration profile are parameters that influence the duration of the dialysis treatment.
Another important dialysis goal is blood purification. The blood purification performance of the dialysis treatment is u. a. characterized by the parameter of clearance. The clearance K of a substance, for example urea, is the partial flow of the total flow through the dialyzer, which is completely freed from the substance in question.
For the effectiveness of a dialysis treatment, various parameters can be evaluated. Of importance, for example, is the so-called dialysis dose K · T / V, which is defined as the product of clearance K for urea and effective treatment time T of the dialysis treatment divided by the volume of distribution V of the patient for urea. The volume of distribution V for urea can, to a good approximation, be assumed to be constant over a dialysis treatment.
As well as for the ultrafiltration volume, the doctor prescribes a specific value for the dialysis dose to be achieved.
From this prescription and the set parameters of the dialysis treatment, such as used dialyzer, blood flow, ultrafiltration rate, dialysate flow, etc. ultimately results in a certain dialysis duration, which is usually between 3 and 5 hours. The duration of treatment represents a compromise between dialysis quality and patient stress. In general, a longer dialysis duration is preferable with regard to the therapeutic effect.
However, since shortening the dialysis time over a wide range does not result in any limitations that are directly noticeable to the patient, the short-term interest of the patients is usually to shorten the dialysis time.
The medical staff, for example, a dialysis nurse, during dialysis has an interest in adapting the actual duration of dialysis flexible to their workflow. Usually there are three parties (doctor, patient, nurse) in recurrent negotiation about the correct assessment of the dialysis time.
Beschreibungdescription
It is therefore the object of the present invention to make the adjustment of the treatment duration of a treatment, which is carried out with a medical device device, more flexible and to adapt it to the interests of the parties involved in the treatment, without restricting the treatment quality in the medium term.
This problem is solved by a method according to claim 1 and a medical device according to claim 10. The subclaims describe preferred embodiments.
According to the teaching of the present invention, the patient is allowed to change the duration of the dialysis automatically. For this purpose, the patient can actively change the duration of the dialysis treatment prescribed by the physician or resulting from the therapeutic target via an input device, for example via a touchscreen display as a combined input and output device.
Thus, the dialysis machine via an output device, a first dialysis duration and / or a first end time starting from suggest a specific starting time of the treatment. In the following, the patient has the option of confirming the dialysis treatment as proposed by the dialysis machine, but alternatively he can shorten or lengthen the treatment within certain limits. For this purpose, the patient can enter a second treatment duration deviating from the proposed first treatment period, or a second time of the end of treatment deviating from the proposed first time of the end of treatment.
In each case, the actual duration of dialysis is documented.
For this purpose, an individual time account is created for each patient. Such a time account is, according to the teachings of the present invention, a file in which the actual duration of dialysis over a predetermined number of treatments, for example, over the last three treatments, is set in proportion to the dialysis durations prescribed by the physician. This can be done, for example, by storing actual and target dialysis periods; it is also possible to store only the deviation, ie the difference between actual and target dialysis duration. It is essential that information can be derived from the time account to which extent the actual duration of dialysis deviates from the medical prescription. In accordance with the teachings of the present invention, an evaluation of the stored deviations takes place.
A net balance, in accordance with the teachings of the present invention, is understood to be a sum or sum of the deviations of the parameter under consideration that correlates with the sum.
Such a patient-specific file can be stored, for example, on a patient card designed as a memory card (SmartCard). Other embodiments provide the storage of the time account file in the dialysis machine itself or in third, with this example, by network connection communicating devices (server) before. It is only essential that the information of the time account is patient-specific and specifically linked and retrievable with a specific patient.
It is provided in one embodiment of the invention, from the stored information of the net balance of the dialysis time deviations for a patient to derive a limit, the maximum possible deviation of the actual treatment time, which is used to set up the dialysis machine for performing the treatment of the prescribed or limited to the proposed first treatment period. This takes into account that the net balance of the time account must never fall below a certain value.
It is also conceivable introduction of a maximum possible time credit, so a maximum possible excess of cumulative dialysis duration compared to the prescriptions. In general, however, the extension of the duration of dialysis is considered positive for the treatment effect.
Also, in accordance with the teachings of the present invention, it is possible to automatically extend the doctor's prescription with respect to dialysis duration by a certain amount of time if it is determined, for example, by reading a patient-individual file that the previous treatment has been shortened or Net balance of the time account is negative.
An automatic extension of the current dialysis duration can also be made dependent on how long, d. H. over which number of treatments the net balance of the time account is negative and / or also how strong the net balance of the time account is negative, or also of combinations of these two quantities.
Thus, only a slightly negative net balance of the time account can be tolerated over a longer period of time (without resulting in an automatic extension of the current duration of the dialysis), as a strongly negative net balance of the time account, for example, on the edge of the limit for the maximum possible shortening of the time account Cumulative dialysis duration moves.
The proposed first treatment time or the proposed first time of the end of treatment thus depends on the evaluation of at least one stored deviation of the Dialysis duration of a previous treatment of the proposed for this previous treatment from the dialysis machine first treatment period for each patient.
