Abstract
Description
Die Erfindung betrifft eine Vorrichtung zur Erfassung einer Fluoreszenz. The The invention relates to a device for detecting a fluorescence.
Sie erlaubt bei Kenntnis der jeweiligen Fluoreszenz-Eigenschaften die Ermittlung der molekularen Beschaffenheit eines jeweiligen untersuchten Stoffes.devices for fluorescence detection, subsequently also as fluorescence scanner referred to be used to detect a variety of molecular factors. Namely, substances with different molecular properties can be different Have fluorescence properties that can be detected selectively. The Fluorescence detection is optically based and therefore not or only minimally invasive. It allows the knowledge of the respective fluorescence properties Determination of the molecular nature of a respective investigated Substance.
In In medicine, molecular properties, e.g. referred to as "molecular signature", information about the health condition of a living thing or patient and therefore can be evaluated diagnostically. In particular, molecular signatures can be used for the detection of cancer. Also other diseases like e.g. Rheumatoid arthritis or arteriosclerosis of the carotid artery can be identified.
The excitation light may be e.g. in the infrared range (IR) or in the near infrared (NIR) range. Which frequency range is suitable, does not hang last also from the substance to be examined. substances which themselves have no molecular or chemical properties, the for the Fluorescence detection would be appropriate can in suitable molecular "labeled". For example, can be markers can be used, which bind with appropriate preparation only to very specific molecules or attach. So such a marker works so therefore after a mechanism that figuratively conceived as a key-lock mechanism can be. Marker and molecule to prove fit like key and lock to each other while the marker does not bind to other substances. Indicates the marker known fluorescence properties, it may after binding or attachments are optically detected. The detection of the marker then lets to indicate the presence of the labeled specific substance. to Detection so only one detector is needed, the light in those wavelength can detect the substance in question or even the used marker on excitation radiates.
When Application of such fluorescence methods are primarily investigations near-surface Areas or examinations of the open body (intraoperative applications) at. examples for such investigations would be the detection of fluorescently labeled skin cancer or detection of tumor boundaries in the resection of fluorescently labeled tumors. For example, The company NOVADAQ has developed a system to perform intraoperatively Coronary arteries and the function (i.e., flow) of bypasses are visible close.
One Research subject of biotechnology are fluorescent metabolic Markers that are either exclusively in certain regions (eg tumors, inflammations or other specific disease sites), or anywhere in the body distributed but only specifically in certain regions, e.g. by tumor-specific enzyme activities (and for example by additional Irradiation by light).
Daneben gewinnen so genannte "smart contrast agents" zunehmend an Bedeutung. 780 nm (Emission). In of medical diagnosis are so-called marker substances Fluorophores are known, e.g. Indocianin green (ICG), e.g. bind to vessels and are optically detectable, so that in an imaging process the contrast, represented by the vessels be increased can be. In addition, so-called "smart contrast agents" are becoming increasingly important. These are activatable fluorescent labels, e.g. on tumor tissue bind and their fluorescent properties by binding first be activated on the substance to be marked. Such substances can from self-quenched stains, e.g. Cy5.5, persist, to larger molecules via specific ones Be bound to peptides. In turn, the peptides can be targeted by specific proteases, the e.g. be produced in tumors, recognized and split become. Splitting releases the fluorophores and are no longer self-inhibited but develop their fluorescent Properties. The liberated fluorophores may be e.g. in the near IR wavelength range be activated at 740 nm. An example of a marker on this basis is AF 750 (Alexa Fluor 750) with a defined absorption and emission spectrum in the wavelength range of 750 nm (excitation) or 780 nm (emission).
In medical diagnosis, such activatable markers can be used, for example, for the intraoperative detection of tumor tissue in order to be able to identify the diseased tissue exactly and then be able to remove it. A typical application is the surgical treatment of ovarian cancer. The diseased tissue is typically surgically removed and then chemotherapy. Due to the increased sensitivity of a fluorescence detection, the diseased tissue including various surrounding disease centers could be better recognized and thereby fully be removed more permanently.
In Breast cancer therapy are typical operative lumpectomies (or mastectomies) and lymph node sections and lymph node withdrawals. Lymph nodes are typically trapped by 99mTc sulfur colloid Optically recognized compound with low molecular weight methylene blue. The radioactive mTc sulfur colloid could be detected by the use of fluorescence detection be avoided, with correspondingly positive effects on the Health of the patients.
