Abstract

Description

Die Harninkontinenz ist auch beim Mann leider ein weit verbreitetes Leiden in der Bevölkerung. Besonders häufig ist sie bei alten Menschen durch eine Erschlaffung des Schließmuskels, nach Schlaganfall, bei Parkinson- und Alzheimererkrankung, operativen Eingriffen an der Prostata und Ouerschnittslähmung bei Unfallopfern, um nur einige wichtige Beispiele zu nennen. Wenn irgend möglich sollten hier, sehr komplikationsarmen, externen Harn ableitenden Methoden, wie über Kondom-Urinale oder auch Harn aufsaugenden Vorlagen der Vorzug vor invasiven Methoden, wie Blasen- oder Harnröhrenkathetern, mit ihren hohen Infektions- und Komplikationsraten, gegeben werden. Bei der Harninkontinenz des Mannes ist, wenn sich physiologisch und medizinisch keine Kontraindikation ergibt, das Kondom-Urinal eine sehr sichere, einfache und hilfreiche Methode zur externen, hygienischen Ableitung des Urins in einen Auffangbeutel. Die heute in unterschiedlichen, optimalen Passgrößen erhältlichen Kondom-Urinale, vor allem aus Latex, synthetischem Kunststoff-Elastomer und Silicon können, wenn schon mit einer einheitlichen Kleberschicht versehen, direkt auf die Haut des Penis aufgerollt und durch Andrücken verklebt werden.. Da Kleber jedoch recht unterschiedlich, vor allem bei empfindlicher Haut vertragen werden, stehen auch unbeschichtete Kondom-Urinale zur Verfügung, die individuell angepasst, mit verschiedensten Klebern oder Klebstreifen auf der Haut fixiert werden können. Alle diese heute auf dem Markt erhältlichen Kondom-Urinale sind aber nur für den Einmalgebrauch und kontinuierliche Anwendungszeiten von 12–max. 48 Stunden konzipiert. Trotz kurzer Anwendungszeiten können lokale Probleme, wie vorzeitige Ablösung von der Haut, Harnleckagen und vor allem Hautentzündungen, Pilz- und Bakterienbesiedlungen, auftreten. Die unphysiologischen Belastungen der Penishaut mit Harn und die notwendigen häufigen Kondom-Urinalwechsel bedeuten immer, vor allem auch durch das Ablösen der Kleber eine zusätzliche Hautbelastung mit erhöhter Entzündungsgefahr und nicht nur Unbequemlichkeiten und nicht zuletzt auch nicht unerhebliche Kosten. Kondom-Urinale für den Einmal- und Mehrmalgebrauch zur externen Harnableitung, die zur Fixierung am Penis keine Klebstoffe benötigen sind bis jetzt noch nicht erhältlich, obwohl besonders bei Senioren eine atrophische, leicht verletzliche Haut sehr häufig ist noch viel zu viel invasive Harnableitsysteme wie Blasen- oder Harnröhrenkatheter, mit ihren bekannt hohen Infektion- und Komplikationsraten, oft ohne strenge medizinische Indikation, aus rein pflegerischen Gründen verwendet werden. Es wäre deshalb aus den genannten Gründen ein externes Harnableitungssystem wünschenswert, das aber folgende Forderungen erfüllen sollte:

1. die sichere mechanische Fixierung am Penis ohne Harnleckagen intermittierend über mehrere Tage, Wochen, Monate bei hoher Hautverträglichkeit und ohne die Verwendung von Hautklebern.
2. Ein einfaches Anlegen und Entfernen der externen Harn-Ableitungsvorrichtung bei geringster Traumatisierung des Penis und der Haut durch Entfernung der Kleber, einer notwendigen Rasur von Scharnhaaren und Mazeration durch Harn.
3. Vermeidung eines Harnrückflusses in Harnröhre oder Harnblase
4. Einfache und bakteriologische und hygienische Kontrollmöglichkeiten
5. und wenn möglich eine Reduzierung der Kosten im Vergleich zu den bei der heute üblichen Verwendung von Einmalartikeln.

Diese Arbeitsstellung kann durch ein über die Glans Penis gestülptes, kleines gummielastisches, dünne Lochplatte gehalten werden. Die Kondom-Urinal-Haltevorrichtung (s. 3) wird nun, mit ihrem Fortsatz über dem Eichelkranz 22 s. 4 gelegt und dabei deren Schnappleiste 16 in den Sulcus 23 des Penis eingehängt. Die untere Öffnung 11 der Haltevorrichtung 18 wird nun mit den Finger gespreizt und auf den vorderen Teil der Glans aufgeschnappt. Das Vorhautbändchen (Frenulum Praeputii) kommt dabei in die breite Spaltöffnung 13 zu liegen. Nun wird die Glansspitze 20 mit der Harnröhrenmündung 21 manuell etwa in die Mitte der unteren Öffnung 11 gebracht und nun mit den Fingern einer Hand die Glansspitze 20 und Haltevorrichtung fest umschlossen und das Kondom-Urinal mit Hilfe des Applikators 1 aufgesetzt oder bei der Ausführungsform von 7 ohne Applikator direkt auf Teil 25 aufgesetzt und anschließend über die Spange 26 mit ihrer abgeschrägten Kante 27 abgerollt. Das Kondom-Urinal wird nun faltenfrei auf die gesamte Haltevorrichtung 18 bzw. 30 abgerollt, bis mit Überschreitung des Fortsatzes 14 und dem Eichelkranz 22 die verbleibende noch aufgerollte Wand des Kondom-Urinals als elastischer Halte- und Dichtwulst 24 in den Sulcus (Furche) 23 einschnappt (s. 4 und 5). Das vor allem bei ungenügend zurückgezogener Vorhaut mögliche Einklemmen derselben im Sulcus 23, muss vorsichtig durch schrittweises Zurückziehen der eingeklemmten Vorhautteile behoben werden. Einfacher ist es, diese Schwierigkeit zu vermeiden durch die Verwendung einer gummielastischen, dünnen Lochplatte, die über die Glans gestülpt wird und dann die Vorhaut sicher zurückhält. Nur bei freiem Sulcus 23 ist ein beschwerdefreier und sicherer Halt des Halte- und Dichtwulstes 24 gewährleistet und nachfolgend das Vorschieben der Vorhaut über den Dichtwulst und die nachfolgenden Teile der externen Harn-Ableitungs-Vorrichtung bis über den sich verjüngenden Teil 19 des Kondom-Urinals leicht möglich s. 6. Die entscheidende Sicherung des Halte- und Dichtwulstes 24 gegen ein Herausspringen aus dem Sulcus (Furche) 23 des Penis und ein Abrutschen über die Glans z. B. beim Einwirken von Zugkräften und die dann daraus folgende Harnleckage, stellt die Haltevorrichtung 18 dar.