The ability to customize the dialysis time creates a number of advantages for the patient.
Thus, a lower waiting time of the patient can be achieved after dialysis, if several patients are driven home together, since the dialysis duration can be flexibly coordinated by the invention.
The patient gains greater freedom to reduce dialysis in cases of discomfort. Thus, the patient receives an albeit limited influence on his own treatment. His personal responsibility is thereby strengthened, which can have a psychological effect.
There is also the possibility for the patient to extend the treatment time in the event of well-being and to thus possibly create a credit on a time account, which he can resort to in a subsequent treatment. For example, the next treatment time could conflict with a private appointment. With a time credit, the patient has the opportunity to shorten this treatment and to relax the temporal situation.
However, the invention also offers the care-giving medical staff the opportunity to change the dialysis duration individually, for example if it is foreseeable that, without changing the dialysis duration, several patients will terminate the dialysis simultaneously or within a short period of time. In order to avoid stress situations when caring for patients at the end of treatment, the individual dialysis duration of several patients can be adjusted so that they no longer stop their treatment at the same time or within a short period of time. Such an adaptation is preferably an extension of the duration of dialysis, so as not to jeopardize therapy goals. In addition, an extension of the duration of dialysis over the prescribed duration also means the establishment of a time credit, which can be used in subsequent treatments.
For the time of treatment end, various variants are conceivable. Although the end of dialysis treatment as an endpoint definition is obvious, in practice, the time at which a patient can leave the dialysis center can be much more practical.
This point in time is determined by the time required for the blood to flow back into the extracorporeal blood circulation, to the patient and the time for the patient to be separated from the extracorporeal blood circulation, and to the wound care time at the puncture sites.
For each patient, the sum of these two times can be stored and taken into account. The actual dialysis treatment is thus terminated by the sum of these two times, rather than the specified time of the end of treatment, to ensure that the patient can leave the dialysis room at the desired time. If necessary, the operator can select by means of a corresponding input on the dialysis machine whether the stated time of the end of treatment should relate to the dialysis treatment or to the time from which he can leave the dialysis center.
In the individual change of the dialysis duration, the ultrafiltration volume must be paid particular attention. In practice, the patient equates the weight loss, which corresponds to the total ultrafiltration volume, the permitted amount of drinking until the next treatment.
Shortening the duration of dialysis leads to over-hydration in the patient, which in turn can lead to increased blood pressure with negative consequences.
An extension of the duration of dialysis, however, leads to an increased weight loss, which is physically not tolerated by some patients, for example, due to adjusting low blood pressure.
With differences in dialysis duration of less than about 5-10% compared to the prescription, these aspects can be neglected in most patients. For higher differences, or in the case of physically robust patients, the adjustment of the ultrafiltration rate or the ultrafiltration volume can be adapted to the duration of the dialysis.
- a) If the dialysis duration is shortened compared to the prescription, dialysis is stopped without the prescribed dry weight being exactly reached. The patient remains slightly overhydrated. He can compensate for this with a smaller amount of drink until the next treatment. Optionally, the ultrafiltration volume of the next treatment may be automatically increased to fully or partially compensate for the overhydration.
- b) If dialysis duration is shortened compared to the prescription, dialysis is stopped and the prescribed dry weight is reached. This is achieved by increasing the ultrafiltration rate after adjusting the dialysis duration so that the prescribed ultrafiltration volume can be withdrawn within the shortened duration of the dialysis. It should be noted that maximum values exist for the ultrafiltration rate, which must not be exceeded. Optionally, despite increasing the ultrafiltration rate within the permitted limits, the prescribed ultrafiltration volume within the shortened dialysis duration can not be withdrawn.
- c) If the duration of dialysis is prolonged compared to the prescription, more water is withdrawn than is prescribed (the prescribed dry weight is thus undershot) with the same ultrafiltration rate unchanged. At typical dialysis settings, a 5-10% time extension means 150-300 ml additional volume of water taken from the patient. Given the usual uncertainty in prescribing the dry weight of more than 500 g, this can be tolerated by many patients.
- d) In contrast to the prescription of prolonged dialysis duration, the ultrafiltration rate is reduced so that exactly the prescribed weight target is achieved. This is particularly advantageous if patients react sensitively to the final weight achieved (eg with severe fatigue or hypotonia). The dialysis treatment is thus gentler for the patient.
Similarly, in accordance with the teachings of the present invention, other parameters of the dialysis treatment may be adjusted depending on the current duration of the dialysis or the time of termination of the dialysis treatment in order to achieve therapy goals despite the dialysis duration deviating from the medical prescription or to match the patient's condition , In addition to the ultrafiltration volume and the ultrafiltration rate, the blood flow rate, the dialysate flow rate and the dialysate composition, as well as the substitution flow rate and the substitution volume, are also conceivable.
In the treatment with other medical devices, the skilled person knows a variety of parameters of the treatment, which can be made equally dependent on the duration of treatment or the time of treatment.
