Abstract
The invention serves for the temporary electrical stimulation of the heart, especially in emergency situations. The aim of the invention is to control when using oesophageal placed pacemaker pulse delivery by R-waves (chamber potentials). According to the invention, this is achieved in that, in addition to the stimulation electrodes positioned in the vicinity of the heart, two ring-shaped discharge electrodes are arranged at a distance of at least 20 cm apart, the aboral discharge electrode being closer than the oral discharge electrode to the different stimulation electrode. This positioning of the lead - off electrodes allows safe R - wave inhibition of the external pacemaker.
Description
Titel: CsophaguselektrodensondeTitle: Csophageal electrode probe
Anwendungsgebiet der Erfindung:Field of application of the invention:
The invention relates to a multipolar Csophaguselektrodensonde for temporary electrical stimulation of the heart, in particular in 5Testituationen.
Wochenschrift 99, 1969, S. 1661). Characteristics of the known technical solutions: Sine Slektrostimulation of the heart via ösophagesl placed pacemaker electrodes is known. The electrodes, which are arranged annularly on a esophageal probe, are brought to a position close to the heart for stimulation (Roth,.; Salzmann, C. and Gärtner, H * -.; Electrical stimulation via a cesophageal electrode, Switzerland, med. Wochenschrift 99 , 1969, p. 1661 ).
In order to allow a fast and safe positioning of the sttmuiation electrodes, an inflatable gastric balloon was provided on the aboral end and several annular electrodes were arranged evenly spaced along the probe (DD-WP 133 4CO).
It is also known to use multipolar esophageal electrode probes, in particular in emergency situations, simultaneously to the ECG department.
Alle bekannten Csophaguselektrodensonden haben den Nachteil, daß sie für eine R-Wellen inhibierte Elektrostimulation nicht eingesetzt werden können, da die über dieAll known esophageal Csophaguselektrodensonden have the disadvantage that they can not be used for an R-wave inhibited electrostimulation, since the on the

Stimulation electrodes allow derivable SKG signals no differentiation between P-wave (atrial potential) and R-wave (Xaminergic potential). However, inhibition of P-wave pacing (atrial potentials) is undesirable and can lead to threatening situations, especially if cardiac pacemaker responses are triggered in the event of atrioventricular block. In this situation, despite cardmer arrest occurs no pacemaker impulse delivery, since there still existing 7orhofpotential inhibits the stimulation and dasau3 the life-threatening condition of a cardiac arrest results. However Η-wave inhibited Elektrostiaulation of the heart is also in lotfall for suppressing the Herzschrittmacherimpulsaogabe upon the occurrence of self-reactions of the heart chamber au request, to prevent the incidence of an electrical stimulation pulse in the vulnerable phase of a cardiac action with the risk of induction of KammerfІілздегп.
Ziel der Erfindung:Object of the invention:
The aim of the invention is to control the pacing pulse delivery by B-waves (Xaiamer potentisles) when using esophageal placed pacing electrodes.
Darlegung des Wesens der Erfindung:Explanation of the essence of the invention:
The invention has for its object to develop a Csophaguselektrodensonde, with a inhib - 'Tellen inhibited Slektrostiiaalation the heart without? -Wellen inhibition is achieved.
According to the invention, the object is achieved by arranging two annular discharge electrodes at a distance of at least 20 cm from each other on the Cscphagussonde in addition to the well-known Stlmulationselektroden located near the heart, wherein the aboral Ableitelektrode closer to the oral Ableitelek-
A particularly safe R-wave IchiDieräug is achieved if the distance of the drainage electrodes from each other is 23 s and the distance of the aboral drainage electrode from the different stimulation electrode 10 ca. It is expedient that an inflatable gastric balloon be provided for the rapid and secure positioning of the differentiation stimulation electrode between it and the aboral discharge electrode. Another further embodiment is that the stimulation electrodes have a relatively long length in order to guarantee that, despite anatomical differences At least a portion of the differentiation stimulation electrode should be in the optimal stimulation position in which a minimum stimulation energy is required. In order to avoid anodal stimulation, it has been found to be favorable that for the formation of the electric field the effective area of the different stimulation electrode is smaller than that of the indifferent stimulation electrode.
Gleichzeitig wird die Inhibierung de3 Herzschrittmachers durch ?-The invented esophageal electrode probe was found by extensive experiments and represents a simple and effective solution that could not logically be derived from the field of cardiac stimulation. It is characterized by an optimal arrangement of the drainage and stimulation electrodes both to each other and to each other and allows for a safe electrical stimulation of the heart to be challenged R-WeI-1en inhibition of the pacemaker. The 3-ellen inhibition prevents the occurrence of энеггепз-3 action in the epileptic stimulation pulse in the vulnerable phase of cardiac action with the risk of triggering Karsner's fibrillation as a pacemaker complication. At the same time the inhibition of the pacemaker by? -

3P wave inhibition could lead to cardiac arrest as a result of ventricular arrest in Уаііе total atrioventricular block. Due to the arrangement of the discharge electrodes according to the invention, a pacemaker pulse delivery is triggered immediately only if there is no chamber potential. This significantly increases the safety and efficacy of heart stimulation by means of esophageal pacemaker electrodes, especially in emergencies.