Die Behandlung rheumatoider arthritischer Gelenks-Erkrankungen könnte das Medikamenten-Monitoring dahingehend verbessert werden, dass das Ausmaß der Protease-Überproduktion quantitativ detektiert werden und die als Gegenmittel vorgesehene Medikamentierung quantitativ angepasst werden könnte.at the removal of brain tumors is the precise narrowing of the tumor tissue, which is achievable by using fluorescence detection, of more obvious Importance. The treatment of pancreatic tumors could be of additional lymph node withdrawals, which would be identified by fluorescence detection, im With regard to possible bowel cancer benefit. In the treatment of skin cancer could improved detection of skin neoplasms by fluorescence detection become. The treatment of rheumatoid arthritic joint disease could the drug monitoring will be improved so that the extent of Protease overproduction be quantitatively detected and provided as an antidote Medication could be quantitatively adjusted.
Eine Vorrichtung zur Fluoreszenz-Detektion sollte daher für den Operateur gut handhabbar und im sterilen OP-Bereich einsetzbar sein.at the treatment of the exemplified diseases and others Illnesses are typically performed an operation in which diseased tissue is surgically removed. In support of the Surgery, a fluorescence detection can be performed the detection of the diseased tissue parts to be removed while running Surgery, so if necessary at the open wound, to improve. To do this the tissue parts before surgery with a suitable substance be marked, which then by connection to the diseased tissue parts is activated. A device for fluorescence detection should therefore for The surgeon can be handled easily and used in the sterile surgical area be.
The Recognition of a region fluorescently labeled in this way takes place via irradiation of the region with light in the special excitation wavelength of the Fluorescent dye and detection of the emitted light in the corresponding emission wavelength of the fluorophore. A fluorescence scan will created in which a fluorescence image based on fluorescent light is recorded and an optical image based on visible light. Subsequently Both optical and fluorescence image are superimposed to fluorescence in the context of the optical image. Based on the superimposed Representation (fusion) of optical and fluorescence image on one Display device allows the surgeon to recognize the tumor tissue and at the actual patient's body locate. The fused image with the fluorescently labeled tissue is displayed on a small screen on the fluorescence scanner or on an external computer with image processing software.
Usually the excitation of the fluorescence of the marker by light and the detection device must have a light source of sufficient strength, around the tissue to be examined to a depth of 0.5 to 1 cm to penetrate. additionally An optical detector is required, which is the fluorescent light and, second, if the fluorescent light is not in the visible wavelength range, also be able to take a picture in the visible wavelength range.
The Fluorescent light of interest is often in the infrared wavelength range (IR) or near infrared (NIR). Excitation light suitable Wavelength, for the Fluorescence typically in the near IR range up to 700nm, and more adequate intensity to the sufficient Penetration of tissue is with the known bulbs only with relatively little Efficiency can be generated. With sufficient intensity in the interest Wavelength range Due to the low efficiency, the heat development is enormous. simultaneously the energy required to generate the excitation light is considerable. A wired power supply to provide the required Energy would however, allow the device to become unwieldy and just in the surgical area in which cramped space must be extremely obstructive. Furthermore separates active cooling in the sterile field Lighting means, e.g. by blower, because the sufficient sterilization of an actively cooled device hardly possible is.
The The object of the invention is therefore an easy to handle and in the sterile surgical field usable device for fluorescence detection specify.
The Invention solves this object by a device having the features of the main claim.
According to the main claim, the device comprises an energy source, at least one light source for generating excitation light, at least one detector for detecting fluorescence light and a data acquisition unit, wherein the excitation light source is operated pulsed. A basic idea of the invention is pulsed operation the excitation light source. The pulsed operation simultaneously reduces the energy requirement and the loss heat generated. Due to the pulsed operation can therefore be dispensed with both a wired power supply and active cooling. In addition, pulsed operation causes short exposure times for the fluorescence images, thus reducing motion artifacts. In summary, the pulsed operation of the excitation light source therefore makes the device mobile and also sterilizable. The image detector can be embodied, for example, as a CCD camera, but other image acquisition technologies can also be used.
